Purpose: To evaluate the effectiveness and safety of postoperative radiotherapy for the treatment of keloid scars administered immediately after Cesarean section. Materials and Methods: A total of 26 postpartum patients with confirmed keloids resulting from previous Cesarean sections received either 12 or 15 Gy radiotherapy. The radiotherapy was divided into three 6 MeV electron beam fractions administered during the postpartum period immediately following the final Cesarean section. To evaluate ovarian safety, designated doses of radiation were estimated at the calculated depth of the ovaries using a solid plate phantom and an ionization chamber with the same lead cutout as was used for the treatment of Cesarean section operative scars and a tissue equivalent bolus. Results: In total, the control rate was 77% (20 patients), while six (23%) developed focally elevated keloids (ranging from 0.5 to 2 cm in length) in the middle of the primary abdominal scar. Five patients experienced mild hyperpigmentation. Nonetheless, most patients (96%) were satisfied with the treatment results. The estimated percentage of the applied radiation doses that reached the calculated depth of the ovaries ranged from 0.0033% to 0.0062%. Conclusion: When administered during the immediate postpartum period, postoperative electron beam radiotherapy for repeated Cesarean section scars is generally safe and produces good cosmetic results with minimal toxicity.
Purpose: This prospective cohort study was done to investigate recall bias to antepartum variables measured at postpartum periods and predictors of postpartum depression. Methods: Participants were 215 women who answered a self-administered questionnaire which included demographics, Postpartum Depression Predictors Inventory-Revised and Korean version of Edinburgh Postpartum Depression Scale at antepartum 36-40 weeks and postpartum 2 weeks and 6 weeks. Data were analyzed using kappa, and hierarchical multiple logistic regression. Results: Agreement between antepartum variables at both antepartum and two postpartum periods was relatively high (${\kappa}$=.55- .95). Postpartum depression rates were 36.3% and 36.7% at two follow-up points. In hierarchical multiple logistic regression analysis, prenatal depression (OR=4.32, 95% CI: 1.41-13.19; OR=5.19, 95% CI: 1.41-19.08), social support (OR=1.40, 95% CI: 1.18-1.66; OR=1.27, 95% CI: 1.06-1.53) and maternity blues (OR=4.75, 95% CI: 1.89-11.98; OR=4.22, 95% CI: 1.60-11.12) were commonly associated with postpartum depression at two follow-up points. Child care stress (OR=1.85, 95% CI: 1.01-3.37) was only associated with postpartum depression at 2 weeks postpartum and pregnancy intendedness (OR=1.57, 95% CI: 1.09-2.27) was only associated with postpartum depression at 6 weeks postpartum. Conclusions: The results indicate a need to apply nursing interventions such as prenatal education and counseling with families from antenatal period.
This study explored the relationships between Husband's emotional support and the mother's postpartum depression. The purpose of this study was to contribute to theoretical understanding of the relationships among these two variables and eventually to direct the planning of nursing care that enhance the health status of mothers in the postpartum period. Data were collected from January to June, 1995, with 47 postpartum Korean mothers in NewHampshire state in The United States of America. Postpartum depression was measured by Jung's Self-rating Depression Scale and husband's support was measured by Husband's support scale developed by Cho, Young Sook. Postpartum depression and husband's support were measured by postpartum mothers on the 3rd postpartum day and 2 weeks. The data were analysed by an SPSS computerized program and Percentages, t-test, paired t-test. ANOVA, Pearson correlation coefficients, and Stepwise multiple regression were used. The result of this study were summerized as follows ; 1. Mother's postpartum depression was greater at two weeks than the 3rd day (t=-2.64, p<.05). 2. Mother's percieved husband's supportive behavior was lower at 2 weeks than the 3rd day(t=-3.14,p<.01) 3. The higher the husband's supportive behavior, the lower the postpartum mother's depression (r=-2650,-2845;p<.01) 4. In this study, variables related to the mother's postpartum depression were husband's support, how much the husband liked the baby, occupation, and the knowledge of how to care for the baby. 5. In addition, by using Stepwise multiple regression analysis, it was determined that the main influencing factors on the postpartum depression. The variables - husband's support, how much the husband liked the baby and job made it possible to explain 48.61% of variance in postpartum depression. In conclusion, this study revealed that husband's support is an important factor that decrease the immigrant mother's postpartum depression.
