Septic arthritis of the temporomandibular joint: a case report

  • Septic arthritis of the temporomandibular joint (TMJ) is a rare disease. The most common symptoms of this disease are acute malocclusion, limited mouth opening, swelling, and tenderness of affected TMJ. These symptoms are often confused with internal derangement of the articular disc, rheumatoid arthritis, retrodiscitis, or osteoarthritis. Therefore, differential diagnosis by image examination is required. Usually, antimicrobial treatment and surgical drainage by needle aspiration, arthroscopy, or arthrotomy are effective treatment approaches. In this study, a patient who was diagnosed with septic arthritis was treated with arthrocentesis and antibiotics without significant complications. We present a case report with a review of the literature.

Analysis of Case Reports for Korean medical treatment of Irritable Bowel Syndrome (과민성 장증후군의 한방치료 증례논문 분석)

  • Objectives : The purpose of this study was to present the clinical guidelines for Korean medical treatment of Irritable Bowel Syndrome. Methods : Reports for Korean medical treatment of Irritable Bowel Syndrome were collected, analyzed and summarized from the Electronic journal search since 2000. Results : In Korean medicine treatment of irritable bowel syndrome used herbs, acupuncture, moxibustion, herbal acupuncture, and auricular acupuncture. Herb medicines were administered according to the patient's case by a prescription of considering physical condition and symptoms. Acupuncture and moxibustion therapies stimulate the median points of LR(足厥陰肝經), ST(足陽明胃經), LI(手陽明大腸經), SI(手太陽小腸經). The assessment items for Korean medical treatment of Irritable Bowel Syndrome were abdominal symptoms, defecation, autonomic nervous system changes, stress, Live blood condition and mental health. Conclusions : Through continued research need to develop better treatment strategies for Korean treatment of Irritable Bowel Syndrome.

Whole Lung Irradiation for Metastatic Lung Malignancy (전이성 폐암의 전폐 방사선치료)

  • The author reviewed four cases of multiple lung metastases who were treated by whole lung irradiation alone for palliation. All patients tolerated the treatment well without complication. Two patients who had symptoms from pulmonary metastases had subjective relief of symptoms and all four patients had objective regression of lung metastases on repeat chest X-rays. Present report and review of literature support that some selected patients with multiple lung metastases can be benefited by whole lung Irradiation. However, systemic chemotherapy should be main modality of treatment for this kind of disseminated disease whenever possible.

A Clinical Report of Adult Acute Viral Hepatitis Type A Treated with Herbal Prescriptions

  • Owing to improvement of socioeconomic status during recent decades in Korea, incidence of hepatitis A has rapidly decreased, especially among children. However, this status has paradoxically caused a steady increase of adult patients with HAV infection, causing new medical issues associated with aggravated clinical symptoms. The present study reports an adult case of acute viral hepatitis type A treated with oriental medicine. The elevated biochemical findings(AST, ALT, gamma-GTP, bilirubin), physical symptoms (general weakness, nausea, right flank pain, itching sign), and serological makers (anti-HAV IgM and anti-HAV IgG) were normalized within four weeks. Also, sonographic examination showed a normal pattern on an enlarged liver image. This study informed us about the clinical capacity of oriental medicine for adult patients with acute viral hepatitis type A.

Intrathoracic Goiter (흉곽내 갑상선종 1예 보고)

  • Substernal goiter may be defined as any thyroid enlargement that has 50 to 100 % of its mass inferior to the thoracic inlet. Ectopic substernal goiters are rare, and most substernal gaiters arise from cervical thyroid gland. Fifteen to fifty percent of these patients are asymptomatic. Symptoms, when present, are usually the result of tracheal or esophageal compression. Symptoms are often positional. Standard chest roentgenograms are often diagnostic, but computed tomographic or radioactive iodine scans may be helpful. In symptomatic patients or those in whom explorations are undertaken for diagnostic purpose or exclude carcinoma, surgical removal is indicated. Although cervical thyroids with substernal extension may be safely and successfully removed through a cervical incision, primary substernal goiters by definition derive their blood supply from within the thorax and are better approached by splitting the sternum or through a posterolateral thoracotomy. Recently we experienced a child fist sized secondary posterior mediastinal goiter in 55-year old female. The mass was completely removed through right posterolateral thoracotomy without any complications. The postoperative courses were uneventful.

