농촌지역(農村地域) 주민(住民)의 만성퇴행성질환(慢性退行性疾患) 유병률(有病率) 및 이용의료기관(利用醫療機關) (The Prevalence of Chronic Degenerative Disease and Utilization of Medical Facility in Rural Population)

  • 경상북도 문경시지역 중 9개 읍면에 거주하는 30세이상 주민 28.883명을 대상으로 1993년 10월 1일부터 1994년 2월 28일까지 건강면접조사한 자료중 5,797명을 무작위로 추출하여 만성퇴행성질환 유병률과 이용의료기관실태를 분석하였다. 인당유병률은 1,000명 당 336이었고 남녀별로는 남자는 278, 여자는 388이었다. 건당유병률은 367이었으며 남자 300, 여자 425였다. 건당유병률은 여자가 남자보다 유의하게 높았다(p<0.05). 연령별 인당 유병률은 30대는 106, 40대 223, 50대 366, 60대 407, 70세 이상 457로서 연령이 높아짐에 따라 유병률도 증가하였다. 만성퇴행성질환 건당유병률 1위는 신경통(128), 2위 만성위염(64), 3위 관절염(54), 4위 고혈압(44), 5위 당뇨(14) 순이었다. 남자는 1위가 신경통, 2위 위염, 3위 관절염, 4위 고혈압, 5이 천식이었고, 여자는 1위 신경통, 2위 관절염, 3위 위염, 4위 고혈압, 5위 당뇨였다. 신경통, 관절염, 고혈압 등의 질환은 남자보다 여자의 유병률이 유의하게 높았으며, 주풍(뇌졸증), 결핵, 간경화증 등의 유병률은 여자보다 남자가 유의하게 높았다. 만성퇴행성질환자의 이용의료기관은 최초에는 병 의원 이용율이 높았고, 최종 또는 현재는 보건(지)소, 의원 및 보건진료소를 많이 이용하였다. 질병별로는 신경통, 관절염 천식은 최초에는 의원을, 최종 또는 현재는 보건(지)소, 조건진료소를 많이 이용하였다. 고혈압과 결핵은 최초, 최종 모두 보건(지)소를 가장 많이 이용하였고, 당뇨, 중풍, 암, 심부전증, 간 경화증, 신부전증은 병원을 가장 많이 이용하였다.

Effect of diabetes, hypertension, and obesity management on visual acuity in geriatric cataract surgery patients

  • Method : Using annual statistics of major surgeries provided by the National Health Insurance Corporation, it figured out what changes come in visual acuity by the number of cataract surgeries nationwide plus age, gender, and geriatric disease every 3 years from 2013 to 2019 through joint point regression for statistics. Objective : This study is intended to identify the relationship between geriatric diseases (diabetes, hypertension, obesity) and visual acuity in geriatric cataract surgery patients. Result : First, geriatric diseases of cataract surgery patients were closely related to diabetes, hypertension, obesity, smoking, and drinking. In particular, diabetes, hypertension, and smoking had a high prevalence rate. There was no difference in gender and age. Second, 72% of all geriatric cataract surgeries were performed at the clinic level, and intraocular lens that was used after geriatric cataract surgery accounted for the majority of monofocal intraocular lens as 96.6%. Third, the visual acuity in geriatric cataract surgery patients improved from an average of 0.40 before surgery to 0.06 after surgery, and visual acuity improvement was found in 95% of them. These results suggest that geriatric cataract patients can expect visual acuity stabilization and positive visual acuity improvement through early surgery.

