Long Term Results and Clinical Evaluation of Lung Cancer (폐암의 임상적 고찰과 장기 성적)

  • From May 1986 to May 1992, 72 patients were diagnosed and operated for primary lung cancer, among them 65 patients were clinically evaluated at the department of Thoracic & Cardiovascular Surgery, Masan Koryo General Hospital. 1. There were 52 males 13 females[M:F=4:1], and 5th, 6th decade of life[72%] was peak incidence. 2. The preoperative diagnosis and its positive rate were sputum cytology 35%, bronchoscopy 47%, pleural effusion cytology 80%, and pleural biopsy 50%. 3. The classification histologic types were squamous cell cancer 71%, adenocarcinoma 17%, undifferentiated cell carcinoma 4.6%, and staging classification were Stage I 31%, Stage II 22%, Stage IIIa 26%, and Stage IIIb 20%. 4. The operative methods were lobectomy 52%, pneumonectomy 36%, and open biopsy 12%, and operability was 89%, resectability was 88%. 5. The postoperative complications developed 13 patients[22%], and operative mortality was 5%. 6. The overall actuarial survival rate was 1year 70%, 2year 42%, 3year 32%, 4year 26%, and 5year 22%, according to Stage 5year survival rate was Stage I 37%, Stage II 22%, Stage IIIa 3year 12%, Stage IIIb 2year 23%. And according to operative method lobectomy 23%, pneumonectomy 19%.

Clinical Evaluation of 32 Cases of Aortocoronary Bypass with Sapheneous Vein (복재정맥을 이용한 관상동맥 우회로 술 32례의 임상적 고찰)

  • From January 1990 to May 1993, 32 patients were underwent aortocoronary bypass with venous conduit using saphenous vein at Department of Thoracic and Cardiovascular Surgery, Masan Koryo General Hospital. There were 19 men, 13 women and a mean age was 57.8 years[range from 46 to 68]. The involved risk factors were as follows: smoking 15 cases, hypertension 14 cases, diabetic mellitus 6 cases, cholesterol[>240mg/dl] 13 cases, and triglyceride[>180mg/dl] 11 cases. The preoperative clinical status were chronic stable angina 4 cases, unstable angina 24 cases and myocardial infarction 4 cases. At angiogram, numbers that involved in coronary artery of narrowing more than 50% luminal diameter were 54 vessels[left anterior descending 30, right coronary artery 17, circumflex 7]. We performed 53 grafts in32 patients, average was 1.7 grafts per patient, 16 patients were single bypass, 11 patients were double bypass and 5 patients were triple bypass. The overall postoperative mortality was 3 %.

Postoperative Clinical Evaluation of Mechanical Valve Replacement (기계적판막의 임상적 고찰)

  • In the department of chest surgery of WonKwang university hospital, mechanical valve replacement was performed in 51 cases from June 1985 to September 1987. Among these, 32 cases were mitral valve replacement, 4 cases were aortic valve replacement, and 15 cases were double valve replacement. 26 cases were male and 25 cases were female and age distribution ranged from 16 years old to 63 years old. Early death within 30 days after operation was 2 cases [3.9%] and causes of death were right heart failure [1] and right ventricular wall rupture [1]. Among 49 early survivors, 2 cases of late death were developed and the causes of death were cardiomyopathy [1] and ventricular arrhythmia [1] Anticoagulant therapy was done with warfarin sodium to the level of 1.5-2 times of normal prothrombin time [20-40%] in 47 survivors. Symptomatically, 93.6% of preoperative NYHA functional class III or IV were converted to the NYHA functional class I or II during follow up.

Evaluation of Vocal Efficiency for the Polyps and Nodules

  • The vocal efficiency(VE) can be calculated as the ratio of acoustic power to aerodynamic power. It relates to the vocal intensity, air Sow rate and subglottic pressure. In this study, we treated 20 cases of vocal polyps and 10 cases of vocal nodules by way of laryngo-microsurgery or laser laryngo-microsurgery. The VE was measured preoperatively and postoperatively in all cases. The results showed that there was a significant improvement of VE postoperatively than that of preoperative measurement(p<0.01) in vocal Polyp group. However, there was no significant difference(p>0.05) in vocal nodule group. Through comparing the results, we obtained the conclusion : The laryngo-microsurgery is the reliable method of management for the vocal polyps, but f3r the vocal nodules, the laryngo-microsurgery should be selected after other more conservative approaches fail to produce the desired results.

