Purpose: The purpose of this study was to describe married female migrants' experiences of health care services and to help nursing researchers, nursing educators, and clinical nurses understand married female migrants' experiences. Method: A conventional content analysis method was utilized. Individual in-depth interviews with 15 married female migrants were conducted. Subject recruitment was performed at a district in Seoul. Results: Ten categories were induced: language barrier, financial burden, insufficient time with a physician, complexity of utilization process, lack of support from peer group, health care providers' discrimination, anxiety regarding lack of information about children's health, health care providers' concerns and efforts to minimize the language barrier, family support, and advanced health care service environment. Conclusion: This study provides basic knowledge regarding married female migrants' experiences related to health care services. Future research should designate and utilize valid instruments to measure the positive and negative experiences and to explore strategies to strengthen the positive features while eliminating the negative ones. Finally, the Korean nursing education curriculum should include cultural competence and knowledge about an ethnic minority's right to health service utilization.

The increase in the edentulous jaw which occurs in the aged population has led to personal dental health concerns. In the case of dental implant surgery, the duration of a patient's recovery depends on the surgical plan and their physicical ability. A device may be required to assist a physician in controlling vibration reduction of free-hand drilling and prescribing a good treatment plan that is suitable for the patient's condition. In this work, an artificial tooth-root implant assistant manipulator was studied. The structure and the vibration analysis of the dynamic restraint manipulator that is for drilling the alveolar bone in the mandible bone were performed, and the structural stability was analyzed. Further, a virtual prototype of an artificial tooth-root implant assisted manipulator was fabricated and tested. Hence, the state of the Remote Center of Motion (RCM) point and the driving state of the manipulator were confirmed. Furthermore, the drilling experiments were performed by using materials similar to a human jawbone in order to evaluate the performance of the drilling process that is operated using the assistant manipulator.
The resource-based relative value scale (RBRVS) compares the value of a medical practice to the consumption of resources, which consist of the work of the physician, practice expenses, and professional liability insurance. At the time of the 2nd revision of RBRVS, the fee for radiological examinations had been reduced due to the high preservation rate. In RBRVS, practice expenses account for most of the compensation of radiological examinations, and physicians' work is relatively undervalued. A new healthcare policy (Moon Jae-In care) consists of the expansion of the National Health Insurance (NHI) coverage, reduction of patient charges for the vulnerable class, and support for catastrophic medical expenses. However, Moon Jae-In care is expected to negatively affect the NHI in Korea financially. The expansion of the insurance coverage for ultrasonography and MRI examinations is a significant part of the Moon Jae-In care, and radiological societies should establish fair compensations for physicians' work within the field of radiology while implementing the Moon Jae-In care.
Objectives : The Major Essentials of Huangdi's Internal Classics Plain Questions (黃帝內經素問大要, MEHP) is one of the late-Chosun's literate physician Lee Gyoojoon (李圭晙, 1885-1923)'s main work, which is known to have logically proofread the Huangdi's Internal Classics Plain Questions (黃帝內經素問, HP). This study aims to examine two elements of the text: (1) the types of MEHP currently in circulation; (2) the types of publications of HP to be the MEHP's original script. Methods : In this study, basically bibliographical analyses of the form and contents was utilized about the types of MEHP and HP. However, to compare the sentences and phrases between prints, I've used 20 examples that Qian Chaochen (钱超尘) had proposed in his preceding studies. Also, regarding Lee Wonse (李元世)'s proofreading on the MEHP in 1999, I've used interviews of his students. Results : First, I've discovered that there are three versions MEHP in circulation: the woodblock printed version; Lee Wonse's handwritten version; Lee Wonse's proofreading version; and confirmed that Lee's proofreading version should be regarded as good version of MEHP. Also, I've discovered the possibility of other types of printed versions of the MEHP in existence, which is considered Lee's handwritten version's original draft. Second, I've confirmed that the original script of HP, which Lee Gyoojoon utilized for MEHP, is indeed not Gu Congde (顧從德) printed version HP, however, is the Chosun's bureau for military drill (訓練都監) printed version HP. Conclusion : Through this study, I've provided strong evidence that Lee Gyoojoon's MEHP is a unique and original research completed within the traditional realm of Korean medicine, which possesses the universality of Eastern Asian medicine represented by Huangdi's Internal Classics (黃帝內經).
