Surgical correction in annuloaortic ectasia associated with ascending aortic aneurysm: one case report (거대상행핵대동맥루를 동반한 대동맥륜확장증 수술 치험: Cabrol씨 수술 1례 보)

  • Most patients having annuloaortic ectasia are associated with marked dilatation of the sinuses of Valsalva and the aortic annulus as well as the huge aneurysm of the ascending aorta. A 19 year old male patient complaining of tightness on left posterior chest wall underwent cardiac angiography in which demonstrated annuloaortic ectasia with ascending aortic aneurysm and aortic insufficiency. The patient had corrective operation replacing the ascending aorta and aortic valve with a composite graft[Dacron prosthesis containing a Bjork-Shiley aortic valve] within the aneurysmal sac. The coronary orifices were anastomosed to the tubular Dacron prosthesis [30 mm in diameter] by means of a second smaller Gore-Tex tube [8mm in diameter]. The aneurysmal sac was trimmed by removing the redundant wall and then wrapped outer wall of the Dacron prosthesis. Postoperatively, mediastinal bleeding was temporarily observed in the operative day and satisfactory blood pressure was maintained with small dose of dopamine. One week later, large amount of serous effusion was drained out of the retrosternal space making partial disruption of the skin which was healed well by daily local dressing. The patient discharged in good condition on postoperative 29th day with no residual complications and is doing very well on the 4 months follow-up.

Surgical correction of Total Anomalous Pulmonary Venous Connection - Review of 37 Cases treated surgically during 10 years (총폐정맥환류이상증에 대한 외과적 요법 및 장기 성적)

  • This report provides follow - up data on 37 patients, aged 7 days to 25 years [median: 6.5 months], who underwent repair of total anomalous pulmonary venous connection at Seoul national University Hospital between May, 1978 and June, 1987. The patients were 22 males and 17 females and the sex ratio was 1.6 to 1, showing a male predominance. Sixteen patients had supracardiac, 13 cardiac, 3 infracardiac and 5 had a mixed type. The duration of follow up was from 1 month to 60 months [median: 14 months] There were eight early and one late deaths, and the overall mortality was 24%. The deaths during 1 year of life were eight [89%] and only one death [11%] occurred above 1 year of age. The mortality of cardiac type was unusually high, accounting for 56 percent of the total death, which was probably due to the preoperative poor clinical condition such as pulmonary edema and congestive heart failure. The major cause of death was the perioperative myocardial failure, and the survival was closely related to the preoperative clinical status, age and moderately elevated pulmonary arterial pressure, the sign of the elevated pulmonary vascular resistance and pulmonary venous obstruction. Early diagnosis and early application of surgical intervention is essential to the improved postoperative survival

Right ventricular failure in congenital heart disease

  • Despite developments in surgical techniques and other interventions, right ventricular (RV) failure remains an important clinical problem in several congenital heart diseases (CHD). RV function is one of the most important predictors of mortality and morbidity in patients with CHD. RV failure is a progressive disorder that begins with myocardial injury or stress, neurohormonal activation, cytokine activation, altered gene expression, and ventricular remodeling. Pressure-overload RV failure caused by RV outflow tract obstruction after total correction of tetralogy of Fallot, pulmonary stenosis, atrial switch operation for transposition of the great arteries, congenitally corrected transposition of the great arteries, and systemic RV failure after the Fontan operation. Volume-overload RV failure may be caused by atrial septal defect, pulmonary regurgitation, or tricuspid regurgitation. Although the measurement of RV function is difficult because of many reasons, the right ventricle can be evaluated using both imaging and functional modalities. In clinical practice, echocardiography is the primary mode for the evaluation of RV structure and function. Cardiac magnetic resonance imaging is increasingly used for evaluating RV structure and function. A comprehensive evaluation of RV function may lead to early and optimal management of RV failure in patients with CHD.

  • Positioning accuracy by the Global Positioning System (GPS) is of great concern in a variety of research tasks. It is limited due to error sources such as ionospheric effect, orbital uncertainty, antenna phase center variation, signal multipath, and tropospheric influence. In this study, the tropospheric influence, primarily due to water vapour inhomogeneity, on GPS positioning height is investigated. The data collected by the GPS receivers along with co-located surface meteorological instruments in 2003 are utilized. The GPS receivers are established as continuously operating reference stations by the Ministry of the Interior (MOI), Central Weather Bureau (CWB), and Industrial Technology Research Institute (ITRI) of Taiwan, and International GNSS Service (IGS). The total number of GPS receivers is 21. The surface meteorological measurements include temperature, pressure, and humidity. They are introduced to GPS data processing with 24 troposphere parameters for the station heights, which are compared with those obtained without a priori knowledge of surface meteorological measurements. The results suggest that surface meteorological measurements have an expected impact on the GPS height. The daily correction maximum with the meteorological effect may be as large as 9.3 mm for the cases of concern.

