전열침(傳熱鍼)이 급성 족관절 염좌로 인한 전거비·종비 인대 손상에 미치는 임상적 효과 (The Clinical Effects of Heating-Conduction Acupuncture Therapy for Anterior Talofibular and Calcaneofibular Ligament Injury Induced by Acute Ankle Sprain)

  • Objectives : The purpose of this study is to investigate the clinical effects of heating-conduction acupuncture therapy on ligament injury induced by acute ankle sprain. Methods : From september 16, 2008 to April 17, 2010, the 28 outpatients who had admitted to department of oriental rehabilitation medicine, Dae-Jeon oriental Medical hospital, Dae-Jeon university with acute ankle sprain were performed heating-conduction acupuncture therapy on anterior talofibular and calcaneofibular ligament injury. To evaluate the efficiency of heating-conduction acupuncture therapy, pain threshold with pressure algometer and visual analogue scale(VAS) were applied before treatment and after 1st, 2nd treatment. Results : 1. The pain threshold and the VAS score showed statistical significant improvement after 1st treatment on calcaneofibular and anterior talofibular ligament injury. 2. The VAS score after 2nd treatment on calcaneofibular and anterior talofibular ligament injury showed statistical significant improvement compared with that after 1st treatment. 3. The pain threshold after 2nd treatment on calcaneofibular and anterior talofibular ligament injury did not show statistical significant improvement compared with that after 1st treatment. 4. The difference between anterior talofibular and calcaneofibular ligament injury was not statistical significant in pain threshold and VAS score. Conclusions : Heating-conduction acupuncture therapy has clinical effects of pain reduction on patient with calcaneofibular ligament injury as well as anterior talofibular ligament induced by acute ankle sprain.

근막통증 증후군 환자에게 체외충격파와 근막이완술 병행 치료가 통증, 움직임, 기능에 미치는 영향 (Effects of Extracorporeal Shock Wave Therapy with Myofascial Release Techniques on Pain, Movement, and Function in Patients with Myofascial Pain Syndrome)

  • Purpose: The study investigated the effects of extracorporeal shock wave therapy with myofascial release techniques (ESWT+MFR) on pain, movement, and function in patients with myofascial pain syndrome. Methods: Forty participants with upper trapezius trigger points were recruited and randomly allocated to two groups: an experimental group (n = 20) and a control group (n = 20). The experimental group performed the ESWT+MFR, and the control group performed only myofascial release techniques. Each group was treated for 15 minutes, twice a week for four weeks. Pain was assessed using a visual analogue scale and a pressure pain threshold measure. Movement was assessed by cervical range of motion, and cervical and shoulder function were assessed on the Constant-Murley Scale and the Neck Disability Index before and after treatment. Results: The results indicate statistically significant improvements in the two groups on all parameters after intervention as compared to baseline (p < 0.05). As compared to the control group, the experimental group showed statistically significant improvements on the visual analogue scale and pressure pain threshold, cervical range of motion (except rotation), and on the Neck Disability Index (p < 0.05). Conclusion: The ESWT+MFR is more effective than myofascial release techniques for pain, movement, and function in patients with myofascial pain syndrome and would be clinically useful for physical therapists treating myofascial pain syndrome.

GaAsAl 레이저 자극이 흰쥐의 압통역치에 미치는 영향 (Effects of GaAsAl Laser on the Pressure Pain Threshold in Rats)

