Systemic review: The study on Rhinitis with acupuncture in PubMed and Chinese medical journals (비염(鼻炎)의 침구치료(鍼灸治療)에 대한 최근(最近) 연구(硏究) 동향(動向) -Pubmed와 중의학(中醫學) 학술지(學術紙)를 중심으로-)

  • Objective: To research the trends of study related to rhinitis and acupuncture in PubMed, and to establish the hereafter direction of treating rhinitis with acupuncture Methods: We searched PubMed and chinese medical journals related to rhinitis and acupuncture. Results: 1. The pattern of the study was as follows: Review article(3), Randomized controlled trials(2), Clinical trial(11), Case report(12). 2. The effect of acupuncture on rhinitis is reported as follows: Acupucture treatment improves the scale of symptoms, nasal airways resistance and velocity of the mucociliary transport, decreasing absolute numbers of blood eosinophils, serum IgE and percentage of nasal eosinophils. Immunologically acupuncture treatment could reduce plasma IL-10 level, control IL-2, and balance between cell-specific pro-inflammatory and anti-inflammatory cytokines, TNF-${\alpha}$ and IL-10. After acupuncture treatment, there is statistically significant changes in IgA, IgE, E-rosette formative rate. 3. Many of these article have affirmative view for therapeutic effect of rhinitis with acupuncture. Statistical test was done only in 6 papers. There showed statically significant results in 4 articles, and in 2 article there showed some clinical improvement but no statically significant changes. 4. In Western countries, alternative treatments are frequent among adults with rhinitis or other allergic disease, and affirmative tendency for acupuncture treatment is increased.

The Prevalence and Associated Factors of the Metabolic Syndrome in Pre-menopausal Housewives: An Analysis of the 2010~2015 Korean National Health and Nutrition Examination Survey (폐경 전 전업주부의 대사증후군 관련 요인: 2010~2015년 국민건강영양조사자료분석)

  • Purpose: The purpose of this study is to estimate the prevalence of the metabolic syndrome in pre-menopausal housewives and to explore controllable and uncontrollable factors regarding metabolic syndrome. Methods: The study population of this cross-sectional survey was from the Korean Health and Nutrition Examination Survey (KHANES) 2010 through 2015, including the fifth and sixth population-based studies. The criteria for metabolic syndrome include waist circumference, blood pressure, fasting plasma glucose, triglyceride, high-density lipoprotein (HDL) based on Korean Clinical Practice Guideline for Metabolic Syndrome by the Korean Academy of Family Medicine 2015. Results: Among the 2,498 subjects, 247 subjects had metabolic syndrome and the prevalence was estimated to be 9.9%. The number of subjects who met the criterion of HDL was 936 (36.2%), which was the most prevalent among the criteria for metabolic syndrome. Statistically significant (p<.05) factors include age, livinghood benefit group, perceived health status, obesity, family history of DM, sleeping time, awareness of stress,leukocyte, and erythrocyte count. The odds ratio of obesity in the BMI ${\geq}25$ group was 12.59 times as high as that of the BMI <25 group (p<.001) for metabolic syndrome. Conclusion: The prevalence of metabolic syndrome in pre-menopausal housewives in the survey was not low, and it is necessary to develop and apply comprehensive health habit management programs to improve controllable factors including exercise and food intake.

The Influence of the Risk Factors and Nutritional Status on the Development of Pressure Sores for the Risk Patients in ICU (욕창발생위험요인과 영양상태가 중환자실 입원환자의 욕창발생에 미치는 영향 - 욕창발생위험집단을 중심으로 -)

  • Pressure sores are a serious concerns in that respect to increasing risk of medical complications and medical costs. Prevention and care of pressure sores is an essential area of nursing practice. The nurse at ICU should be more careful of maintaining the skin integrity of patients especially than at any other place. This study was conducted to determine if the risk facotrs of pressure sores and nutritional status of the patients at risk for pressure sores is related the occurance of pressure sore. The risk group refers the patients having the below 14 scores of the braden scale. The 100 subjects were recruited from the ICU ward at an university hospital in Choongnam. The parameters for nutritional status are the blood chemistry including plasma protein, albumin, hemoglobin and the anthropometric measurements consisting of weight, BMI, LBM, the proportion of body fat, body fluid and triceps skin fold using bioimpedence analizer and caliper. The results are as follows : 1. The subjects were 55 years and stayed 8 days on average. Of the 100 subjects, males were 61%, neurologic/neurosurgical diseases were 68% and the incidence of pressure sores was 17% mainly occuring within 3days after the admission. 2. The present paralysis(or paraplegia) and edema(arm, leg, trunk) were showed more significantly the subjects with pressure sores than those without pressure sores. 3. Regarding with the nutritional status, the subjects with pressure sores had significantly lower the weight, BMI, LBM, body fluid, albumin than the ones without pressure sores. This results were supported the reports of previous studies that the decreased weight and albumin could be the important predictors of pressure sores. Thereafter we should encourage these factors to be utilized in predicting pressure sores for a comprehensive assessment. Nurse should identify patients at risk of the development of pressure sores, assess their nutritional status and dietary intake at regular intervals.

