교통사고 후 두통을 호소하는 환자들의 다면적 인성검사 프로파일(Profile) (Minnesota Multiphasic Personality Inventory Profile of Headache Patients after Traffic Accident)

  • Objectives : This study was investigated to find out psychological characteristics of headache patients after traffic accident(TA). also It was identified headache after TA as predictor of posttraumatic stress disorder(PTSD). Methods : Minnesota Multiphasic Personality Inventory(MMPI) was administered to 26 TA patients, We divided them into four groups according to headache and sex and headache duration. Results : 3-1 profile was found by total mean of T score of each MMPI scale in all patients. 3-4 profile was found in headache group, 1-3 profile was found in female Group, female to have headache group. 3-1 profile was found in headache to continue more than 7 days. Conclusions : Female group, female to have a headache group, headache to continue more 7 days group showed 1-3/3-1 profile. it was similar to PTSD profile. The results indicate that in the above three groups, headache was predictor of PTSD and they must be treated more carefully.

외상후 스트레스 장애를 가진 만성요통 환자에게 도수치료가 외상후 스트레스와 수면 장애 및 통증에 미치는 영향-사례 연구 (A Case Study of the Effect of the Manual Therapy on Patient with Posttraumatic Stress Disorder, Sleep Disturbance and CLBP)

  • Background: This case study is to describe the manual therapy for the patient with posttraumatic stress (PTSD), sleep disturbance, and pain such as chronic low back pain (CLBP). Methods: The patient who participated in this study was a 60 year-old male PTSD patient with CLBP. His CLBP is not from any genetic or family history but a sequelae of torture that he had gone through about 30 years ago. Prior to the intervention, it was assessed that the PDS-K score was 16, PQSI-K was 12, SLR-90-R was low, VAS score was 10, and KODI socre was 25. The intervention was conducted through manual therapy (myofascial release, muscle energy technique, lumbar stabilization) twice a week for eight weeks in total. Results: After eight week-intervention, the PDS-K, SLR-90-R, VAS, and KODI score were improved whereas PQSI-K was not sufficiently fast improved. Conclusions: The manual therapy is substantially effective in dealing with PTSD and CLBP.

  • Objectives : Since prostitution is multitraumatic phenomena, it is known that most women in prostitution have symptoms of posttraumatic stress disorder (PTSD) and other mental illness. In Korea, new anti-prostitution law launched in the year of 2004 imposes protecting prostituted victims at government-supported shelter. The objective of this study was to compare the mental symptoms, especially PTSD features of women who escaped from prostitution with those of control subjects. Methods : We assessed 113 ex-prostituted women who live in shelter for 8 months on the average and 65 normal control subjects. Demographic data, questionnaire for sleep, physical illness, smoking, alcohol and drug use, Stress Response Inventory, Davidson Trauma Scale (DTS), Impact of Event Scale-Revised (IES-R) were used. Results : Ex-prostituted women had significantly higher scores of DTS, IES-R, stress related responses, sleep problems, smoking problems, and alcohol problems than control group. Age and duration of sex trade of the subjects were significantly correlated with the severity or frequency of PTSD symptoms. The degree of smoking, alcohol drinking and sleep problems were also significantly correlated with the PTSD symptoms. Conclusion : These results suggested mental symptoms did not disappear even after women escaped from prostitution.

'PTSD 시대'의 고통 인식과 대응: 외상 회복의 대안 패러다임 모색 (The awareness and coping of human suffering in the "PTSD era": Searching for an alternative paradigm of trauma recovery)

  • 본 연구는 외상후 스트레스 장애(Posttraumatic stress disorder, PTSD)가 등장한 이후 현대 사회에서 외상 및 고통을 인식하고 이에 대응하는 방법을 조명하였고, 한국 사회의 외상 인식과 대응 방식을 검토하면서 외상 회복을 위한 대안 패러다임의 원칙을 제시하였다. 외상은 기억을 매개로 만성적인 고통을 야기하는 외부 스트레스 사건으로서, 미국정신의학회는 1980년 정신장애 진단 및 통계 편람에서 외상후 스트레스 장애를 공식 인정하였다. 외상후 스트레스 장애 진단의 개발은 피해자에게 도덕적 정당성을 부여하였고, 성공적인 치료 개입의 길을 열었으며, 신경생리학과 인지신경과학 분야에서 연구 성과 축적에 기여하였다. 그러나 이와 동시에, 고통에 대한 인식이 협소해졌고, 기술 개입의 한계를 넘어서는 대응 방안의 중요성이 간과되고 있다. 특히 한국사회의 분리와 부인의 역사적 맥락, 의료 및 전문가 중심 관료주의 기저에는 외상을 개인의 문제로 치환하려는 전략이 발견된다. 따라서 사회적 고통으로서 외상을 인식하고 대안 패러다임을 모색할 필요가 있다. 이 글은 진실 규명과 정의 회복, 회복 주체로서 생존자와 공동체의 책임, 최신 생물-심리학적 성과의 생태학적 적용, 그리고 회복이란 무엇인가에 관한 지속적인 논의를 중시하는 대안 패러다임의 원칙을 제시하였다.