  • This study was designed to evaluate the analgesic effect of low power GaAsAl laser on the pain threshold of mechanical stimulation using different treatment points, acupuncture point (zusanli) and non-acupuncture points(back). Furthermore, we investigated the analgesic effect of low power GaAsAl laser using the different duration and intensity of laser in mechanical stimulation induced pain behavior. The results were summarized as follows: 1. The threshold of mechanical stimulation was significantly increased by GaAsAl laser stimulation into zusanli point after 15 and 30 min after laser stimulation(P<05). However, the laser stimulation into non-acupoint did not affect the pain threshold of mechanical stimulation. with dose dependent manner. 2. In order to investigate the analgesic effects of BV depending upon different intensities of laser stimulation, the experimental animals were divided into three groups: 3 mW treated group, 6 mW treated group and 10 mW treated group. The low power GaAsAl laser stimulation was applied into zusanli acupoint for 30 min with different intensity of laser stimulation. Six and ten mW of laser stimulation significantly increased the pain threshold of mechanical stimulation at 15 min after laser stimulation as compared to that of control group(P<.05). Moreover, the analgesic effect of 10 mW laser stimulation was maintained for 30 min after laser stimulation (P<.05). 3. Finally, we tested the analgesic effect of 10 mW laser stimulation using different duration such as 10 min, 30 min or 1 hr after application of mechanical stimulation. In 30 min treatment group, the pain threshold of mechanical stimulation was increased at 15min and 30min after laser stimulation(P<.05). However, laser stimulation for 60 min dramatically increased the pain threshold of mechanical stimulation at 0 min after laser stimulation and the analgesic effect of laser stimulation was observed until 1 hr after laser stimulation. In conclusion, these data apparently demonstrate that low power GaAsAl laser has analgesic effect on mechanical induced pain model in rats. In addition, the treated point, intensity and duration of laser stimulation should be concerned before clinical application for pain management purpose.

만성 허리통증에 대한 Kinesio taping 적용이 허리압통과 균형능력에 미치는 영향 (Effect of Kinesio Taping on Lower Back Pressure Pain and Balance ability in Chronic Lower Back Pain)

  • Background: This study investigated the effects of Kinesio taping application on lower back pressure pain and balance ability among university students with chronic lower back pain. Methods: A total of thirty university students between 20 to 30 years of age with chronic lower back pain were divided randomly into two groups, the control and the experimental group. In the control group (n=15), placebo taping was applied to the lumbar region. In the experimental group (n=15), Kinesio taping was applied to the erector spine muscles of the lower back. The groups were assessed for lower back pressure pain and balance ability, before and after the taping application. Pain was measured by the pain pressure threshold (PPT), and balance was measured using the good balance system (GBS). Results: There were significant improvements in both the PPT and GBS of the Kinesio taping group compared to pre-treatment values (p<.05), while the placebo taping group showed no significant change (p>.05). In addition, the Kinesio taping group had a statistically significant difference in PPT and GBS compared to the placebo taping group (p<.05). Conclusion: The Kinesio taping application is more effective than the placebo taping application in the improvement of lower back pressure pain and balance ability among university students with chronic lower back pain.

시청각 매체를 활용한 목, 어깨 자가신장운동이 만성 목통증 여성의 통증, 자세정렬과 관절위치감각에 미치는 영향 (The Effect of Neck and Shoulder Self-Stretching Exercise Using Audiovisual Media on Neck Pain, Postural Alignment, and Joint Position Error in Women with Chronic Neck Pain)

  • Background: The purpose of this study is to investigate the effect of neck and shoulder self-stretching exercise using audiovisual media on neck pain, postural alignment, and joint position error in women with chronic neck pain. Methods: The subjects included 20 women that gave consent to participate in the study voluntarily. They performed the self-stretching exercises using audiovisual media was carried out 20 minutes 5 times a week during 3 weeks. Neck disability index (NDI) and visual analogue scale (VAS) were used to measure the functional disability and pain, A pressure pain threshold was measured using an algometer, and a cervical range of motion (CROM) measurement tool was used to measure the range of motion and error of proprioceptive position sense of the cervical spine. To assess posture alignment, forward head angle (FHA), forward shoulder angle (FSA) were measured using image J software. Results: The neck pain intensity was statistically significantly within group (p<.05). Neck and shoulder functional disability were a statistically significant difference within group (p<.05). Splenius capitis and upper trapezius pressure pain threshold were statistically significant difference in within group (p<.05). The postural alignment was statistically significantly within group (p<.05). The cervical range of motion in neck extension, right and left lateral flexion were statistically significantly within group (p<.05). The joint position error in neck flexion, extension, right and left lateral flexion decreased statistically significantly within group (p<.05). Conclusion: Self-stretching exercise using audiovisual media increased the mobility of the neck, decreased neck pain and joint position error, and improved posture alignment. As a result, there was a positive effect by applying the self-stretching exercise using audiovisual media to people with neck pain. Based on this, it is thought that it can be used as the basis for research related to home training programs for healthy self-management.

