Pathogenesis and Prevention of Intraventricular Hemorrhage in Preterm Infants

  • Intraventricular hemorrhage (IVH) is a serious concern for preterm infants and can predispose such infants to brain injury and poor neurodevelopmental outcomes. IVH is particularly common in preterm infants. Although advances in obstetric management and neonatal care have led to a lower mortality rate for preterm infants with IVH, the IVH-related morbidity rate in this population remains high. Therefore, the present review investigated the pathophysiology of IVH and the evidence related to interventions for prevention. The analysis of the pathophysiology of IVH was conducted with a focus on the factors associated with cerebral hemodynamics, vulnerabilities in the structure of cerebral vessels, and host or genetic predisposing factors. The findings presented in the literature indicate that fluctuations in cerebral blood flow, the presence of hemodynamic significant patent ductus arteriosus, arterial carbon dioxide tension, and impaired cerebral venous drainage; a vulnerable or fragile capillary network; and a genetic variant associated with a mechanism underlying IVH development may lead to preterm infants developing IVH. Therefore, strategies focused on antenatal management, such as routine corticosteroid administration and magnesium sulfate use; perinatal management, such as maternal transfer to a specialized center; and postnatal management, including pharmacological agent administration and circulatory management involving prevention of extreme blood pressure, hemodynamic significant patent ductus arteriosus management, and optimization of cardiac function, can lower the likelihood of IVH development in preterm infants. Incorporating neuroprotective care bundles into routine care for such infants may also reduce the likelihood of IVH development. The findings regarding the pathogenesis of IVH further indicate that cerebrovascular status and systemic hemodynamic changes must be analyzed and monitored in preterm infants and that individualized management strategies must be developed with consideration of the risk factors for and physiological status of each preterm infant.

Postoperative delirium after cholecystectomy in older patients: A retrospective study

  • Backgrounds/Aims: Postoperative delirium (POD) is a common complication that increases mortality and morbidity in older patients. This study aimed to evaluate the clinical significance of post-cholecystectomy delirium in older patients. Methods: This retrospective study included 201 patients aged > 75 years who underwent cholecystectomy for acute or chronic cholecystitis between January 2016 and December 2019. Patients were divided into the POD (n = 21) and non-POD (n = 180) groups, and their demographic features and clinical results were compared. Results: The mean patient age was 78.88 years; the female/male ratio was 44.8%/55.2%. Laparoscopic surgery was performed in 93.5% of patients. The univariate analysis showed that lower body mass index (BMI), immobilized admission status, neuropsychiatric disease history, preoperative intervention (percutaneous drainage), high C-reactive protein, hypoalbuminemia, neutrophilia, hypo-/hyperkalemia, and longer operative time were more frequently observed in the POD group. The multivariate analysis showed that lower BMI (odds ratio [OR], 2.796; p = 0.024), neuropsychiatric disease history (OR, 3.019; p = 0.049), hyperkalemia (OR, 5.972; p = 0.007), and longer operative time (OR, 1.011; p = 0.013) were significant risk factors for POD. Conclusions: POD was associated with inflammation degree, general condition, poor nutritional status, electrolyte imbalance, and stressful conditions. Recognizing risk factors requiring multidisciplinary team approaches is important to prevent and treat POD.

Effect of Organic Substrates Mixture Ratio on 2-year-old Highbush Blueberry Growth and Soil Chemical Properties (유기자재 종류별 혼합비율이 2년생 하이부시 블루베리의 유목 생육과 토양환경에 미치는 영향)

