식이 Vitamin E가 급성 카드뮴중독 흰쥐 간조직의 Cytochrome $P_{450}$ 함량과 Xanthine Oxidase 활성에 미치는 영향 (Effects of Vitamin E on Liver Cytochrome $P_{450}$ Content and Xanthine Oxidase Activity in Acute Cadmium-Poisoned Rats)

  • The purpose of this study was to investigate the effects of vitamin E on liver microsomal cytochrome P450 contents and xanthine oxidase activity in acute cadmium poisoned rats. Sprague Dawley male rats weighing 100$\pm$10g were randomly assigned to one normal and three cadmium injected groups. Cadmium injected groups were fed vitamin E free diet(0E Cd group), 40mg vitamin E per kg diet(40E Cd group) or 400mg vitamin E per kg diet(400E Cd group). Vitamin E level of normal group was 40mg per kg diet. Animals were injected intraperitoneally with 2.0mg Cd2+/kg bw for 4 days after the rats were fed diets with three different levels of vitamin E for 2 and 4 weeks. Body weight, food intake and feed efficiency ratio of cadmium injected animals, were decreased compared with those of normal group. The weights of liver and kidney in cadmium injected groups were not different from those of normal group. Cadmium contents of liver in cadmium groups were 160 fold higher those that of normal group. Accumulation of cadmium poisoned rat liver was reduced by vitamin E supplementation. Contents of blood hemoglobin and hematocrit in 0E Cd groups were decreased to 2~ 14% of those of the other groups. Contents of serum triglyceride in all experimental groups were not significantly different each other. Levels of serum total cholesterol, LDL cholesterol and antherogenic index in 0E Cd and 40E Cd groups were higher than those of normal group, while the contents of HDL cholesterol in 0E Cd and 40E Cd groups were lower than those of normal group. Xanthine oxidase(XOD) activity and cytochrome P450 contents in the liver were significantly increased in cadmium injected groups but these were reduced by vitamin E supplementations. The present results indicate that acute cadmium poisoning in rats causes increasing free radical generation systems in the liver and that leads to liver tissue damage. But these abnormalities can be reduced by dietary vitamin E supplementations.

납 중독된 흰쥐에서 식이 단백질 수준 및 제독기간에 따른 제독효과 비교 연구 (Detoxification Study with Different Dietary Protein Levels and Detoxifying Periods in Lead Poisoned Rats)

  • This study was performed to investigate lead(Pb) detoxification with different dietary protein levels and detoxifying periods in Pb poisoned rats. Forty nine male rats of Sprague-Dawley strain weighing 114$\pm$4g were used and they were fed 1% Pb-15% casein diet ad libitum for 2 weeks. After sacrifying 7 animals as control group, remaining forty two rats were blocked into 6 groups according to body weight. Three groups were fed each detoxifying diet(5%, 15% or 40% casein diet without Pb) for 1 week and the other 3 groups were fed same diets for 3 weeks. The results obtained were summerized as follows ; 1) Liver and kidney weight, bone weight, and bone length were increased with increasing dietary protein level and detoxifying period. 2) Hematocrit values were recovered to normal level during detoxifiying periods but hemoglobin contents in detoxifying diet groups showed no significant difference from those in control group. 3)Pb contents in blood were decreased with increasing dietary protein level, and Pb contents in liver were significantly decreased in high protein diet groups. Pb contents in kidney were significantly decreased during detoxifying period compared to control group, but no significant difference was shown within the detoxifying diet groups. Pb contents in femur were significantly decreased after 3 weeks of detoxifying period but no difference was shown with different dietary protein level. 4) Urinary Pb excretions were significantly increased with increasing dietary protein level. Fecal Pb excretions were remarkably decreased after 3 weeks of detoxifying period, but no difference was showen in fecal Pb excretions with different dietary protein level. Above results suggest that high protein diet improves Pb detoxification by increasing urinary Pb excretion.

