The Efficacy of Three-Dimensional Sweeping Mode Extracorporeal Shockwave Treatment for Plantar Fasciitis (3차원 동적집속모드 체외충격파 기기를 이용한 족저근막염 치료의 유용성)

  • Purpose: This was a pilot study to examine the clinical usefulness of the newly developed three-dimensional sweep mode extracorporeal shockwave treatment (ESWT) in patients with plantar fasciitis. Materials and Methods: Three-dimensional sweep mode ESWT was performed once a week for 5 weeks in patients with plantar fasciitis who showed no improvement with the conventional conservative treatment. A 100-mm visual analogue scale (VAS) reading for pain from walking and at rest after walking were collected before the treatment and 8 and 16 weeks after the initial treatment. In addition, the Foot and Ankle Outcome Score (FAOS) and EuroQol-5-dimension (EQ-5D) scores before and 16 weeks after the treatment were evaluated. Results: VAS for pain for walking improved from 50.60±8.38 to 19.80±15.61 at 8 weeks after the initial treatment (p=0.008) and 9.80±9.62 at 16 weeks after the treatment (p<0.001). VAS for pain at rest after walking improved from 36.60±19.55 to 11.80±12.95 at 8 weeks after the initial treatment (p=0.052) and 8.80±8.87 at 16 weeks after the treatment (p=0.024). Preoperative FAOS increased from an average of 74.80±9.73 before the treatment to an average of 81.00±8.86 at week 16 after the procedure (p=0.49) and compared to pre-treatment levels, there was a decrease of one level in the anxiety/depression domain of the EQ-5D, post-treatment. Conclusion: The results of this preliminary study confirmed that the newly developed EWST with the smart forging sweep mode was effective in improving pain and function in plantar fasciitis.

Effects of Extracorporeal Shock Wave Therapy in Pain Point on Range of Motion, Pain and Mechanical Muscle Properties in Myofascial Pain Syndrome (근막통증증후군 환자에게 통증점 체외충격파 치료가 경추의 가동 범위, 통증, 근육의 기계적 특성에 미치는 영향)

  • Background: To evaluate whether extracorporeal shock wave therapy (ESWT) in the pain point is a more effective treatment than the trigger point for myofascial pain syndrome (MPS) of the upper trapezius. Objects: The purpose of this study was to compare the most effective areas when applying extracorporeal shock wave therapy. Methods: A total of 30 patients with MPS were randomly assigned to the trigger point in the ESWT (n = 15) and pain point ESWT (n = 15) groups. Interventions in both groups were performed in one session, i.e., 2,000 shocks with 1.5 bar intensity. Pain and function were assessed using the visual analog scale (VAS) and cervical range of motion (ROM) and based on mechanical muscle properties. Statistical analysis was performed using the repeated measures two-way analysis of variance to determine the significance probability between pre- and post-test. Results: Changes in mechanical muscle properties were not statistically significant between the two groups. However, VAS and cervical ROM showed statistically significant differences at pre- and post-intervention, regardless of the group (p < 0.05). Conclusion: Although no significant difference was observed in the intervention effect, applying an extracorporeal shock wave to the pain point rather than the pain trigger point should be considered in order to save time in effectively and accurately identifying the pain trigger point and site.

A Clinical Study of Bee Venom Acupuncture Therapy on Shoulder Pain Patients in Stroke Sequelae (중풍후견증(中風後遣症)으로 인한 견비통(肩譬痛) 환자(患者)의 봉약침치료(蜂藥鍼治療)에 대한 임상적(臨床的) 고찰(考察))

  • Objectives : The occurrence of shoulder pain after attack of stroke varies from 15% to about 80% in patients. Hemiplegic shoulder pain has been shown to affect stroke outcome in a negative way that it interferes with recovery after a stroke. These following processes have been all postulated as causes of a shoulder pain: glenohumeral subluxation, spasticity, impingement, soft tissue trauma, glenohumeral capsulitis, shoulder hand syndrome. And stroke patients may suffer from pain caused by stroke itself(central post-stroke pain). The aim of this study is to investigate the effectiveness of Bee venom therapy for shoulder pain in stroke patients. Methods : To evaluate the effectiveness of Bee Venom Acupuncture Therapy, 40 patients were allocated into control and treatment group. They were monitored for 3 weeks and followed up with VAS score(with the interval of Initial(YAS1)), 1 week later(VAS2), 2 weeks later(VAS3), 3 weeks later(VAS4), Motor Grade and Passive ROM. Results : VAS score decrease in treatment group compared to control group. Bee Venom Acupuncture Therapy seems to decrease hemiplegic shoulder pain and this effect was statistically significant after 3 weeks. Therefore this therapy could be recommended for the treatment of patients with shoulder pain after stroke and further extensive clinical studies are expected. Conclusion : We suggest that GDS oral administration and electro-acupuncture at $BL_{52}$ & $GB_{39}$ are available for prevention and curing about the postmenopausal osteoporosis.

