수지진동에서의 진동강도, 손잡이온도, 소음 및 미는 힘의 복합효과에 따른 악력 및 지단피부온의 변화 (Combined Effect of Vibration Intensity, Grip Temperature, Noise and Pushing Power on Grip Forces and Skin Temperatures of Fingers)

  • Recent studies reveal that grip forces during the hand-arm vibration are most significant for the genesis of vibration-induced white linger syndrome. Therefore, exerted grip forces and skin temperatures of fingers were regarded as dependent variables in experiments and the effects of grip temperature, noise, pushing force, vibration and the combined effect of vibration and pushing force were studied. The objectives of the present study were, first, to varify and compare the changes of grip force affected by grip temperature, noise, pushing force, vibration and the combined effect of vibration and pushing force and, second, to observe the reaction of finger skin temperature affected by above factors. Forty-six healthy male students ($25.07{\pm}2.85$) participated in five systematically permuted trials, which endured 4 minutes each other. Experiments were executed in a special chamber with an air temperature of 21C. In each experiments, the subjects were exposed to five experiment types: (1) grip force of 25N only, (2) pushing force of 50N, (3) acceleration of vibration $7.1m/sec^2(z-direction)$, (4) pink noise of 95 dB (A) and (5) combination of pushing force 50N and acceleration of vibration $7.1m/sec^2$. A repeated-measures analysis of variance (ANOVA) was performed on the grip force to test whether it was affected by noise, pushing force, vibration and pushing force. The present results show that vibration was significantly related to the increase of grip force, but the other factors, such as pushing force, noise and grip temperature had no signigicant influence on the increase of grip force, and that the reaction of finger skin temperature were significantly affected by the skin temperature at start of experiment and grip temperature, not grip force and other experimental conditions. Therefore, we suggest that the management for decreasing the grip force is meaningful to prevent the occurrence of Hand-arm vibration syndrome (HAVS).

The Plasma Level of Insulin-like Growth Factor-I (IGF-I) in Relation to Mammary Circulation and Milk Yield in Two Different Types of Crossbred Holstein Cattle

  • The objective of the present study was to determine the plasma level of insulin-like growth factor-I (IGF-I) in relation to mammary blood flow and milk yield including biological variables of relevance to milk synthesis in two different types of crossbred Holstein cattle at 3 different stages of lactation. Eight heifers were 87.5% HF and eight 50% HF animals were selected for the experiments. The three stages of lactation tested were: early lactation (30 days postpartum), mid-lactation (120 days postpartum) and late lactation (210 days postpartum). Animals in each group were fed a concentrate and rice straw treated with 5% urea as the source of roughage throughout the experiments. In early lactation, mammary blood flow and milk yield of 87.5% HF animals were significantly higher than those of 50% HF animals. In mid- and late lactation, both mammary blood flow and milk yield showed a proportional decrease from the early lactating period of 87.5% HF animals. The trends for persistency were observed in 50% HF animals as for udder blood flow and milk yield throughout the experimental periods. The plasma glucose level of the 50% HF animals was significantly higher than those of 87.5% HF animals in both early and mid-lactation. The concentrations of arterial plasma free fatty acids ($C_{16}\;to\;C_{18}$) were higher in 50% HF animals as compared with 87.5% HF animals in all periods of study. In early lactation, the concentration of plasma growth hormone (GH) of 87.5% HF animals was higher than those of the 50% HF animals, thereafter the mean level of plasma growth hormone declined in both mid- and late lactation. The concentration of plasma IGF-I of 50% HF animals was significantly higher than those of 87.5% HF animals in all stages of lactation. There were no differences among stages of lactation for the levels of plasma IGF-I, insulin and growth hormone in 50% HF animals. In 87.5% HF animals, the plasma levels of both IGF-I and insulin were lower in early lactating period while it showed an increase during mid- and late lactation. The present results indicated that the regulatory role for the higher mammary blood flow and milk yield during lactation in 87.5% HF are not mediated via the higher level of circulating IGF-I. Differences in mammary blood flow and milk yield between 50% HF and 87.5% HF animals are in part due to a higher concentration of circulating growth hormone. The lower level of circulating growth hormone in 50% HF animals would be regulated by higher levels of IGF-I, free fatty acid and glucose in plasma.