북한이탈주민의 삶의 질이 외상 경험 이후 심리적 성장에 미치는 영향 (A Study on the Quality of life and Posttraumatic growth among North Korean refugees)

  • 본 연구는 남한에 장기 거주하는 북한 이탈 주민의 삶의 질이 외상 및 스트레스와 같은 역경 경험 이후 심리적 성장(Posttraumatic Growth)에 어떠한 영향을 주는지를 이해하기 위해서 실시되었다. 이를 위해 남한 거주기간이 평균 7년 이상 되는 탈북 주민 105명을 대상으로 PTG의 예측 요인으로서 삶의 질을 분석하였다. 또한 삶의 질이 외상 및 스트레스를 어느 정도 조절하여 PTG에 영향을 미치는지 파악하여 북한이탈주민 개인이 PTG에 이를 수 있는 조절변수를 예측해 볼 수 있는 탐색적 연구를 실행하였다. 결과적으로 남한 이주 7년차인 북한이탈주민의 경우 문화적응스트레스는 삶의 질에 부정적인 영향을 미치나 미래에 대한 희망과 인식된 만족이 삶의 질에 긍정적인 영향을 미치는 것으로 나타났다. 남한 이주기간에 따른 삶의 질과 PTG의 상관관계를 조사한 결과 초기 이주 3년 정도에까지는 총 4개의 삶의 질 영역 중 심리적 영역과 사회적 영역에서의 삶의 질 향상이 PTG와 긍정적인 상관관계가 있음이 제시되었으나 그 두 개의 영역 중 특히 심리적 영역이 PTG에 특히 유의한 영향을 미치는 것으로 밝혀졌다. 이주 7년이 경과된 상태에서 조사된 결과로는 신체적, 심리적, 사회적, 환경적 영역 모두가 PTG에 긍정적인 상관은 있으나 인과관계를 설명하는데 있어서 그 중 특히 심리적 영역 뿐 아니라 사회적 영역으로 확장된 삶의 질 향상이 PTG에 긍정적인 영향을 미치는 것으로 파악되었다.

음악치료가 외상 후 스트레스를 받은 청소년의 뇌파 변화 및 정서, 우울, 불안 감소에 미치는 영향 (The Effects of Music Therapy on the Changes in Brain Waves and Emotion, and Reduction of Depression and Anxiety for the Post-traumatic Stressed Adolescent)

  • This study of a music therapy program for the 18 subjects residing in Jeollanam-do province was conducted two times per week during 4 months totalling 17 times in order to evaluate the reduction of the characteristics of patients suffering from stress disorder patients such as depression, and anxiety, Though the experiment for non-symmetric level the concentration and appearance of Alpha wave were determine to be stable but the findings from non-symmetric level according to the left-right brain activity were not significant for T-test, though its effective intermission of music therapy due to variation of the mean values was demonstrated, and the variation of the Alpha wave appearance rate was also significant for the statistics of the T-test for the effectiveness of the music therapy. And for the reduction of depression, emotion, and state-trait anxiety between groups divided into the control group and the experiment, pre-measurement and post-measurement comparisons through psychological tess, validated the effectiveness of music therapy for Posttraumatic stress disorder patients. First of all, this study was conducted in the situation of scarce study cases and program developments for the efficacy of music therapy for the patients suffering from Posttraumatic stress disorders without precedent studies of scientific identification through brain wave measurement. Because this study is insightful in the context of validating the efficacy of musical treatment, we expect that these kinds of studises will be continued.

학교폭력 가해자에서 외상후 스트레스 증후군의 증상과 휴대폰 중독적 사용과의 관계 (The Relationship between Symptoms of Posttraumatic Stress Disorder and Addictive Use of Mobile Phone in Bullies of School Violence)

  • Objective : This study aims to investigate the relationship between addictive mobile phone use and post-traumatic stress disorder (PTSD) symptoms in bullies who experienced school violence. Methods : The study subjects consisted of 97 adolescents in The Foundation for Preventing Youth Violence after school violence. The participants conducted self-report questionnaire for addictive mobile phone use and Impacted Event Scale-revised-Korean version (IES-R-K). Partial correlation and multiple logistic regressions were performed. Results : The scores related to addictive mobile phone use were correlated with total score (r=0.21, p=0.04), avoidance score (r=0.21, p=0.04) and intrusive score (r=0.26, p=0.01) related to PTSD symptoms on the IES-R-K. Intrusive score related to PTSD symptoms on the IES-R-K were significant risk factor for addictive mobile phone use (O.R=1.25, 95% CI=1.12-1.39). Conclusion : This data supports that PTSD intrusive symptoms could be predictable factor of addictive mobile phone use. An appropriate intervention about PTSD symptoms might play a useful role in preventing addictive mobile phone use in bullies of school violence.

