Predicting Arachnoid Membrane Descent in the Chiasmatic Cistern in the Treatment of Pituitary Macroadenoma

  • Objective : Preoperative prediction of the arachnoid membrane descent in pituitary surgery is useful for achieving gross total removal and avoiding cerebrospinal fluid leakage resulting from tearing of the arachnoid membrane in the chiasmatic cistern. In this study, we analyzed the patterns of arachnoid membrane descent during or after pituitary tumor surgery and identified the factors related to this descent. Methods : Analysis was restricted to pituitary macroadenomas not extending into the third ventricle or over the internal carotid artery. To minimize confounding factors, patients who underwent revision surgery, those who had a torn arachnoid during operation or small medial diaphragma sellae (DS) opening, and subtotal resections were excluded. We enrolled 41 consecutive patients in this retrospective analysis. The degree of arachnoid descent was categorized using intraoperative videos. Preoperative magnetic resonance findings, including tumor height, suprasellar extension, and variables including DS area and medial opening size, tumor composition, and displacement of the pituitary stalk and gland were evaluated to determine their correlations with arachnoid membrane descent. Results : Arachnoid membrane descent was significantly correlated with DS area and medial opening size. Based on T2-weighted images (T2WI) magnetic resonance (MR) images, tumor composition was significantly associated with arachnoid membrane descent. Other factors were not significantly correlated with arachnoid membrane descent. Conclusion : T2WI of tumor composition and preoperative MR imaging of DS area and medial opening provided valuable information regarding arachnoid membrane descent. These parameters may serve as fundamental measures to facilitate complete resection of pituitary macroadenomas.

A Case of Unusual Pituitary Apoplexy Presented as Aseptic Meningitis (무균성 수막염으로 발현한 흔하지 않은 뇌하수체졸중)

  • We encountered a case of pituitary apoplexy who presented with isolated headache and vomiting without visual disturbance or ophthalmoplegia. The cerebrospinal fluid examination was compatible with aseptic meningitis. A computed tomography revealed slightly high density in the pituitary fossa and suprasella area, but the signal change was very faint. Our case suggests that clinicians should take into account the possibility of pituitary apoplexy without visual disturbance or ophthalmoplegia, when aseptic meningitis is suspected.

Variations of Gonadotropin Subunits mRNA Levels at Different Stages of Ovarian Development in Masu Salmon, Oncorhynchus masou

  • The variations of gene expression and pituitary contents of GTH subunits during the ovarian development of masu salmon, Oncorhynchus masou, were investigated. The pituitary GTHs contents was measured by radioimmunoassays (RIAs) using purified GTH subunits and their antibodies. Pituitary contents of GTH $I\beta$ gradually increased from April through August, and reached the maximum in October. On the other hand, pituitary contents of GTH $II\beta$ remained low until August, but they rapidly increased in October. Total RNAs were prepared from pooled pituitaries and the GTH subunits mRNA in pituitaries was quantified by Northern blot hybridization using masu salmon cDNA probes for each GTH subunit. GTH $I\beta$ mRNA level increased with the progression of ovarian maturity. However, GTH $II\beta$ mRNA was detected only at a more advanced stage, and were extremly high at ovulation. A high levels for GTH a mRNA was detected only at ovulation stage. The synchronous increase in pituitary contents and mRNA levels suggested that ovarian maturity in masu salmon was regulated by both GTH I and GTH II.

  • Gonadotropin (GTH) is a pituitary glycoprotein hormone that regulates gonadal development in vertebrates. In teleosts, two types of gonadotropins, GTH-I and GTH-II, are produced in the pituitary, and they comprised of common ${\alpha}$ and distinct ${\beta}$ subunits. In the present study, the cDNAs encoding GTH ${\alpha}\;and\;GTH-II{\beta}$ subunits were cloned and sequenced from flounder (Paralichthys olivaceus) pituitary cDNA library. The nucleotide sequence of the a subunit was 619 bp long, encoding 124 amino acids, and that of the $GTH-II{\beta}$ subunit was 538 bp long, encoding 145 amino acids. GTH subunits had well conserved cysteines, when aligned with other members of the glycoprotein family. The ${\beta}$ subunit of gonadotropin II ($GTH-II{\beta}$) had a different N-linked glycosylation site. RT-PCR analysis showed an increase of GTH II mRNA levels in association with gonadal development, and also showed that the mRNA expression of the ${\alpha}$ subunit was detected only in tissues from pituitary glands.

