뇌졸중 후유증 환자에서 발병한 견관절 아탈구 치료 2례 (A Clinical cases Study of Shoulder Dislocation in Patients Undergoing Stroke)

  • Objective: There are some case reports on treatment of shoulder dislocation in Patients Undergoing Stroke. We report the electronic needle and the Bee venom Acupunure Theraphy about under using shoulder dislocation in patients Undergoing Stroke got a good remedial value. Methods : The patients diagnosed as shoulder dislocation and treated mainly with Bee venom Acupunture Therapy and electronic needle Results & Conclusions : Symptoms of the patientes such as shoulder pain, Range Of Motion(ROM) were improved after above treatments. The interval in X-ray film comes to be narrow. So, it is suggested that oriental medical treatment are effective on post stroke with shoulder dislocation.

PNF 매트 운동이 뇌졸중 환자의 균형수행력 향상에 미치는 효과 (The Effect of Mat Activities in PNF on Improvement of Balance Performance in Stroke Patients)

  • Purpose : The purpose of this study was to investigate the effect of mat activities in PNF on improvement of balance performance in stroke patients. Methods : Included 12 persons with stroke who were living in the community. They participated in exercise class for 60minutes duration three times a week for 6 weeks. Mat activities in PNF program was modified PNF patterns and techniques on various posture. Balance indexes were measured at pre-treatment, post-treatment and follow-up by K.A.T.3000. Results : Balance index scores had larger improvements after 6 weekends of treatment(p<.05) than pre-treatment, also follow-up test(p<.05). Conclusion : The result of this study showed that PNF mat activities intervention can improve balance performance in disabled persons after stroke.

뇌졸중 환자의 흡인유무에 따른 후두닫힘 지속시간 (Laryngeal Closure Duration in Post-stroke Patients)

  • As bolus enters the pharynx during the swallow, laryngeal closure takes place by approximating the epiglottis to the arytenoid Laryngeal Closure Duration(LCD) is the duration of contact between the arytenoids and the epiglottis from the first contact to the last(Logemann et al, 2000). Epiglottic inversion continues pharyngeal swallow stage is completed in order to protect the airway. The purpose of this study is to measure layrngeal closure duration (LCD) in three groups of subjects: a) 10 stroke patients who aspirate before and during the swallow(aspirators), b) 10 stroke patients who do not aspirate during the swallow c)10 normal control subjects. Means and standard deviation of LCD was analyzed in both 5ml and 10 ml thin liquids using 100msec timer in videoflouroscopic swallowing examination. The mean for each group was 0.15 seconds shorter from aspirators to control group. There was a significant difference between aspirators and normal subjects for laryngeal closure duration during the swallow. Laryngeal closure duration after a stroke lead to aspiration. However, only one of this temporal problem may not be enough to aspiration.

라이프케어 증진을 위한 동적탄력튜빙 보행훈련이 뇌졸중 환자의 균형에 미치는 영향 (Effect of Dynamic Tubing Gait Training for Life-Care on Balance of Stroke Patients)

  • 본 연구는 라이프케어 증진을 위한 동적탄력튜빙 보행훈련(Dynamic Tubing Gait, DTG II)이 만성 뇌졸중 환자의 균형에 미치는 영향을 알아보았다. 본 연구는 10명의 만성 뇌졸중 환자를 대상으로, DTG II 프로그램을 일일 30분씩 1회, 주 5회, 5주간 총 25회 실시하였다. 균형향상을 위한 DTG II 프로그램의 효과를 알아보기 위해 표면근전도(배바깥빗근, 척주세움근, 엉덩허리근, 큰볼기근), 골반 3축의 대칭지수 검사(경사, 대각선, 회전), 동적보행지수 검사(DGI)를 중재 전과 후에 시행하였다. 본 연구의 결과, 초기디딤기와 중간디딤기에서 큰 볼기근의 중재 전·후 비교에서 통계적으로 유의한 차이를 보였다(p<.05). 골반의 대칭지수(SI)는 대각선 SI와 회전 SI에서 중재 전·후 비교에서 통계적으로 유의한 차이를 보였다(p<.05). 동적보행지수(DGI)에서 중재 전·후 비교에서 통계적으로 유의한 차이를 보였다(p<.05). 본 연구결과를 통해 라이프케어 증진을 위한 DTG II 프로그램이 만성 뇌졸중 환자의 몸통근육의 근활성과 골반의 대각선 움직임과 회전 움직임의 대칭성을 향상시킴으로써 만성 뇌졸중 환자의 동적 균형 능력을 향상된 것을 알 수 있었다. 따라서 만성 뇌졸중 환자의 라이프케어 증진을 위한 균형능력 향상을 위해 동적탄력튜빙 보행훈련을 중재법의 하나로 권장한다.

