목 적: 기계적 환기 요법 시 삽입된 비위관으로 인한 식도 운동 기능의 저하와 가해진 양압이 위식도 압력차를 감소시켜 미숙아에서 위식도 역류가 증가한다고 알려져 있으나, 아직 국내에서는 보고된 바가 없기에 본 연구에서는 미숙아에서 기계적 환기 요법이 위식도 역류에 미치는 영향과 유발 요인을 조사하였다. 방 법: 2000년부터 2004년까지 부산대학교병원 신생아 집중치료실에 입원하여 신생아 호흡 곤란 증후군, 신생아 가사 등의 원인으로 기계적 환기 요법을 받은 11명을 대상으로 하였다. 24시간 식도 산도 측정은 DIGITRAPPER MK III (Synectics Medical AB, Stockholm, Sweden)를 이용하였고, 탐촉자로는 antimony electrode를 사용하였다. 역류의 정도를 알아보는 지표들로는 역류 지수, 역류 횟수, 5분 이상 지속되는 역류 횟수, 가장 긴 역류의 지속 시간, 총 역류 횟수 중 5분 이상 지속된 역류 횟수의 비 등 5개의 지표 중 2개 이상이 기준에 합당할 경우 의미있는 위식도 역류로 정의하였다. 결 과: 전체 환아의 평균 재태 기간은 30.9주, 출생시의 평균 체중은 1,568 g, 검사 당시의 평균 나이는 2.8일이었다. 미숙아 중 4명(남아 3명, 여아 1명)에서 의미있는 위식도 역류가 있었다. 역류가 없었던 군이 재태 기간이 짧았고, 평균 체중이 작았으며, 검사 당시의 평균 나이가 많은 경향은 보였지만 대상 수가 적어 통계학적인 의미를 두기 어려웠다. 의미있는 위식도 역류가 동반된 군과 역류가 없었던 군 사이에 호흡기 설정에서는 의미있는 차이가 없었다. 결 론: 본 연구에서 위식도 역류를 증가시키는 요인을 발견할 수는 없었으며, 기계적 환기 요법시 위식도 역류의 빈도가 이전 연구들과 비슷한 결과를 보여 기계적 환기 요법이 미숙아의 위식도 역류에 큰 위험 요인은 아닌 것으로 판단된다.

Objectives: Dental or maxillofacial emergencies are uncommon during pregnancy, but if they occur, they are challenging to treat due to potential risks. The mother should not be denied necessary medical or dental care because of pregnancy. The aim of the study is to observe outcomes of pregnancy in patients requiring emergency minor oral surgical procedures during gestation and to determine the safety of the pregnant woman undergoing the procedure and the fetus. Materials and Methods: The study was conducted on 52 pregnant women requiring emergency oral surgical procedures. A standard treatment protocol for treatment of specific entities was followed. Close monitoring and observation were the primary goal of treatment. All patients were followed postoperatively until complete recovery from the surgical procedures and then until birth of the baby. A control group of 52 healthy pregnant patients who did not require oral surgical procedures was considered for statistical analysis. The measurements to calculate observation were fetal loss (spontaneous abortion), preterm birth, low-birth weight, or incidence of any congenital anomalies in the baby and its association with surgical procedures. Results: No fetal loss occurred in any of the cases. However, four patients experienced preterm birth and seven neonates exhibited low birth weights. No congenital abnormalities were discovered. In one instance, a patient who underwent surgery for a mandibular symphysis fracture under general anesthesia in the 31st week of pregnancy experienced labor pain on the fourth postoperative day, requiring an emergency Caesarean section. Conclusion: The results of our study demonstrate that, compared to the control group, minor emergency surgeries performed during pregnancy have no discernible negative effects on the fetus. These procedures can safely be performed by adhering to our described protocols.
The 16p11.2 microdeletion has been reported in patients with developmental delays and intellectual disability. The distal 220- kb deletion in 16p11.2 is associated with developmental delay, autism spectrum disorder, epilepsy, and obesity at a young age. We have reported a case of distal 16p11.2 deletion syndrome in a preterm infant with unusual facial morphology and congenital heart disease. We suggest using chromosome microarray analysis to detect chromosomal abnormalities in newborns, especially preterm infants with unusual morphologies.
