교각운동시 팔의 보상작용에 따른 몸통 및 하지근육 활성도 분석 (Analysis of trunk and lower extremity muscle activity according to the compensation of arm during bridge exercise)

  • Background: The purpose of this study was to investigate the changes of trunk and lower extremity muscle activity according to the compensation of arm during bridge movement of healthy subject. Design: Cross-sectional Study. Methods: Twenty healthy subjects participated in this study. The subjects performed bridge exercise with 3 different arm positions(arm abduction 45°, 90° and cross-arms) and measured the muscle activity of the trunk and lower extrimity. During bridge exercise with 3 different arm positions, trunk (rectus abdominis, erector spinae) and lower extrimity muscle activity (gluteus medius, biceps femoris, tibialis anterior) were measured using wireless surface EMG. Results: Rectus abdominis and gluteus medius muscle were most activated during bridge exercise with arm abduction 90° and erector spinae and biceps femoris muscle were most activated during bridge exercise with arm abduction 45°. In addition, tibialis anterior muscle was most activated during bridge exercise with arm cross. However, these difference in muscle activity according to the arm position was not statistically significant. Conclusion: As a result of this study, we think that the change in arm position does not induce sufficient instability to increase the muscle activity of the trunk and lower extremity muscles. Therefore, various approaches for inducing instability of the support surface for increasing muscle activity when applying bridge movement in clinical practice should be explored.

국내 리듬청각자극(RAS) 기법 활용 연구 분석: 음악치료와 물리치료 비교를 중심으로 (An Analysis of Research Using the Rhythmic Auditory Stimulation Technique: A Comparison of Music Therapy and Physical Therapy Approaches)

  • 본 연구는 국내 음악치료 및 물리치료 분야에서 리듬청각자극(RAS)을 적용하여 수행된 연구를 비교분석하였다. 이를 위해 1999년부터 2018년 11월까지 발표된 학위 논문과 학술지 게재 연구 중 RAS 및 보행기능 관련 주제어를 검색하여 기준에 부합한 45개의 연구를 선정하였고, 세부중재방법과 리듬자극의 유형, 기술된 치료 원리의 측면에서 분석하였다. 분석 결과, 두 영역에서의 연구 모두 일정한 간격의 리듬을 핵심적인 치료 자극으로 사용하고 RAS의 기본 절차를 따른 점에서는 공통적이었지만, 목표기능이나 구체적인 리듬제공방법에 있어서는 차이가 있었다. 음악치료 연구에서는 모두 보행기능이 목표 변인이었던 데 비해, 물리치료 연구에서는 균형이나 근력, 고유수용성감각 등 관련 신체기능이 목표에 포함되었다. 또한 음악치료 연구에서는 다양한 요소를 포함한 '음악'이 리듬자극으로 활용된 반면, 물리치료 연구에서는 메트로놈을 리듬자극으로 활용한 연구가 음악을 활용한 연구보다 많았다. RAS 원리와 관련하여 단순한 기법 설명 외에 동조화와 같이 치료의 원리가 충분히 기술된 연구는 물리치료보다는 음악치료 분야에서 많이 확인되었다. 이러한 결과는 음악치료 연구에서는 리듬과 더불어 음악의 다양한 요소가 보행에 미칠 수 있는 영향이 고려되어 음악의 역할이 적극적으로 활용된 반면, 물리치료 연구에서는 보행에 영향을 미치는 다양한 신체기능에 대한 개입이 포함된 경향이 있음을 보여준다. 두 가지 측면 모두 재활에 있어 중요한 부분임을 고려할 때, 본 연구는 다학제간 연구를 기반으로 한 RAS 연구의 기법의 확장 적용에 대한 시사점을 제시한다고 할 수 있다.