자조활동 훈련 프로그램이 만성 뇌졸중 환자의 상지 운동기능 및 자기효능감에 미치는 영향 (Effects of Self Care Training Program on Motor Function of Upper Limb and Self-Efficacy in Chronic Stroke Patients)

  • 본 연구는 만성 뇌졸중 환자를 대상으로 과제 지향적 훈련을 기반으로 하는 자조활동 프로그램이 상지 운동기능 및 자기효능감에 미치는 영향을 알아보고자 하였다. 지역사회에 거주하는 만성 뇌졸중 환자 20명을 대상으로 연구를 진행하였다. 자조활동 훈련 프로그램은 상지 과제 지향적 훈련으로 총 6주간, 주 4회 40분의 프로그램으로 구성하였으며, 매주 1회 집단훈련을 통해 훈련 프로그램을 점검하였다. 상지 운동기능의 변화를 알아보기 위해 맨손기능 검사, 악력 검사를 실시하였으며, 자기효능감 변화를 알아보기 위해 자기효능감 척도를 이용하여 설문 조사를 실시하였다. 상지 운동기능 검사 결과, 자조활동 훈련 전에 비해 자조활동 훈련 후 맨손기능과 악력이 모두 증가되었으며, 통계적으로 유의한 차이를 나타내었다(p<.05). 자기효능감 변화를 관찰한 결과, 자조활동 훈련 전에 비해 자조활동 훈련 후 자기효능감이 향상되었으며, 통계적으로 유의한 차이를 나타내었다(p<.05). 이상의 결과를 종합해 볼 때 과제 지향적 훈련을 기반으로 하는 자조활동 훈련 프로그램이 만성 뇌졸중 환자의 상지 운동기능 향상에 효과적이며, 과제 성취감을 경험하게 함으로써 신체의 운동기능 회복뿐만 아니라 자기효능감에도 긍정적인 영향을 미친 것을 알 수 있었다.

신전반사에 의해 유발된 휴지기성 진전 1예 (Stretch Reflex Induced Resting Tremor(SRIRT))

  • It has been said that variable anatomical structures and neural circuits are related to the generation of tremor. There are cerebral cortex, thalamus, basal ganglia, inferior olivary nucleus, midbrain tegmentum, stretch reflex, and musculoskeletal structures. The stretch reflex is related with the physiologic tremor and various peripherally originated tremors. We experienced a case with the post-stroke resting tremor which was induced and aggravated by mechanical stretching stimulation. In the present case, stretch reflex has a major role in the generation and exacerbation of tremor. It is presumed that the development of tremor is attributed to the increased rhythmicity of ventral intermedius nucleus of thalamus. The enhancement of thalamic rhythmicity may be due to the increasement of long latency reflex by post-stroke rigidity. This case suggests that stretch reflex may have a major role in the pathophysiologic mechanisms of a certain centrally originated tremor.

Chronic Central Post-Stroke Pain Treated with Scalp Acupuncture and Traditional Korean Medicine: A Case Report

  • A patient with central post-stroke pain was treated for 4 weeks with scalp acupuncture and traditional Korean medicine (following a cerebral infarction in 2013). The patient presented at Chungju hospital in January 2019 with left side weakness and tingling, numbness in the left hemisphere, chronic pain and dysarthria. Initially, herbal medicine, acupuncture, pharmacupunture, indirect moxibustion, and physiotherapy were administered together with Western medicine, with no improvement in the patient's condition. On Day 5, scalp electroacupuncture (MS1, MS5, MS10, MS11) was introduced. The numbness feeling in the patient's head resolved, and the pain in his upper body decreased. Grip force difference between the left and right hand improved from 3 kg to 0-0.5 kg. Sleep disturbance was resolved after 4 weeks treatment, and his average numeric rating scale score for pain improved from an admission score of 10, to a discharge score of 5. The patient could walk unaided after treatment.