Purpose: This study investigated the parenting experiences of mothers of young children born moderate-to-late preterm (MLPT) in South Korea. Methods: In this qualitative study, semi-structured focus group interviews were conducted with 10 mothers of MLPT children from infancy to preschool age. The interviews were video-recorded, transcribed verbatim, and analyzed using qualitative content analysis. Results: Four categories resulted from the analysis of parenting experiences of mothers with young MLPT children, as follows: "becoming a mother of an early-born child", "difficulties as the primary caregiver for a high-risk child", "helpful social support, but still a lack of professional support for parenting a high-risk child", and "mothers and children growing together". Conclusion: Mothers of young MLPT children experienced difficulties due to concerns about their child's health, growth and development, and insufficient child-rearing support. Therefore, social support systems should be strengthened and more aggressive nursing strategies should be adopted for mothers of young MLPT children.
Purpose: Preterm infants are at high risk for adverse neurodevelopmental outcomes. Magnetic resonance imaging (MRI) has been proposed as a means of predicting neurodevelopmental outcomes in this population. It is controversial whether diffuse excessive high signal intensity (DEHSI) represents damage to the white matter or delayed myelination in preterm infants. This study investigated MRI findings for predicting the severity of neurodevelopmental outcomes and assessing whether preterm infants with DEHSI near term-equivalent age have abnormal neurodevelopmental outcomes. Materials and Methods: Preterm infants (n = 64, gestational age at birth < 35 weeks) undergoing brain MRI near term-equivalent age and subsequent neurodevelopmental outcomes were evaluated between 18 and 24 months of age. The associations of MRI findings and the risk of severe cognitive delay, severe psychomotor delay, cerebral palsy (CP), and neurosensory impairment were analyzed. The associations of DEHSI with risks of severe cognitive delay, severe psychomotor delay, CP, and neurosensory impairment (hearing or visual impairment) were analyzed. Outcome data were evaluated by logistic regression and the Fisher's exact test. Results: There were significant associations between abnormal white matter findings and delayed mental development, delayed psychomotor development, neurosensory impairment, and presence of CP. The presence of DEHSI was not correlated with delayed neurodevelopmental outcomes or presence of CP. In multivariate logistic regression analyses, cystic encephalomalacia, punctate lesion, loss of white matter volume and ventricular dilation were significantly associated with CP. Conclusion: Abnormal MRI findings near term-equivalent age in preterm infants predict adverse neurodevelopmental outcomes. No significant association between DEHSI and adverse neurodevelopmental outcomes was demonstrated.

Purpose: In this study, we aimed to review the clinical presentation of preterm infants with gastrointestinal perforations and compare the clinical features of gastric perforation with other intestinal perforations. Methods: The medical records of preterm neonates with pneumoperitoneum, admitted to the neonatal intensive care unit (NICU) between January 1994 and December 2013, were retrospectively reviewed. Results: Twenty-one preterm infants underwent exploratory laparotomy to investigate the cause of the pneumoperitoneum. The sample consisted of five patients (23.8%) with gastric perforation and 16 patients (76.2%) with intestinal perforation. No statistical differences were found in the birth history and other perinatal factors between the two groups. Underlying necrotizing enterocolitis, bilious vomiting, and paralytic ileus preceding the perforation were statistically more common in the intestinal perforation group. All preterm infants with gastric perforation survived to discharge; however, six preterm infants with intestinal perforation expired during treatment in the NICU. In the gastric perforation group, sudden pneumoperitoneum was the most common finding, and the mean age at diagnosis was $4.4{\pm}1.7days$ of life. The location and size of the perforations varied, and simple closure or partial gastrectomy was performed. Conclusion: Patients with gastric perforation did not have a common clinical finding preceding the perforation diagnosis. Although mortality in previous studies was high, all patients survived to discharge in the present study. When a preterm infant aged less than one week presents with sudden abdominal distension and pneumoperitoneum, gastric perforation should first be excluded. Prompt exploratory laparotomy will increase the survival rates of these infants.
While the survival rate of preterm infants and employment rates of mothers with infants have been dramatically increasing, little is known about the impact of maternal employment and having premature infants on maternal health recovery after delivery. The purpose of this longitudinal study was to examine differences in postpartum health by mother's employment and infant's prematurity over time during the first 3 months postpartum. The study used a longitudinal design and was secondary data analysis from a large study. A convenience sample of 132 mothers was recruited with mothers who had fullterm or preterm infants through the postpartum unit. They were followed up for 3 times (at 1-2 days, 6 weeks, and 3 months) during the first 3 months postpartum period. Level of fatigue and hysical symptoms were indicators for postpartum physical health; Positive affect and negative affect were assessed for postpartum mental health, and role functional status after childbirth was for role performance. Finding indicated that health indicators changed significantly over time but there was no main or interaction effect for maternal employment status. There were main effects of infant's prematurity on positive affect, negative affect, and role functional status (self-care and social and community activities). This study enhanced the understanding of postpartum health of mothers with preterm infants as well as those with fullterm infants during the postpartum period.