Changes of Cerebral Metabolism and the Related Factors during Cardiac Surgery

  • The effect of cardiopulmonary bypass (CPB) on cerebral physiology during heart surgery remains incompletely understood. This study was carried out to investigate changes of cerebral metabolism and the association between the changes and clinical factors during heart surgery. Seventy adult patients (n=70) scheduled for elective cardiac surgery were participated in the present study. Middle cerebral artery blood flow velocity (V$_{MCA}$), cerebral arteriovenous oxygen content difference (C(a-v)O$_2$), cerebral oxygen extraction (COE), and modified cerebral metabolic rate for oxygen (MCMRO$_2$) were measured during six phases of the operation; Pre-CPB, CPB-10 min, Rewarm-1 (nasopharyngeal temperature 34$^{\circ}C$), Rewarm-2 (nasopharyngeal temperature 37$^{\circ}C$), CPB-off, and Post-OP (at skin closure after CPB-off). Each relationship of age, arterial blood gas parameters, or other variables to V$_{MCA}2$, C(a-v)O$_2$, COE, or MCMRO$_2$ was evaluated. V$_{MCA}$ increased (P<0.0001) whereas C(a-v)O$_2$ decreased (P<0.01) throughout the five phases of the operation compared to Pre-CPB value (control). COE diminished at CPB-10, Rewarm-1, and CPB-off (P<0.05) while MCMRO$_2$ reduced at CPB-10 and Rewarm-1 (P<0.05) compared to Pre-CPB value. Positive correlation was found between age and cerebral metabolic parameters (V$_{MCA}$, C(a-v)O$_2$, COE, or MCMRO$_2$) during CPB (range r=0.24 to 0.38, p<0.05). Four cerebral metabolic parameters had partially negative or positive correlation with arterial blood gas parameters and other variables (arterial blood pH, $O_2$ tension, $O_2$ content, $CO_2$ tension, blood pressure, blood flow, temperature, or hematocrit) during the operation. In conclusion, CPB led to marked alterations of cerebral metabolism and age, pH, and $CO_2$ tension profoundly influenced the changes during cardiac surgery.

  • Intrathecal drug delivery is an effective and safe option for the treatment of chronic pathology refractory to conventional pain therapies. Typical intrathecal administered drugs are opioids, baclofen, local anesthetics and adjuvant medications. Although knowledge about mechanisms of action of intrathecal drugs are every day more clear many doubt remain respect the correct location of intrathecal catheter in order to achieve the best therapeutic result. We analyze the factors that can affect drug distribution within the cerebrospinal fluid. Three categories of variables were identified: drug features, cerebrospinal fluid (CSF) dynamics and patients features. First category includes physicochemical properties and pharmacological features of intrathecal administered drugs with special attention to drug lipophilicity. In the second category, the variables in CSF flow, are considered that can modify the drug distribution within the CSF with special attention to the new theories of liquoral circulation. Last category try to explain inter-individual difference in baclofen response with difference that are specific for each patients such as the anatomical area to treat, patient posture or reaction to inflammatory stimulus. We conclude that a comprehensive evaluation of the patients, including imaging techniques to study the anatomy and physiology of intrathecal environment and CSF dynamics, could become essential in the future to the purpose of optimize the clinical outcome of intrathecal therapy.

Differential Effects of Typical and Atypical Antipsychotics on MK-801-induced EEG Changes in Rats

  • We examined whether the abnormal EEG state by NMDA receptor blocker MK-801 can be reversed by typical and atypical antipsychotics differentially by comparing their spectral profiles after drug treatment in rats. The spectral profiles produced by typical antipsychotics chlorpromazine (5 mg/kg, i.p.) and haloperidol (0.5 mg/kg, i.p.) were differ from that by atypical antipsychotic clozapine (5 mg/kg, i.p.) in the rats treated with or without MK-801 treatment (0.2 mg/kg, i.p.) which produce behavioral abnormalities like hyperlocomotion and stereotypy. The dissimilarity between the states produced by antipsychotics and the control state was examined with the distance of the location of the canonical variables calculated by stepwise discriminant analysis with the relative band powers as input variables. Although clozapine produced more different state from normal state than typical antipsychotics, clozapine could reverse the abnormal schizophrenic state induced by MK-801 to the state closer to the normal state than the typical antipsychotics. The results suggest that atypical anesthetic can reverse the abnormal schizophrenic state with negative symptom to the normal state better than typical antipsychotic. The results indicate that the multivariate discriminant analysis using the spectral parameters can help differentiate the antipsychotics with different actions.

적혈구 용적이 요골 동맥의 맥파에 미치는 영향 (Effect of Hematocrit Level on the Radial Pulse Wave)

  • In this work, we investigate the effect of blood hematocrit level on the radial pulse wave to study the clinical application of the pulse analyzer. For this purpose, we measured the radial pulse wave at the left Gwan for 15 males with abnormal high hematocrit level and 47 males with normal hematocrit level at the age of thirties and forties. Various variables of the radial pulse wave between two groups were analyzed by Student's T test. We found significant differences in several characteristic variables in the amplitude, time-span and the integrated area of the amplitude and time of the pulse wave. The systolic peak in the amplitude of the radial pulse wave was higher in abnormal high hematocrit group. In contrast, the third peak from the second incisure was higher and longer in normal hematocrit group. Our study suggests that the radial pulse wave can be useful in distinguishing the patient group with high hematocrit level and thus with high blood viscosity. Our finding may motivate research activities towards diverse clinical applications of the pulse wave.