한국산 희귀조류 수 종에 대한 중금속 축적 (Heavy Metal Accumulation in Some Korean Rare Bird Species)

  • 본 연구는 여러 가지 원인으로 사망한 한국산 희귀조류 4목 16종 76개체를 이용하여 납과 카드뮴의 조직 축적레벨을 밝히고 오염수준을 비교 검토하였다. 남의 경우 전반적으로 간보다 신장과 뼈에서 높은 농도를 나타냈다. 간조직에는 평균농도는 0.47 ㎍/wet g(큰고니 Cygnus cygnus)∼3.74 ㎍/et g(쇠부엉이 Asio flammeus)의 범위를 나타냈고, 납의 독성 여부를 수준별로 보면 비오염수준인 2 ㎍/wet g 이하에 해당하는 개체가 51.3%(39개체), 오염수준이 10 ㎍/wet g 이상에 해당하는 개체가 2.6%(2개체)로 나타났다. 따라서 상당수의 한국산 희귀조류가 만성적인 납 오염에 노출되어 있는 것으로 판단된다. 카드뮴의 경우 신장조직에서 가장 높게 나타났으며 간과 뼈에서는 비슷한 경향을 보였다. 신장조직에서의 평균농도는 0.06 ㎍/wet g(수리부엉이)∼1.75 ㎍/wet g(새홀리기 Falco subbuteo)의 범위를 나타냈고 소쩍새 1개체를 제외하고 모두 비오염 수준에 해당하였다.

모 산업장에서 발생한 연중독자에 대한 D-Penicillamine의 치료효과 (Effects of Oral D-Penicillamine in Treatment of Industrial Lead Poisoning)

  • For the purpose of the curative effects of oral D-penicillamine in lead poisoning, D-penicillamine was orally administered to 7 lead poisoned workers which were employed in glaze product industry dealing with the lead oxide ($Pb_3O_4$). The doses of D-penicillamine was 1,200mg per day which was administered by oral 7days schedules, taking for 5 days and stopping for the following 2days, repeatedly during 3 months period. (All the poisoned workers started working again in that industry after 1 month treatment, and were treated by oral D-penicillamine for 2 months still being exposed to contaminated environment.) In order to evaluate the curative effects of D-penicillamine, 10gm of whole blood and 24 hours urine were collected every 14 days during the curative period for laboratory analysis(hemoglobin, blood lead, urine $\sigma$-aminolevulinic acid, urine coproporphyrin, and urine lead levels) with the observation of the clinical symptoms. The results were as follows; 1. Oral D-penicillamine effected good curative results as that hemoglobin, blood lead, urine $\sigma$-aminolevulinic acid, and urine coproporphyrin levels were decreased below the critical level within 1 month treatment. 2. After re-exposure, oral D-penicillamine effected to some extent as that urine lead level was decreased below the critical level after 3 months treatment with disappearence of the clinical symptoms after 2 months treatment. However, the curative effects of oral D-penicillamine in the lead exposure state is questionable since increasement of blood lead level and remarkable decreasement of urine lead level after 3 months treatment can be observed.

식이 Vitamin E가 급성 카드뮴중독 흰쥐 간조직의 항산화계에 미치는 영향 (Effects of Vitamin E on Antioxidative Defense System of Liver in Acute Cadmium-Poisoned Rats)

