A Study on the Effect of Pelvic Tilting Exercise in Hemiplegic Patients (골반운동이 뇌졸중 환자의 보행특성에 마치는 효과)

  • The objective of this study was to identify the effects of pelvic tilting exercise on gait patterns of hemiplegic patients. The subjects of this study were 31 hemiplegic in- and out-patients of the Rehabilitation Hospital, Yonsei University Medical Center, from September 24, 1997 through November 5, 1997. Pre- and post-treatment change in gait patterns were measured using a ink foot-print. The data were analyzed by the paired t-test, one-way ANOVA, and independent t-test. The findings were as follows: The difference in gait patterns between pre- and post-treatment was statistically significant, with an increase in gait velocity to 7.98 cm/sec post-treatment; an increase in cadence to 7.29 steps/min; a narrowing of the base of support to 1.33 cm; an increase in step length of 3.92 cm on the less affected side and 3.73 cm on the more affected side; an increase in stride length of 5.82 cm on the less affected side and 5.92 cm on the more affected side(statistically not significant in foot angle). In relation to sex, age, cause of stroke, and laterality of paralysis, the difference in gait patterns between pre- and post-treatment was not statistically significant. Where there was no significant difference of the effects of pelvic exercise regarding the degree of spasticity, the presence of a decrease in proprioception, and the duration of treatment. In conclusion, hemiplegic pelvic tilting exercise was found to have transmitting positive effect in improving gait patterns.

The Effect of The Heat Treatment Condition and the Oxidation Process on the Microstructure of Ag-CdO Contact Materials (Ag-CdO계 전기접점재료의 미세조직에 미치는 열처리 조건과 산화 공정의 영향)

  • Contact material is widely used in the field of electrical parts. Ag-CdO material has a good wear resistance and stable contact resistance. In order to establish optimizing heat treatment condition, rolling temperature and oxidation process, we studied the microstructure of Ag-CdO material with various conditions. The experimental procedure were melting using high frequency induction, heat treatment, rolling and internal oxidation. And we experimented on difference process, Post-oxidaion. In this study, we obtained the optimizing heat treatment condition was $700^{\circ}C$ for 15 min. and the optimizing rolling temperature was $730^{\circ}C$. In investigation of the microstructure of oxidized material, coarse oxide and depleted oxidation layer existed. The hardness was average Hv 70. When we used Post-oxidation, oxides were finer than prior process and depleted oxidation layer did not exist. The hardness of Post-oxidation material was average Hv 80. And the optimizing rolling temperature was $800^{\circ}C$.

Maxillomandibular advancement surgery after long-term use of a mandibular advancement device in a post-adolescent patient with obstructive sleep apnea

  • Patients with obstructive sleep apnea (OSA) whose phenotype belongs to a craniofacial vulnerability are referred from sleep doctors to orthodontists. In adults, for osseo-pharyngeal reconstruction (OPR) treatment, permanent maxillomandibular advancement (MMA) surgery and use of a temporary mandibular advancement device (MAD) are applied. This case report demonstrates successful treatment of OSA through application of phased MAD and MMA in a 16-year-old male with craniofacial deformity and residual growth potential. This patient showed skeletal and dentoalveolar changes after 7-year MAD use throughout post-adolescence, which affected the design and timing of subsequent MMA surgery, as well as post-surgical orthodontic strategy. This case report suggests that OPR treatment can be useful for treatment of OSA in post-adolescent patients, from an orthodontic point of view, in close collaboration with sleep doctors for interdisciplinary diagnosis and treatment.

  • Objectives : The purpose of this study is to search for more effective methods of diagnosis and treatment of Communication Disorder with Post-Stroke. Methods : Literature review on Communication Disorder with Post-Stroke in view of oriental and western medicine. Conclusions : Communication Disorder with Post-Stroke is relative with aphasia in western medicine, Sul-um(舌瘖) oriental medicine Aphasia, apraxia of speech, and dysarthria come after strock Each of them has its own mechanism. Yomchon(CV23), Amun(GV15), Chuldol(CV22), Tongni(HT5), Pungnyung(ST40), Pungbu(GV16), Chigu(TE6), Yongchon(KI1), Hapkok(L14), Peakoe(GV20), Kokchi(LI11), Puryu(KI7), Shinsu(BL23), Kumjin-okaek, and Chohea(KI6) was used on Communication Disorder with Post-Stroke treatment.