  • The distribution of barbiturates in the model membranes of total lipids (SPMVTL) and total phospholipids (SPMVPL) extracted from synaptosomal plasma membrane vesicles was determined by employing a fluorescent probe technique. The two fluorescent probes 2-(9-anthroyl)stearic acid and 12-(9-anthroyl)stearic acid were utilized as probes for the surface and the hydrocarbon interior of the outer monolayer of the SPMVTL and SPMVPL, respectively. The Stern-Volmer equation of fluorescent quenching was modified to calculate the relative distribution. The analysis of preferential quenching of these probes by barbiturates indicates that pentobarbital, hexobarbital, amobarbital and phenobarbital are predominantly distributed on the surface area, while thiopental sodium has an accessibility to the hydrocarbon interior of the outer monolayer of the SPMVTL and SPMVPL. From these results, it is strongly suggested that the more effective penetration into the hydrocarbon interior of the outer monolayer of the membrane lipid bilayer could result in a higher general anesthetic activity.

  • Noninvasive prenatal test (NIPT) is a novel screening method for the diagnosis of fetal chromosomal aneuploidies. NIPT is based on technology that detects cell-free fetal DNA in maternal plasma and analyzes it with massively parallel sequencing technology to determine whether the fetus is at risk of trisomy 21, trisomy 18, trisomy 13 or sex chromosome abnormalities (SCAs). NIPT has been reported to have sensitivity of 99% and a false positive rate of less than 1% for detecting trisomy 21 and trisomy 18. Although extension of the application of NIPT to other SCAs has been attempted, there are concerns in extending NIPT to SCAs because of maternal or fetal mosaicism, undetected maternal SCAs, and multiple pregnancies. Recently, we assessed a pregnancy with the rare Turner syndrome mosaicism 45, X/47, XXX, which was reported as 45, X with NIPT. We present the case here and briefly review the current literatures on NIPT in testing for fetal monosomy X. To the best of our knowledge, this is the first report of the 45, X/47, XXX mosaicism in Korea to be reported as 45, X by NIPT with whole genome sequencing. This case report will provide valuable information for counseling women who want to undergo NIPT.

Conformational Study of Human Serum Albumin in Pre-denaturation Temperatures by Differential Scanning Calorimetry, Circular Dichroism and UV Spectroscopy

  • Thermal conformational changes of human serum albumin (HSA) in phosphate buffer, 10 mM at pH = 7 are investigated using differential scanning calorimetric (DSC), circular dichroism (CD) and UV spectroscopic methods. The results indicate that temperature increment from $25^{\circ}C$ to $55^{\circ}C$ induces reversible conformational changes in the structure of HSA. Conformational change of HSA are shown to be a three-step process. Interestingly, melting temperature of the last domain is equal to the maximum value of fever in pathological conditions, i.e. $42^{\circ}C$. These conformational alterations are accompanied by a mild alteration of secondary structures. Study of HSA-SDS (sodium dodecyl sulphate) interaction at $45^{\circ}C$ and $35^{\circ}C$ reveals that SDS affects the HSA structure at least in three steps: the first two steps result in more stabilization and compactness of HSA structure, while the last one induces the unfolding of HSA. Since HSA has a more affinity for SDS at $45^{\circ}C$ compared to $35^{\circ}C$, It is suggested that the net negative charge of HSA is decreased in fever, which results in the decrease of HSA-associated cations and plasma osmolarity, and consequently, heat removal via the increase in urine volume.

  • Podocytopathy is glomerular lesions characterized by podocyte injury. It is observed in various glomerular diseases, but minimal change disease and focal segmental glomerulosclerosis (FSGS) are the prototypes. In this review, morphologic features of podocyte injury and subtypes of FSGS will be reviewed briefly. Effacement of podocyte foot processes is the most common feature of podocyte injury. As podocytic injury progresses, intracytoplasmic vacuoles, subpodocytic cyst, detachment of podocytes from the glomerular basement membrane and apoptosis develop. Glomerular capillary loops in epithelium-denuded area undergo capillary collapse. Synechia and hyalinosis may accompany this lesion. To manifest segmental sclerosis, podocyte loss above a threshold level may be required. Injured podocytes can injure neighboring intact podocytes, and thereby spread injury within the same lobule. FSGS can be categorized into five subtypes by morphologic characteristics; not otherwise specified (NOS), perihilar, cellular, tip, and collapsing types. Each subtype has been reported to show different clinical courses and associated conditions, but there are controversies on its significance. With recent progress in the discovery of genetic abnormalities causing FSGS and plasma permeability factors, we expect to unravel pathophysiology of FSGS and to understand histological sequences leading to FSGS in near future.