Purpose: This study aims to determine the factors that affect the breastfeeding of late preterm infants (gestation age 34+0~36+6) in South Korea. Method: A cross-sectional and questionnaire-based survey was conducted on 178 mothers of 209 late preterm infants discharged from a university hospital. We collected data on participants' demographics, breastfeeding-related characteristics and current status of breastfeeding and analyzed them using SPSS. Results: Breastfeeding duration varied significantly according to the number of babies born (t=3.691, p<.001), birth order (F=6.416, p=.002), type of feeding planned (F=8.691, p<.001), planned breastfeeding period(F=24.779, p<.001), previous baby's breastfeeding type(F= 8.510, p<.001), previous baby's breastfeeding duration(F=10.589, p<.001). The mothers with incomes of 3-5 million won a month were less likely to continue breastfeeding than those with incomes over 5 million won (CI: 0.049-0.086: p=0.035). Conclusion: Our results are meaningful in that we revealed that mother's monthly income influenced breastfeeding continuation, first-time mothers and mothers of multiples were at risk of breastfeeding difficulties. Interventions for helping late preterm infants' mothers establish breastfeeding and maintain an adequate milk supply are vital.
목 적 : 최근 모유수유의 장점이 부각되고 있으나 모유가 미숙아 전용분유에 비해 열량을 포함한 영양소들이 미숙아들의 적절한 성장을 유도하기에는 낮아 이를 극복하기 위한 모유강화제가 개발되었다. 본원에서는 극소저체중출생아들을 대상으로 모유강화제를 첨가한 모유를 수유한 경우와 미숙아 전용분유를 수유한 경우를 비교하여 성장에 어떠한 효과를 미치는지 알아보기로 했다. 방 법 : 2005년 10월부터 2006년 12월까지 동아대학교의료원에서 출생하여 신생아 집중치료실에 입원한 출생체중 1,500 g 이하, 재태주령 33주 미만인 극소저체중출생아들을 대상으로 강화된 모유수유군과 미숙아 전용분유 수유군으로 나누어 전반적인 특징, 영양상태, 성장 추이 및 합병증의 발생 등을 전향적으로 비교 조사하였다. 대상 환자들 중 본원에서 추적관찰이 가능했던 환자들의 경우 외래에서 1, 3, 6개월에 걸쳐 체중, 신장, 두위를 측정하여 비교하였다. 결 과 : 두 군의 평균 재태주령 및 출생체중은 각각 $30^{+1}{\pm}2^{+1}$주과 $29^{+4}{\pm}2^{+1}$주, $1,281.30{\pm}178g$과 $1,234.32{\pm}193g$으로 통계학적으로 차이가 없었으며(P=0.26, P=0.38), 성비, 괴사성 대장염, 패혈증, 신생아호흡곤란증후군, 동맥관개존증, 뇌 내출혈 및 뇌실 내 출혈 등의 합병증의 비율, 평균 재원 기간에서도 유의한 차이는 없었다. 수유 시작 시기는 강화된 모유수유군과 미숙아 전용분유 수유군에서 각각 생후 $8.00{\pm}3.27$일 및 $8.86{\pm}5.37$일(P=0.99), 완전수유 도달 시기는 각각 수유시작 후 $41.78{\pm}20.47$일 및 $36.86{\pm}20.63$일로(P=0.55) 통계적으로 유의한 차이가 없었으며, 총정맥영양 기간 및 수유 곤란증상 횟수에도 유의한 차이는 없었다. 수유시작 시점부터의 체중, 신장, 두위의 성장 속도 및 절대치도 두 군 간에 유의한 차이가 없었으나 각 시점의 평균치를 비교하였을 때 수유 시작 후 7주째 체중은 강화된 모유수유군에서 더 빨리 증가하였다(P=0.03). 환자들의 초기 체중 감소 후 출생체중까지 회복되는데 걸리는 기간도 각각 $22.3{\pm}6.92$일 및 $24.32{\pm}10.68$일로 통계학적 차이는 없었으며(P=0.41), 체중 회복 후부터 퇴원까지의 체중 및 두위, 신장의 증가속도는 두 군에서 유의한 차이는 없었다. 퇴원 후 외래 추적 관찰을 한 각 군의 환자들의 체중, 신장, 두위의 평균치에도 유의한 차이는 없었다. 결 론 : 극소저체중출생아에서 모유강화제를 포함한 모유수유군과 미숙아 전용분유 수유군 간의 전반적인 성장 상태, 생화학적 검사 수치 및 합병증 발생에는 유의한 차이가 없어 모유 강화제는 고열량 수유를 필요로 하는 극소저체중출생아에서 부가적인 선택 사항이라고 하겠다.