  • The blueberry farming requires the soil condition of well-drainage, pH of 4.5 to 5.2, and high in organic matters for stable growth and development. Most of soil type of cultivated land in Korea, however, belongs to alkaline soils with low organic matter content and poor drainage. Therefore, the blueberry farmers use peat moss heavily to improve the soil condition, but the guideline on the effective and economic ratio of peat moss is not established yet. This study was performed to determine the cost effective peat moss ratio for amending soils, and to investigate the feasibility of using sawdust and coco peat as soil amendments. Peat moss, coco peat and sawdust are mixed with soil at the ratio of 0, 12.5, 50 and 100% (v/v). Among 3 organic materials with various mixture ratios, the pH of soil was the lowest in 100% peat moss and sawdust mixtures (pH 3.67 and pH 3.73, respectively), followed by pH 5.30 at 50% peat moss. The soil organic matter content are directly proportional to the mixture ratios in all three organic materials and the same trend was observed in the variation of content of exchangeable potassium in the coco peat treatments. On the contrary, the content of available phosphate, exchangeable calcium and magnesium decreased with increasing the ratio of organic materials. The nitrogen content in the leaves decreased as increasing the ratio of peat moss and coco peat in soil, but not of sawdust. The content of phosphate decreased but potassium increased as the ratio of sawdust and coco peat increased. There was no clear difference in the contents of magnesium and calcium among 3 organic materials. The plant height, stem diameter and dry weight of blueberry plants were the highest in 50 % peat moss, followed by 12.5% peat moss and 12.5% coco peat. The plants in 100% peat moss showed very poor growth. It can be concluded that peatmoss, when applied and managed appropriately, will be a good material for improving soil condition as well as securing desirable growth for blueberry. Upon coupling economic aspect, the optimum mixing ratio of peatmoss for blueberry farming is approximately 25-50%.

Effect of Soil Salinity on Growth, Yield and Nutrients Uptake of Whole Crop Barley in Newly Reclaimed Land (신간척지에서 토양 염농도가 청보리 생육, 수량 및 양분 흡수에 미치는 영향)

  • BACKGROUND: Newly reclaimed land has poor soil environment for crop growth since it is high in salt concentration but low in organic content compared with ordinary soil. It is known that whole-crop-barley can grow better in the soil of relatively high salt concentration than other crops but, the growth is poor at the concentration if higher than certain amount and it is a difficulty to secure productivity. Hence, the level of soil salt concentration suitable for the production of bulky feed in newly reclaimed land has been investigated. METHODS AND RESULTS: At Saemanguem reclaimed land, the land for the soil salt concentration electrical conductivity (EC) 0.8, 3.1, 6.5, 11.0 dS/m was selected; and chemical fertilizer $N-P_2O_5-K_2O$ (150-100-100kg/ha) was tested; and forage barley 220kg/ha were sown. The soil salt concentration during the cultivation period decreased in the order of harvest season>earing season>sowing season>wintering season, and the salt concentration in harvest season is 1.4-4.2 times higher than that of the sowing season. The higher the salt concentration, the poorer the over ground growth due to poor rooting; especially at EC 11.0 ds/m there was emergence but, it blighted after wintering. The Yield from the soil salt concentration 3.1dS/m and 6.5 dS/m was 68% and 35% from that of the soil salt concentration 0.8 dS/m (8.8 MT/ha) respectively. The proline content in early life stage was more than that of the harvest season, and it increased with salt concentration. The higher salt concentration, the more $Na_2O$ and MgO content in harvest season; but the higher the salt concentration, the less the content of N, $P_2O_5$, $K_2O$ and CaO. CONCLUSION(S): When the soil salt concentration becomes higher than 3.1 dS/m, the yield becomes poor because there is serious growth inhibition of forage barley both in root part and above aerial part that results in unbalanced absorption of nutrients. Therefore, it is recommended that the salt concentration should be lowered below 3.1 dS/m by underground drainage facilities or irrigating water for the stable production of whole-crop-barley.

Impact of Pulmonary Vascular Compliance on the Duration of Pleural Effusion Duration after Extracardiac Fontan Procedure (수술 전 폐혈관 유순도가 심장 외 도판을 이용한 Fontan 수술 후 늑막 삼출 기간에 미치는 영향)

