  • Background & Object : Postprandial epigastric fullness is one of common symptoms in functional dyspepsia, but its pathophysiological mechanism has still been unknown. In this study, its association between postprandial epigastric fullness and pyloric valve disturbance was studied through analysis of bowel sounds and electrogastrography. Methods : Bowel sounds and electrogastrography were recorded together for fasting 15 min and postprandial 40 min. Parameters of bowel soundincluding motility index, sound to sound interval, standard deviation of sound to sound interval, sounds per minute, % of bowel sound, sound length, intensity, dominant frequency of sounds, and postprandial / fasting ratio of dominant frequency of sounds were analyzed with a specialized program. By electrogastrography, regularity of slow waves and power ratio were obtained. Results : Significances of bowel sounds appeared in motility index (p=0.046), dominant frequency of fasting (p=0.048), postprandial (p=0.003), and the ratio of postprandial/fasting (p=0.000); those of EGG parameters were shown in postprandial regularity of slow waves (p=0.006) and power ratio (p=0.011). Conclusion : Pyloric valve disturbance was a cause of postprandial epigastric fullness in patients with functional dyspepsia. Analysis of bowel sound might be useful in diagnosing its existence.

  • New biochemical and genetic markers will be required to be more successful in the prevention of coronary heart disease. Postprandial lipid metabolism has received considerable attention since it was shown that postprandial triglyceride-rich lipoproteins are independently involved in the development of atherosclerosis. Multiple genes and environmental factors work in concert to alter these lipid. In this paper, postprandial lipemia, genetic variation and cardiovascular risk will be reviewed.

재가 노인의 식사후 저혈압 발생에 관한 연구 (A Study on the Prevalence and Risk Factors of Postprandial Hypotension among the Community-Dwelling Aged)

  • Purpose: This study was to identify the prevalence of postprandial hypotension and its related risk factors, and investigate the relationship between the related symptoms and the occurrence of postprandial hypotension. Method: The data were collected from the 22th of August, 2000 to 7th May, 2001. The participants were 74 aged over 60 in Seoul and Chungju, Korea. Blood pressure was taken in the sitting Position at 15mins, 30mins and 60mins after starting meals. Result: 1) The prevalence of postprandial hypotension was 12.2% at 15mins, 25.7% at 30mins, 25.7% at 60mins after starting meals. 2) There was no significant difference in a9e, but there was significant difference in basal systolic blood pressure between responders and nonresponders to postprandial hypotension over the three different times. 3) There was no significant relation between the occurrence of postprandial hypotension and its related symptoms over the three different times. Conclusion: Findings indicate that this study will contribute to develop an assessment protocol and nursing strategies to identify risk factors and to prevent postrprandial hypotension for the aged.

Sodium Alginate와 Cellulose 가 식후 혈장 Lipoprotein 조성과 콜레스테롤 대사에 미치는 영향 (II) (Effects of Sodium Alginate and Cellulose on Postprandial Plasma Lipoprotein and Cholesterol Metabolism in Rats(II))

  • This study was undertaken to dtermine whether dietary fibers had an effect on postprandial plasma lipoprotein and liver lipid composition in rats. Each experimental diet ocntained 10 % dietary fiber by weight. All animals were sacrificed in postprandial state. Sodium alginate-fed animals has significantly lower plasma cholesterol, VLDL-, LDL-cholesterol, TG, and protein levels than did the fiber-free control group. In addition, liver cholesterol and TG concentrations were the lowest in sodium alginate -fed animals. There was no significant change in HDL-cholesterol levels among experimental groups. Cellulose-fed animals also have lower plasma cholesterol and TG levels than fiber-free controls, but liver TG concentrations were not different from those in fiber-free controls. This study demonstrates that dietary fiber included in the diet of rats is able to alter postprandial lipoportein cholesterol and TG, and that sodium alginate, a soluble fiber, was the most effective in lowering plasma and liver cholesterol and TG.