Morphine에 의한 저작근 통증의 조절 효과 (Effect of Masticatory Muscle Pain Control by Morphine)

  • 이 연구는 Morphine을 이용한 저작근통증의 조절 효과를 확인하기 위해서 시행되었으며, 경희대학교 치과대학병원 구강내과에 내원한 환자 중 RDC/TMD로 진단된 지원자를 saline 주사군, lidocaine 주사군, morphine 주사군 각각 10명씩 배정하였다. 통증부위에 주사 전, 주사 후 10분, 30분, 60분에 각각 주관적인 통증 평가인 시각유추척도검사, 맥길통증설문지검사 그리고 통증부위표시검사와 객관적인 통증 평가인 압력통증역치검사와 압력통증한계검사를 실시하였다. 검사 후 평가된 자료를 통계 처리하여 다음과 같은 결과를 얻었다. 1. 주관적인 통증 평가와 객관적인 통증 평가 모두 집단 내 효과가 있었다. (p<0.001) 2. 주관적인 통증 평가인 통증부위표시검사(p<0.001)에서 집단 간 효과가 있었다. 3. 객관적인 통증 평가인 압력통증역치검사(p=0.025)에서 집단 간 효과가 있었다. 4. 주관적인 통증 평가인 통증부위표시검사는 morphine 주사군(p=0.001)이 saline 주사군에 비해서 효과가 있었다. 이상의 연구결과로 저작근에 통증이 있는 환자에게 morphine 주사 시 60분 이내에는 주관적인 평가에서 통증 조절 효과가 있었으며, 향후 시간 연장에 따른 지속적인 추가 연구가 필요 할 것으로 생각된다.

동통을 가진 신체형장애 환자에서 감정표현능력과 압통역치 (Alexithymia and Pressure Pain Threshold in Patients with Somatoform Disorder who have Pain)

  • 저자들은 동통을 가진 신체형장애 환자들에서 물리적인 외부자극에 대한 지각과 외부자극에 따른 언어행동의 관계성을 파악하기 위해서 본 연구를 시행하였다. 환자 34명 (남 10, 여 24)과 건강대조군 37명 (남 19, 여 18)을 대상으로, 물리 적 자극에 대한 지각도의 측정은 압통역치 측정기(algometer)를 이용하였으며, 감정표현능력의 측정은 Toronto Alexithymia Scale을 이용하였다. 한편 신체증상의 정도와 부모-자녀 결합형태검사를 시행하였다. 전체환자의 81.4%는 6개월이상 증상을 지속적으로 많이 호소하는 만성화 특성을 보였다. 그리고 alexithymia의 정도, 신체증상 호소정도, 압통역치가 모두 환자군에서 유의하게 높았다. 그리고 환자군의 44.1%는 alexithymia라고 할 수 있었다. 한편 압통역치와 alexithymia간에 유의한 상관성은 없었다. 결론적으로, 동통을 가진 신체형장애환자들이 정상인에 비해 외부 동통자극에 덜 민감하며, 이는 병의 만성화와 관련되는 것 같다. 이들은 또 외부자극에 따른 적절한 감정표현능력도 떨어져 있는 경우가 많은데, 이것은 신체화의 한 조건이 되며, 한편으로는 정신치료적 접근을 방해하는 한 요소가 될 수 있음을 시사한다.