  • Background: Preoperative risk analysis for Fontan candidates is still less than optimal in that patients with apparently low risks may have poor surgical outcome; prolonged pleural drainage, protein losing enteropathy, pulmonary thromboembolism and death. We hypothesized that low pulmonary vascular compliance (PVC) is a risk factor for prolonged pleural effusion drainage after the Fontan operation. Material and Method: A retrospective review of 96 consecutive patients who underwent the Extracardiac Fontan procedures (median age: 3.9 years) was performed. Fontan risk score (FRS) was calculated from 12 categorized preoperative anatomic and physiologic variables. PVC $(mm^2/m^2{\cdot}mmHg)$ was defined as pulmonary artery index $(mm^2/m^2)$ divided by total pulmonary resistance $(W.U{\cdot}/m^2)$ and pulmonary blood flow $(L/min/m^2)$ based on the electrical circuit analogue of the pulmonary circulation. Chest tube indwelling time was log-transformed (log indwelling time, LIT) to fit normal distribution, and the relationship between preoperative predictors and LIT was analyzed by multiple linear regression. Result: Preoperative PVC, chest tube indwelling time and LIT ranged from 6 to 94.8 $mm^2/mmHg/m^2$ (median: 24.8), 3 to 268 days (median: 20 days), and 1.1 to 5.6 (mean: 2.9, standard deviation: 0.8), respectively. FRS, PVC, cardiopulmonary bypass time (CPB) and central venous pressure at postoperative 12 hours were correlated with LIT by univariable analyses. By multiple linear regression, PVC (p=0.0018) and CPB (p=0.0024) independently predicted LIT, explaining 21.7% of the variation. The regression equation was LIT=2.74-0.0158 PVC+0.00658 CPB. Conclusion: Low pulmonary vascular compliance is an important risk factor for prolonged pleural effusion drainage after the extracardiac Fontan procedure.

Effect of Cattle Manure and Chemical Fertilizer Application on Productivity and Nutritive Value of Silage Corn in Reclaimed Paddy Field (우분퇴비와 화학비료 시용이 간척지 논에서 사일리지용 옥수수의 생산성 및 사료가치에 미치는 영향)

  • This study was carried out to examine the effect of cattle manure and chemical fertilizer application on productivity and nutritive value of silage corn in the reclaimed paddy field of Sukmoon at Dangjin Province in Korea. The cultivars of silage corn used in this experiment were Kwangpyungok and Pioneer (P32B33). The fertilizer was applied according to recommended dose of application. The reclaimed paddy field of Sukmoon was created for farming in 1985 and approximately 5 to 10 years ago rice was cultivated in full-scale. Recently, reclaimed paddy field of Sukmoon has a limitation to cultivate the crops, due to increased salinity caused by drought and poor drainage system. The total area of reclaimed paddy field of Sukmoon was 305 ha, and silage corn was sown on 149 ha of the reclaimed paddy field in nine regions, such as Songsan 28 A and B, Songsan 27 A and B, Songsan 42 A, B and C, and Songsan 43 A and B. The productivity and nutritive value of corn were evaluated collected from each of nine experimental regions. Dry matter (DM) yields of P32B33 in Songsan 28 A and B, and Songsan 27 A were 13 to 15 ton/ha and it was increased 3 to 5 ton/ha as compared to those of Songsan 27 B. DM yields of Kwangpyungok in Songsan 42 A, and Songsan 43 A and B were 23 to 27 ton/ha and it was increased 15 to 16 ton/ha as compared to Songsan 42 A and B. The nutritive value of corn was not significantly different between Kwangpyungok and P32B33, but the yield of Kwangpyungok corn silage was higher compared to P32B33. Therefore, this study suggests that good drainage system is important to maintain normal growth and productivity of corn in the reclaimed paddy field.

  • Low-Humic Gley Soils occur very commonly in Korea and constitute important paddy soils of the country. These soils are developed either on alluvial or fluvio-marine materials. The soils in the present invest igation are derived from the latter. The interesting morphological features of these two soils are the presence of $FeCO_3$ concretions in the subsoil and the occurrence of a buried black organic horizon of variable thickness(25-100 cm). The organic horizon overlies the greenish marine material and underlies the continental material. These soils have poor drainage and are wet most of the year. The presence of $FeCO_3$ concretions in poorly drained paddy soils has previously been reported in North America(8), Japan(9) and West Europe(7). The purpose of this investigation is to study the chemical and mineralogical characteristics of these soils; and the genesis of $FeCO_3$ concretions. Profile samples of both these soils were collected from Gimjae area, in co-operation with the Soil Survey Party. These profiles were described as follows:

Capillary Characteristics of Water and Cations in Multi-layered Reclaimed Soil with Macroporous Subsurface Layer Utilizing Coal Bottom Ash