Apoptosis-Inducing Activity of Galloylglucoses from Juglans mandshurica in Human Promyeloid Leukemic HL-60 Cells

  • Two galloyl monosaccharides, 1,2,6-trigalloylglucose (1, TRgG) and 1,2,3,6- tetragalloylglucose (2, TEgG), were isolated from the stem-bark of Juglans mandshurica. Two galloylglucoses showed cytotoxic effects on human promyelocytic leukemia HL-60 cells. In order to elucidate their mechanism of action, we have investigated the flow cytometric analysis after Annexin V-FITC and PI staining, caspase-3 activity, and internucleosomal DNA fragmentation in HL-60 cells. HL-60 cells treated with both compounds 1 and 2 at 150 and $100\;{\mu}M$, respectively, led to a morphological features of apoptosis, such as plasma membrane blebbing and cell shrinkage. TRgG (1) and TEgG (2) increased the percentage of $FITC^+\;and\;FITC^+PI^+$ cells in flow cytometry after Annexin V-FITC and PI staining. The increase of apoptotic cells was preceded by the activation of caspase-3 reported to play a central role in apoptotic process and inducing internucleosomal DNA fragmentation. TEgG (2) showed to have stronger apoptosis inducing activity in HL-60 cell lines as compared with TRgG (1).

Hepatitis C Virus Associations with Non Hodgkin's Lymphoma: Insights on Inflammation/Angiogenesis and CD Markers

  • We aimed to investigate any association between hepatitis C virus (HCV) infection and non-Hodgkin's lymphoma (NHL) in the view of cytokines that control inflammation/angiogenesis and their correlation with certain CD markers. NHL patients with or without HCV infection were studied. CD5, CD30, CD3, CD20 and CD45 were immunohistochemically evaluated. Plasma levels of vascular endothelial and platelet derived growth factors (VEGF, and PDGF), tumor necrosis factor (TNF-${\alpha}$), transforming growth factor (TGF-${\beta}$), interleukin-6 (IL-6), IL-8, IL-4, IL-12 and interferon gamma (IFN-${\gamma}$) were detected by enzyme-linked immunosorbent assay (ELISA). HCV+ve NHL patients showed a significant reduction in VEGF, PDGF, IFN-${\gamma}$, CD5 and CD45 and a significant increase in IL-12 and IL-8. In conclusion, there was a significant change in cytokine secretion and expression of CD markers in HCV+ve NHL patients. Based on our results, HCV infection in NHL patients requires more in-depth investigations to explore any role in lymphoma progression.

Development of Anti-Obesity Agent from Resource Plants

  • Obesity is a physical condition that results from excessive storage of fat in the body. The present study examined the anti-obesity effects of the selected natural medicine, Galla rhois extract (GRE) and solvent fractions on 3T3-L1 preadipocytes and in vivo studies. Here, we show that EtOAc fraction of Galla rhois inhibits the differentiation of the 3T3-L1 preadipocytes induced by differentiated medium in a dose-dependent manner. To investigate the effect of the GRE-EtOAc fraction on obesity in high fat diet-fed C57BL/6 mice, which included a normal diet (ND), high-fat diet (HFD) and HFD+GRE concentration-dependent, were fed to the mice for 6 weeks. The GRE-EtOAc fraction was inhibited the highest adipocyte differentiation in vitro, the GRE supplement significantly decreased body weight and visceral fat mass compared to the HFD group. The total cholesterol and triglyceride levels in the plasma were significantly decreased by GRE supplementation compared with those of the HFD group. Also, we aimed to determine the differentiation inhibition and the modulation of differentiation genes brought about by the Galla rhois in adipocyte. A cDNA microarray-based method was introduced for the high contents screening (HCS) of gene expressions. This technology has revolutionized gene expression studies by providing the means to measure mRNA levels in thousands of genes simultaneously in simple and complex biological samples. 13 genes were founded to be affected in their expression levels by more than 5-fold up-regulation after 4 days treatment with the EtOAc fraction from Galla rhois. Otherwise, 21 genes were founded to be affected in their expression levels by more than 5-fold down-regulation treated with the EtOAc fraction. Therefore, Galla rhois extract may be considered for use in a therapeutic agent to control obesity.
