코로나 19 후 폐섬유화(Post COVID-19 pulmonary fibrosis)에 대한 복합 한의치험 1례 (Korean Medicine Treatment for a Patient with Post-COVID-19 Pulmonary Fibrosis: A Case Report)

  • Background: Post-COVID-19 pulmonary fibrosis (PCPF) is a common complication in severe COVID-19 cases, often associated with acute respiratory distress syndrome or mechanical ventilation. Patients with PCPF frequently experience a decline in their quality of life due to persistent COVID-19 sequelae, including cough and chest pain. However, there is currently no established standard treatment, and the efficacy of existing medications remains uncertain. Case Report: A 65-year-old female patient presenting with cough, dyspnea, chest pain, and fatigue due to PCPF received Korean medicine treatment for 25 days. Symptom evaluation utilized the modified Medical Research Council scale, the Leicester Cough Questionnaire, and the Numeral Rating Scale. Quality of life and functional status were assessed using the Post-COVID-19 Functional Status and the EuroQol 5-Dimensional 5-Level. The extent of pulmonary fibrosis was assessed by comparing chest computed tomography (chest CT) scans before and after hospitalization. Following treatment, the patient demonstrated clinically meaningful improvement in clinical symptoms, enhanced quality of life, and decreased fibrotic lesions on CT scans. Conclusion: This case report suggests that Korean medicine treatment may be effective in improving clinical symptoms, such as cough and dyspnea caused by PCPF, while also enhancing post-COVID-19 quality of life and ameliorating pulmonary fibrotic lesions.

코로나19 확진 간호사의 롱코비드(long COVID) 증상과 관련 요인 (Long COVID symptoms and associated factors in registered nurses with COVID-19)

  • Purpose: The coronavirus disease 2019 (COVID-19) pandemic has had significant physical and psychological impacts on registered nurses (RNs). This study aimed to identify long COVID symptoms and their associated factors specifically among RNs. Methods: This descriptive correlational study's sample comprised 189 nurses (31.57±5.98 years, 93.7% female) in Korea. Self-reported long COVID symptoms were assessed using the COVID-19 Yorkshire Rehabilitation Scale. Data were collected from December 31, 2022, to January 13, 2023, using the online survey method and were analyzed using independent t-test, Wilcoxon signed-rank test, one-way ANOVA, Pearson's correlation, and a multiple linear regression analysis with the IBM SPSS Statistics 26.0 program. Results: A total of 179 participants (94.7%) experienced one or more long COVID symptoms. The most prevalent symptoms were weakness (77.8%), fatigue (68.3%), breathlessness (67.7%), cough/throat sensitivity/voice change (50.3%), and sleep problems (50.3%). The factors related to long COVID symptoms were marital status, type of institution, working time, acute COVID-19 symptoms, and vaccination status. The quarantine period (β=.26, p<.001) and the nursing workforce after COVID-19 (β=-.17, p=.018) were significantly associated with long COVID symptoms (Adjusted R2 =.33). Conclusion: Providing comprehensive recognition is necessary for the understanding of long COVID symptoms and their associated factors among nurses and could promote a long COVID symptom management education program targeted at nurses. Moreover, it could facilitate effective nursing care and education plans for long COVID patients.

Clinical Features and Risk Factors of Post-COVID-19 Condition in Korea

  • Objectives: Numerous studies have explored the causes and spread of outbreaks, yet there is a lack of research on post-coronavirus disease 2019 condition (PCC) in Korea. The goal of this study was to identify the various types of PCC and associated factors in discharged patients and to provide directions for the ongoing health management of confirmed patients. Methods: A telephone survey was conducted among 680 coronavirus disease 2019 (COVID-19) patients diagnosed between July 7, 2021 and August 26, 2021, in Dangjin, Chungnam, Korea. A descriptive analysis of characteristics, univariate analysis, and regression were performed using data from basic epidemiological surveys conducted at the time of diagnosis and post-discharge questionnaires. Results: Of the 585 patients who responded, 159 (27.2%) developed PCC. Of the 211 patients with no initial symptoms, 27 (12.8%) developed PCC, versus 132 (35.3%) of the 374 patients with initial symptoms. Among the initial symptoms, fever or chills, cough or sputum, loss of smell, and sore throat were associated with PCC. Compared to patients with less than 10 days of hospitalization, those with a hospitalization period of 21 days to 30 days (odds ratio [OR], 2.3; 95% confidence interval [CI], 1.0 to 5.2) and 31 days or more (OR, 5.8; 95% CI, 1.9 to 18.1) had a higher risk of PCC. Conclusions: More than a quarter of COVID-19 patients, including those who had no initial symptoms, experienced PCC in Korea. People with the initial symptoms of fever, chills, and respiratory symptoms and those who had prolonged hospital stays had a high risk of PCC.

Viral Load Dynamics After Symptomatic COVID-19 in Children With Underlying Malignancies During the Omicron Wave

