Frequency of Post-Concussion Syndrome in Korean Patients with Minor Head Injury

  • Purpose: To determine the incidence of post-concussion syndrome (PCS) in Korean patients after minor traumatic brain injury. Methods: We conducted an observational cohort study of a convenience sample of patients presenting to the emergency department of a major academic Korean hospital. Patients who visited the Emergency Department for head trauma were screened. A researcher questioned the subject regarding his or her symptoms. Subjects were contacted by phone approximately 2 weeks after their Emergency Department visit and questioned about subsequent symptoms and subsequent visits for medical care. Results: Only 8% of subjects reported any post-concussion symptoms. Only 0.4% had three or more symptoms which might have met criteria for PCS. The median peak onset of symptoms was 3 days after injury. Conclusion: The incidence of PCS is Korean patients is much lower than that documented for patients in the United States or other western countries. On the other hand, this study results could give an idea that mild trauma could also cause the PCS. Further study is needed to replicate this finding and investigate possible explanations for this difference.

What Should Be Considered to Cause the Early Post-Craniotomy Seizure : Antibiotics (Cefazolin) Irrigation

  • Objective : Post-craniotomy seizure (PCS) is reported only rarely. However, our department noted a 433% increase in PCS for a year beginning September 2010, especially after cerebrovascular surgery. Our goal was to identify the cause of our unusual outbreak of PCS. Methods : For almost one year after September 2010, cases of PCS increased significantly in our department. We analyzed 973 patients who had received a major craniotomy between January 2009 and November 2011. We included seizures that occurred only in the first 24 postoperative hours, which we defined as early PCS. After verifying the presence of PCS, we analyzed multiple seizure-provoking factors and their relation to the duration and character of seizure activity. Results : Overall PCS incidence was 7.2% (70/973). Cefazolin (2 g/L saline) was the antibiotic drug used for intraoperative irrigation in 88.4% of the operations, and no PCS occurred without intraoperative cefazolin irrigation. When analyzed by operation type, clipping surgery for unruptured aneurysms was the most frequently associated with PCS (80%). Using logistic regression, only 2 g cefazolin intraoperative irrigation (p=0.024) and unruptured aneurysm clipping surgery (p<0.001) were associated with early PCS. The seizure rate of unruptured aneurysm clipping surgery using 2 g cefazolin intraoperative irrigation was 32.9%. Conclusion : Intraoperative cefazolin irrigation must be avoided in patients undergoing craniotomy, especially for clipping of unruptured aneurysms, because of the increased risk of early PCS.

  • With an explosive growth of internet and a change of consumer's demand, Post PC is emerging as a substitute for the existing desktop computer and laptop computer. Post PC is a new computer which is portable with wireless networking, cheaper than the existing PCs and specialized in the specific function that consumers want. Furthermore, the shape and the ability of Post PC are dependent on its specific function. The rudimentary Post PCs such as PDA, web-pad and smart-phone are already usable. And in the future, owing to the progress of technologies, the heterogeneity of relate companies and the low entry barrier, various Post PCs will be developed. In this paper, we analyze consumers' willingness-to-pay for Post PC with the attributes such as the portability, the speed of CPU, the data input method (using key board, or notepad etc), the size of monitor and price. To estimating consumers' willingness-to-pay for the attributes of Post PC, we apply a contingent ranking method, which makes the respondents rank hypothetical Post the portability, the speed of CPU, the data input method (using key board, or notepad etc),the size of monitor and price. CCAAT/enhancer binding protein β Induces Post-Switched B Cells to Produce Blimp1 and Differentiate into Plasma Cells

  • Long-lasting post-switched plasma cells (PCs) arise mainly from germinal center (GC) reactions, but little is known about the mechanism by which GC B cells differentiate into PCs. Based on our observation that the expression of the transcription factor CCAAT/enhancer binding protein β (C/EPBβ) is associated with the emergence of post-switched PCs, we enquired whether a cell-autonomous function of C/EPBβ is involved in the program for PC development. To address this, we generated C/EPBβ-deficient mice in which the Cebpb locus was specifically deleted in B cells after transcription of the Ig γ1 constant gene segment (Cγ1). In response to in vitro stimulation, B cells from these Cebpbfl/flCγ1Cre/+ mice had defects in the induction of B lymphocyte-induced maturation protein 1 (Blimp1) and the formation of IgG1+ PCs, but not in proliferation and survival. At steady state, the Cebpbfl/flCγ1Cre/+ mice had reduced serum IgG1 titers but normal IgG2c and IgM titers. Moreover, upon immunization with T-dependent Ag, the mice produced reduced levels of Ag-specific IgG1 Ab, and were defective in the production of Ag-specific IgG1 Ab-secreting cells. These results suggest that a cell-autonomous function of C/EPBβ is crucial for differentiation of post-switched GC B cells into PCs through a Blimp1-dependent pathway.

  • Mobile Internet Service has been accelerated in ubiquitous computing environment. The purpose of this study is to suggest the patterns of mobile internet services based upon the current and future environments. The types of mobile internet service are explicated on the basis of current and future usage. A two-dimensional framework using level of timeliness and service purpose is utilized for classifying mobile internet service. Future trends of mobile internet service are carefully suggested.