  • Even after testing negative for COVID-19, some patients continue to struggle with a variety of symptoms such as fatigue, shortness of breath, gastrointestinal problems and neurological problems. The World Health Organization (WHO) defined long COVID (Post COVID-19 conditions) as "A disease occurs in individuals with a history of probable or confirmed SARS-CoV-2 infection, usually 3 months from the onset of COVID-19 with symptoms that last for at least 2 months, that cannot be explained by an alternative diagnosis." As a possible pathological mechanism of long COVID, three hypotheses are proposed: the persistence of the infectious state due to the residual virus, the persistent inflammatory response, and the autoimmune response. The main symptoms of long COVID are shortness of breath (dyspnea), abdominal pain and dyspepsia, fatigue, cognitive problems (brain fog), anosmia and dysgeusia, and chest pain, palpitations and tachycardia. In the Chinese guidelines, COVID-19 patients were divided into mild, moderate, severe, and recovery, and prescriptions with effective therapeutic effects were summarized to encourage combined treatment of chinese and western medicine. Globally, only symptomatic therapy is recommended for long COVID, but a specific treatment has not yet been proposed. Recently, morbidity code for post COVID-19 conditions was created, and it is planned to announce guidelines for long COVID treatment and management in the first half of 2023. In line with this trend, the Korean medical community needs to make efforts to prepare treatment guidelines for patients with long COVID.

Clinical Analysis of Anterior Lumbar Interbody Fusion for Failed Back Surgery Syndrome (Failed Back Surgery Syndrome에서 전방 요추체간 유합술의 치료성적분석)

  • Objective : To evaluate the role of anterior lumbar interbody fusion in treatment of failed back surgery syndrome, the authors retrospectively analyzed the result of anterior lumbar interbody fusion performed in our institute. Methods : Fifteen FBSS patients due to variable causes have been treated with anterior lumbar interbody fusion in our institute from April 1994 to June 1999. We analyzed clinical changes in 15 patients who were followed up for an average of 23 months. Results : The etiologies of FBSS were post operative discitis(6 cases), post operative instability(3 cases), post operative adhesion(5 cases), and recurrence(1 case). These fifteen FBSS patients were treated with anterior lumbar interbody fusion. The overall treatment outcome was satisfactory(excellent and good) in 11 cases. Three patients were slightly improved, but post operative low back pain was remained. One patient who had underwent nerve root injury due to pedicle screw insertion showed no improvement. Conclusion : We conclude that the anterior lumbar interbody fusion for FBSS seems to be safe and favorable treatment in selective patients, because low incidence of nerve injury risk and post-operative infection.

The Effects of Moxibustion on Chronic Back Pain and Activities of Daily Living in Aged (뜸 요법이 노인의 만성 요통과 일상생활 활동장애에 미치는 효과)

  • Purpose: This study was attempted in order to verify the effects of moxibustion on aged back pain and activities of daily living. Methods: A research design is quasi-experiment with pre-post test design in nonequivalent control group. As for data collection and experimental treatment, it accidentally sampled totally 50 people with 24 people for the experimental group and 26 people for the control group who appeal for chronic back pain aged over 60 who visited 4 senior centers where are located in N city from November 7, 2012 to December 3, 2012. Experimental treatment was carried out moxibustion totally 12 times by 3 times for 4 weeks in the experimental group. Data analysis was made by using SPSS program. As an analytical technique, the experimental group and the control group were analyzed the homogeneity verification with t-test, Fisher's exact test and $x^2$ test and the hypothesis verification with t-test. Results: The back pain level was reduced in the experimental group than the control group. The disability of daily living activities was reduced in the experimental group than in the control group. Conclusion: Moxibustion was considered to be likely useful therapeutic method for effects of chronic back pain and disability of daily living activities in aged.

