Long-term outcomes after salvage radiotherapy for postoperative locoregionally recurrent non-small-cell lung cancer

  • Purpose: The outcomes and toxicities of locoregionally recurrent non-small-cell lung cancer (NSCLC) patients treated with curative radiotherapy were evaluated in the modern era. Materials and Methods: Fifty-seven patients receiving radical radiotherapy for locoregionally recurrent NSCLC without distant metastasis after surgery from 2004 to 2014 were reviewed. Forty-two patients were treated with concurrent chemoradiotherapy (CCRT), and 15 patients with radiotherapy alone. The median radiation dose was 66 Gy (range, 45 to 70 Gy). Lung function change after radiotherapy was evaluated by comparing pulmonary function tests before and at 1, 6, and 12 months after radiotherapy. Results: Median follow-up was 53.6 months (range, 12.0 to 107.5 months) among the survivors. The median overall survival (OS) and progression-free survival (PFS) were 54.8 months (range, 3.0 to 116.9 months) and 12.2 months (range, 0.8 to 100.2 months), respectively. Multivariate analyses revealed that single locoregional recurrence focus and use of concurrent chemotherapy were significant prognostic factors for OS (p = 0.048 and p = 0.001, respectively) and PFS (p = 0.002 and p = 0.026, respectively). There was no significant change in predicted forced expiratory volume in one second after radiotherapy. Although diffusing lung capacity for carbon monoxide decreased significantly at 1 month after radiotherapy (p < 0.001), it recovered to pretreatment levels within 12 months. Acute grade 3 radiation pneumonitis and esophagitis were observed in 3 and 2 patients, respectively. There was no chronic complication observed in all patients. Conclusion: Salvage radiotherapy showed good survival outcomes without severe complications in postoperative locoregionally recurrent NSCLC patients. A single locoregional recurrent focus and the use of CCRT chemotherapy were associated with improved survival. CCRT should be considered as a salvage treatment in patients with good prognostic factors.

Isolated Unilateral Absence of Pulmonary Artery Associated with Contralateral Lung Cancer

  • Unilateral absence of a pulmonary artery (UAPA) is a rare congenital anomaly that may present with various symptoms, depending on the nature and severity of other cardiovascular anomalies. Furthermore, contralateral lung surgery in patients with UAPA is extremely rare, and clinical experience is limited. This report describes a case of surgical treatment of contralateral primary lung cancer in a patient with isolated UAPA. A 56-year-old man was diagnosed with primary lung cancer accompanied by isolated UAPA on the contralateral side. He underwent meticulous cardiorespiratory function tests preoperatively. We performed a right lower lobectomy. Although in the immediate postoperative period, the patient suffered from a mild decline in his respiratory function, he recovered uneventfully. The present case shows that preoperative awareness of UAPA and meticulous perioperative management enable contralateral lung surgery to be performed safely.

Thoracic Epidural Anesthesia and Analgesia (TEA) in Patients with Rib Fractures

  • Background: We analyzed the results of surgical reduction and fixation of ribs under thoracic epidural anesthesia and analgesia (TEA) in patients who had no more than 3 consecutive rib fractures with severe displacement to examine the clinical usefulness of this method. Materials and Methods: From May 2008 to March 2010, 35 patients underwent surgical reduction and fixation of ribs under TEA. We reviewed the indications for this technique, number of fixed ribs, combined surgical procedures for thoracic trauma, intraoperative cardiopulmonary events, postoperative complications, reestablishment of enteral nutrition, and ambulation. Results: The indications of TEA were malunion or nonunion of fractured ribs in 29 (82.9%; first operation) and incompletely ribs under previous general anesthesia in 6 (17.1%; second operation). The average number of fixed ribs per patient was 1.7 (range: 1~3). As a combined operation for thoracic trauma, 17 patients (48.6%) underwent removal of intrathoracic hematomas, and we performed repair of lung parenchyma (2), wedge resection of lung (1) for accompanying lung injury and pericardiostomy (1) for delayed hemopericardium. No patient had any intraoperative cardiopulmonary event nor did any need to switch to general anesthesia. We experienced 3 postoperative complications (8.6%): 2 extrapleural hematomas that spontaneously resolved without treatment and 1 wound infection treated with secondary closure of the wound. All patients reestablished oral feeding immediately after awakening and resumed walking ambulation the day after operation. Conclusion: Thoracic epidural anesthesia and analgesia (TEA) may positively affect cardiopulmonary function in the perioperative period. Moreover, this technique leads to an earlier return of gastrointestinal function and early ambulation without severe postoperative complications, resulting in a shortened hospital stay and lowered costs.