  • In the multi-pass welding of pressure vessels or ships, the mechanical touch sensor system is generally used together with a manipulator to measure the gap and depth of the narrow gap to perform seam tracking. Unfortunately, such mechanical touch sensors may commit measuring errors caused by the eterioration of the measuring device. An automation system of narrow gap multi-pass welding using a laser vision system which can track the seam line of narrow gap and which can control welding power has been developed. The joint profile of the narrow gap, with 250mm depth and 28mm width, can be captured by laser vision camera. The image is then processed for defining tracking positions of the torch during welding. Then, the real-time correction of lateral and vertical position of the torch can be done by the laser vision system. The adaptive control of welding conditions like welding Currents and welding speeds, can also be performed by the laser vision system, which cannot be done by conventional mechanical touch systems. The developed automation system will be adopted to reduce the idle time of welders, which happens frequently in conventional long welding processes, and to improve the reliability of the weld quality as well.

An Experimental Study on the Performance Prediction Logic for a Regenerative Cooling System (재생냉각시스템의 성능예측기법에 관한 실험적 연구)

  • The experimental research was conducted to setup a performance prediction logic for the regenerative cooling system on a small scale liquid rocket engine using kerosene and LOX. Total heat flux of the combustion gas side was determined for the flow rate of the coolant, combustion pressure using the calorimeter thrust chamber. Based on the experimental investigation, a performance prediction scheme for the regenerative cooling system is setup in our own way. A performance prediction logic for the regenerative cooling system has been developed by the correction scheme of the combustion gas side. The key parameters determining the temperature limitation of the coolant are the mass flow rate of the coolant and the length of the combustion chamber and the nozzle. And the parameters to control the limitation of the usable wall temperature are the number of channels and wall thickness.

Accuracy Comparison of GPT and SBAS Troposphere Models for GNSS Data Processing

  • The Global Navigation Satellite System (GNSS) signal gets delayed as it goes through the troposphere before reaching the GNSS antenna. Various tropospheric models are being used to correct the tropospheric delay. In this study, we compared effectiveness of two popular troposphere correction models: Global Pressure and Temperature (GPT) and Satellite-Based Augmentation System (SBAS). One-year data from a particular site was chosen as the test case. Tropospheric delays were computed using the GPT and SBAS models and compared with the International GNSS Service tropospheric product. The bias of SBAS model computations was 3.4 cm, which is four times lower than that of the GPT model. The cause of higher biases observed in the GPT model is the fact that one cannot get wet delays from the model. If SBAS-based wet delays are added to the hydrostatic delays computed using the GPT model, then the accuracy is similar to that of the full SBAS model. From this study, one can conclude that it is better to use the SBAS model than to use the GPT model in the standard code-pseudorange data processing.

  • In this paper, the response surface method using three-dimensional Navier-Stokes analysis to optimize the shape of a forward-curved blades centrifugal fan, is described. For numerical analysis, Reynolds-averaged Navier-Stokes equations with standard k-e turbulence model are transformed into non-orthogonal curvilinear coordinate system, and are discretized with finite volume approximations. Due to the large number of blades in forward-curved blades centrifugal fan, the flow inside of the fan is regarded as steady flow by introducing the impeller force models for economic calculations. Linear Upwind Differencing Scheme(LUDS) is used to approximate the convection terms in the governing equations. SIMPLEC algorithm is used as a velocity-pressure correction procedure. Design variables, location of cur off, radius of cut off, expansion angle of scroll and width of impeller were selected to optimize the shapes of scroll and blades. Data points for response evaluations were selected by D-optimal design, and linear programming method was used for the optimization on the response surface. As a main result of the optimization, the efficiency was successfully improved. It was found that the optimization process provides reliable design of this kind of fans with reasonable computing time

  • This paper describes numerical modeling of transcritical and supercritical fluid flows within a liquid propellant rocket engine. In the present paper, turbulence is modeled by standard $k-{\varepsilon}$ model. A conserved scalar approach in conjunction with multi-environment probability density function model is used to account for the turbulent mixing of real-fluids in the transcritical and supercritical region. The two real-fluid equations of state and dense-fluid correction schemes for mixtures are used to construct thermodynamic data library based on the conserved scalar. In this study, calculations are made on two cryogenic nitrogen jets under different chamber pressures. Sensitivity analysis for two different real-fluid equations of sate is particularly emphasized. Based on numerical results, precise structures of cryogenic nitrogen jets are discussed in detail. Numerical results show that the current real-fluid model can predict the essential features of the cryogenic liquid nitrogen jets.

An Analysis for Irregularity of Tropospheric Delay due to Local Weather Change Effects on Network RTK (지역적 기상 차이에 의한 대류권 지연 변칙이 네트워크 RTK 환경에 미치는 영향 분석)

  • Network RTK generates spatial corrections by using differenced measurements from reference stations in the network, and the corrections are then provided to a rover. The rover, generally, uses linear interpolation, which assumes that the corrections at each reference station are spatially correlated, to obtain a precise correction of its location. However, an irregularity of the tropospheric delay is a real-world factor that violates this assumption. Tropospheric delay is a result of weather conditions, such as humidity, temperature and pressure, and it can cause spatial decorrelation when there are changes in the local climate. In this paper, we have defined the non-linear characteristics of the tropospheric delay between reference stations or user within a region as the "irregularity of tropospheric delay". Such an irregularity can negatively impact the network RTK performance. Therefore, we analyze the influence of the irregularity of tropospheric delay in network RTK based on meteorological data.