A Case of Cerebral Infarction Patient Complaining Post-Stroke Aggression Accompanying Anxiety Treated with Combined Korean Medical Treatment (뇌졸중 후 불안을 동반한 공격성을 호소하는 뇌경색 환자에 대한 복합 한의치험 1례)

  • Post-stroke aggression refers to aggressive behaviors and languages which are occurred after stroke. In stroke patients, over 15% suffer from the anger and the symptom lowers the quality of life of the patients, family, and caregivers. After diagnosing post-stroke aggression, selective serotonin re-uptake inhibitor(SSRI) and beta-blocker, beta adrenergic agonist treatment usually underwent to treat the anger and anxiety of the patient. In the present case, a 81-year-old female patient with post-stroke aggression underwent Korean medical treatment by Jodeungsan and Ukgansan for 49 days. The effect of the treatment was assessed with State-Trait Anger Expression Inventory(STAXI) per a week, roaming time per a week and roaming frequency per a week. After the treatment, STAXI score, roaming time, and roaming frequecy decreased, and the patient could successfully cease to take beta-blocker, which she started to take for treating her aggression, without any side-effects. This case report suggests that Ukgansan might be an effective option with post-stroke aggression patients taking medication including beta-blocker.

  • ZnO thin films on silicon substrates have been deposited by pulsed laser deposition technique(PLD). A Nd:YAG laser was used with the wavelength of 355 nm. In order to investigate the effect of oxygen post-annealing treatment on the property of ZnO thin films, deposited film has been annealed at the substrate temperature of $440^{\circ}C$. After post-annealing treatment in the oxygen ambient, the stoichiometry of ZnO film has been characterized be improved which results in higher UV emission intensity of photoluminescence.

Effects of Heat Treatment on Corrosion Resistance Properties of Gas Nitrided Stainless Steels (스테인레스강의 가스질화 후 내식특성에 미치는 열처리조건의 영향)

  • Gas nitriding and post oxidation were performed on stainless steels and SACM 645 steel. With increasing gas nitriding time, the increasing rate of nitrided layer was most rapid on SACM 645 steel and the nitriding depth of nitrided layer was most narrow on STS 304 steel among three steels. Corrosion resistance was increased with post oxidation on stainless steels and with increasing time the effect of corrosion resistance was decreased to compare with relatively short gas nitriding time. An improvement effect of corrosion resistance was consisted of predominantly on austenitic stainless steel by post oxidation after gas nitriding among three steels and it was relatively less influenced on martensitic stainless steel.

  • ZnO thin films on (001) sapphire substrates have been deposited by pulsed laser deposition(PLD) technique at the oxygen pressure of 350 mTorr. In order to investigate the effect of post-annealing treatment with oxygenn pressure of 350 mTorr on the optical property of ZnO thin films, films have been annealed at various substrate temperatures after deposition. After post-annealing treatment in the oxygen ambient, the optical properties of the ZnO thin films were characterized by PL(Photoluminescence) and structural properties of the ZnO were characterized by XRD, and have investigated structural property and optical property for application of light emission device.

  • A college student who exhibited several post traumatic stress disorder (PTSD) symptoms was used as a case study in order to investigate the therapeutic effects of sandplay therapy for PTSD sufferers. In total, 27 sessions of the sandplay treatment program were used to conduct psychological tests before and after the sessions. Any changes in the manner of expressions on the sand box during the treatment, and the results of pre and post psychological tests on aspects of depression, anxiety and interrelationships were analyzed. Overall results showed that the subject exhibited progressive behavioral improvement and signs of recovery. Psychological tests revealed reduced levels of depression and anxiety, and improved interrelationship scores. In conclusion, sandplay therapy does exhibit positive benefits as an effective treatment modality for PTSD in adolescents and young adults.

Clinical and Electromyographic Study of the Effects of Ultrasonic Wave and Microwave Diathermy Treatment on the Craniomandibular Disorder Patients (두개하악장애 환자에 대한 초음파와 극초단파 심부투열치료 효과의 임상 및 근전도학적 연구)

  • This study was performed to observe the effect of micro-wave diathermy and ultrasonic-wave diathermy on the craniomandibular disorder patients. 19 patients were classified into 12 acute and 7 chronic groups according to the duration of 6 months. They were treated with micro-wave diathermy and ultrasonic-wave diathermy for 2 weeks and pain, maximum comfortable opening, active range of motion were checked before and after therapy. Electromyographic activities of temporal and masseter muscles were also measured at physiologic rest position, clenching and mastication before and after therapy. The obtained results were as follows : 1. After treatment, pain were reduced and active range of motion and maximum comfortable opening were increased. 2. Temporoal and masseter muscle activities of post-treatment in rest position, clenching and mastication were lower than those of pre-treatment. 3. In rest position, temporal and masseter muscle activities of pre-treatment on affected sides were higher than those on unaffected sides, but there were no differences in muscle activities between affected ad unaffected sides on clenching and mastication in pre and post-treatment respectively. 4. There were no significant differences in active range of motion, pain and maximum comfortable opening between acute and chronic groups in pre and post-treatment but there were significant differences between pre-treatment and post-treatment in acute and chronic groups respectively. 5. Muscle activities of masseter and temporal muscles in acute and chronic patients were reduced in rest position after treatment.
