Acupuncture appears to be a clinically effective treatment for acute and chronic pain. A considerable amount of research has been conducted to evaluate the role that acupuncture plays in pain suppression; however, few studies have been conducted to evaluate the side effects of the acupuncture procedure. This case report describes a suspected postdural puncture headache following acupuncture for lower back pain. Considering the high opening pressure, cerebrospinal fluid leakage, and the patient's history of acupuncture in the lower back area, our diagnosis was iatrogenic postdural puncture headache. Full relief of the headache was achieved after administration of an epidural blood patch.

목 적 : 수술용 장갑의 분말이 아이들에서 요추천자 후 요통의 원인인자가 될 수 있는지에 대한 조사를 하고자 하였다. 방 법 : 본 병원 소아과에서는 1997년 이전에는 수술용 장갑에 대한 특별한 처치 없이 요추천자를 시행하였으며, 1998년 이후부터 분말을 제거하는 방식으로 요추천자를 시행하였기 때문에 1997년까지와 1998년 이후로 자연적인 대조군이 이루어 졌다. 뇌수막염의 대유행이 있었던 1997년 7월부터 9월까지와 2001년 5월부터 10월까지 마산삼성병원 소아과에서 뇌수막염을 진단 받고 입원한 3세에서 14세 사이의 환아들을 대상으로 요추천자 후 요통의 비율을 각각 조사하였다. 결 과 : 수술용 장갑에 분말을 제거하지 않은 1997년에는 164례 중 41례(25.00%)에서 요추천자 후 요통이 발생한 반면 분말을 제거한 2001년에는 149례 중 8례(5.36%)에서만 요통이 발생하였다(P<0.001). 결 론 : 수술용 장갑의 분말이 아이들에서 발생하는 요추천자 후 요통의 주요한 원인임을 짐작할 수 있었고 앞으로 요추천자시 수술용 장갑의 분말을 제거한 후 시술을 시행해야 할 것으로 사료된다.

We performed the autologous epidural blood patch (AEBP) for the relief of headache and other related symptoms following dural punctures of the lumbar region during 8 years from 1981 to 1988. The total of 37 patients with the patching consisted of 9 cases in 3007 of spinal anesthesia, 12 accidental dural punctures in 4283 cases of lumbar epidural anesthesia, 12 cases in 4747 of epidural analgesia for back pain control, 3 cases of myelography and a case of diagnostic lumbar puncture. The headaches were relieved completely in 35 cases following the first AEBP and the remaining two were also relieved following the second AEBP. We think that the AEBP for postdural-puncture headache is the treatment of choice.