서울시 교직원 당뇨병의 역학적 특성 및 관련위험요인분석 (A Study of the Epidemiological Characteristics and Related Factors in School personnel with Diabetes Mellitus)

  • Purpose: To describe the prevalence, epidemiological characteristics, and related factors of diabetes mellitus in school personnel. Method:5,384 school personnel (2,638 males 2,746 females) received physical examinations at the Seoul School Health Center in 2002. Using the American Diabetes Association criteria, and fasting blood sugar(FBS) was classified as normal at 110>=FBS, borderline 111-125, and the diabetes group 126<=FBS. Related factors of each group were evaluated by gender. Results: In the diabetes group, there were 4.74% males and 0.8% females, and the borderline group was 6.37% males and 2.22% females. The older age group gad a higher distribution in both males and females in the diabetes group. In the diabetes group, the mean and abnormal rate of BMI, systolic hypertension, diastolic hypertension, total cholesterol, GOT, GPT, and GTP were higher than in the normal group. In life style factor, the diabetes group gad a higher distribution of meat eating and smoking more than 20 years than the normal group. The effect of FBS on BMI, BP(blood pressure), total cholesterol, GOT, GPT, and GTP were investigated after controlling for confounding variables. In the borderline and diabetes group, the mean of BMI, BP, total cholesterol, GOT, GPT, and GTP was higher in dose-response effect. In the logistic multiple regression, the related factors of the diabetes group were hypertension and abnormal GTP in males, and abnormal total cholesterol and GTP in females. In the borderline group, the related factors were meat eating, hypertension, and high GOT in males and the older age group, and hypertension, total cholesterol, and abnormal GTP in females. Conclusions: It is possible to manage all related factors of diabetes mellitus except for age. An appropriate program for health promotion is necessary.

한국인의 이상지질혈증 발생 위험 요인 및 약물복용이행 영향 요인 평가: 2013-2015 국민건강영양조사 자료 이용 (Risk Factors of Dyslipidemia and Related Factors of Medication Adherence in Korea Adults: KNHANES 2013-2015)

  • Purpose: Dyslipidemia was a known risk factor for cardiovascular disease and was a leading cause of mortality in worldwide. This study aimed to determine the factors associated with prevalence and medication treatment of dyslipidemia in Korean adult population. Methods: In this study, based on the criteria set by the Korean Society of Lipidology and Atherosclerosis, the factors associated with prevalence and medication treatment of dyslipidemia was evaluated in a population of 12,506 people ($age{\geq}20$), who participated in the Korean National Health and Nutrition Examination Survey (2013-2015). The findings were tested by using multivariate logistic regression. Results: Dyslipidemia prevalence rate was 36.5%. Among populations with dyslipidemia, 17.5% were treated with lipid-lowering drugs. In the multivariate logistic regression model, male, increase in age, lower education level, non-drinker, current smoking, less physical activity, increase of body mass index, hypertension, and diabetes were associated with an increased odd of dyslipidemia. Female, increase in age, higher income, excess fat intake, hypertension, diabetes, myocardial infarction, and angina were associated with an increased odd of medication treat. Conclusion: The results of this study could be used to screen patients at the high risk for dyslipidemia or to predict medication adherence.

지방자치단체의 보건재정역량과 지역주민의 건강행태 간 관련성에 대한 단면조사연구 (Association between health financial capacity of local governments and health behaviors of local residents: a cross-sectional study)

  • Objectives: The budget gap in the health sector of local governments affects the supply of health services, which can cause the health gap. This study classified local governments according to their financial characteristics, such as local financial independence and health budget level. It analyzed the health behaviors and disease prevalence of local residents to examine the effect of local government financial investment on the health of local residents. Methods: To classify types according to the financial characteristics of local governments, financial independence and the health budget data for 17 local governments were collected from the local fiscal yearbook of the Ministry of Public Administration and Security. The prevalence of chronic diseases and healthy behavior was compared using the 16,333 data of adults between the ages of 30 and 65 years among the original data of the National Health and Nutrition Examination Survey (2016-2020). Results: Cluster analysis was used to classify local governments into five clusters according to the health financial capacity type. A comparison of the prevalence of local residents by cluster revealed a similar prevalence of hypertension, diabetes, and hypercholesterolemia. On the other hand, the obesity rate (P < 0.01), high-risk drinking rate (P < 0.01), aerobic physical activity rate (P < 0.001), and healthy eating practice rate (P < 0.001) were significantly different. In addition, an analysis of the odds ratio based on the Seoul area revealed a higher risk of health behavior of non-Seoul residents. Conclusions: It is necessary to review the universal health promotion project budget considering the degree of regional financial vulnerability from the viewpoint of health equity to narrow the health gap among regions.