Fall experience and dual-task during gait performance for community-dwelling persons with stroke

  • Objective: The purpose of this study was to investigate the effects of fall experience and task complexity on gait performance in community-dwelling persons with chronic hemiparetic stroke. Design: Cross-sectional study. Methods: Thirty-three persons who had a history of stroke participated in this study. The participants included 18 persons (aged mean 54.0, mean score of 24.6 points on the Montreal Cognitive Assessment, MoCA) with fall experience (faller group) and 15 persons (aged mean 53.7, mean score of 24.7 points on the MoCA) without fall experience (non-faller group) in the previous six months. This study measured balance and gait performance at two different conditions (with/without 70% of water filled in a 200 cc cup). The participants were clinically assessed using the 10-meter walk test (10MWT), 6-minute walk test (6MWT), Berg Balance scale (BBS), Dynamic Gait Index (DGI), and Timed Up-and-Go (TUG) test. Results: After analyzation, persons in the faller group performed significantly better on the 10MWT, 6MWT, BBS, DGI, and the TUG test in the no-cup-carrying condition than those in the cup-carrying condition (p<0.05). The persons in the non-faller group also performed significantly better in all outcome measures with the no-cup-carrying condition than those in the cup-carrying condition (p<0.05). However, there was no interaction between fall experience and task complexity in the two groups. Conclusions: Our results showed that balance and gait performance depended on fall experience and task complexity but fall experience did not interact with task complexity. Clinicians should consider fall prevention and task complexity during therapeutic approaches in persons with hemiparetic stroke.

노인장기요양보험제도 시행에 따른 등급판정의 중요성에 대한 연구 (Study of Importance of Grade Decision to Enforce the Insurance Policy for Long-Term Care)

  • Purpose: The purpose of this study is to investigate the importance of grade decision and role of physical therapist which follows enforcement of elderly long-term residential care insurance system. Methods: One of the data from grade decision meeting in Bukgu, Busan on 2007 which was 88 case of attached finding of doctor was compared with findings of visited investigator. Result: Eighty-four investigation subjects had 186 diseases that included stroke and arthritis, requiring the need for physical therapeutic approaches. In addition, the results of the door-to-door research project in the northern district of Busan showed that there was no match out of 88 subjects who submitted the viewpoint of the doctor. Such a result was produced as the doctors did not diagnose the patient directly, but rather the diagnoses were obtained from guardians and a door-to-door researcher who had a poor understanding of geriatric motion and function. Conclusion: To enforce long-term care successfully, a re-investigation should be performed for the welfare of the aged.

Diagnosis and treatment of positional plagiocephaly

  • Positional plagiocephaly is increasing in infants. Positional plagiocephaly is an asymmetric deformation of skull due to various reasons; first birth, assisted labor, multiple pregnancy, prematurity, congenital muscular torticollis and position of head. Positional plagiocephaly can mostly be diagnosed clinically and by physical examinations. The simplest way to assess the severity of plagiocephaly is to use a diagonal caliper during physical examination, which measures the difference between the diagonal lengths on each side of the head. Plagiocephaly can be treated surgically or conservatively. Positional plagiocephaly, which is not accompanied by craniosynostosis, is treated conservatively. Conservative treatments involve a variety of treatments, such as change of positions, physiotherapy, massage therapy, and helmet therapy. Systematic approaches to clinical examination, diagnosis and treatment of positional plagiocephaly can be necessary and the age-appropriate treatment is recommended for patients with positional plagiocephaly.

Feasibility of the Depth Camera-based Physical Health Monitoring System for Elderly Living Alone