  • 목적: 본 연구의 목적은 혈액종양 기저질환으로 치료 중 유증상 코로나바이러스감염증-19(COVID-19)으로 확진 된 소아청소년에서 바이러스 부하(viral load)의 변화를 확인하고자 하였다. 방법: 후향적 종단 코호트연구(retrospective longitudinal cohort study)로, 19세 미만 소아청소년 중 악성 빈혈, 혈액암, 또는 고형암으로 치료 중인 상태에서 2022년 3월 1일부터 8월 30일 사이에 SARS-CoV-2 PCR 양성으로 유증상 코로나바이러스감염증-19가 확진 된 환자를 대상으로 하였다. 환자들의 의무 기록과 전화 문진으로 임상 증상과 전파경로, 그리고 증상의 경과에 대한 자료를 얻었고, 서울성모병원에서 시행했던 SARS-CoV-2 PCR titer 값을 분석하였다. 확진 이후 E gene RT-PCR Ct value ≥25을 전파가능성이 낮은 상태로 정의하였다. 결과: 6개월의 연구 기간 동안 총 43명의 환자에서 44번의 COVID-19 확진 사례가 포함되었다. 환자의 평균 연령은 8세(interquartile range, 4.9-10.5)였으며, 가장 흔한 기저 질환은 급성 림프구성 백혈병(n=30, 68.2%)이었고, 다음으로 조혈모세포이식 후(n=8, 18.2%) 상태인 환자들이었다. 대부분 경증 COVID-19 (n=32, 72.7%)에 해당이 되었고, 3명의 환자(7.0%)는 중증/위중증 COVID-19에 해당되어 산소 치료를 받았다. 2.3% (n=1)는 COVID-19 관련 급성 호흡곤란 증후군으로 사망하였다. 확진 이후 E gene RT-PCR Ct값이 ≥25을 도달한 시점이 15-21일인 환자는 총 39.4%(n=13)이었고, 22-28일에 도달한 환자는 30.3% (n=10)이었다. 15.2% (n=5)의 환자에서는 확진 후 28일이 지난 시점에서도 Ct값 <25를 유지하였다. E gene Ct값이 <25 장기간 지속되는 위험인자로 난치성 악성 종양 상태(β, 67.0; 95% CI, 7.0-17.0; P=0.030)가 유의한 관련이 있었다. 한 환자는 확진 후 Ct 값이 <25으로 유지되던 중, 확진 후 86일 째 보호자로 상주하던 어머니에게 바이러스를 전파하였다. 결론: 난치성 악성 종양 상태에서 유증상 COVID-19에 확진 되는 경우 바이러스를 장기간 배출 할 수 있기 때문에, 이런 환자군에서는 PCR 기반 바이러스 전파 예방 조치가 도움이 될 수 있다.

코로나19 감염과 그 이후의 정신신체증상 (Psychosomatic Symptoms Following COVID-19 Infection)

  • 연구목적 코로나19 감염으로 인한 다양한 정신과적 증상, 정신신체증상을 파악하고 장기적 영향을 조사하였다. 방 법 체계적 문헌고찰을 통해 국내외 데이터베이스에서 논문을 선정하고, "COVID-19", "psychosomatic" 등의 검색어를 활용하였다. 정신신체증상에 대한 구조화된 측정도구를 사용한 연구를 포함하여, 총 16편의 논문이 최종 분석 대상으로 포함되었다. 결 과 코로나19 급성 감염과 관련된 정신증상으로는 불안, 우울, 신체증상 등이 보고되고 있다. 장기간 지속되는 포스트 코로나증후군의 증상으로는 흉통, 피로 등이 보고되었고, 이와 관련된 정신신체증상의 발생 빈도는 10%-20%로 파악되었다. 감염병으로 인한 심리사회적 스트레스, 여성, 노인, 정신과적 기왕력이나 동반 정신질환 등의 요인이 관련을 미친다. 전신염증, 자가면역, 자율신경계의 이상반응 등이 관련이 있을 것으로 생각되고 있다. 결 론 코로나19 감염 이후 발생하는 정신신체증상은 삶의 질과 심리사회적 기능에 부정적인 영향을 미칠 뿐 아니라 증상에 대한 정신과적 이해와 접근은 예방과 치료에도 중요하다.

Drug-Induced Bullous Pemphigoid Associated with the Severe Acute Respiratory Syndrome-Coronavirus Disease 2019 Vaccine: Case Report

  • In this study, we investigate the emergence of bullous pemphigoid (BP) after the administration of the Severe Acute Respiratory Syndrome-Coronavirus Disease 2019 (SARSCOVID-19) vaccine. The study presents two cases of women, aged 47 and 53, diagnosed with BP following SARS-COVID-19 vaccination. BP is a common autoimmune blistering disorder prevalent among older populations, with an incidence rate ranging from 2 to 40 cases per million individuals. This condition arises when autoantibodies target adhesive proteins in the skin, resulting in blister formation and mucosal erosion. Drug-induced bullous pemphigoid (DIBP) shares similarities with the classic form of BP but may be influenced by medication usage. Notably, DIBP exhibits distinct characteristics, such as affecting a younger demographic and involving mucosal regions more prominently than classic BP. The growing incidence of BP is linked to factors such as an aging population and the rise of drug-induced cases. This case report provides valuable insights into comprehending DIBP, elucidating post-vaccination discomforts, particularly those related to oral lesions and the exacerbation of existing lesions. By elucidating these aspects, we aim to advance the understanding of DIBP within the medical community.

Sustained SARS-CoV-2 antibody response in domestic pets: Insights from a longitudinal study

  • The COVID-19 pandemic, triggered by the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), has not only impacted human health on a global scale but also raised concerns about the vulnerability of a wide array of animals that are in close contact with humans. Particularly, the potential for infection and the subsequent immune response in domestic pets such as dogs and cats remain largely unexplored under natural living conditions. In this study, we have undertaken the task of detecting and tracking the presence of antibodies against SARS-CoV-2 in a small cohort of household pets-specifically, two dogs and two cats. Employing techniques such as the indirect ELISA and plaque reduction neutralization tests, we observed that the neutralizing antibodies against SARS-CoV-2 in these animals were maintained for a duration of up to six months following their initial positive test result. This duration mirrors the antibody response documented in human cases of COVID-19, suggesting a comparable post-infection immune response timeline between humans and these domestic animals.
