Therapeutic effect of Shinkiwhan, herbal medicine, regulates OPG/RANKL/RANK system on ovariectomy-induced bone loss rat

  • [Purpose] Although physical activity is required to prevent or ameliorate osteoporosis, medicine prescription should precede it, since it may be limited in severe osteoporosis patients. Furthermore, osteoporosis has a great effect on physical activity disorders that accompany fractures and pain, and therefore, research on treatment or prevention to decrease the number of patients is required. The purpose of this study was to discover candidate substances from natural products with an effective pharmacological action and to prepare basic data to help patients. [Methods] To prepare the osteoporosis model, ovariectomy (OVX) was performed using surgical methods. The prepared prescription [Shinkiwhan (SKH), a Korean medicine] was administered orally at a dose of 210 mg/kg/day for 8 weeks. After completion of the animal experiment, the bone mineral density (BMD) was analyzed using double-energy X-ray absorptiometry. The analysis of the effect of drugs on bones was performed using histological analysis and immunostaining. [Results] SKH increased the BMD in the OVX rats. Furthermore, SKH significantly increased the expression of osteoprotegerin and downregulated receptor activator of nuclear factor kappa B ligand and phosphorylation of c-jun N-terminal kinases in the bones of the OVX model. [Conclusion] Our findings suggest a protective effect of SKH against BMD loss in the OVX model.

Exploring the Mechanisms and Target Diseases of Sasang Constitutional Prescription based on Multiscale Interactome (다계층 상호작용 네트워크 기반 사상처방의 작용 기전과 대상 질환 탐색 연구)

  • Objectives The aim of this study is to explore the mechanism of action and target diseases of Sasang constitutional prescriptions using a multiscale interactome approach. Methods The compound and target information of Sasang constitutional prescriptions were retrieved from various databases such as the TM-MC, STITCH, and TTD. Key targets for Sasang constitutional prescriptions were identified by selecting the top 100 targets based on the number of simple paths within the constructed network. Diffusion profiles for Sasang constitutional prescriptions and diseases were calculated based on a biased random walk algorithm. Potential diseases and key mechanisms of Sasang constitutional prescriptions were identified by analyzing diffusion profiles. Results We identified 144 Sasang constitutional prescriptions and their targets, finding 80 herbs with effective biological targets. A cluster analysis based on selecting up to 100 key targets for each prescription revealed a more cohesive grouping of prescriptions according to Sasang constitution. We then predicted potential diseases for 62 Sasang constitutional prescriptions using diffusion profiles calculated on a multiscale interactome. Finally, our analysis of diffusion profiles revealed key targets and biological functions of prescriptions in obesity and diabetes. Conclusions This study demonstrates the effectiveness of a multiscale interactome approach in elucidating the complex mechanisms and potential therapeutic applications of prescriptions in Sasang constitutional medicine.

Development of Personalized Exercise Prescription System based on Kinect Sensor (Kinect Sensor 기반의 개인 맞춤형 운동 처방 시스템 개발)

  • The purpose of this study is to investigate the personalized treacmill exercise analysis using a smart mirror based on Kinect sensor. To evaluate the performance of the development system, 10 health males were used to measure the range of the hip joint, knee joint, and ankle joint using a smart mirror when walking on a treadmill. For the validity and reliability of the development system, the validity and reliability were analyzed by comparing the human movement data measured by the Kinect sensor with the human movement data measured by the infrared motion capture device. As a result of validity verification, the correlation coefficient r=0.871~0.919 showed a high positive correlation, and through linear regression analysis, the validity of the smart mirror system was 88%. Reliability verification was conducted by ICC analysis. As a result of reliability verification, the correlation coefficient r=0.743~0.916 showed high correlation between subjects, and the consistency for repeated measurement was also very high at ICC=0.937. In conclusion, despite the disadvantage that Kinect sensor is less accurate than the motion capture system, Kinect is it has the advantage of low price and real-time information feedback. This means that the Kinect sensor is likely to be used as a tool for evaluating exercise prescription through human motion measurement and analysis.

Analysis on the herbal combinations in Korean medicine for atopic dermatitis (아토피 치료 처방의 본초 활용 분석)

  • Objective : The aim of this study was to search and analyze the herbal combinations in formulas used to treat atopic dermatitis in Korean medicine. Methods : Articles on the atopy were searched for from among those registered on the OASIS up to December, 2015. After the articles were reviewed, information on both internal and external formulas for atopy was constructed. Then, the herbal combination in the formulas was analyzed by using frequency distribution, network analysis, and data mining. Results : The combination of 'Saposhnikovia Radix - Schizonepetae Spica' and 'Scutellariae Radix - Phellodendri Cortex ' was mostly used in both internal and external formulas to treat atopy in articles. By using the network analysis and data mining, twelve combinations including ' (Saposhnikovia Radix, Ponciri Fructus Pericarpium) - (Platycodi Radix, Schizonepetae Spica) ' were discovered. Five effective combinations were also searched for external formulas.Conclusions : This study could help researchers to analyze the formulas in various ways. Moreover, the herbal combination in atopy formulas could be used to search for atopy formulas in other databases or make a new prescription.