Purpose: The purpose of this study was to investigate the effects of the professional nurse's postpartum breast-feeding support on breast-feeding prevalence for mothers who delivered in Baby-Friendly Hospitals (BFH). Methods: This quasi experimental study was designed with a nonequivalent control group post test. The subjects of this study were 55 mothers who were hospitalized in the delivery room of a university hospital which was selected as a BFH in Daegu from October 1, 2005 to June 30, 2006. Twenty nine mothers were assigned to the experimental group and 26 mothers to the control group. Postpartum breast-feeding support by nurses' telephone calls to the experimental group was provided once a week for 4 weeks postpartum and then once a month for 16 weeks postpartum. Four post tests were given at postpartum week 8, 12, and 16. The control group was given a telephone call at postpartum week 4, 8, 12, and 16. Results: The breast-feeding prevalence of the experimental group was significantly higher than that of the control group for each period. Conclusion: Postpartum breast-feeding support by nurses may be a useful intervention to increase breast-feeding prevalence.
The objective of this study was to determine cow characteristics and farm management factors during the dry period associated with early postparturient intramammary infection (IMI) and subsequent clinical mastitis (CM). Data were collected three times: before drying off (P1), during the dry period (P2), and 5 to 14 days after calving (P3), using questionnaires and farm investigation. Milk samples were aseptically collected for bacterial identification at P1 and P3. Factors associated with IMI and CM were analyzed using multiple logistic regression models. The final model showed that IMI in early postpartum was significantly associated with full insertion of dry cow antibiotic, dry cows in barns with a combination of tie and free stalls, body condition score (BCS) in dry period and after calving, and milk yield before drying off. For IMI cows, factors significantly associated with clinical expression of mastitis were having daily barn cleaning, teat disinfected with alcohol before administration of dry cow therapy, BCS before drying off, milk yield before drying off, and days in milk at drying off. In conclusion, both cow and farm management factors are associated with the IMI rate and subsequent expression of clinical signs of mastitis in early postpartum cows.
The purpose of this study was to assess for re-breeding concentrate period in postpartum in milking cows. The 48 cows aged $3.5{\sim}5.5$ years and of $400{\sim}600\;kg$ body weight were examined every 3rd day from 15 to 36 day postpartum. Blood samples for progesterone and estradiol $17{\beta}$ hormone analyses were withdrawn from the coccygeal vein every third day until the end of the experiment. The ovarian follicular numbers were verified and measured using a multi frequency probe. The least squares means are presented for each day by GLM of SAS. The results showed that ovary lengths (right ovary; $1.64{\pm}0.62\;cm$, left ovary; $1.44{\pm}0.46\;cm$) were similar in right and left ovary activity level during estrous cycle of postpartum cows. We were judged completed uterus on day at $2.31{\pm}0.17\;cm$ level of cervix diameter. And we were monitoring started at $6.44{\pm}2.03\;cm$ from day 15 after postpartum. The results showed that mean plasma concentration of progesterone (3.28 ng/ml) in large follicle gradually increased days 30 in postpartum. And, monitoring of estradiol 176 (22.18 pg/ml) hormone during postpartum period would be useful to predict the ovarian and uterus activity for re-breeding in postpartum milking cows. From these results, we conclude that cervix diameter (mean: 2.31 cm) was very important for reproductive organ recovery standard level of postpartum milking cows, hormone secretion level ($P_4$: 3.28 ng/ml, $E_2$: 22.18 pg/ml) and body condition score ($2.5{\sim}2.75$) level about 30 days in postpartum period.