  • Pulmonary lymphangioleiomyomatosis is a chronic destruct8ive disease of the lung affecting women of childbearing ages which eventually leads to respiratory failure. Lung transplantation is the only conclusive therapeutic measure because this disease responds poorly to other therapies, To date only a few reports in the literature describes the clinical experience of the bilateral sequential lung transplantation of this rare condition. We performed a bilateral sequential lung transplantation on a 32-year-old woman suffering from lymphangioleiomyo-matosisw. The heart-lung block was harvested from a 51-year-old donor. We transplanted the left lung first through the clam-shell incision. As the hemodynamics deteriorated suddenly during the dissection of the right lung the right lung was transplanted under the cardio-pulmonary bypass. Although the patient's lung function was initially satisfactory the patient died of sepsis and subsequent cardiogenic shock at the postoperative 18th day. Autopsy findings showed infection of Candida albicans on the pericardium and the left lung which had been initiated possibly from the left bronchial anastomosis site,. Through detailed review of the clinical course we concluded that lung transplantation could have been performed safely on this disease provided that early diagnosis and proper management or the oppor-tunistic infection have been carried out.

일측폐 적출술후의 폐기능의 평가 (Evaluation of Pulmonary Function after Pneumonectomy)

  • Studies of pulmonary function using spirometry were performed before and after pneumonectomy for inflammatory lung diseases from 1985 to 1990 at the Pusan Paik Hospital, Inje Medical College. Fifty-two patients were evaluated ; 33 tuberculosis, 17 bronchiectasis, 2 abscess, and 1 actinomycosis. All patients had preoperative and postoperative FVC, FVC[% predicted], FEV1, %FEV1, MVV and MVV[%predicted] determinations. And above datas were compared each other statistically with applying of the paired t-test. The results were obtained as follows : there were significant decreased after surgery in the values of FVC, FVC[% predicted], MVV, and MVV[% predicted], but the values of FEV1, and %FEV1 were no significant changes after surgery.

Sleeve 폐엽절제술 후 폐기능 보존에 대한 연구 (Preservation of Pulmonary Function after Sleeve Lobectomy in Patients with Lung Cancer)

  • 연구배경: 비소세포성 폐암 수술시 sleeve 폐엽절제술은 가능한한 폐실질을 보존하기 위한 수술방법으로, 폐기능이 감소되어 있는 환자에서도 폐절제술을 시행할 수 있다. 그러나 sleeve 폐엽절제술 후 폐기능이 기대했던 것 만큼 보존되는지에 대한 국내의 보고가 없어 sleeve 폐엽절제술 후 폐기능의 보존정도에 대해 연구 하였다. 방법: 1995년부터 1998년까지 삼성서울병원에서 sleeve 폐엽절제술을 받은 폐암 환자중 수술 후 3개월 이후에 기관지내시경 검사, 폐기능검사를 시행받은 15명의 환자를 대상으로 수술 전 예상 $FEV_1$과 수술 후 실제 $FEV_1$과의 차이를 비교 분석하였다. 수술 전 예상 $FEV_1$은 수술 전 $FEV_1$에 수술 후 잔류폐엽의 폐관류 정도(%)를 곱한 값으로 계산하였다. 결과: 15명 환자의 연령의 중앙값은 58세(44세-70세)였고, 남녀비는 14:1이었다. 모두 비소세포성폐암이었으며 상피세포암이 13명, 선암이 2 명이었다. 우측 상엽 sleeve 폐업절제술이 8명, 우측 중엽과 하엽을 동시에 제거하는 양엽 sleeve 폐엽절제술이 3명, 좌측 상엽 sleeve 폐엽절제술이 4명이었다. 환자들의 수술 전 평균 $FEV_1$$2417{\pm}694mL$ 였고, sleeve 폐엽절제술을 받을 경우 예상 $FEV_1$$2180{\pm}570mL$ 였다. Sleeve 폐엽절제술 후 실제 $FEV_1$$2293{\pm}499mL$로 수술 전 예상 $FEV_1$과 의미있는 상관관계을 보여 (r = 0.67, P<0.05), 전폐절제술을 시행했을 때보다 폐기능을 보존할 수 있었다. 각 환자의 수술 전 $FEV_1$과 수술 후 실제 $FEV_1$을 비교해 보았을 때 수술 전 $FEV_1$이 적었던 (<2L) 4명의 환자에서는 오히려 수술 전보다 수술 후 $FEV_1$이 증가하였다. 결론: 폐암의 수술에 있어 sleeve 폐업절제술은 예상했던 것 만큼 폐기능이 잘 보존되었고, 특히 수술 전 폐기능이 저하되어 있는 환자에서 유용한 수술수기로 이용될 수 있을 것으로 생각 되었다.