소시호탕(小柴胡湯)을 투여한 오심, 구역, 구토 및 식후포만감 1례 (A Case Report on Nausea, Retching, Vomiting and Postprandial Fullness Treated with Soshiho-tang)

  • Objectives : "The purpose of this study is to report the effectiveness of Soshiho-tang(小柴胡湯)" on the patient suffered from nausea, retching, vomiting and postprandial fullness. Methods : We diagnosed the symptoms and the signs of the patient as Soshiho-tang syndrome(小柴胡湯證) according to "Treatise on Cold Damage Disease(傷寒論)". We prescribed Soshiho-tang with acupuncture and moxibustion for about 2 weeks. Results : The subjective assessments of the nausea and postprandial fullness had improved. And the frequencies of the retching and vomiting had decreased. Conclusions : We suggest that Soshiho-tang has effectiveness on nausea, retching, vomiting and postprandial fullness in patient diagnosed as Soshiho-tang symptom.

Sargassum yezoense Extract Inhibits Carbohydrate Digestive Enzymes In Vitro and Alleviates Postprandial Hyperglycemia in Diabetic Mice.

  • In this study, we investigated whether Sargassum yezoense extract (SYE) could inhibit ${\alpha}-glucosidase$ and ${\alpha}-amylase$ activities, and alleviate postprandial hyperglycemia in streptozotocin (STZ)-induced diabetic mice. Freeze-dried S. yezoense was extracted with 80% ethanol and concentrated for use in this study. The hypoglycemic effect was determined by evaluating the inhibitory activities of SYE against ${\alpha}-glucosidase$ and ${\alpha}-amylase$ as well as its ability to decrease postprandial blood glucose levels. The half-maximal inhibitory concentrations of SYE against ${\alpha}-glucosidase$ and ${\alpha}-amylase$ were $0.078{\pm}0.004$ and $0.212{\pm}0.064mg/mL$, respectively. SYE was a more effective inhibitor of ${\alpha}-glucosidase$ and ${\alpha}-amylase$ activities than the positive control, acarbose. The increase in postprandial blood glucose levels was significantly alleviated in the SYE group compared with that in the control group of STZ-induced diabetic mice. Furthermore, the area under the curves significantly decreased with SYE administration in STZ-induced diabetic mice. These results suggest that SYE is a potent inhibitor of ${\alpha}-glucosidase$ and ${\alpha}-amylase$ activities and alleviates postprandial hyperglycemia caused by dietary carbohydrates.

식후저혈압 위험 노인의 식전 물 섭취가 식후 혈압변화에 미치는 효과 (Effect of Water Drinking on the Postprandial Fall of Blood Pressure in the Elderly)

  • Purpose: The purpose of this study was to examine preventive effects of water drinking on postprandial fall of blood pressure in the elderly. Method: Participants were 25 elders who had experienced postprandial fall of systolic blood pressure of more than 15mmHg in a previous study. Within subject repeated measures design using random order allocation was used to control extraneous variables among participants. During the experiment, each participant had to drink 400mL water before lunch, whereas when in the control, they only had their lunch. Blood pressure and heart rate were measured for both times before lunch and at 15 minutes intervals up to 90 minutes after lunch. Data were analyzed using descriptive statistics, repeated measures of ANOVA, paired t-test and Bonferroni adjustment as Post-hoc analysis. Results: Water drinking before lunch elicited significant pressor effects by increasing systolic and diastolic pressure $13.9{\pm}4.7/5.2{\pm}2.2mmHg$ compared blood pressure during the control p<.005, p<.022. Heart rate, however, did not change significantly at either time. Conclusion: Drinking water before meal is recommended for elders who experience postprandial hypotension. However, the appropriate volume of water to prevent postprandial fall in blood pressure should be further studied.