  • The purpose of this study was to investigate the effects of vitamin E on antioxidative defense system of liver in acute cadmium poisoned rats. Sprague-Dawley male rats weighing 100$\pm$10gm were randomly assigned to one control and three cadmium injected groups. Cadmium injected groups were fed vitamin E free diet(OE-Cd group), 40mg vitamin E per kg diet(40E-Cd group) or 400 mg vitamin E per kg diet(400E-Cd group). Vitamin E level of normal group was 40mg per kg diet. Animals were injected intraperitoneally with 2.0mg Cd$^2$$\^$+//kg bw for 4 days after the rats were fed diets with three different levels of vitamin E for 2 and 4weeks. Activities of superoxide dismutase(SOD), glutathione peroxidase(GSHPx) and glutathione S-transferase(GST) were decreased in cadmium injected groups but those were significantly improved by dietary vitamin I supplementations. Vitamin E contents reduced glutathione(GSH) in the live were decreased in cadmium injected groups, but we., not significantly different among three groups with different levels of vitamin E supplementations. Contents of liver thiobarbituric acid reactive substance (TBARS) of 0E-Cd group were higher than those of 400E-Cd and 400E-Cd groups, but those were markedly alleviated according to vitamin E supplementations. These results indicate that cadmium poisoning in rats causes decreasing antioxidative defense system and increasing peroxidative damage in liver, however can be restored by vitamin E supplements. Pb 중독에 Fe, Cu가 흰쥐의 체내에 미치는 효과에 관한 연구 (The Effect of Fe and Cu on the Pb Toxicity in Rats)

  • This study was undertaken to investigate the effects of dietary Fe and Cu levels on Pb accumulation of Pb poisoned rats. 80 male Sprague weaning Dawley rate weighing 80-90g were divided into 8 groups(Pb groups: LFLCPb-low Fe, low Cu and Pb group, LFACPB-low Fe, adequate Cu and Pb group, AFLCPb-adequate Fe, low Cu and Pb group, AFACPb-adequate Fe, adequate Cu and Pb group, without Pb gorups: LFLC-low Fe, low Cu and without Pb grooup, LFAC-low Fe, adequate Cu and without Pb group, AFLC-adequate Fe, low Cu and without Pb group, AFAC-adequate Fe, adequate Cu and without Pb group) according to Pb administration (0, 500ppm in drinking water) and Fe and Cu levels(Fe :6ppm, 40ppm, Cu : 0.5ppm, 0.8ppm) for 12 weeks. The food intake, body weight, gain, and FER of Pb groups were significantly lower than those of without Pb groups(p<0.01, p<0.001, p<0.001). The weights of liver in Pb groups were significantly lower than that of adequate Fe and Cu group in without Pb group(AFAC) (p<0.001). The Cu contents of liver in Pb groups were significantly lower than that of AFAC (p<0.01). The liver Pb accumulation of LFLCPb was significantly higher than those of LFACPb, AFLCPb, AFACPb(p<0.05). The serum Pb content of LFLCPb was significantly higher than those of LFACPb, AFLCPb, AFACPb(p<0.05). The levels of Hb, and Hct, of 12 weeks in Pb poisoned rats were lower than those of 8 weeks, in other words, long term Pb poisoned rats were affected more serious on hematoopoiesis. The fecal excretions of Fe and Cu in Pb groups were significantly higher than those of without Pb groups(p<0.001, p<0.001). The urinary Fe and Cu excretions were significantly higher than those of AFAC(p<0.001, p<0.001). There were significant differences between Pb groups and without Pb groups according to dietary Fe and Cu levels. So, dietary levels of adequate Fe and Cu were effective to reduce Pb accumulation in rats. It were showed that the decrease of food intake, FER, body weight gain, serum level of Fe and Cu in Pb groups than those of without Pb groups. And the increase of liver Pb accumulation, serum Pb levels, fecal and urinary Fe and Cu excretions were showed in Pb groups than those of without Pb groups by long term lead poisoning. Otherwise, adequate Fe and Cu administrations in Pb groups have preventive effects on the Pb poisoned rats.

