Dissolution and Transformation of Crystal Forms of Piroxicam (피록시캄 결정형의 용출과 형전환)

  • The polymorphism of piroxicam was studied. Form I, II, III and monohydrate designated as Form IX were prepared by recrystallization from different solvents. Depending on the coo ling rate of the piroxicam melt, Form IV, V, VI, VII and VIII were prepared. The crystal forms were characterized by DTA, TGA and UV spectroscopy. Their dissolution patterns were also investigated. During storage at ambient condition. Form VIII was transformed into Form I and it was accelerated by milling. The other crystal forms were also transformed into Form I by milling. Form I and Form IX were very stable.

  • The pharmacological activities and pharmacokinetic aspects of a topical microemulsion (KDPM) containing 0.5% piroxicam were evaluated after its topical application compared with a commercially available 0.5% piroxicam gel (R gel). When the pharmacological activities were evaluated with the carrageenan-induced paw edema model, KDPM showed 55.6% edema inhibition, while R gel resulted in 37.1%. With the adjuvant-induced arthritis model, KDFM also resulted in the better pharmacological activities than R gel. The relative bioavailability of KDPM based on R gel was 176% in rabbits.

  • Piroxicam is known to be a new nonsteroidal anti-inflammatory drug which has anti-inflammatory, antiedema, antigranuloma and analgesic activities through the mechanism of inhibition of prostaglandin synthetase. Clinical survey for piroxicam was carried cut on 40 cases of rheumatoid arthritis and 30 cases of degenerative arthritis who visited University Hospital from Feb. to June, 1979. Following results could be obtained: 1) In rheumatoid and degenerative arthritis, the effect of piroxicam was significant in decrease of joint pain, shortening of duration of morning stiffness, decrease in number of swollen joints, improvement of joint stiffness, decrease of E.S.R., increase of mean grip strength and decrease of articular index. 2) Side effects, almost all in G-I troubles were encountered in 6 cases in 40 cases of rheumatoid arthritis and 4 cases in 30 cases of degenerative arthritis.

  • Piroxicam is one of the NSAID, which is used in the systemic and topical treatment of a variety of inflammatory conditions. Conventionally, for topical use, the drug is formulated in gel. We designed an phonophoretic drug delivery system to investigate the piroxicam permeability and the influence of ultrasound application (continuous mode, pulsed mode), frequency (1.0 MHz, 3.0 MHz) and intensity $(1.0\;w/cm^2,\;1.5\;w/cm^2,\;2.0\;w/cm^2)$ with 0.5% piroxicam gel. Per cutaneous absorption studies were performed in vitro models to determine the rate of drug absorption via the skin. Permeation study using hairless mouse skin was performed at $37^{\circ}C$ using buffered saline (pH 7.4, 10% propylene glycol solution) as the receptor solution. Anti-inflammatory activity was determined using carrageenan-induced foot edema model in rat. A pronounced effect of ultrasound on the skin absorption of the piroxicam was observed at all ultrasound energy level studied. Ultrasound was carried out for 10 hr. The highest permeation was observed at intensity of $2.0\;w/cm^2$, frequency of 1.0 MHz and continuous output. The inclusion of phonophoresis was found to improve significantly the skin permeation in vitro and the anti-inflammatory activity in vivo.

Pre-emptive analgesia efficacy of piroxicam versus tramadol in oral surgery

  • Background: This double-blind randomized controlled trial (RCT) was conducted to evaluate the pre-emptive analgesia and anti-inflammatory efficacy of piroxicam compared with tramadol in patients undergoing oral surgery. Methods: Seventy-eight patients who required extraction of impacted mandibular third molars were randomized into three treatment groups of 26 patients each: group I received 100 mg of tramadol, group II received 20 mg of piroxicam, and group III received a placebo. Drugs were administered intramuscularly 30 min prior to the extraction procedure. Results: Pain intensity, time to first analgesic administration, total analgesic consumption, facial edema, and trismus were the outcomes of interest. The group receiving 20 mg of piroxicam showed significantly lower pain intensity, increased time to first analgesic, and reduced edema from preoperative to postoperative day seven than those in the tramadol and placebo groups. Conclusion: The findings of this study showed that piroxicam had significant pain relief efficacy after third molar surgery compared with that in tramadol.