개심술후 폐기능 -수술직후 및 장기간의 추이에 대하여- (Pulmonary Function Following Open Heart Surgery -early and late postoperative changes-)

  • Twenty-two patients were selected for evaluation of pre-and postoperative pulmonary function. These patients were performed open cardiac surgery with the extracorporeal circulation from March 1979 to July 1980 at the Department of Thoracic and Cardiovascular Surgery, Kyungbook National University Hospital. Patients were classified with ventricular septal defect 5 cases, atrial septal defect 5 cases, tetralogy of Fallot 5 cases, mitral stenosis 4 cases, rupture of aneurysm of sinus Valsalva 1 case, left atrial myxoma I case, and aortic insufficiency 1 case. The pulmonary function tests were performed and listed: [1] respiratory rate, tidal volume [TV], and minute volume[MV], [2] forced vital capacity [FVC] and forced expiratory volume[FEV 0.5 & FEV 1.0], [3] forced expiratory flow [FEF 200-1200 ml & FEF 25-75%]. [4] Maximal voluntary ventilation [MVV], [5] residual volume [RV] and functional residual capacity[FRC], measured by a helium dilution technique. Respiratory rate increased during the early postoperative days and tidal volume decreased significantly. These values returned to the preoperative levels after postoperative 5-6 days. Minute volume decreased slightly, but essentially unchanged. Preoperative mean values of the forced vital capacity, functional residual capacity and total lung capacity decreased [63.2%, 87.2% & 77.3% predicted, respectively], and early postoperatively these values decreased further [19.6%, 76.0% & 38.0% predicted], but later progressively increased to the preoperative levels. In residual volume, there was no decline in the preoperative mean values [100.9% predicted] and postoperatively the value rather increased [106.3-161.7% predicted]. Forced expiratory volume [FEV 0.5 & FEV 1.0] and forced expiratory flow [FEF 200-1200 ml & FEF 25-75%] also revealed significant declines in the early postoperative period. There was no significant difference in values of the spirometric pulmonary function tests, such as FEF 1.O and FEF 25-75% between successful weaning group [17 cases] extubated within 24 hrs post-operatively and unsuccessful weaning group [5 cases] extubated beyond 24 hrs. Static compliance and airway resistance measured for the two cases during assisted ventilation, however, any information was not obtained. Long term follow-up pulmonary function studies were carried out for 8 cases in 9 months post-operatively. All of the results returned to the pre-operative or to normal predicted levels except FVC, FEV 1.0, and FEF 25-75% those showed minimal declines compared to the pre-operative figures.

Factors Associated with Lung Function Recovery at the First Year after Lung Transplantation

  • Purpose: Post-operative pulmonary function is an important prognostic factor for lung transplantation. The purpose of this study was to identify factors affecting recovery of forced expiratory volume in 1 second (FEV1) at the first year after lung transplantation. Materials and Methods: We retrospectively reviewed the medical records of lung transplantation patients between October 2012 and June 2016. Patients who survived for longer than one year and who underwent pulmonary function test at the first year of lung transplantation were enrolled. Patients were divided into two groups according to whether they recovered to a normal range of FEV1 (FEV1 ${\geq}80%$ of predicted value vs. <80%). We compared the two groups and analyzed factors associated with lung function recovery. Results: Fifty-eight patients were enrolled in this study: 28 patients (48%) recovered to a FEV1 ${\geq}80%$ of the predicted value, whereas 30 patients (52%) did not. Younger recipients [odds ratio (OR), 0.92; 95% confidence interval (CI), 0.87-0.98; p=0.010], longer duration of mechanical ventilator use after surgery (OR, 1.14; 95% CI, 1.03-1.26; p=0.015), and high-grade primary graft dysfunction (OR, 8.08; 95% CI, 1.67-39.18; p=0.009) were identified as independent risk factors associated with a lack of full recovery of lung function at 1 year after lung transplantation. Conclusion: Immediate postoperative status may be associated with recovery of lung function after lung transplantation.