An Update on Accumulating Exercise and Postprandial Lipaemia: Translating Theory Into Practice

  • Over the last two decades, significant research attention has been given to the acute effect of a single bout of exercise on postprandial lipaemia. A large body of evidence supports the notion that an acute bout of aerobic exercise can reduce postprandial triacylglycerol (TAG) concentrations. However, this effect is short-lived emphasising the important role of regular physical activity for lowering TAG concentrations through an active lifestyle. In 1995, the concept of accumulating physical activity was introduced in expert recommendations with the advice that activity can be performed in several short bouts throughout the day with a minimum duration of 10 minutes per activity bout. Although the concept of accumulation has been widely publicised, there is still limited scientific evidence to support it but several studies have investigated the effects of accumulated activity on health-related outcomes to support the recommendations in physical activity guidelines. One area, which is the focus of this review, is the effect of accumulating exercise on postprandial lipaemia. We propose that accumulating exercise will provide additional physical activity options for lowering postprandial TAG concentrations relevant to individuals with limited time or exercise capacity to engage in more structured forms of exercise, or longer bouts of physical activity. The benefits of accumulated physical activity might translate to a reduced risk of cardiovascular disease in the long-term.

순환여과식 내 수온에 따른 돌돔(Oplegnathus fasciatus) 치어의 절식 및 식후 암모니아 배설 (Fasting and Postprandial Ammonia Nitrogen Excretion of Juvenile Oplegnathus fasciatus at Various Temperatures in a Recirculating System)

  • This study investigated diurnal fasting and postprandial total ammonia nitrogen (TAN) excretion rates in juvenile Oplegnathus fasciatus (rock bream; mean body weight 45.8±1.1 g) at three temperatures (15, 20, and 25℃) in a recirculating system (three replicates). The fish were hand-fed twice daily (09:00 and 17:00 h) with commercial food (46.7% protein) until satiation. The results showed that the fasting and postprandial TAN excretion rates were temperature-dependent. The mean hourly rates were 7.7 (15℃), 10.2 (20℃), and 11.9 (25℃) mg kg fish-1 h-1 for fasting and 34.0 (15℃), 47.8 (20℃), and 60.2 (25℃) mg kg fish-1 h-1 for postprandial. At each temperature, two postprandial TAN excretion peaks were observed 2-4 h after feeding; the second peak was always higher. The TAN loss to nitrogen consumption ratio was 41.1-46.8 % and increased significantly with increasing temperature. Temperature affected fasting and postprandial TAN excretion in juvenile rock bream, providing insight for culture management.

당뇨환자의 식사 전후에 따른 혈중 Insulin-like growth factor(IGF-I), IGF-II 및 Insulin-like growth factor binding proteins(IGFBP)-3의 변화 (Changes of Insulin-like Growth factor-I, II and IGF-Binding Protein-3 on Fasting and Postprandial state in Diabetes)

  • IGFs and IGFBPs have an important role in controlling glucose homeostasis. This study was conducted to investigate the changes of insulin-like growth factor(IGF)-I. IGF-II and IGF binding proteins (IGFBPs) on fasting and postprandial state in Korean diabetes, Twenty eight healthy subjects and fifty seven diabetic patients participated in this study. The healthy subjects were not knowingly suffered from any disease and were not receiving any medical treatment, and diabetic subjects were undergo medical treatment, continuously. Weight and height were measured and body mass index (BMI) was calculated as weight (kg) divided by the square of height (m2). Blood pressure was measured. Plasma lipid profiles were analyzed by enzymatic methods, plasma Insulin and glucose levels were measured in fasting and postprandial state, respectively. The levels of serum IGFs and IGFBP-3 were measured by radioimmunoassay (RIA). The levels of glucose and insulin were significantly higher in diabetes than normal subjects on fasting as well as postprandial state (p<0.0l). The levels of IGF-I was significantly lower in diabetes than normal subjects, however in postprandial state, there was no significant difference between diabetes and control subjects, The levels of IGF-II were significantly lower in diabetes than control subjects both fasting and postpradial state, The level of IGFBP-3 were not significantly different between diabetes and normal subjects. Fasting IGF-I, IGF-II and IGFBP-3 levels were positively correlated with those levels on postprandial state, fasting IGe levels of IGF-I levels were positively correlated with fasting insulin levels, and postprandial IGF-I levels were positively correlated with fasting glucose, postprandial insulin and postprandial insulin levels, plasma triglyceride levels were correlated with plasma triglyceride levels. The IGFBP-3 levels were not correlated with IGF components, glucose, insulin and plasma lipids, These results demonstrate that in diabetes, the components IGF-I/IGFBPs system were significantly correlated with plsma glucose and insulin levels both fasting and postprandial state.