The Purpose of this study was to build a substantive theory about the experience of postpartum depression. The qualitative research method used was rounded theory. The interviewees were eight others who had experienced postpartum depression. The data were collected through in-depth interviews with audiotape recording done by the investigator over a period of twelve months. he data were analyzed simultaneously by a constant comparative method in which new data were continuously coded into categories and properties according to Strauss and Corbin's methodology. Analysis the grounded data resulted in 28 concepts being identified. Eight categories emerged from the analysis. The categories were regret, loss of freedom, isolation of oneself, heartache, loss, emotional upset, avoidance, recovery. These substantive categories are consistent with precious research results. Causal conditions included : regret, loss of freedom. Phenomena : heartache, loss, emotional upset. Context : isolation oneself. Intervention condition : avoidance. Action/interaction strategies : desire for recovery. Consequences : recovery. These categories were synthesized into the core concept-The process of filling the empty loss of self. The process of the experienced postpartum depression was ① change after delivery, ② searching for a reason for depression, ③ effort to recover from postpartum depression, ④ recovery from postpartum depression and return to previous life. The process of recovery from postpartum depression was proceeded by ① support from others, especially husband, ② resolution of stressful life events, ③ reconstructing of life goals and resolution strategies, ④ acceptance of depression and seeking psychiatric treatment. Seven hypotheses were derived from the analysis. ① Mothers who experienced stressful life event and economic problem are more depressive. ② Mothers who have conflict with parents are more depressive. ③ The more somatic symptoms, the more depression. ④ Social support facilitates recovery from postpartum depression. ⑤ Mothers who have lower self-esteem are more depressive. ⑥ Mother's role overload disturbs recovery from postpartum depression. ⑦ Ideal maternal identity facilitates recovery from postpartum depression. Through this substantive theory, nurses can understand the importance of postpartum depression management.

The purpose of present study was to investigate the progesterone concentrations throughout gestation and peripartum period, and the return to the first estrus postpartum for improvement of reproductive efficiency in Korean native goats. The average length of gestation was 148 days(range : 144~154 days) and the average number of live births was 2 kids(range : 1~5 kids) in 12 Korean native goats. Progesterone concentrations were measured in blood samples taken from 12 goats every 5 days during gestation period. Plasma progesterone concentrations were 0.10 ng/ml at Day 0 of pregnancy and increased gradually until Day 20(6.58 ng/ml). Then they decreased slightly from Day 30 to 40(range : 4.32~4.82 ng/ml), increased again after Day 40 and remained thereafter until Day 140(range : 4.32~10.36 ng/ml). The progesterone levels declined sharply to basal levels at parturitum. Plasma progesterone concentrations during the pestpartum were 6.98 ng/ml at 10 days, 4.86 ng/ml at 6 days 3.18 ng/ml at 2 days before parturition, and 0.10 ng/ml at parturition, respectively. The basal levels were maintained until the first estrus postpartum. The mean intervals from parturition to the first estrus postpartum on the basis of progesterone determination and estrus detection were $100{\pm}64(mean{\pm}S.D.)$ days in 7 Korean native goats.
Purpose: For the importance of the postpartum period not only for the women delivered of child but also the newborn babies, the multitude of health professionals have emphasized increasingly special care during this period. However, there has been no decisional consensus both in the concepts and the ways of postpartum cares between health professionals and non-professionals. In this study, we explored the ethnical and racial diversities concerning postpartum period focusing on their beliefs and practices. Methods: Literature review was used as main research method for topic of the study. To carry out the method, papers related with postnatal care were searched at the Pubmed, the Korea Education & Research Information Service. Results: 1. Differences between Western and non-Western cultures in terms of postnatal care were found out in the study. 2. The study has proved that non-Western cultures have shared the similar beliefs and practices in postnatal care, although some ethnical and racial diversities specially in duration for care and diets for mother etc. 3. It was found that most prominent similarities among non-Western cultures in postnatal care have centered mainly on the issues such as 'Hot and Cold balance', postnatal confinement etc. Conclusion: Based on the results of the study, it should be suggested that the health professionals do their efforts in understanding deeply the intrinsic diversities on the beliefs and the practices about postnatal care and providing the adequate services and cares reflecting them properly.
Purpose: During postpartum period, many women complain multiple pain, sweating, cold hypersensitivity, abdominal pain, hot flush and so on. We called the postpartum disease as San Hu Pung. The San Hu Pung could related with dysautonomia. So we studied the character of the postpartum disease patients as heart rate variability(H.R.V) Methods: The subjects were 63 women who visited Kangnam Kyunghee hospital for postpartum management from February 2006 to June 2007. From history taking, 42 symptomatic women was divided at the postpartum disease group, 21 asymptomatic women was divided at the control group. All women was measured HRV, we studied the difference of HRV between two group. Results: Compared with the control group, SDNN of the disease group was significant low. TP, LF and HF of the disease group were significantly lower than the control group. Conclusion: The difference of HRV between the disease group and the control group was significant. The results of HRV on the disease group could mean that San hu pung is related dysautonomia. The more studies to diagnose San hu pung would be needed.
