The mediastinal lipoma is an extremely rare tumor in children. Since the orininal description by Fothergill in 1781, about 120 cases have been reported in the world literature. Of these, less than 10 have been reported in children below the age of 12. We had experienced a posterior mediastinal lipoma in 3 year old boy. He did not complain of any subjective symptoms. Routine X-ray film revealed a huge round homogenous mass density in the posterior mediastinum. On posterior thoracotomy incision, bright yellow, well encapsulated, partly 1obular adipose mass was found in the posterior mediastinum. The tumor mass was removed easily, being proved to be lipoma on histopathologic examination. His postoperative course was uneventful and discharged on the 12th postoperative day.
Recently we experienced a case of Takayasu`s arteritis involving the major aortic branches. A 30 year-old female patient admitted with the complaints of dizziness, visual disturbance, headache and tingling sensation of upper extremities. Aortogram revealed nearly complete obstruction of the origin site of both common carotid arteries and right vertebral artery, and irregular luminal narrowing of the origin site of innominate artery and left subclavian artery, but opacification of right subclavian artery and left vertebral artery. Successful surgical treatment was accomplished with a bypass from the ascending aorta to the left common carotid artery using a tube graft. The left subclavian artery and right axillary artery were revascularized distal to the stenosis with tube grafts that extended from the aortic graft. Postoperative complications were atelectasis, lymph leakage and left phrenic nerve palsy. She discharged uneventually at postoperative 22 days and most of symptoms were relieved.
Pulmonary aspergillosis is a rare disease, most commonly presenting pre-existing cavitary disease. In Department of thoracic and cardiovascular surgery, Chonbuk National University, 5 patients have been recognized as having this disorder from 1988 to 1990, June. Of the 5 patients, three were female and two were male. Age ranged from 30 to 53 years, the average age was 38.4 years. The most common presenting symptoms were blood-tinged sputum, hemoptysis, coughing, and chest pain Pulmonary tuberculosis occupied 8-% of underlying pulmonary disease. The locations of lesion were right upper lobe in 3 cases, left upper lobe in 1, and right lower lobe in l. All of these patients were treated by surgical resection. The operative procedures were as follows: lobectomy, 3 cases; segmentectomy, 1 case; lobectomy and segmentectomy, 1 case. There was no death in early and late postoperative period. Empyema and dead space developed in two cases, respectively. The postoperative empyema was treated with open thoracostomy and the dead space was carefully observed. During follow-up, there was no recurrence.
Sixteen patients with esophageal diverticulum operated on between July 1979 and September 1988 at the Department of Thoracic Surgery of National University Hospital have been studied. There were 2 cases of the pharyngoesophageal diverticula, 12 cases of the midesophageal diverticula, and 2 cases of the epiphrenic diverticula. Twelve cases of midesophageal diverticula consisted of 9 cases of pulsion type and 3 cases of traction type. There were 13 women and three men, whose ages ranged from 25 to 65 years with an average age of 45.5 years. Diverticulectomy alone in three cases, diverticulopexy with myotomy in two cases, and diverticulectomy with myotomy in 11 cases were performed. There were no deaths or morbidity and all patients have achieved marked improvement of their symptoms except four patients who had a concomitant hypopharyngeal carcinoma, had a postoperative recurrence in epiphrenic diverticulum, and had two cases of postoperative transient regurgitation which subsided spontaneously one and one and half year later.
Objectives: This study assessed the effect of a combination of Korean medicine and immunotherapy on three papillary thyroid cancer patients following thyroidectomy. Methods: The three patients who underwent thyroidectomy received Korean medical treatments, including acupuncture, moxibustion, pharmacopuncture, and immunotherapy. To evaluate the patients, symptoms were measured using the Numerical Rating Scale (NRS) and Karnofsky Performance Status Scale (KPS). Blood tests, including thyroid function tests, were conducted during treatment. Results: After treatment, postoperative pain and general weakness were gradually alleviated. Conclusions: These cases provide evidence that treatment with a combination of Korean medicine and immunotherapy can have substantial benefits for postoperative complications following thyroidectomy.
Objectives: The purpose of this study is to report the effect of Korean medicine on endometrial cancer patient after laparotomy. Methods: The patient with endometrial cancer who underwent Total Abdominal Hysterectomy (TAH), Bilateral Salpingo Oophorectomy (BSO), Bilateral Paraaortic Lymph Node Dissection (BPLND) was treated by Korean medicine such as herbal medicine, acupuncture, moxibustion. To evaluate the patient, symptoms were measured by Numeric Rating Scale (NRS) and Eastern Cooperative Oncology Group (ECOG). Blood tests including cancer biomarker were conducted during treatment. Results: After treatment, postoperative pain and general weakness were gradually relieved. Conclusions: This case provides us treatment with Korean medicine have substantial benefit on postoperative complications after laparotomy.

Cerebral ischemic complications after pituitary surgery are not frequently reported. Multiple mechanisms have been proposed, including vasospasm, and delayed cerebral ischemia resulting from postoperative subarachnoid bleeding. Given the unknown etiology of vasospasm following these situations, little is known about its prevention. Through a case report and bibliographic review, the authors warn about the importance of recognizing key signs postoperatively that could indicate increased risk for cerebral vasospasm and must be recognized in a timely manner, with appropriate treatment strategies implemented once these symptoms present.