만성 목-어깨 통증이 있는 여성 성인에게 시청각 매체를 활용한 탄력밴드 저항운동이 통증, 고유수용성 감각과 운동기능에 미치는 영향 (Effects of Elastic Band-Resistive Exercise using Audio-visual Medium on Pain, Proprioceptive Sense, and Motor Function in Adult Females with Chronic Neck and Shoulder Pain)

  • Background: This study aimed to investigate the effect of elastic band-resistive exercise using audio-visual medium on pain, proprioception, and motor function in adults with chronic neck and shoulder pain. Design: One group pretest-posttest follow-up experimental design. Method: Twenty adult women with neck and shoulder pain voluntarily participated in this study. Elastic band-resistive exercise using audio-visual medium including cervical flexion and extension, shoulder external rotation, and scapular retraction-protraction motions was conducted 5 times a week for 3 weeks. The Numerical Rating Scale, pressure threshold tool, CROM goniometer, and Image J software were used to assess subjective pain level, tenderness threshold (pain), joint position sense error (proprioception), joint range of motion, and postural alignment (motor function), respectively. Result:: The pain intensity and threshold and joint position sense error showed significant decreases after the intervention, whereas the joint range of motion angle revealed significant increases. The postural alignment including forward head posture and rounded shoulder revealed significant improvements after the intervention. Conclusions: Therefore, we suggest that elastic band-resistive exercise through audio-visual medium would be helpful in preventing and managing pain and physical dysfunction in individuals with chronic neck and shoulder pain, and then it would support the development of health management-related online education content.

The passive stretching, massage, and muscle energy technique effects on range of motion, strength, and pressure pain threshold in musculoskeletal neck pain of young adults

  • Objective: Musculoskeletal neck pain have many symptoms which include decreased range of motion (ROM) and muscle strength, and increased pain. However, the management methods are controversial. The purpose of this study was to examine the effects of three interventions on ROM, strength, and pressure pain threshold (PPT) with musculoskeletal neck pain. Design: Pretest-posttest design. Methods: Thirty subjects participated in this experiment. They were randomly assigned to thefollowing groups: passive stretching (PS) group (n=10), massage (MASS) group (n=10), and muscle energy technique (MET) group (n=10). The treatment were applied bilaterally on the upper trapezius. The PS was applied 3 times for 30 seconds each time. The MASS was applied using two different techniques for 2 minutes per technique. For MET, the subjects performed 2 sets of 3 repetitions of isometric resistance exercise that was maintained for 10 seconds, followed by 10 seconds of rest. ROM, strength, and PPT parameters were measured after intervention. Results: In the MASS group, there was a significant improvement in all outcomes except for muscle strength (p<0.05). In the MET group, ROM and strength significantly improved compared to the pre-treatment results (p<0.05). As result of measuring the amount of change in each group, there was a significant difference in ROM (flexion) in the PS group compared with the MASS and MET group, a significant difference in strength in the MET group compared with the PS and MASS groups, and a significant difference in PPT in the MASS groups compared with the PS and MET groups (p<0.05). Conclusions: This study showed that PS, MASS, and MET are effective methods for improving ROM, strength, and PPT for musculoskeletal neck pain. Therefore, various therapeutic interventions for improving ROM, strength, and pain are suggested.

안정화 운동, 체외충격파, 테이핑이 상승모근 근막통증 증후군에 미치는 효과 비교 (Comparing the Effects of Stability Exercise, ESWT, and Taping for Patients with Myofascial Pain Syndrome of Upper Trapezius)

  • Purpose: In this study, the effects of stability exercise, extracorporeal shock wave therapy, and taping on pain and function in patients with myofascial pain syndrome of upper trapezius were compared. Methods: The subjects were divided into the stability exercise, ESWT and the taping treatment group and the clinical outcomes were evaluated by visual analog scale (VAS), pressure pain threshold (PPT) and a constant-murley scale (CMS) at pre-treatment and post-treatment. Paired t-test and ANOVA was used for statistical analysis. Results: All groups were statistical significance in the change in visual analog scale (p<0.05). The difference between the ESWT group and taping group was statistical significance in the change in pressure pain threshold (p<0.05) except for the taping group. Using the constant-murley scale, the stability exercise group showed a significant decrease in pain, and a significant increase in ROM, ADL, strength, total score of shoulder (p<0.05); however, the ESWT group showed no difference on ADL. In addition, there was no difference in strength for the taping group. The comparison of the effect between the stability exercise group, ESWT group and taping group in CMS showed a statistical significant difference in pain, ADL and ROM (p<0.05). Conclusion: These results indicate that stability exercise, ESWT and taping could be considered an effective and efficient treatment modality for myofascial pain syndrome of upper trapezius.