Electroacupuncture Treatment for Post-Stroke Foot Drop: A Systemic Review of Randomized Controlled Trials

  • A review of randomized controlled trials (RCTs) using electroacupuncture (EA) to treat patients with foot drop was performed to analyze the effectiveness of EA for this condition. Relevant studies (n = 183) from 7 databases (Cochrane Library, Excerpta Medica Database, PubMed, China National Knowledge Infrastructure, Korean Studies Information Service System, Research Information Sharing Service, and Oriental Medicine Advanced Searching Integrated System) were selected based on the inclusion and exclusion criteria, and 12 RCTs met the selection criteria. In all 12 studies, EA showed significantly positive changes. In most indicators, positive changes were observed in the EA group compared with that in the control group. Significant increases were confirmed in muscle strength-related indicators such as the Fugl-Meyer motor scale, surface electromyography, active range of motion, and gait-related indicators such as the Tinetti score, maximum walking speed, and Berg balance scale. No notable adverse events were reported. EA is suggested as an effective treatment for post-stroke foot drop; however, more RCTs are required.

뇌졸중 환자에서 8자 모양 경로 보행 검사의 동시 타당도 연구 (Concurrent Validity between Figure-of-8 Walking Test and Functional Tests Included Tasks for Dynamic Balance and Walking in Patient with Stroke)

  • Purpose: The purpose of this study was to determine the concurrent validity between Figure-of-8 Walking Test (F8W), Berg Balance Scale (BBS), Four Squared Step Test (FSST), and Timed UP and GO Test (TUG) in patients with stroke. Methods: Forty two participants (26 men, 16 women, $55.0{\pm}11.72$) with at least three months post stroke who were able to walk at least 10 m without walking aid participated in this study. Assessment of concurrent validity between the F8W (time and steps) and BBS was performed using Spearman rank order correlation and between the F8W (time and steps), FSST and TUG assessed using Pearson correlation. Results: The time of the F8W showed correlation with BBS (r=-0.46, p<0.01), FSST (r=0.64, p<0.01), and TUG (r=0.81, p<0.01), and steps of the F8W showed correlation with BBS (r=-0.43, p<0.01), FSST (r=0.47, p<0.01), and TUG (r=0.51, p<0.01). Conclusion: The F8W is a valid measure of balance and walking skill among patients with stroke and may provide complementary information with regard to dynamic balance and functional walking for the real life of stroke patients.

Effects of Sympathetic Conversation on Electroencephalogram, Stress, Anxiety-Depression, and Muscle Tone in Chronic Stroke Patients

  • PURPOSE: This study was conducted to identify the effects of sympathetic conversation on stress, anxiety and depression, and muscle tone in chronic stroke patients. METHODS: Patients were randomly assigned to either an experimental group (EG, n=7) or control group (CG, n=7). Both groups participated in a pretest before intervention. Subjects were asked to undergo: 1) electroencephalogram, 2) Stress Response Inventory, 3) Hospital Anxiety and Depression Scale, 4) muscle tone and stiffness testing. After the pretest, EG received sympathetic conversation and CG received a simple explanation about stroke recovery and rehabilitation. Following the intervention, both groups were immediately administered a post test. RESULTS: In EG, the electroencephalogram relative alpha power was significantly increased (p<.05), while the electroencephalogram relative gamma power was significantly decreased (p<.05). The Stress Response Inventory and Hospital Anxiety and Depression Scale scores decreased significantly in both groups (p<.05). In addition, muscle tone and stiffness decreased significantly in the EG (p<.05) CONCLUSION: The results of the present study indicate that sympathetic conversation had a positive effect on stress, anxiety and depression, and muscle tone in patients with chronic stroke. Therefore, sympathetic conversation could be used to improve not only psychological problems in chronic stroke patients including stress and anxiety, but also physical conditions including muscle tone.