The development and management of epidemiology intelligence service (EIS) officer with more specialized competence to cope with and prepare for health threats, including pandemic of emerging and re-emerging infectious diseases, is a high priority policy issue in Korea. First of all, we need to establish the training goal of EIS officer. It is necessary to establish manpower training and management system with at least three tiers including quantitative and qualitative targets. Second, at least 50% of all EIS officer must secure a physician and secure expertise and competence for epidemic. Third, for the ultimate purpose of EIS officer, the establishment of a public health expert should expand the scope of epidemiologist's work to health and medical care, occupational environment, and various disasters. Fourth, it is essential to expand the epidemiologist training and education program to the level of advanced countries. Especially, the training course should be expanded at least twice of current times. Fifth, it is necessary to independently install and operate the 'EIS Officer Training Center' as a mid- and long-term goal. Stewardship and governance are secured with the organization, personnel, etc. that can fully manage the planning, management, and evaluation of the EIS system. In the future, it will be necessary to establish a systematic and phased operational base of education and training programs for EIS officer, and establish a sustainable implementation system for strategy development. In addition, it is urgent to revise the guidelines for training public health professionals and strengthening competencies, and for establishing professional educational institutions.
Background: The number and curriculum of dental hygienists in Korea have dramatically increased. Controversies have since resulted from insufficient job descriptions of the work performed by a dental hygienist. A dentist's perception was examined to legally reflect the actual work of dental hygienists. Methods: Four hundred and nineteen dentists were surveyed about the duties of a dental hygienist. Their views on the career and availability of each job were examined. The duties of the dental hygienist include 13 items in dental treatment preparation, 14 items of radiography, 21 items of preventive dentistry, 6 items of periodontal treatment, 12 items of oral medicine, 12 items of conservative dentistry, 8 items of prosthetics, 10 items of orthodontics, 7 items of oral and maxillofacial surgery, 6 items of implantation, 6 items of impression taking and model fabrication, 5 items of anesthesia and injection, 11 items of management and administrative, and 3 items of self-development. Results: Most of the duties were doable by a dental hygienist. Many dentists reported that managing implants, oral hygiene of special patients, some duties in oral medicine, teeth brightening, making temporary crowns, making individual trays, selecting shades, ligaturing, and precision impressions need ≥3 years of experience. Duties perceived by dentists not to be performed by dental hygienists were reading radiographs (55.4%), suture and stitch out (48.0%), intramuscular injection (36.0%), root planning (27.2%), cementation and removal of prostheses (23.2%), and examining pulp vitality (22.0%). Conclusion: Current laws are to be revised to include, the care provided by dental hygienists and under a physician's supervision. Flexibility is also needed to cope with rapidly changing dental technology.
As a descriptive survey, this study was attempted to get basic data necessary to recognize the factors that disturb the therapeutic atmosphere of hospital wards as perceived by nurses and hospitalized patients, to identify differences between the perceptions of the nurses and of patients. The subjects, 159 patients in Pusan National Hospital and 68 nurses working there were sampled between March 18 and April 13, 1996. The tool used to measure the disturbing factors was an amended form of the one developed by Kim, Mae Ja(1983). The differences between each subject's score for each factor were analyzed using means & SD. and the highest 3 items above the mean score for each factor were collected and compared. The results are described below : 1. Subject's perception of main disturbing factors : patients reported that the main factors were 'loss of role & economic trouble', 'the prognosis of disease', 'the change of daily life' but nurses replied that the main factors were' the prognosis of disease', 'the communication trouble with the medical team & interpersonal relationships'. 'The change of daily life' was not a perceved factor by nurses, but ranked third by the patients. 2. Subject's perception degree of each disturbing factor : (1) among the items related to interpersonal relationship. the patient group reported that the worst disturbance was dur to severely ill patients in the same room' but the nurse group regarded 'greed to monopolize wheelchairs or other supplies' as the worst disturbance. (2) among the items related to physical factors. the patient group regarded 'limitations to wash their body, physical pain and limitations in physical activity' as the worst disturbance, but the nurse group regarded' physical pain', and 'limitations to activity or change of appearance' as the worst disturbance. (3) among the items related to the change of daily activity, the patient group regarded 'the boredom of hospitalization or infavorable diet' as the worst disturbance, but the nurse group regarded 'too much noise or unclean room' as the worst disturbance. (4) among the items related to the communication trouble with medical team, the patient group regarded 'the ignorance of their disease due to poor information. the inability to understand the language of the medical team or the difficulty in seeing physician in time' as the worst disturbance, but the nurse group regarded 'the inability to trust physicians and physician's poor attention to patients' as the worst disturbance.