The effect of Myofascial Release and Infrared on the Range of Motion and Pain in Persons with Neck Pain (적외선치료와 근막이완술이 경부통증환자의 경추가동범위와 통증에 미치는 영향)

  • Purpose : The purpose of this study was to evaluate the Influence of Infra red(IR) and myofascial release(MRF) on the range of motion and pain in persons with neck pain. Methods : 24 subjects with neck pain participated in the experiment. All subject randomly assigned to the IR group and MFR group. Both groups receive 10minutes, 3 times per week during 3 weeks period. laser exercise(LEX) used to measure range of motion of neck and visual analog scale(VAS) used to measure pain. All measurements of each subject were measured at pre-treatment and post-treatment. Results : 1. The neck flexion, extension, right-sidebending, left-rotation, right-rotation range of motion of MFR group was significantly increased.(P<0.05). 2. The neck left-sidebending, left-rotation, right-rotation range of motion of IR group was significantly increased.(P<0.05). 3. As to compare two group, MFR group increases ROM more than IR group. Conclusion : These data suggests that MFR is more beneficial than IR.

Extracorporeal Shock Wave Therapy for Myofascial Pain Syndrome of Upper Trapezius (충격파치료를 이용한 상부 등세모근의 근막통증증후군 치료)

  • Purpose : The purpose of this study was to evaluate whether extracorporeal shock wave therapy (ESWT) is an effective treatment for myofascial pain syndrome (MPS) of upper trapezius. Methods : Study subject were 20 patients (7 male and 13 female). The patients were evaluated by assessing of pain and function using visual analog scale (VAS) and pressure pain threshold (PPT) and Constant-Murley scale (CMS). Statistical analysis was used paired T-test to know significance probability between pre-test and post-test. Results : There was no statistical significance in the change in PPT. But, VAS scores were significantly improved after ESWT (p<.05). Using the Constant-Murley score, this study showed a significant decrease in pain and a significant increase in range of motion of shoulder (p<.05). Conclusion : These results indicate that extracorporeal shock waves therapy could be considered as an effective and efficient treatment modality for myofascial pain syndrome of upper trapezius.

The Effect of Intrapleural Injection of Bupivacaine for Pain Relief Following Thoracotomy (늑막강내 Bupivacaine의 투여가 개흉술후 동통 감소에 미치는 영향)

  • An approach to the treatment of post-operative pain by the injection of bupivacaine into the pleural space through an intrapleural cathter has been studied. Among 24 thoracotomy patients, bupivacaine was injected only to experimental group[ 12 patients ] when the patient was able to head up for oneself during recovery from anesthesia. The pain and ROM[ range of motion ] scores, respiration rate, PaCO2 level of both experimental and control group were measured at the time of head-up and 30 and 120 minutes thereafter.The scores of pain and ROM of experimental group were significantly[ P value < 0.05 ] decreased in 30 minutes and 120 minutes after bupivacaine injection compared with those of control group but respiration rate and PaCO2 level were not changed significantly. With this result, we can suggest that intrapleural injection of bupivacaine is useful for pain relief following thoracotomy.

  • Background: This study clinically evaluated the effect of botulinum toxin type A (BTX-A) in the temporomandibular disorder (TMD) treatment using Research Diagnostic Criteria for Temporomandibular Disorders (RDC/TMD). Methods: A total of 21 TMD patients were recruited to be treated with BTX-A injections on the bilateral masseter and temporalis muscles and were followed up by an oral and maxillofacial surgeon highly experienced in the TMD treatment. For each patient, diagnostic data gathering were conducted according to the RDC/TMD. Characteristic pain intensity, disability points, chronic pain grade, depression index, and grade of nonspecific physical symptoms were evaluated. Wilcoxon signed-rank test was applied for statistical analysis. Results: The results showed that more than half of the participants (85.7 %) had parafunctional oral habits such as bruxism or clenching. In comparison between pre- and post-treatment results, graded pain score, characteristic pain intensity, disability points, chronic pain grade, and grade of nonspecific physical symptoms showed statistically significant differences after the BTX-A injection therapy (p < 0.05). Most patients experienced collective decrease in clinical manifestations of TMD including pain relief and improved masticatory functions after the treatment. Conclusions: Within the limitation of our study, BTX-A injections in masticatory musculatures of TMD patients could be considered as a useful option for controlling complex TMD and helping its associated symptoms.