  • Objective: This study aimed to evaluate the validity of a depth camera-based system for monitoring physical function, assessing its feasibility for accurately monitoring activities of daily living. Design: A cross-sectional study. Methods: Twenty-three participants were enlisted to perform fifteen activities of daily living within a living laboratory designed to simulate a home environment. Activities were monitored using a depth camera system capable of classifying actions into standing, sitting, and lying down, with a conventional video camera employed for activity recording. The duration of each activity, as measured by the system, was compared to direct observations made by a physical therapist which were analyzed using a motion analysis software. The association between these two measurement approaches was assessed through correlation analysis, coefficient of determination, intraclass correlation coefficient (ICC), and Bland-Altman plots. Results: Our findings indicated that standing activities exhibited the highest correlation (r=0.847) between the system measurements and physical therapist observations, followed by sitting (r=0.817) and lying down (r=0.734), which demonstrated lower correlations. However, the ICC and Bland-Altman plots revealed notable variances between the two measurement methods, particularly for activities involving lying down. Conclusions: In this study, the depth camera-based physical monitoring system showed promise feasibility in distinguishing standing, sitting, and lying down activities at home environments. However, the current study also underlined some necessities of enhancements in capturing lying down activities.

Global Postural Re-education 운동과 일반적인 요부 안정화 운동(LSE)의 비교 -여성노인을 대상으로- (Comparisons between Global Postural Re-education Exercise and Lumbar Stabilization - Focuhing on Eldery Women)

  • Purpose : The purpose of this study was to compare 2 different interventions, global postural re-education (GPR) and specific spinal stabilization exercise in the healthy elderly of women. Methods : A total of 28 subjects were randomized into 2 treatment groups: GPR, where therapy involved muscle global chain stretching, or specific spinal stabilization exercise with conventional static stretching (GPR group: n=14, specific spinal stabilization exercise group: n=14). Both groups received exercise intervention 3 times a week for 12-weeks. Results : Outcome was based on trunk endurance, trunk flexibility, Pressure Bio Feedback, Oswestry Disability Index measured immediately before and after intervention. Trunk flexibility, Pressure Bio Feedback, Oswestry Disability Index improved more in GPR group after intervention. There were no differences between the 2 exercise approaches for any of the trunk endurance. Conclusion : As the above results, GPR appear to improve Trunk flexibility, Pressure Bio Feedback, Oswestry Disability Index to the elderly of women. The efficacy of the GPR program used in this study should be further investigated in a long period study and objective outcomes.

플라스틱 단하지 보조기와 신발 착용이 편마비 환자의 정적 선자세 균형에 미치는 영향 (Effects of Plastic AFO and Shoes on Static Standing Balance in Hemiplegic Patients)

  • In the patients with hemiplegia caused by stroke and TBI. postural sway is increased and open displaced laterally over the non-affected leg, reflecting asymmetry in weight bearing on lower extremities during standing balance. Recovery of symmetric weight bearing and postural stability is an important aim in physical therapy. Plastic AFO has been used for hemiplegic patients in order to help their abnormal walking patterns. Past studies have mainly focused on the AFO influences on hemiplegic walking patterns without balance function approaches. The purpose of this study was to identify the immediate effects of plastic AFO and shoes on the static balance in hemiplegic patients. The scale for static balance were weight bearing on affected leg(%), sway area(mm2), sway path(mm), maximal sway velocity(mm/s), anteroposterior sway angle($^{\circ}$ ), and lateral angle($^{\circ}$ ). Seventeen hemiplegic patients participated in this study: 13 men and 4 women, with an average age of 50.18 years. Static balance was measured using BPM(balance performance monitor; dataprint software version 5.3) under four standing condition namely bare-foot standing. standing in shoes, standing with AFO, and standing in shoes with AFO. In order to assure the statistical significance of the results, an one-way ANOVA, the independent t-test. and a pearson's correlation were applied at the .05 level of significant. The results of this study were as follows: 1) There were statistically significant differences in weight bearing(%) on the static balance between affected leg and non-affected leg(p<.01). 2) There were statistically significant differences in sway reverse frequence(Hz) in standing with AFO between affected leg and non-affected leg(p<.05). 3) Sway area(mm2) on standing in shoes with AFO was lower than bare-foot standing(p<.05), Lateral sway angle($^{\circ}$ ) on standing in shoes with AFO was lower than bare-foot standing and standing in shoes(p<.05). 4) Weight bearing in affected leg was not significantly correlated with postural sway.