삼사십대 성인에서 대사증후군 관련 영향 요인 (The Affecting Factors of Metabolic Syndrome in Korean Adults in Their 30s and 40s)

  • Objectives: The aim of this study was to determine the affecting factors of metabolic syndrome (MetS) and it's components in Korean adults in their 30s and 40s. Methods: The present study assessed 1,215 subjects who participated in the Korea National Health and Nutrition Examination Survey in 2014. We analyzed the prevalence rate of MetS and it's components and their affecting factors. Results: The prevalence rate of MetS was 16%. Family history of diabetes mellitus and, low or high saturated fat intake were associated with MetS in males. Low income, low education, excessive sleep, excessive carbohydrate and low saturated fat intake were associated with MetS in females. Triglycerides showed the highest prevalence in males and were related to white collar socioeconomic status, smoking, and obesity. Triglycerides and high-density lipoprotein cholesterol were high in females, and triglycerides were affected by excessive sleep, family history, low saturated fat intake, and obesity. High-density lipoprotein cholesterol was affected by obesity. Conclusions: Because sex differences exist in MetS, it is necessary to intervene through diverse approaches. In particular, a continuous management and preventive intervention for obesity is needed.

교사의 간장질환 유병률 및 질환발생의 위험요인에 관한 연구 (The Prevalence Rate and the Risk Factors for Liver Disease among Schoolteachers)

  • This study examined the health conditions of schoolteachers in terms of the prevalence rate and risk factors for liver disease. A cohort design was conducted for 21,319 teachers who were born from 1953 to 1957. The cohort was constructed for teachers who had no disease history such as liver disease, hypertension, cerebral vascular disease, heart disease, diabetes mellitus and cancer, and had 'normal' results from liver disease examinations in 1998. They were followed up from 1998 to 2000. SAS 6.12 was used for the data analysis. The results were as follows; (1) Prevalence rates of liver disease per 1,000 people in 1998 were 43.0. (2) The 2-year cumulative incidence of liver disease was 433/16,103(26.9/1000 persons). (3) Factors such as sex, age, school type(private or public), drinking, smoking, exercise, BMI, weight gain, fasting-blood sugar levels and total cholesterol levels were statistically significant. The significant risk factors of liver disease be identified from the multiple logistic regression analysis were age, sex(male), drinking, smoking, BMI, weight gain, fasting-blood sugar levels, and total cholesterol levels.

The Association Between Smoking Tobacco After a Diagnosis of Diabetes and the Prevalence of Diabetic Nephropathy in the Korean Male Population

  • Objectives: Smoking is known to be associated with nephropathy in patients with diabetes. The distinct effects of smoking before and after diabetes has been diagnosed, however, are not well characterized. We evaluated the association of cigarette smoking before and after a diagnosis of diabetes with the presence of diabetic nephropathy. Methods: We analyzed data from the 2011-2013 editions of the Korea National Health and Nutrition Examination Survey. A total of 629 male patients diagnosed with diabetes were classified as non-smokers (90 patients), former smokers (225 patients), or continuing smokers (314 patients). A "former smoker" was a patient who smoked only before receiving his diagnosis of diabetes. A "continuing smoker" was a patient who smoked at any time after his diabetes had been diagnosed. Diabetic nephropathy was defined as the presence of albuminuria (spot urine albumin/creatinine ratio ${\geq}30mg/g$) or low estimated glomerular filtration rate ($<60mL/min/1.73m^2$). Multiple logistic regression models were used to assess the independent association after adjusting for age, duration of diabetes, hemoglobin A1c, body mass index, systolic blood pressure, medication for hypertension, and medication for dyslipidemia. Female patients were excluded from the study due to the small proportion of females in the survey who smoked. Results: Compared to non-smokers, continuing smokers had significantly higher odds ratio ([OR], 2.17; 95% confidence interval [CI], 1.23 to 3.83) of suffering from diabetic nephropathy. The corresponding OR (95% CI) for former smokers was 1.26 (0.70 to 2.29). Conclusions: Smoking after diagnosis of diabetes is significantly associated with the presence of diabetic nephropathy in the Korean male population.

