Objectives: The purpose of this retrospective cohort study was to investigate the relationship between exposure of Korean workers to petrochemicals in the refinery/petrochemical industry and lymphohematopoietic cancers. Methods: The cohort consisted of 8,866 male workers who had worked from the 1960s to 2007 at one refinery and six petrochemical companies located in a refinery/petrochemical complex in Korea that produce benzene or use benzene as a raw material. Standardized mortality ratios (SMRs) and standardized incidence ratios (SIRs) were calculated for 1992-2007 and 1997-2005 based on the death rate and cancer incidence rate of the Korean male population according to job title (production, maintenance, laboratory, and office workers). Results: The overall mortality and most cause-specific mortalities were lower among these workers than those of the general Korean population. Increased SMRs were observed for leukemia (4/1.45; SMR 2.77, 95% CI: 0.75-7.09) and lymphohematopoietic cancers (5/2.51; SMR 2, 95% CI: 0.65-4.66) in production workers, and increased SIRs were also observed in leukemia (3/1.34; SIR 2.24, 95% CI: 0.46-6.54) and lymphohematopoietic cancers (5/3.39; SIR 1.47, 95% CI: 0.48-3.44) in production workers, but the results were not statistically significant. Conclusion: The results showed a potential relationship between leukemia and lymphohematopoietic cancers and exposure to benzene in refinery/petrochemical complex workers. This study yielded limited results due to a short observational period; therefore, a follow-up study must be performed to elucidate the relationship between petrochemical exposure and cancer rates.
Metal contamination of medicinal herbs is of growing concern because of their potential adverse health effects. In this study, metal exposures were estimated and their potential health risks were preliminarily evaluated using available data including metal contamination levels and consumption estimates of herbal medicine in Korea. Consumption and contamination data of 34 medicinal herbs abundantly used in Oriental medicine in Korea were used in this study. Lead, mercury, arsenic, cadmium, cobalt, and chrome were identified as contaminants of potential health concerns. Even based on a conservative exposure scenario, i.e., consuming 5 times more herbal medicine with 95th percentile contamination levels, health risks associated with herbal medicine consumption were estimated to be minuscule. Herbal consumption was 0.3% of the provisional tolerable daily intake levels recommended by Joint Expert Committee on Food Additives (JECFA) of WHO/FAO. However, it should be noted that there are several important assumptions and uncertainties associated with this evaluation: This study was conducted for only 34 types of medicinal herbs of which consumption and metal contamination data were available. In addition, there are no reliable herbal medicine consumption data among Korean population. The pattern and amount of herbal (medicine) consumption in Korea need to be investigated in order to conduct more refined risk assessment associated with metal contamination in medicinal herbs.

Background: Messaging surrounding skin cancer prevention has previously focused on the general public and emphasized how or when activities should be undertaken to reduce solar ultraviolet radiation (UVR) exposure. Generic messages may not be applicable to all settings, and should be tailored to protect unique and/or highly susceptible subpopulations, such as outdoor workers. The primary objective of this study was to develop a set of tailored, practical, harm-reducing sun safety messages that will better support outdoor workers and their employers in reducing the risk of solar UVR exposure and UVR-related occupational illnesses. Methods: We adapted a core set of sun safety messages previously developed for the general population to be more applicable and actionable by outdoor workers and their employers. This study used an integrated knowledge translation approach and a modified Delphi method (which uses a survey-based consensus process) to tailor the established set of sun safety messages for use for outdoor worker populations. Results: The tailored messages were created with a consideration for what is feasible for outdoor workers, and provide users with key facts, recommendations, and tips related to preventing skin cancer, eye damage, and heat stress, specifically when working outdoors. Conclusion: The resulting tailored messages are a set of evidence-based, expert- approved, and stakeholder-workshopped messages that can be used in a variety of work settings as part of an exposure control plan for employers with outdoor workers.