Effect of Chlorella on Metallothionein Synthesis and Binding Capacity of Cadmium in Cadmium Poisoned Rat Liver and Kidney

  • The rate of metallothionein synthesis on cadmium-poisoned rats reflects the level of toxicity, and also it reduces the toxicity which is caused by the uptake of cadmium. Chlorella supplementation in the diets of the cadmium-poisoned rats decreased the concentration of cadmium in blood and urine compared with the control group. Although the liver and kidneys of rats are major target organs of cadmium and coherence of metallothionein and cadmium, no previous study has determined the correlation between the rate of metallothionein synthesis in the liver and kidneys of rats and dietary supplementation of chlorella with cadmium uptake. This study analyzed total metallothionein level on the tissue of the liver and kidneys, the concentration of cadmium bound to the metallothionein, and the total concentration of cadmium on the tissue of the liver and kidneys after dietary supplementation with 1%, 5%, and 10% dried chlorella and 40 ppm of cadmium to 46 male SD rats (mean weight: $150\pm20\;g$) for 4 weeks. According to the data analysis of the total rate of metallothionein synthesis in the liver and kidneys, the group of SD rats on the supplementation with 1% chlorella and 40 ppm of cadmium showed a rate of $93.2\pm8.9\;ng/g$, a significant decrease of 58.8% compared to that of the control group of SD rats on the supplementation with cadmium only, which showed a rate of $227.3\pm32.5 ng/g$ (P=0.0001). In contrast, no significant difference was observed through the changing of chlorella concentrations between 5% and 10% chlorella supplementation with cadmium. The group supplemented with 1% or greater chlorella levels represented a greater decrease in the total cadmium concentration of the kidney and liver tissues, the amount of total metallothionein synthesis, the amount of metallothionein with binding to cadmium, and the concentration of free cadmium without binding to metallothionein. Consequently, the supplementation of 1% and 5% chlorella was effective in reducing the synthesis of metallothionein for cadmium uptake, but increased the rate of binding of cadmium to metallothionein.

Effects of Vitamin E on the Changes of Mineral Contents in Chronic Cadmium-Poisoned Rats

  • This study was carried out to investigate the effects of vitamin E on the cadmium contents of bone and on the calcium and phosphorous contents of the blood, urine and feces. Male Sprague-Dawley rats weighing 100$\pm$10g were randomly assigned to one normal group and three cadmium poisoned groups. The cadmium poisoned groups consisted of a vitamin E free diet (Cd-0E) group; a 40 mg vitamin E /kg diet (Cd-40E) group; and a 400 mg/kg diet (Cd-400E) group. Experimental animals were maintained on their respective diets for 20 weeks and were simultaneously administered 50 ppm $Cd^{2+}$ dissolved in the drinking water. At the end of the trial, the average hematocrit value in the Cd-0E group was 28.13% lower than in the normal group. However, the average hematocrit value in the Cd-400E group was significantly higher than in the Cd-0E and Cd-40E groups. WBC levels in the cadmium-poisoned groups were lower than in the normal group, but Cd-400E group levels were significantly higher than in the Cd-0E and Cd-40E groups. The contents of calcium of tibia has no significant difference between normal group and cadmium exposed group at $10^{th}$ week After 20 weeks, the calcium contents of the tibia in the Cd-0E and Cd-40E groups were lower than in the normal group by 25.5% and 22.1 %, respectively, although the calcium contents of the tibia in the Cd-400E group were higher than in the normal group. After 10 weeks, the calcium contents of the femur in the Cd-0E and Cd-40E groups were 19.25% and 15.45% lower than in the normal group, respectively, but the calcium contents of the femur in the Cd-400E group were at the same levels as in the normal group. The levels of calcium in the femur after 20 weeks were similar to the 10-week levels. Calcium levels of the urine in the Cd-0E and Cd-40E groups were 3.92 fold and 2.92 fold higher, respectively, than in the normal group, but levels in the Cd-400E group were significantly lower than in either the Cd-0E group or the Cd-40E group. Calcium levels of the feces in cadmium-poisoned groups were significantly higher than in the normal group, although levels in the Cd-400E group were significantly lower than in the Cd-0E and Cd-40E groups. Phosphorous levels of the blood in the Cd-0E group were 17% lower than in the normal group, although levels in the Cd-400E group were significantly higher than in the Cd-0E group. Phosphorous levels of the urine in the Cd-0E and Cd-40E groups were significantly higher than in the normal group, while Cd-400E group levels were found to be at the same level as in the normal group. Cadmium contents of the tibia in the Cd-40E and Cd-400E groups were 13% and 17% lower, respectively, than in the Cd-0E group. Regarding cadmium levels in the femur, only the Cd-400E group achieved lower levels (10% lower) than the Cd-0E group. In conclusion, vitamin E supplementation resulted in a suppression of the release of calcium from bone, and a reduction in the excretion of calcium via the urine and feces, thus having a normalizing effect on calcium metabolism in rats with chronic cadmium poisoning.