Assessing Health Impacts of Pictorial Health Warning Labels on Cigarette Packs in Korea Using DYNAMO-HIA

  • Objectives: This study aimed to predict the 10-year impacts of the introduction of pictorial warning labels (PWLs) on cigarette packaging in 2016 in Korea for adults using DYNAMO-HIA. Methods: In total, four scenarios were constructed to better understand the potential health impacts of PWLs: two for PWLs and the other two for a hypothetical cigarette tax increase. In both policies, an optimistic and a conservative scenario were constructed. The reference scenario assumed the 2015 smoking rate would remain the same. Demographic data and epidemiological data were obtained from various sources. Differences in the predicted smoking prevalence and prevalence, incidence, and mortality from diseases were compared between the reference scenario and the four policy scenarios. Results: It was predicted that the optimistic PWLs scenario (PWO) would lower the smoking rate by 4.79% in males and 0.66% in females compared to the reference scenario in 2017. However, the impact on the reduction of the smoking rate was expected to diminish over time. PWO will prevent 85 238 cases of diabetes, 67 948 of chronic obstructive pulmonary disease, 31 526 of ischemic heart disease, 21 036 of lung cancer, and 3972 prevalent cases of oral cancer in total over the 10-year span due to the reductions in smoking prevalence. The impacts of PWO are expected to be between the impact of the optimistic and the conservative cigarette tax increase scenarios. The results were sensitive to the transition probability of smoking status. Conclusions: The introduction of PWLs in 2016 in Korea is expected reduce smoking prevalence and disease cases for the next 10 years, but regular replacements of PWLs are needed for persistent impacts.

Changes in dietary habits and chronic diseases before and after COVID-19 by regions using data from the 2018-2020 Korea Community Health Survey and Consumer Behavior Survey for Foods: a cross-sectional study

  • Objectives: This study examined the changes in dietary habits, the prevalence of chronic diseases, and mental health problems in the regional areas of the Republic of Korea before and after the COVID-19 pandemic to provide evidence of the status of regional health inequalities. Methods: This study analyzed Korean adults aged 19 or older who participated in the Korea Community Health Survey (n = 686,708) and Consumer Behavior Survey for Foods (n = 19,109) from 2018 to 2020. The participants were classified according to their residence area (Seoul metropolitan area, Metropolitan cities, Provinces); 2018-2019 were defined as before COVID-19, and 2020 as after COVID-19. The dietary behaviors, chronic diseases, and mental health problems were measured using a self-report questionnaire. Results: After COVID-19, the eating-out usage rate in the Seoul metropolitan area and Provinces decreased compared to before COVID-19 (P < 0.001), and when responding that they eat out, the frequency of eating out with household members in the Seoul metropolitan area increased (P = 0.024). The deliveries/takeout usage rate in the Provinces decreased after COVID-19 compared to before (P < 0.001). After COVID-19, the prevalence of obesity decreased in all regions (P < 0.001), and the prevalence of hypertension increased significantly in the Provinces (P = 0.015). The prevalence of diabetes mellitus increased continuously before and after COVID-19 in all regions (P < 0.002). High-risk subjective stress levels increased significantly in the Seoul metropolitan area (P < 0.001), and sleep duration significantly increased in all regions (P < 0.001). Major depressive disorder was reduced significantly in Metropolitan cities (P = 0.042) and Provinces (P < 0.001). Conclusions: After the COVID-19 pandemic, the prevalence of chronic diseases and mental health problems showed regional differences along with changes in dietary habits. It is necessary to reflect the regional differences in dietary habits in future policies resolving regional health inequalities.