골관절염 여성의 임상적 특성 및 자기효능감이 기능수행에 미치는 영향 (The Influence of Clinical Symptoms and Self-Efficacy on Function in Women With Osteoarthritis)

  • This study was designed to examine the relationship between clinical symptoms, self-efficacy, and performance of women with osteoarthritis. It is a survey study of 60 women who were diagnosed as osteoarthritis and given medical treatments from September, 2005 to October, 2005 in hospital 'H' located in Yongin-si. For clinical symptoms, radiographs of the subjects' knees were taken and evaluated the pathology grade by the Kellgren-Lawrence grade. Pain and stiffness was measured by the measure of WOMAC (Western Ontario and McMaster Universities Osteoarthritis Index), and functional reach test was measured in order to examine balancing abilities. Self-efficacy was measured by a tool which has revised the ASES (Arthritis Self-Efficacy Scale), and performance was measured by recording the difficulty of the WOMAC measurements themselves, and the time taken for 20 m walking, going up and down 9 stairs, and 5 sit-down and stand-up repetitions. The resulting differences in the other variables according to performance and the relationship between performance with variables are the following. First, an increase in pain in women with osteoarthritis led to decreased functional ability. Second, an increase in stiffness in women with osteoarthritis led to a decrease in functional ability. Third, a decrease in balance in women with osteoarthritis led to a decrease in functional ability. Fourth, a decrease in self-efficacy in women with osteoarthritis led to a decrease in functional ability. Fifth, the variables for estimating the performance by self-report were pain and self-efficacy. The variables for estimating the performance by recording the time taken was balance and self-efficacy. As a result factors such as pain, balance and self-efficacy in women with osteoarthritis were closely related to performance. Based on the results, it seems that physical therapy programs to decrease pain and to increase the balance in women with osteoarthritis, and psychological approaches to increasing self-efficacy are needed. I hope that the results of this study will be useful data for clinical management and intervention for women with osteoarthritis.

Effect of a Combined Functional Electrical Stimulation with Action Observation Training on the Upper Limb Global Synkinesis and Function of Patients with Stroke

  • Background: Multifaceted approaches will be needed, such as global synkinesis (GS) achieve functional improvements in the arms of stroke patients from involuntary movements during exercise. Objective: To identify changes in arm GS and muscle activity, functional evaluation and the correlation with variables through action observation training, combined with functional electrical stimulation (FES), thereby verifying the effect on stroke patients. Design: A quasi-experimental study. Methods: The subjects of this study were 20 stroke patients who were divided into two groups: Control group (n=10) and experimental group (n=10). Before the intervention, arm GS and muscle activity were measured using surface electromyography (EMG), and arm function was evaluated using the Fugl-Meyer Assessment (FMA) scale. At the end of the intervention, which lasted 4-wk, arm GS and muscle activity were measured again using the same scale. Results: There was a decrease statistically significant difference in GS during the bending action in experimental group (P<.01). Both groups showed a significant difference increased only in the activity of the anterior deltoid (AD) and biceps brachii (BB) (P<.05). The results of the arm functional assessment revealed a significant difference increase in both groups (P<.05). In the between-group comparison, there was a significant difference decrease in GS during the bending action (P<.05). Only the muscle activity of the AD and BB were significantly increase different (P<.05). There was a significant between-group difference increase in the arm functional assessment (P<.05). There was a positive correlation between GS and muscle activity on the FMA in the control group (r=.678, P<.05). In experimental group, GS during the bending arm action exhibited a negative correlation (r=-.749, P<.05), and the muscle activity of the AD and BB showed a positive correlation (r=.701, P<.05). Furthermore, in experimental group, the activity of the extensor carpi radialis increased, and the activity of the flexor carpi radialis decreased, which exhibited a negative correlation (r=-.708, P<.05). Conclusion: These results suggest that brain plasticity could be more efficiently stimulated by combining surface stimulation in the affected arm of stroke patients.