Purpose: The emergency medical service (EMS) system in Japan is similar to that in Korea. This study reviewed the development of paramedics who are major personnel in the respective EMS system of their country. Methods: We searched law for paramedics and publications in J-stage, MEDLINE and the webpage of Japan's Fire and Disaster Management Agency. Results: Paramedic training had started in Japan in 1991, and most paramedics were employed in fire departments. In Japan, ambulances belonging to fire department are staffed by a three-person unit, including at least one paramedic. Medical control advisory boards were established in all Japanese prefectures in 2003 with the purpose of upgrading the activities of ambulance personnel, including paramedics. The scope of paramedics' work has been widened owing to the continued efforts of medical control advisory boards. This allows them to perform invasive procedures including tracheal intubation, intravenous line placement, and epinephrine administration under medical control of a physician. Conclusion: As paramedics' responsibilities expand, further development of the paramedic role and an effective medical control advisory board system are imperative both Korea and Japan.
First of all, this study shows the legal issues of hospice and palliative care, and the legal basis for lifelong medical practice is generally derived from medical, civil and criminal law regulations, and is applied to patients who are severely ill and dying in principle. In addition, those what is particularly meaningful about hospice and palliative care in terms of legal aspects are discussed the determination of the purpose of care and the provision of medical adaptability and adult guardianship, in particular the legal criteria for the work and status of patient representatives. As such, the purpose of care is to form part of the contract of care and to be agreed between the patient and the physician. In addition, the patient may not write to his/her agent in advance, and the patient may admit discretionary powers to his/her agent, but the patient's will is to be considered. In conclusion, the medical institutional ethics committee should play an active role, especially in the case of no-agents/family or no intention of the patient.
Objective: The purpose of this study was to investigate the effects of 5 different personalities on job stress in physical therapists. Design: Descriptive cross-sectional study. Methods: Structured self-report questionnaires consisted of demographic items, 5 types of personalities, and occupational stress. A questionnaire was distributed to 420 physical therapists working in Seoul and Incheon who had voluntarily agreed to participate in the study after the purpose of the study was explained. Of the 420 questionnaires, 405 questionnaires were collected and showed a recovery rate of 96.4%, of which 28 cases were excluded, leaving a total of 377 questionnaires being used for analyses. Pearson correlation analysis was performed to investigate the correlation of job stress and five personalities types. In addition, multiple regression analysis was performed to investigate the effect of general and occupational characteristics of physical therapist on job stress and the effect of personality type on occupational stress. All statistical significance levels were set at p<0.05. Results: The highest number of subjects, which were the physical therapists, showed the tendency to have a sincere personality, followed by affinity, openness, extroversion, and neuroticism. Physical therapists reported to be the most stressed in the order of professional role conflict, overload of work, work relation with a physician and supervisor, interpersonal relationship with patient and caregiver. The higher the level of affinity and sincerity, the lower the amount of stress received by the therapist from the interpersonal relationships between the patient and caregivers. On the other hand, the greater the level of openness and sincerity, the lower the stress levels related to professional role conflict. Conclusions: This study showed that the personality type and job stress of physiotherapists had a statistically significant effect. It is very important for physiotherapists to find their own way of coping with stress, which is satisfactory and appropriate for their job, because it is related not only to individual problems but also to the quality of patient care and medical services. Therefore, it is necessary to continue conducting research on how to relieve the stress levels of physical therapists according to their personality characteristics.