Inhibitory effect of Chelidonii Herba water extract on production of Nitric Oxide, Expression of iNOS and COX-2 in lipopolysaccharide-activated Raw 264.7 cells (백굴채(白屈菜)의 물추출물이 lipopolysaccharide로 유도된 Nitric Oxide의 생성 및 iNOS와 COX-2의 발현에 미치는 영향)

  • Chelidonii Herba (CHE, Baek-gul-chae in Korean), which has its original description in Gu-Hwang-Bon-Cho, a classic book of oriental Herbal book, is widely used in the treatment of stomach cancer, jaundice, gasrtic ulcer, edema and stomach pain, in Korea, Japan and China. The present study was conducted to evaluate the effect of CHE on the nitric oxide (NO) production, iNOS and COX-2 expression in lipopolysaccharide - activated Raw 264.7 cells. After the treatment of CHE, NO production was monitored by measuring the nitrite content in culture medium, cell viability was measured by MIT assay. COX-2 and iNOS were determined by lmmunoblot analysis. The production of nitric oxide was significantly inhibited by pretreatment (1h) with CHE (0.1-0.3 mg/ml) on LPS-activated Raw264.7 cells. The expression of inducible nitric oxide synthase (iNOS) and cyclooxygenase 2 (COX-2) protein were up-regulated by LPS, but the increased levels of iNOS and COX-2 were inhibited by pretreatment of CHE (0.1-0.3 mg/ml), respectively. Thus, the present data suggest that CHE may play an important role in adjunctive therapy in Gram-negative bacterial infections.

Intravenous fluid prescription practices among pediatric residents in Korea

  • Purpose: Recent studies have established the association between hypotonic fluids administration and hospital-acquired hyponatremia in children, and have contended that hypotonic fluids be removed from routine practice. To assess current intravenous fluid prescription practices among Korean pediatric residents and to call for updated clinical practice education Methods: A survey-based analysis was carried out. Pediatric residents at six university hospitals in Korea completed a survey consisting of four questions. Each question supposed a unique scenario in which the respondents were to prescribe either a hypotonic or an isotonic fluid for the patient. Results: Ninety-one responses were collected and analyzed. In three of the four scenarios, a significant majority prescribed the hypotonic fluids (98.9%, 85.7%, and 69.2%, respectively). Notably, 69.2% of the respondents selected the hypotonic fluids for postoperative management. Almost all (96.7%) selected the isotonic fluids for hydration therapy. Conclusion: In the given scenarios, the majority of Korean pediatric residents would prescribe a hypotonic fluid, except for initial hydration. The current state of pediatric fluid management, notably, heightens the risk of hospital-acquired hyponatremia. Updated clinical practice education on intravenous fluid prescription, therefore, is urgently required.

Effects of Rhei Rhizoma and Moutan Cortex on Inflammation and Insulin Resistance in Endothelial Cells Stimulated with Palmitic Acid (팔미트 지방산으로 자극된 혈관내피세포에서 대황 및 목단피가 염증 및 인슐린 저항성에 미치는 효과)

  • Rhei Rhizoma (RR) and Moutan cortex (MC) have been reported to have anti-inflammatory effects. However, little is known about the effects of RR and MC on endothelial inflammation and insulin resistance (IR). This study aims to investigate whether the water extracts of RR and MC could exert protection against palmitic acid (PA)-induced inflammation and IR in human umbilical vein endothelial cells (HUVECs). HUVECs were pretreated for 6 h with RR or MC, and then exposed to PA for 24 h. The levels of interleukin-6 (IL-6) and tumor necrosis factor-${\alpha}$ (TNF-${\alpha}$) were determined by enzyme-linked immunosorbant assay kits. Western blot analysis was performed for activation of nuclear factor-${\kappa}B$ (NF-${\kappa}B$) and insulin receptor substrate-1 (IRS-1). In HUVECs stimulated with PA, both RR and MC significantly inhibited the production of TNF-${\alpha}$ and IL-6 and the activation of NF-${\kappa}B$. At the same concentrations, the inhibitory effects of RR were more potent than those of MC. PA reduced insulin-induced phosphorylation of IRS-1, which was reversed by RR and MC. The results suggest that RR and MC are effective in inhibiting PA-associated endothelial inflammation and ameliorating IR by beneficial regulation of NF-${\kappa}B$ and IRS-1 activation.