Objectives: In the present study, the effects of sagittal split ramus osteotomy (SSRO) combined with intraoral vertical ramus osteotomy (IVRO) for the treatment of asymmetric mandible in class III malocclusion patients were assessed and the postoperative stability of the mandibular condyle and the symptoms of temporomandibular joint disorder (TMD) evaluated. Materials and Methods: A total of 82 patients who underwent orthognathic surgery for the treatment of facial asymmetry or mandibular asymmetry at the Department of Oral and Maxillofacial Surgery, Dong-A University Hospital, from 2016 to 2021 were selected. The patients that underwent SSRO with IVRO were assigned to Group I (n=8) and patients that received bilateral SSRO (BSSRO) to Group II (n=10, simple random sampling). Preoperative and postoperative three-dimensional computed tomography (CT) axial images obtained for each group were superimposed. The condylar position changes and degree of rotation on the superimposed images were measured, and the changes in condyle based on the amount of chin movement for each surgical method were statistically analyzed. Results: Group I showed a greater amount of postoperative chin movement. For the amount of mediolateral condylar displacement on the deviated side, Groups I and II showed an average lateral displacement of 0.07 mm and 1.62 mm, respectively, and statistically significantly correlated with the amount of chin movement (P=0.004). Most of the TMD symptoms in Group I patients who underwent SSRO with IVRO showed improvement. Conclusion: When a large amount of mandibular rotation is required to match the menton to the midline of the face, IVRO on the deviated side is considered a technique to prevent condylar torque. In the present study, worsening of TMD symptoms did not occur after orthognathic surgery in any of the 18 patients.

Objective : Laminoplasty is an effective surgical method for treating cervical degenerative disease. However, postoperative complications such as kyphosis, restriction of neck motion, and instability are often reported. Despite sufficient preoperative lordosis, this procedure often aggravates the lordotic curve of the cervical spine and straightens cervical alignment. Hence, it is important to examine preoperative risk factors associated with postoperative kyphotic alignment changes. Our study aimed to investigate preoperative radiologic parameters associated with kyphotic deformity post laminoplasty. Methods : We retrospectively reviewed the medical records of 49 patients who underwent open door laminoplasty for cervical spondylotic myelopathy (CSM) or ossification of the posterior longitudinal ligament (OPLL) at Pusan National University Yangsan Hospital between January 2011 and December 2015. Inclusion criteria were as follows : 1) preoperative diagnosis of OPLL or CSM, 2) no previous history of cervical spinal surgery, cervical trauma, tumor, or infection, 3) minimum of one-year follow-up post laminoplasty with proper radiologic examinations performed in outpatient clinics, and 4) cases showing C7 and T1 vertebral body in the preoperative cervical sagittal plane. The radiologic parameters examined included C2-C7 Cobb angles, T1 slope, C2-C7 sagittal vertical axis (SVA), range of motion (ROM) from C2-C7, segmental instability, and T2 signal change observed on magnetic resonance imaging (MRI). Clinical factors examined included preoperative modified Japanese Orthopedic Association scores, disease classification, duration of symptoms, and the range of operation levels. Results : Mean preoperative sagittal alignment was $13.01^{\circ}$ lordotic; $6.94^{\circ}$ lordotic postoperatively. Percentage of postoperative kyphosis was 80%. Patients were subdivided into two groups according to postoperative Cobb angle change; a control group (n=22) and kyphotic group (n=27). The kyphotic group consisted of patients with more than $5^{\circ}$ kyphotic angle change postoperatively. There were no differences in age, sex, C2-C7 Cobb angle, T1 slope, C2-C7 SVA, ROM from C2-C7, segmental instability, or T2 signal change. Multiple regression analysis revealed T1 slope had a strong relationship with postoperative cervical kyphosis. Likewise, correlation analysis revealed there was a statistical significance between T1 slope and postoperative Cobb angle change (p=0.035), and that there was a statistically significant relationship between T1 slope and C2-C7 SVA (p=0.001). Patients with higher preoperative T1 slope demonstrated loss of lordotic curvature postoperatively. Conclusion : Laminoplasty has a high probability of aggravating sagittal balance of the cervical spine. T1 slope is a good predictor of postoperative kyphotic changes of the cervical spine. Similarly, T1 slope is strongly correlated with C2-C7 SVA.
Myxoma is a benign growth constituting nearly 50% of all primary cardiac tumors. It is important because it can be abolished by surgical therapy and is usually fatal if unrecognized and untreated. Recently a wider use of echocardiogram as the screening test of valve lesions can be expected to increase the number of myxomas found preoperatively. We report a case of left atrial myxoma which was diagnosed by echocardiogram before surgery, and was successfully removed with the aid of extracorporeal circulation. The patient was a 24-year old woman who had suffered from mitral valvular symptoms for 2 months before admission. At operation, a tumor, measuring 5.2 x 4.3 x 4.7 cm, was extremely friable, villous gelatinous mass and it was removed from its origin near the closed fossa ovalis, including its stalk and a portion of the septum. The postoperative course was uneventful. The patient is clinically well and without symptoms of heart disease.