  • The relationship between acute pancreatitis and hyperparathyroidism has been described extensively in the past. Despite the rarity, the clinical progression of pancreatitis associated with hyperparathyroidism has been known to be so rapid and severe that it may cause death. When, furthermore, the pancreatitis is caused by hyperparathyroidism, the recovery from disease can be hardly expected unless the hyperparathyroidism is corrected. We present a case of acute pancreatitis in a 68-year-old man that have been caused by primary hyperparathyroidism. The clues of hyperparathyrodism were hypercalcemia and elevated parathyroid hormone, but he showed subtle or negative symptoms of hypercalcemia. After the excision of parathyroid adenoma, serum calcium level returned to normal and the symptoms and function of pancreas were recovered.

  • Background and Objectives : Singers, the vocal equivalents of Olympic athletes, may present with vocal dysfunction in the singing voice before adverse effects are noted in the speech mode. For the classically trained singers, any imbalance in the respiratory, laryngeal or resonatory subsystems may manifest first in technical compensations. Therefore it is necessary that baseline values be available in order to differentiate technique related dysfunction from abnormalities within the phonatory system. Materials and Methods : Vocal efficiency measures were collected from 55 classically trained singers and 20 untrained adults. All singers was divided into 3 groups : (1) Singers who have normal larynx with out voice symptoms. (2) Singers who have vocal nodule without voice symptoms. (3) Singers who have vocal nodule with voice complaints. Results and Conclusion : Results were compared with each other. Differences in each group wert found, suggesting the need for separate normative data to be used for the evaluation of the classically trained singers.

  • Thiamine, in the form of its diphosphate (TDP), is well known to act as a coenzyme, and during the early stage in the study of thiamine it had been believed that the symptoms of thiamine-deficiency were resulted secondarily from the disturbance of metabolic processes in which TDP participated as a coenzyme. However, the neurological symptoms in thiamine deficiency are now separated from the metabolic disturbances in thiamine deficiency. On the other hand, the specific involvement of phosphorylated thiamine in nerve conduction has been suggested by von Muralt, but nature of this involvement has not been elucidated at a molecular level. Recently the possible significance of thiamine triphosphate (TTP) in nervous tissue was suggested by the demonstration that TTP is not present in the brain of patients with subacute necrotizing encephalomyelitis, a fatal disease associated with an abnormality in thiamine metabolism. Furthermore, the studies using membrane fragments of rat brain strongly indicated that ion movement across the nerve membrane is associated with dephosphorylation of phosphorylated thiamine.

Early Esophageal Carcinoma(2 Cases report) (조기식도암 -2례 보고-)

  • Early esophageal carcinoma is defined as a lesion wherein invasion is confined to the mucosa and submucosa without metastasis to lymph node or other organs. Postoperative 5-year survival rate for early esophageal carcinoma is much superior than advanced carcinoma. Unfortunately, because of the anatomic characteristic of esophagus and absence of specific early symptoms, detection is frequently belated, and advanced disease is present at the time of the initial diagnosis. We experienced 2 cases of early esophageal carcinoma. They complained no specific symptoms. The diagnosis was made by barium esophagogram, esophagofiberscopy with dye staining and endoscopic biopsy. We performed esophagectomy with esophagogastrostomy. All had good postoperative course without any complication. We concluded that the combined use of double contrast radiography, esopagofiberscopy aided by intraluminal staining with Toluidine blue or Lugol`s solution, and endoscopic biopsy is very important in the diagnosis of early esophageal carcinoma in high risk patient group.

  • For the study of the applicability of predictive equation of subjective symptoms of workers using organic solvents, 67 male workers using organic solvents were selected. The predictive equation was quotated from previous Cheon's study. All the datas of necessary variables were gained. The predictive values and observed values were compared. The results were summarized as below; 1. The difference between predictive values and observed values were characteristically large. The coefficient of determination between the two values was 0.0024. 2. The $R^2$ value of multiple stepwise regression equation derived from present study was 0.2827.