  • In this paper, we have designed and fabricated a small size wide-band monopole antenna which can integrate the frequency of previous business mobile communication system by adding 4 PCS of post type parasitic element on top-loaded sleeve monopole antenna. We have observed the properties of return loss upon a parameter change of element, and we also examined radiation properties in the band of PCS, W-CDMA, WiBro, W-LAN and S-DMB in order to make sure the suggested antenna's wide-band properties. We have found that the proposed antenna has omni-direction in horizontal plane and figure eight-direction in vertical plane, and we could have good return loss($Return\;loss{\leq}-10\;dB$) and $1.14{\sim}3.66\;dBi$ gain in $1.67{\sim}3.55\;GHz$ of frequency range($B/W{\fallingdotsep}72%$).

Practical Communication Strategies to Improve the Surgical Outcomes in a Pediatric Cardiac Intensive Care Unit (소아심장외과 중환자실에서의 실무의사소통 프로토콜이 수술 후 성과에 미치는 영향)

  • Purpose: The purpose of this study was to identify the impact of practical communication strategies (PCS) on the reduction of AEs (Adverse Events) in pediatric cardiac ICU (PCICU). Methods: Intra-operative findings and care plans were documented and shared between staff members on a daily basis from the day of operation to the day of general ward transfer. Incidence of AEs was investigated in all patients who were admitted to the PCICU and was compared with incidence of AEs one year after establishment of PCS. Results: The study population consisted of 216 patients in pre-PCS group and 156 patients in post-PCS group. Incidence of readmission decreased from 6.0% (13/216) in pre-PCS group to 0.6% (1/156) in post-PCS group (${\chi}^2=7.23$, p=.010). Incidence of other major complications decreased from 4.2% (9/216) to 0.6% (${\chi}^2=6.66$, p=.012). Minor AEs such as intervention omission, order error, and protocol misunderstanding were reduced from 23.3 cases per 100 patient-days to 7.5 cases per 100 patient-days (${\chi}^2=20.31$, p<.001). Conclusion: Handover protocol is an effective strategy to reduce AEs for critically ill patients after pediatric cardiac surgery. Efforts to develop effective communication strategies should be continued and outcome research about communication strategies for patient safety should be further studied.

  • Post-concussion syndrome (PCS) following mild traumatic brain injury (mTBI) is a major factor that contributes to the increased socioeconomic burden caused by TBI. Myelin loss has been implicated in the development of PCS following mTBI. Diffusion tensor imaging (DTI), a traditional imaging modality for the evaluation of axonal and myelin integrity in mTBI, has intrinsic limitations, including its lack of specificity and its time-consuming and labor-intensive post-processing analysis. More recently, various fast MR techniques based on multicomponent relaxometry (MCR), including QRAPMASTER, mcDESPOT, and MDME sequences, have been developed. These MCR-based sequences can provide myelin water fraction/myelin volume fraction, a quantitative parameter more specific to myelin, which might serve as a surrogate marker of myelin volume, in a clinically feasible time. In this review, we summarize the clinical application of the MCR-based fast MR techniques in mTBI patients.

A Review of Clinical Studies for Chinese Medicine Treatment of Post-concussion Syndrome Using the CNKI Database (뇌진탕 후 증후군의 중의치료에 대한 임상연구 동향 - CNKI검색을 중심으로)

  • Objectives: The aim of this study was to investigate the latest clinical studies on Korean medicine treatment of post-concussion syndrome (PCS) in the Chinese National Knowledge Infrastructure Database (CNKI). Methods: We searched the last 10 years of clinical studies discussing Oriental medicine-based treatments for PCS in the CNKI database. The search focused on the authors, publication year, type of study, purpose of the study, method and duration of treatment, evaluation criteria, and results of each article. Results: Of the 22 selected studies, 1 was a non-randomized controlled trial (nRCT), 2 were case series, and 19 were randomized controlled trials (RCTs). Treatments included oral herbal medicine in 16 studies, oriental medicine through external use in 2 studies, acupuncture in 3 studies, and traditional Chinese medicine injection in 2 studies. The most frequently used herb was Cnidii Rhizoma (川芎). All 22 studies confirmed the efficacy of Oriental medicine treatments. Conclusion: More varied and scientifically designed clinical studies are required to develop treatments for PCS. The results of this study could be used as basic data for further PCS studies.

Physical Activities and Health-related Quality of Life of Individuals Post Stroke

  • PURPOSE: The purpose of this study is to examine the relationship between the physical activities of individuals post-stroke and their HRQL, as well as to determine whether their functional abilities contribute to their amounts of physical activity. METHODS: The study's subjects included 90 individuals post-stroke. Their amounts of physical activity were measured using the International Physical Activity Questionnaire (IPAQ), and their HRQL was measured using the Medical Outcomes Study 36-Item Short-form Health Survey (SF-36). In addition, the functional abilities of the subjects were measured. For the measures of physical activities and the HRQL, Pearson's correlation coefficients were used to identify the strengths of the associations between the measures. A hierarchical linear regression model was used to determine whether physical activities had independent impacts on the HRQL. RESULTS: This study found that the physical activities performed by the subjects affected the SF-36 physical component score (PCS) (12%). However, the physical activities and the SF-36 mental component score (MCS) showed no statistically significant relationship, whereas functional abilities and physical activities had a statistically significant relationship (r = .57~.86, p<.001). CONCLUSION: The present study identified a correlation between physical activity and the PCS. Therefore, individuals post-stroke should be encouraged to carry out more physical activities, including more frequent walking activities.