글라이포세이트 중독 환자에서 포함된 염의 종류에 따른 예후의 차이 (The Prognosis of Glyphosate herbicide intoxicated patients according to their salt types)

  • Purpose: Glyphosate herbicide (GH) is a widely used herbicide and has been associated with significant mortality as poisoned cases increases. One of the reasons for high toxicity is thought to be toxic effect of its ingredient with glyphosate. This study was designed to determine differences in the clinical course with the salt-type contained in GH. Methods: This was a retrospective study conducted at a single hospital between January 2013 and December 2017. We enrolled GH-poisoned patients visited the emergency department. According to salt-type, patients were divided into 4 groups: isopropylamine (IPA), ammonium (Am), potassium (Po), and mixed salts (Mi) groups. The demographics, laboratory variables, complications, and their mortality were analyzed to determine clinical differences associated with each salt-type. Addtionally, we subdivided patients into survivor and non-survivor groups for investigating predictive factors for the mortality. Results: Total of 348 GH-poisoned patients were divided as follows: IPA 248, Am 41, Po 10, and Mi 49 patients. There was no difference in demographic or underlying disease history, but systolic blood pressure (SBP) was low in Po group. The ratio of intentional ingestion was higher in Po and Mi groups. Metabolic acidosis and relatively high lactate level were presented in Po group. As the primary outcome, the mortality rates were as follows: IPA, 26 (10.5%); Am, 2 (4.9%); Po, 1 (10%); and Mi, 1 (2%). There was no statistically significant difference in the mortality and the incidence of complications. Additionally, age, low SBP, low pH, corrected QT (QTc) prolongation, and respiratory failure requiring mechanical ventilation were analyzed as independent predictors for mortality in a regression analysis. Conclusion: There was no statistical difference in their complications and the mortality across the GH-salt groups in this study.

체외 생명유지술을 이용한 중독 환자의 치료: 체계적 고찰 (Extracorporeal Life Support in Treatment of Poisoning Patient: Systematic Review)

  • Purpose: Extracorporeal life support (ECLS) is a term used to describe a number of modalities including extracorporeal membrane oxygenation (ECMO) to support cardiac and/or pulmonary systems. The purpose of this study is to review the available evidence regarding the effect of ECLS in patients with acute poisoning. Methods: Electronic literature searches with PubMed, Embase, Cochrane library, and KoreaMed were conducted for identification of relevant studies addressing ECLS in treatment of acute poisoning. The literature search was conducted by two investigators in March, 2016 with publication language restricted to English and Korean. The toxic substance, technique of ECLS, and final outcome of each case were analyzed. Results: The final analysis included 64 articles including 55 case reports. There were no articles classified according to a high level of evidence such as randomized trial and prospective cohort study. ECLS treatment was used in the management of patients poisoned with 36 unique substances. Venovenous ECMO was performed in 4 cases. Among the reported cases, 10 patients died despite treatment with ECLS. Conclusion: Evidence supporting ECLS for patients with acute poisoning is inadequate. However, many case reports suggest that early consideration of ECLS in poisoned patients with refractory cardiac arrest or hemodynamic compromise refractory to standard therapies may be beneficial.