  • Serious problems in reclaimed land agriculture are high soil salinity and poor vertical drainage, so desalinization in these soils is very difficult. Also, although desalinization is accomplished in reclaimed top soils, before long, soils are resalinized according to capillary rise of salts from the subsurface soils. To resolve these problems, multi-layered soil columns with subsurface layer of macroporous medium utilizing coal bottom ash (CBA) were constructed and the effects of blocked resalinization of these soils were investigated. In this experiment soil samples were collected from Munpo series (coarse-loamy, nonacid, mixed, mesic, typic Fluvaquents). The soil texture was silt loam and the EC was $33.9dS\;m^{-1}$. As for groundwater seawater was used and groundwater level of 1 cm from the bottom was maintained. The overall rate of capillary rise was $2.38cm\;hr^{-1}$ in soil 60 cm column, $0.25cm\;hr^{-1}$ in topsoil (30 cm) + CBA (5 cm) + subsurface soil (10 cm) column and $0.08cm\;hr^{-1}$ in topsoil (30 cm) + CBA (10 cm) + subsurface soil (10 cm) column. In multi-layered soil columns with CBA 20, 30 cm layer, wetting front due to capillary rise could not be seen in top soil layer. After 70 days capillary rise experiment water soluble Na+ accumulated in top soil of soil columns with CBA 20, 30 cm was diminished by 92.8, 96.5% respectively in comparison with Na+ accumulated in top soil of soil 60 cm column because CBA layer cut off capillary rise of salts from the subsurface soil. From these results we could conclude that the macroporous layer utilizing CBA placed at subsurface layer cut off capillary rise of solutes from subsurface soil, resulting in lowered level of salinity in top soil and this method can be more effective in newly reclaimed saline soil.

A case of recurrent respiratory infection resulting from a congenital anomaly of the bronchial tree tracheal bronchus (반복적인 호흡기 감염을 가진 환아에서 진단된선천성 기도 기형, 기관기관지 1예)

  • The term tracheal bronchus refers to an abnormal bronchus that comes directly off of the lateral wall of the trachea (above the carina) and supplies ventilation to the upper lobe. Tracheal bronchi occur almost exclusively on the right trachea and are associated with other congenital anomalies. In addition, tracheai bronchus may be related to other inflammatory conditions with persistent wheezing, such as recurrent pneumonia, chronic bronchitis and bronchiectasis, which is a result of the relatively poor local drainage of the involved bronchi. An infant with recurrent wheezing is likely to be a challenge for a clinician in the evaluation of the etiology of airway obstruction and in the differential diagnosis of wheezy breathing. The authors report a case of an 8-month-old female infant with a ventricular septal defect, who presented with stridor and recurrent respiratory infection and finally was finally diagnosed with a tracheal bronchus using computed tomography and a bronchoscopy. Therefore, tracheal bronchus should be included in the differential diagnosis of any child who presents with chronic or recurrent respiratory tract symptoms such as coughing, wheezing, stridor and recurrent respiratory infection, particularly in children with other congenital deformities.

Physical-chemical Properties and Phosphorus Adsorption Characteristics of Soils in Baicheng, China (중국 길림성 백성지역 흑개토의 이화학성 및 인산 흡착 특성)

  • Soil physical-chemical properties and phosphorous adsorption characteristics were investigated to obtain the informations of the appropriate fertilization and soil management in Baicheng region, China, where agricultural circumstances at present forces to consider the use of land for crop production. Soils were collected from one uncultivated and three cultivated lands on August 1993. Soil $_PH$ was very higher in uncultivated land than in cultivated land, their values were 10.2 and 7.4, respectively. Regardless of cultivation, soil organic matter contents were below 2%, and concentrations of available soil phosphorus expressed as Bray 1 P and Olson P were less than 10 mg P $kg^{-1}$, however, cation exchange capacity was higher than 20 cmol(+) $kg^{-1}$. For uncultivated soil, the values of exchangeable sodium percent and calcium saturation percent were higher than 100%. The major cation of soil saturation paste extracts was Na regardless of land use type. Based on electrical conductivity and sodium adsorption ratio of saturation paste extracts, uncultivated soil was classified as saline-sodic soil and cultivated soil was classified as sodic or normal soil. The maximum adsorption capacity of phosphorus calculated by Langmuir isotherm ranged from 406 to 521 mg P ,$kg^{-1}$. The constraints of soils in Baicheng regions for agricultural cops werw high salt concentration, unfavorable soil chemical composition such as low concentration of available phosphorous, and poor drainage due to soil dispersion by high Na concentration. Therefore, the soil in Baicheng region, need the application of phosphorus fertilizer to increase the soil fertility and the proper soil management to improve the soil physical property especially permeability and soil structure.