Background: A population-based cervical cancer screening program using visual inspection with acetic acid was launched in Maldives in 2014. Our study aimed to assess the knowledge, attitude and practices of women in relation to risk factors of cervical cancer, early detection of the disease and its prevention. Materials and Methods: The questionnaire based survey was conducted among 20 to 50 year old women, systematically sampled to represent three regions of Maldives. Trained investigators interviewed a total of 2,845 women at home. Results: The prevalence of the risk factors of cervical cancer like early age at marriage and childbirth, multiple marriages, multiple marriages of the husbands, and multiple pregnancies was high. More women knew about breast cancer than cervical cancer. Even among the small number of women who knew of cervical cancer, only 34.6% had the knowledge of at least one early symptom. Very few women knew that the cancer could be prevented by any test. Only 6.2% of the women reported having ever undergone a Pap smear. Many women had the misconception that cervical cancer was infectious. In Maldives the younger women have high literacy rate due to the policy of universal free education and those with higher levels of education had improved knowledge of cervical cancer and its risk factors. The prevalence of risk factors also reduced with improved literacy. Conclusions: Awareness about risk factors and prevention of cervical cancer is limited among Maldivian women in spite of having high exposure to some of the risk factors. A universal literacy program in the country has helped to improve the knowledge of cervical cancer prevention and to reduce the exposure to various risk factors in the younger population.
Background: Diarrheal and intestinal infectious disease caused by inadequate drinking water, sanitation, and hygiene (WASH) is not only a great concern in developing countries but also a problem in low-income populations and rural areas in developed countries. In this study, we assessed the exposure to inadequate WASH in Korea and estimated the burden of disease attributable to inadequate WASH. Methods: We used observational data on water supply, drinking water, sewage treatment rate, and hand washing to assess inadequate WASH conditions in Korea, and estimated the level of exposure in the entire population. The disease burden was estimated by applying the cause of death data from death registry and the morbidity data from the national health insurance to the population attributable fraction (PAF) for the disease caused by inappropriate WASH. Results: In 2013, 1.4% of the population were exposed to inadequate drinking water, and 1.0% were living in areas where sewerage was not connected. The frequency of handwashing with soap after contact with excreta was 23.5%. The PAF due to inadequate WASH as a cluster of risk factors was 0.353 (95% confidence interval [CI], 0.275-0.417), among which over 90% were attributable to hand hygiene factors that were significantly worse than those in American and European high-income countries. Conclusion: The level of hand hygiene in Korea has yet to be improved to the extent that it shows a significant difference compared to other high-income countries. Therefore, improving the current situation in Korea requires a continuous hand washing campaign and a program aimed at all people. In addition, continuous policy intervention for improvement of sewage treatment facilities in rural areas is required, and water quality control monitoring should be continuously carried out.
Background: The present study aimed to evaluate the clinical association between cadmium exposure and hearing impairment among the Korean population. Methods: This retrospective cross-sectional study used the data obtained from the Korean National Health and Nutrition Examination Survey were used for our study. Finally, 3,228 participants were included in our study, which were then divided into quartiles based on their blood cadmium levels: first quartile (1Q), second quartile (2Q), third quartile (3Q), and fourth quartile (4Q) groups. The hearing thresholds were measured using an automatic audiometer at 0.5, 1, 2, 3, 4, and 6 kHz. Hearing loss (HL) was defined as >25 dB average hearing threshold (AHT). Results: All the groups had 807 participants each. The area under the receiver operating characteristic curves of cadmium level for HL were 0.634 (95% confidence interval [CI], 0.621-0.646). The participants in the 4Q group had higher Low/Mid-Freq, High-Freq, and AHT values than those in the other groups in the multivariate analysis after adjusting for confounding factors. The logistic regression showed that the OR for HL per $1{\mu}g/L$ increase in cadmium was 1.25 (95% CI, 1.09-1.44; p=0.002) on the multivariate analysis. Moreover, the multivariate logistic regression analyses revealed that the participants in the 4Q group exhibited a 1.59-, 1.38-, and 1.41-fold higher odds for HL than those in the 1Q, 2Q, and 3Q groups, respectively. Conclusion: High cadmium level quartile was associated with increased hearing thresholds and HL among the Korean adult population.