  • Addisonian crisis, also commonly referred to as adrenal crisis, occurs when the cortisol produced by the adrenal gland is insufficient to meet the body's needs. Pituitary apoplexy usually occurs as hemorrhagic and ischemic necrosis in the presence of a pre-existing pituitary adenoma, and is a rare sequela of cardiovascular surgery. Most pituitary apoplexy that happens in cardiovascular surgery has been known to be related to harmful effects of the cardiopulmonary bypass. The case presented herein illustrates occult pituitary apoplexy that occurred after off-pump coronary artery bypass grafting. In this patient, . the initial signs of addisonian crisis was similar to those of septic shock, and were overlooked. However, once recognized, they were reduced dramatically with standard stress-dose cortisone.

  • The release of ovulatory level of LH from the pituitary gland occurred between 51 and 56 hours after PMS treatment in 24-day old female rats. Estradiol given simultaneously with PMS advanced LH release 24 hours. Injections of estradiol $(2.5\\sim 40\\mu g)$ at 0, 24 and 48 hours failed to increase pituitary LH level by 51 hours after the first injection in ovariectomized rats. However, $5 \\mu g$ estradiol at 0, 24 and 48 hours followed by 1 mg progesterone at 48 hours elevated pituitary L level by 51 hours in ovariectomized rats. These results indicate that advancement of PMS-induced ovulation by estradiol in the previous study occurred by means of inducing premature release of LH and estrogen might synergize with progesterone in the regulation of LH in the pituitary gland.

  • The present study is involved with the prolactin secretion from anterior pituitary gland by ginseng saponin since it was handled down by tradition that ginseng might influence the milk secretion when it was given to nursing mother. To investigate the effect of saponin on the prolactin production or release from the anterior pituitary gland, cell culture study and whole animal studies were carried out. For the cell culture study, enzymatically dispersed anterior pituitary cells of rat anterior pituitary gland in HEPES buffers containing trypsin were used. Ginseng saponin was added to the culture media and the amount of prolactin produced in the cell culture media was determined by radloimmunoassay(RIA) technique. Dose-dependent increases of prolactin with ginseng saponin were observed, whereas, no change was observed without ginseng treatment. For the whole animal study, normal and castrated rats which previously cannulated into the heart via the right juglar vein were used. The prolactin concentration in plasma were determined by using the technique of RIA. In normal rats, prolactin concentration in plasma were elevated dramatically after 1 hour of ginseng saponin administration, whereas, instantaneous increases were observed in castrated rats. For prolactin assay by RIA, NIAMDD Rat Prolactin Kit and NIAMDD Rat Prolactin RP-1 were used as standard. The results indicate that ginseng saponins increase the release of prolactin from the anterior pituitary gland and production of prolactin from the cell in rats.

Intrasellar Schwannomas

  • The authors present two cases of intrasellar schwannoma, a condition rarely reported in the literature. The patients presented with symptoms of hypopituitarism. Sellar magnetic resonance imaging(MRI) revealed intrasellar masses extending into the suprasellar region, with the normal pituitary gland also identifiable in the imaging study. The tumors were removed via trans-sphenoidal route, and found to be very firm, unlike usual pituitary adenomas. Tissue diagnosis confirmed the tumors as schwannoma.

Dopaminergic influences on prolactin synthesis and release from rat anterior pituitary cultures

  • Dopaminergic influences on prolactin release from lactotrophs have been studied using the rat anterior pituitary cell culture. The prolactin inhibiting activity of hypothalamic extracts was examined in relation to dopamine. Dopamine inhibited prolactin secretion from the rat anterior pituitary cell culture in a dose dependent fashion. The median effective dose was $2{\times}10^{-7}$ / M and the maximal inhibition (70-90 % of the control value) was shown by 10$^{-5}$ / M dopamine. Further increase in dopamine concentration did not result in any further inhibition of prolactin secretion.nhibition of prolactin secretion.