전폐절제술의 임상적 연구 (Clinical Evaluation of Pneumonectomy)

  • 전북대학교병원 흉부외과학 교실에서는 1979년 8월부터 1995년 8월까지 73례의 전폐절제술을 시행하였으며, 그 중 폐암이 53례, 폐결핵이 10례, 기관지 확장증이 4례, 기타가 6례 이었다. 73례에서 수술사 망율 및 합볏증을 조사하고 술후 6개월에 53명의 환자에서 폐기능 검사를 시행하여 이에 영향을미치는 인자(나이:성별, 병리소견(양성 또는 악성), 동반질환, 술전 폐기능 검사, 수술시간 등)를 통계학적으로 비교 분석하였다. 수술 사망율에 대해서는 술전 최대자발호기량(MW)의 예측치에 대한 백분율(P=0.013)과 수술시간 (P=0.009)이, 술후 합병증에 대해서는 염증성 질환(P=0.015)이, 그리고 술후 6개월후에 시행한 폐기능 검사치에 대해서는 술전 노력성 폐활량의 예측치에 대한 백분율(FVC(%. prod), p=0.0018),술전 1초간 강제 호기량의 예측치에 대한 백분율(FEVI 3%, prod), p=0.0024),술전 최대 자발성 호기량의 예측치에 대한 백분율(MW 3%. prod), p=0.0043) 등이 통계학적으로 의의 있게 나타났다. 결론적으로수술후의 사망율과 합병증을 줄이고 환자의 원활한 활동능력을 얻기 위해 술전의 환자의 전신상태,심혈관계 및 술전의 폐기능과술후의 保맨\ulcorner폐기능에 대한충분한 평가가 있어야 하며, 염증성 질환이 있는 환자는 염증및 분비물의 성상및 정도를 정확하게 평가하여야 할 것으로 사료된다.

허파암 수술 전과 후에 적용한 운동의 효과: PubMed 내 연구에 대한 체계적 문헌고찰 (Effects of Exercise-Based Intervention Before and After Lung Cancer Surgery: A systematic review in pubmed database)

  • Purpose : Lung cancer induces a decrease in physical activity and a deterioration of respiratory ability. Exercise is an effective treatment to reduce side effects of anti-cancer treatments, also influence the survival and successful rehabilitation in lung cancer patients. However, there is insufficient evidence to show which period is the most effective to apply exercise for lung cancer patients. Therefore, this study was conducted to evaluate the efficacy of exercise-based interventions before and after surgery. Methods : Clinical trials (CTs) and randomized controlled trials (RCTs) reported in PubMed database were investigated. The trials investigated in this study were published for 10 years before August 20, 2022. The risk of bias was judged according to the Cochrane guideline. The materials included in this meta-analysis were 6-minute walk test (6MWT), pulmonary function, and quality of life (QOL). Results : 1 CT and 9 RCTs were selected in current study. In the meta-analysis, exercise increased 6MWT in preoperation (mean difference [MD] 29.49; 95 % confidence interval [CI] .99 to 57.99; p=.04; I2=0 %), 3 months postoperation (MD 54.97; 95 % CI 31.85 to 78.09; p<.001; I2=45 %) and 6 months postoperation (MD 85.59; 95 % CI 45.06 to 126.12; p<.001; I2=47 %). Exercise, also enhanced the lung function such as FEV1/FVC (%) in postoperation (MD 7.64; 95 % CI 6.26 to 9.02; p<.001; I2=19 %). Additionally, exercise improved QOL, such as preoperative EORTC-QLQ-C30-LC13 in mental function (MD 3.21; 95 % CI .64 to 5.79; p=.01; I2=0 %) and postoperative SF-36 in mental component summary (MD 9.24; 95 % CI 4.94 to 13.54; p<.001; I2=0%). Conclusion : These results indicate that exercise-based intervention can elevate the ability to exercise and the mental componentof QOL within 3 months.