The Effect of Hydroalcoholic Gel System on Skin Permeation of Piroxicam and its Anti-inflammatory Activity (피록시캄의 피부투과 및 소염효력에 대한 알코올성 하이드로겔 시스템의 영향)

  • These studies were designed to determine the effect of hydroalcoholic gel system (lower alkanol concentration: 40-60%) compared to general hydrogel system (lower alkanol concentration: 10-35%) on transdermal delivery of piroxicam and its anti-inflammatory activity. Piroxicam was incorporated into a hydroalcoholic gel and a hydrogel containing polymers, solvents, and cosolvents. The pH of gel was about 6.3-7.3 and the solvent mixtures were composed of water and various concentrations of ethanol (35, 40, 50, and 60%). For the in vitro study, the skin permeation of piroxicam from the gel formulations was investigated using Franz modified diffusion cells fitted with hairless mouse skin. For the in vivo study, the anti-inflammatory activity of hydroalcoholic gel was compared to other commercial products (piroxicam hydrogel and ketoprofen hydrogel) in rat and human. The anti-inflammatory activity was determined using carrageenan induced foot edema model in rat. For the clinical study, it was evaluated from determining efficacy and acceptability with 98 patients suffering from musculoskeletal pain. A novel piroxicam hydroalcoholic gel was successfully formulated in the range of 40-50% of ethanol as solvent, more than 10% of propylene glycol, 5% of $Transcutol^{\circledR}$ and 1 % of benzyl alcohol. The skin permeation of piroxicam using hydroalcoholic gel system was greater than that of general hydrogel system $(flux\;:\;139.1-148.2\;{\mu}g/cm^2/hr\;vs.43.0-84.5 {\mu}g/cm^2/hr)$ in vitro. HPLC Analysis of Piroxicam in the Rabbit Plasma and its Bioavailability after the Transdermal Administration of Patches (토끼 혈장 중 피록시캄의 HPLC 분석 및 패취제 투여 후 경피흡수)

  • A rapid and sensitive reversed-phase high performance liquid chromatography (HPLC) method was developed for the determination of piroxicam in the rabbit plasma. After a treatment of plasma sample by liquid-liquid extraction, the drug was analyzed on an HPLC system with ultraviolet detection at 330 nm. HPLC was carried out using reversed-phase isocratic elution with a C18 column, a mobile phase of a mixture of acetonitril, doubly deionized water and acetic acid 43.74:56.00:0.26 v/v%) at a flow rate of 1.1 mL/min. The chromatograms showed good resolution and sensitivity and no interference of plasma. The calibration curve for the drug in plasma sample was linear over the concentration range of 0.01-2.0 ${\mu}$g/mL. The intra- and inter-day assay accuracies of this method ranged from 86.82% to 108.33% of normal values and the precision did not exceed 13% of relative standard deviation. The plasma concentration of piroxicam decreased to below the quantifiable limit at 12 hr after the i.v. bolus administration to rabbits following dose of 0.375 mg/kg yielding a apparen t plasma half life of 1.38 hr. The transdermal route prolongs plasma levels of piroxicam. The bioavailability, calculated from the dose-adjusted ratio of the $AUC_{transdermal}$ to the $AUC_{i.v.}$, was 7.44%. The plasma concentration of piroxicam was detected by 48 hr after the transdermal administration of patch at a dose of 32 mg/kg. This method was suitable for cutaneous absorption studies of piroxicam in the rabbit after transdermal administration of different types of dosages of the drug.

Efficacy of Transdermal Piroxicam as Preemptive Analgesia (술후 제통에 사용된 Piroxicam 첩포의 선행진통효과)

  • Background: Preemptive analgesia has been suggested recently as an another technique of postoperative pain control. Combination of low dose opioid and NSAIDs was used to lessen systemic opioid side effect, however, the use of NSAIDs may hinder their side effects in perioperative period. The local application of small dose at the target site can be effective without systemic effect. The aim of this study is evaluating the additive effect and side effect of transdermal piroxicam as preemptive adjuvant to intravenous nalbuphine on pain relief after major abdominal surgery. Methods: We reviewed the records of patients received piroxicam patch for preemptive analgesia before operation and compared it with control group. Two sheets of piroxicam patch to the skin incision site for 12 hours before operation were attached (Group 1, n=20) and no patch were applied (Group 2, n=20). Both groups were received nalbuphine continuously after operation using two days infuser (2 ml/hr) containing 80 mg (96 ml). Pain is evaluated by VAS score at each time; 30 min, 1, 6, 12, 24, 36, 48 hours after operation and side effects of NSAIDs were observed for 3days postoperatively. Results: There was no significant VAS score difference between two groups following time in progress. And no significant side effect was noted in both groups, either. Conclusion: There is no preemptive or synergistic analgesic effect of piroxicam patch attached at planned operation site before operation.

  • Data obtained from X-ray diffractometry, thermal analysis, IR spectroscopy and microscopic observation were used for the identification and characterization of four crystalline modifications of piroxicam. form a was crystallized from sodium hydroxide-hydrochloric acid and from c was obtained by crystallization from toluene. Form b and d was crystallized from methanol under the different temperature conditions. Relative rates of dissolution and solubility of four crystal forms of piroxicam in distilled water were measured.

  • 4-Pivaloyloxy, 4-benzenesulfonyloxy and 4-p-toluenesulfonyloxy compounds of piroxicam (2a, 2b and 2c) and isoxicam(4a, 4b and 4c) were synthesized in fairly good yield. Antiinflammatory and ulcerogenic effects of the 6 compounds were determined. Antiinflammatory effect of 2a was comparable to that of piroxicam and ulcerogenic effect of 2a was less than that of piroxicam.