Dynamic Lumbar Spinal Stenosis : The Usefulness of Axial Loaded MRI in Preoperative Evaluation

  • Two cases of dynamic lumbar spinal stenosis were identified by the authors using axial loaded magnetic resonance image (MRI). In both cases, the patients presented with neurogenic claudication but MRI in decumbency showed no definite pathologic condition associated with their symptoms. In contrast, axial loaded MRI demonstrated constrictive spinal stenosis and a significantly decreased dural sac caused by epidural fat buckling and thickening of the ligamentum flavum in both cases. In the second case, a more prominent disc protrusion was also demonstrated compared with decumbent MRI. After decompressive surgery, both patients had satisfactory outcomes. Axial loaded MRI can therefore give decisive information in dynamic spinal disorders by allowing simulation of an upright position.

  • The cavus foot is a deformity characterized by an elevated medial longitudinal arch and a hindfoot varus with plantarflexed 1st ray. The etiology of cavus foot is usually related to neuromuscular disease or idiopathic cause. Thorough clinical and radiographic evaluation is required for differentiating etiology of the cavus. Most cases of cavus foot are stable and slowly progressive deformities which can initially be managed with conservative treatment including orthoses and physical therapies. Determining whether the deformity is flexible or rigid, the apex of the deformity and any muscle imbalances in foot and ankle is important for achievement of an adequately balanced plantigrade foot. Treatment should include systematic preoperative planning for selection of appropriate procedures for maintaining a functional and flexible foot with combinations of soft-tissue release, osteotomy, tendon transfer, and arthrodesis.

Surgical Outcomes of Arthroscopic Modified Brostr$\ddot{o}$m Procedure in Chronic Lateral Ankle Instability (족관절 만성 외측 불안정증에서 관절경하 변형 Brostr$\ddot{o}$m 술식의 수술적 결과)

  • Purpose: To evaluate the result of arthroscopic modified Brostrom procedure with suture anchor for chronic lateral ankle instability. Materials and Methods: Fifty-two patients with chronic lateral ankle instability were analyzed, who underwent arthroscopic modified Brostrom procedure between December 2010 and May 2012. Clinical evaluation was performed using AOFAS scroring and Sefton grading system. Results: The average AOFAS hind foot score increased from preoperative 61.9 to 88.8 at the last follow up. There were 35 excellent, 9 good, 4 fair, 4 poor results according to Sefton grading system. For one patient, lateral ankle instability recurred. Conclusion: Arthroscopic modified Brostrom procedure is considered to be an effective and satisfactory technique.

  • We describe a backprojection filtering method for limited angle tomography. In order to reduce blur artifacts originated from the out-of-planes and control high-frequency noise, we employed two band-limited window functions, and which were realized by Hann filters in this study. Based on the experimentally measured data, the performance of the developed method is demonstrated by comparing with the conventional shift-and-add and filtered backprojection methods. Application of the developed method to the dental imaging has a potential to be used for the preoperative evaluation of the jaw for dental implants with much reduced patient dose instead of the conventional dental computed tomography.

Surgical treatement of complex cardiac malformations (복잡심기형의 외과적 치료)

  • Thirteen patients with cyanotic cardiac malformations having more complex intracardiac defects, hemodynamics and operative procedures than ones in Tetralogy of Fallot undertaken total surgical corrections from July 1981 to August 1985. The cases of corrective surgery for complex cardiac malformations were 3.9% of all congenital cardiac malformations and 12.6% of cyanotic cardiac malformations. Six patients died within 30 days after surgery. So operative mortality was 46%; Transposition of the great arteries, two of 4 patients, due to low cardiac output syndrome and tracheal bleeding ; Univentricular heart, one of 3 patients, due to bleeding; Corrected transposition of the great arteries, one of 2 patients, due to acute heart failure; Tricuspid atresia, one of 2 patients, due to low cardiac output syndrome; Double outlet right ventricle, one of single patient, due to respiratory failure. The cases of surgical correction for complex cardiac malformations are progressively increasing in numbers. The more accurate evaluation of anatomical condition and hemodynamics in preoperative diagnosis, studies on applicable surgical procedure and perioperative care of patients are necessary in the improvement of clinical and surgical results.

Calcified Giant Thrombosis in the Right Atrium and Coronary Sinus with Persistent Left Superior Vena Cava - Report of 1 case - (우심방과 관정맥동내의 석회화된 거대혈전증을 동반한 좌상공정맥증 - 1례 보고-)

  • We experienced one case of the calcified giant thrombosis in the enlarged right atrium and coronary sinus with markedly dilated persistent left superior vena cava and absent right superior vena cava in 29year old female patient. We supposed that the dilatation of persistent left superior vena cava was due to poststenotic dilatation secondary to obstruction on orifice of coronary sinus by thrombosis. The giant thrombosis in the right atrium and coronary sinus was successfully resected. She had improvement on preoperative chest discomfort but, the moderate hepatomegaly was developed and then she was discharged with incompletely recovered state due to economical poverty on postoperative 6th weeks. The continuous follow-up and study are indeed necessary for further evaluation of pathology and etiology.