A Case Study of the Effects of Posttraumatic Stress Disorder on Operational Fire Service Personnel Within the Lancashire Fire and Rescue Service

  • Background: Lancashire Fire and Rescue Service (LFRS), the subject of this evaluative research document employs 1400 people. 80% of employees are operational firefighters and officers whom operate across a range of duty systems and support functions, providing prevention, protection and emergency response to the communities of Lancashire. Methods: The overarching purpose of this epidemiological study is to assess the prevalence of posttraumatic stress disorder (PTSD) amongst operational LFRS personnel and to analyse the effects upon those who may be suffering from it, whether brought about by a single traumatic event or by repeated exposure to traumatic occurrences over a period of time. A combination of primary and secondary research was carried out. Primary data was collated using two recognised clinical questionnaires and statistical analysis was conducted with the aid of the software package SPSS. Results: The findings and statistical analysis showed that out of the 100 people surveyed, 30% of respondents had signs of probable distress. Of this quota, 4% showed symptoms of PTSD. The study considers how an organisation can recognise and manage PTSD and provides recommendations to assist in better recognising and managing the associated risks. Conclusion: Based upon the findings, the authors conclude that the level of PTSD within LFRS is slightly lower than those found in other studies undertaken within the Fire and Rescue Service sector. The paper provides recommendations for future studies and a series of actions for consideration by LFRS senior management to improve PTSD support services for employees.

소방 공무원에서 간접 외상이 정신 건강에 미치는 영향 (Effects of Vicarious Trauma on Mental Health in Firefighters)

  • Objectives : Indirect contact with trauma may occur when a person empathically listens to detailed descriptions, exposing the person to intense emotional pain from trauma victims. Although less severe than direct trauma, indirectly traumatized persons may experience the same fear, rage, and despair as direct trauma victims. This phenomenon has been variously termed vicarious trauma, traumatic countertransference, burnout, compassion fatigue, and secondary traumatic stress. Using a psychiatric symptoms questionnaires, this study investigated the effects of indirect exposure to co-worker's trauma on the mental health of firefighters who had not directly experienced traumatic events in the previous year. Methods : We administered self-report questionnaires, such as the Impact of Event Scale-Revised (IESR) and Alcohol Use Disorder Identification Test (AUDIT), to firefighters working at two fire stations in a metropolitan city. We analyzed 188 of 232 collected surveys, after excluding falsely entered data (28 cases) and questionnaires by directly-traumatized victims (16 cases). Results : Alcohol consumption and posttraumatic stress disorder (PTSD) symptoms, as determined by the AUDIT and IES-R, were significantly higher in the indirectly-traumatized group. The ratio of high-risk members scoring higher than 23 in IES-R was larger in the indirectly traumatized group, although this was not statistically significant. Conclusion : Alcohol consumption was significantly higher in the indirectly traumatized group. This group also included more members at high risk for PTSD. Further research, with a larger group of indirectly traumatized firefighters and ongoing investigation of PTSD development and other psychiatric symptoms, is needed.

COVID-19 환자 간호한 간호사들의 외상 후 스트레스 장애, 우울 및 사회적 낙인 간의 관계 (Corelation between Nurses' Posttraumatic Stress Disorder, Depression and Social Stigma in Nursing COVID-19 Patients)

  • Purpose: This study aims to examine the levels of posttraumatic stress disorder (PTSD), depression and social stigma among nurses caring for Corona Virus Disease-19 (COVID-19) patients. Methods: 169 nurses caring for COVID-19 patients participated in this study. Data collection was conducted at university hospitals from March 1 to August 31, 2020. Data analysis was performed for descriptive statistics, t-test, ANOVA, and Pearson correlation coefficients using SPSS/WIN 24.0 program. Results: The mean scores of PTSD, depression and social stigma were 25.16±16.80, 17.26±8.63 and 5.83±2.84, respectively. The PTSD scores were significantly different between the department (F=2.89, p=.037). Depressive scores were significantly different between the marital status (t=2.27, p=.024) and the department (F=4.91, p=.003). Social stigma scores were significantly different between age (F=6.49, p=.002), marital status (t=-3.30, p=.008), having or not having children (t=3.82, p=.001), department (F=5.82, p=.001) and clinical experience (F=7.43, p=.001). Positive correlations were found between PTSD and depression (r=.70, p<.001) and social stigma (r=.22, p<.004). Conclusion: Integrated assessment and management are required to address the psychological and emotional problems faced by nurses caring for COVID-19 patients, and active follow-up measures should be considered.