Presentation Delay in Breast Cancer Patients, Identifying the Barriers in North Pakistan

  • Background: There is strong evidence that delayed diagnosis of breast cancer is associated with poor survival. Our objectives were to determine the frequency of breast cancer patients with delayed presentation, the reasons of delay and its association with different socio-demographic variables in our North Pakistan setting. Materials and Methods: We interviewed 315 histologically confirmed breast cancer patients. Delay was defined as more than 3 months from appearance of symptoms to consultation with a doctor. Questions were asked from each patient which could reflect their understanding about the disease and which could be the likely reasons for their delayed presentation. Results: 39.0 % (n=123) of patients presented late and out of those, 40.7% wasted time using alternative medicines; 25.2 % did not having enough resources; 17.1 % presented late due to painless lump; 10.6% felt shyness and 6.5% presented late due to other reasons. Higher age, negative family history, < 8 school years of education and low to middle socio-economic status were significantly associated with delayed presentation (p< 0.05). Education and socioeconomic status were two independent variables related to the delayed presentation after adjustment for others (OR of 2.26, 2.29 and 95%CI was 1.25-4.10, 1.06-4.94 respectively). Conclusions: Significant numbers of women with breast cancer in North Pakistan experience presentation delay due to their misconceptions about the disease. Coordinated efforts with public health departments are needed to educate the focused groups and removing the barriers identified in the study. Long term impact will be reduced overall burden of the disease in the region.

병인변증과 요골동맥 맥상파의 특성 파악을 위한 탐색적 관찰 연구 : 임상시험 프로토콜 개발 (Development of Clinical Protocol on the Correlation Between Disease Cause Pattern Identification and Pulse Wave Variables)

  • The purpose of this clinical study is to develop structured clinical trial protocol and guideline for improvement of safety, useful and effective of pulse diagnosis devices. As a first step, papers on pulse diagnosis and pulse diagnosis devices from 2001 and 2013 were systematically reviewed. In the next step, we have collected the opinions from the specialists, companies, and statistician in pulse diagnosis to evaluate the current condition, the state and problem of domestic clinical trial cases of pulse diagnosis device. And we have to created protocol and case report form (CRF) in regards to site condition and characteristics of pulse diagnosis devices, and showed the guideline of eligibility criteria, operation process, investigation items, evaluation items and so on. This clinical protocol will become a basic information for a researcher in designing or performing a clinical study of pulse diagnosis devices, and be used as a useful material during acquisition of good clinical data. Furthermore, we hope to enhance the invigoration of pulse diagnosis clinical trials and the performance improvement of pulse diagnosis devices.

고연령 한국 남성의 사상 체질별 건강 수준에 따른 안색의 정량적 분석 (Quantitative Analysis of Face Color according to Health Status of Four Constitution Types for Korean Elderly Male)

  • In this paper, we performed a quantitative analysis of face color according to the health status of four constitution types. 205 Korean male in age from 65 to 80 were participated in this study and 85 subjects were finally selected for the analysis. Imaging process techniques were employed to extract feature variables associated with face color from a frontal facial image. Using the extracted feature variables, the correlations between face color and health status, face color and health status in each constitution type, and face color and four constitution types in heath status group were investigated. As the result, it was observed that the face color of healthy group contained more red component and less blue component than unhealthy group. For each constitution type, the face parts showing a significant difference according to health status were different. This is the first work which reports the correlation between the face color and health status of four constitution types with a objective method, and the numerical data for the face color according to the health status of four constitution types will be an objective standard to diagnose a patient's health status.

Transient effects of jumping lunge on sand on balance ability in healthy adults: a preliminary study

  • Objective: The purpose of this study was to show the temporary effects of performing jumping lunges on a sand surface on static balance and dynamic balance. Design: Randomized controlled trial. Methods: Twenty healthy subjects volunteered in the study and was randomly assigned into either the sand group (n=10) or the control group (n=10). The subjects in the sand group performed jumping lunges on a sand surface and the subjects in the control group performed jumping lunges on a firm surface. The intervention was performed for 3 sets of 8 repetitions by both groups. To measure static balance, the force plate was employed to measure the center of pressure (CoP) area, and the CoP velocity during one-legged standing. Anterior, postero-medial, postero-lateral movements was assessed using the Star Excursion Balance Test (SEBT) to measure dynamic balance. Results: After the intervention, the sand group showed statistically significant improvements on all variables (CoP area, CoP velocity) in static balance (p<0.05). There were statistically significant changes in CoP area and CoP velocities between the two groups (p<0.05). In the sand group, there were significant improvements in the postero-medial and postero-lateral directions (p<0.05) except for anterior direction on dynamic balance. In the control group, there was a significant improvement in the postero-lateral and anterior directions (p<0.05). In comparison of the two groups, there was no statistically significant improvement in all variables. Conclusions: This study demonstrated that performing jumping lunges on a sand surface was effective in improving static and dynamic balance temporarily.