To investigate the exposure effect of benzene, we measured airborne benzene as external doses, uninary tt-muconic acid as an internal dose of benzene exposure and analyzed the relationship between tt-muconic acid concentration and benzene exposure. The study population of eight businesses included 157 workers(87 workers in field; exposure group, 70 workers in board; control group) who produce or use benzene in petrochemical industry. The concentrations of airborne benzene were evaluated by personal samples and urine was sampled at the pre and end shift. Urinary t,t-muconic acid as internal dose was to analyze the relationship with airborne benzene. The geometric means(GM) of airborne benzene was 0.0231 ppm(range ND-1.0471 ppm) in exposure group and 0.0147 ppm(range ND-0.3162 ppm) in control group. The geometric means(GM) of urinary t,t-muconic acid at end-shift was $196.8{\pm}2.23{\mu}g/g$ creatinine in exposure group and $149.2{\pm}2.08{\mu}g/g$ creatinine in control group. There was significant correlation between the airborne concentration of benzene and the urinary concentration of t,t-muconic acid( r=0.711, p<0.01). From the results of stepwise multiple regression analysis about t,t-muconic acid at end-shift, significant independents was airborne benzene. In this study, there were significant correlation between the urinary concentration of t,t-muconic acid and the airborne concentration of benzene. More extensive studies ruling out healthy worker effect is needed.
As mercury absorbed into body can cross the blood-brain barrier and react with DNA and RNA. Central nervous system has been known to be affected especially in children. But it was very difficult to know the influences of chronic low-does Hg exposure on the health. Although many studies investigated the affect, most of results were still disparate. In order to investigate the health effects of mercury exposure, several test were conducted for some Korean school children. The general health effects were investigated using blood test, Posturography and computer-based neurobehavioral test was done to examine the affect of Hg into neural responses. About 400 children were chosen for blood test whose blood Hg level were upper and lower 10% of population participated in the nationwide Hg exposure survey. The concentration of calcium and creatine, the number of white and red blood cell showed statistical significance with Hg exposure in blood test. Another 36 children were selected from the same participants for the posturography and neurobehavioral test. The intensity and center frequency of hand tremor which were related to unconsciousness also showed distinct significances. Any general relations with Hg exposure were not found in all test including computer-based neurobehavioral test.
Objectives: The aim of this study is to identify temporal trends in urinary cotinine levels and exposure factors among Non-smokers in Korean adults. Methods: Biological samples and questionnaire data were collected from representative Korean adults recruited in the Korean National Environmental Health Survey from 2009 to 2017. Multiple regression analysis was performed to determine the factors affecting urinary cotinine concentrations of non-smokers. Results: The urinary cotinine levels of Korean non-smokers decreased in Cycle 2 (1.04 ㎍/L, 2012-2014) compared to Cycle 1 (1.93 ㎍/L, 2009-2011) but increased slightly in Cycle 3 (1.37 ㎍/L, 2015-2017). Among the diverse variables, the main factors of cotinine exposure were secondhand smoke exposure and the presence of smokers in the family. Conclusions: The results of this study identified temporal trends in cotinine exposure among non-smokers in the Korean adult population. These findings will be used to develop further assessment and environmental health policies on secondhand smoke exposure.

Environmental tobacco smoke (ETS) is a major public health problem for all ages. Despite the high prevalence of smoking among the Saudi population, there is limited information about levels of urinary cotinine in Saudi children exposed to SHS. The aim of the study was to assess the exposure of schoolchildren to ETS, and measure their urinary cotinine levels. Multistage cluster sampling was carried out, where schoolchildren from 4 schools were randomly chosen from primary schools in Taif city. A questionnaire including questions on SHS exposure and smoking rules in the residence were sent to students parents/guardians. Urine samples were taken and analyzed for total cotinine using chemiluminescent immunoassay. Of the studied children, 38.4% had a smoking father, 61.8%, 41.2% and 49.3% of them were exposed to ETS indoors, outdoors and both indoors and outdoors respectively. The mean urinary cotinine was significantly higher among children exposed to ETS compared to unexposed children. Urinary cotinine levels in children with both indoor and outdoor exposure was significantly higher compared with its level in children with single exposure. A significant positive correlation was found between urinary cotinine concentrations and the number of cigarette packs smoked by parents, and the number of smokers in the residence. The mean urinary cotinine level was significantly higher in children who reported no smoking rules at the residence.. The study revealed a high exposure of Saudi children to ETS. An antismoking media awareness campaign on the harmful effects of ETS should be carried out, in addition to family counseling programs targeted to parents to protect their children from ETS.