The Basic Study for Building the Depression Prescription Guideline of Gamiguibi-Tang - The Evaluation of Reliability and Validity of the Depression Pattern-Identification Questionnaire - (가미귀비탕(加味歸脾湯)의 우울증 투약지침 개발을 위한 기초연구 - 우울증 변증 설문지의 신뢰도 타당도 평가 -)

  • Objectives : As depression falls into the category of Wuljeung, Gamiguibi-Tang(Jiaweiguipitang) is the standard prescription to cure Wuljeung. This study develops a questionnaire for building the guidelines to administer Gamiguibi-Tang to depression and evaluates reliability and validity of questionnaire. Methods : With extracting the text related to depression and Gamiguibi-Tang through total 9 Korean medicine books and consulting the experts, the study selected 80 items and converted them into a questionnaire. It surveyed the neuropsychiatry professors and the medical specialists three times by Delphi method, and lastly selected 21 final items of a questionnaire. On the basis of the questionnaire, it collected total 216 samples and tested their reliability and validity. Results : 21 items all didn't reduce total Cronbach alpha coefficient and satisfied test-retest reliability. As a result of factor analysis, totally 5 factors were extracted such as mental state, sleep, accompaniment, fatigue and weakness. Finally, in comparing a depression group with a normal control group, two groups all showed meaningful difference in each 21 items' point, the sum of factor 1 to 5 items' points, and the sum of 21 items' points. Conclusions : The questionnaire on the updated depression prescription guideline of Gamiguibi-Tang satisfied both of reliability and validity. Later it can help objectifying to apply Gamiguibi-Tang to depression cure.

Analysis of Prescriptions from Taepyeonghyeminhwajegukbang, Somunsunmyungronbang and Nansilbijang based on Herb weight ratio grade (본초 비율의 순위를 이용한 문헌의 특징 분석 방법 - 태평혜민화제국방(太平惠民和劑局方), 난실비장(蘭室秘藏), 소문선명론방(素問宣明論方)을 중심으로 -)

  • Objectives : I'd like to mimic JunChenZuoShe Theory as Herb weight ratio grade. And than analyze the characteristics of the prescription based on the Herb weight ratio grade from Taepyeonghyeminhwajegukbang, Somunsunmyungronbang and Nansilbijang. Methods : At first, we have changed herb informations to Herb weight ratio. The second we have decided the ranking of herb weight ratio. The third we have entered herb weight ratio grade. Finally we have counted frequency of first grade herb from the Taepyeonghyeminhwajegukbang, Somunsunmyungronbang and Nansilbijang. Results : We could use a new search function to find prescription by herb weight ratio grade. And than we could find out prescription efficacy rate of the ancient documents. Conclusions : We have entered herb weight ratio grade values by herb weight ratio ranking of top 10%, 30%, 60%. So we could find out that buqi(補氣) efficacy is used most often in the Taepyeonghyeminhwajegukbang, Somunsunmyungronbang and Nansilbijang. In the rate of qingrezaoshi(淸熱燥濕) efficacy, fasanfengre(發散風熱) efficacy, fasanfenghan(發散風寒) efficacy, Nansilbijang is higher than Taepyeonghyeminhwajegukbang. In the rate of qingrezaoshi(淸熱燥濕) efficacy, gongxia(攻下) efficacy, Somunsunmyungronbang is higher than Taepyeonghyeminhwajegukbang.

A Clinical Practice Guideline for Ma-huang(Ephedra sinica) Prescription in Obesity (비만치료 및 체중감량에서의 적절한 마황 사용에 대한 임상 진료지침 개발)

  • Background Clinical practice guidelines (CPG) are systematically developed statements to assist practitioners and patients on healthcare decisions. They provide recommendations for the average patient, which should take into account individual clinical judgment and the patient's values and expectations. Ephedra has sympathomimetic effect and has been used for weight loss worldwide. However, its safety is controversial especially in autonomic and cardiovascular systems. Therefore, the need of appropriate CPG for ephedra prescription in obesity was advocated in Korean Traditional Medicine. Methods The committee comprised of specialists of obesity, oriental herbology, oriental cardiology, constitutional medicine. The committee collected all relevant references about adverse effect and safety of ephedra in the forms of meta-analysis, systematic review, randomized controlled trial, case-control study and observational study from international and domestic databases and paper journals. 11 English- and 5 Korean-language references were gathered and categorized by PICO (Patient, Intervention, Comparison, Outcome) method. We could not complete strength of recommendation which should be clarified according to the evidence grade estimation. Result The first version of CPG for ephedra prescription in obesity was issued by Korean Oriental Association for Study of Obesity. It includes topics of introduction, pharmacokinetics, side effects and adverse events, constitutional aspect and recommendations for dose, indication and contraindication. Conclusion There should be periodic upgrade of this CPG from now on. Although there are some drawbacks in this version of CPG, it has significance as the first CPG in Korean Traditional Medicine.
