재활요양병원의 재활군 환자의 만족도 요인에 관한 연구 (Satisfaction Determinants of Rehabilitation Care Group Inpatients in Rehabilitation Long-term Care Hospitals)

  • Objectives : The aim of this study was to investigate the satisfaction determinants of rehabilitation care group inpatients in rehabilitation long-term care hospitals. Methods : A survey was conducted with 262 patients from 6 rehabilitation long-term care hospitals located in Busan Kyungnam. Data were collected from June 2016 to July 2016 with a structured questionnaire and analyzed with SPSS 23.0. Results : The significant factors affecting the overall satisfaction score were disease group and 5 quality indices of medical services(Medical service quality, Rehabilitation services quality, Hospital reputation, External physical environment, and Internal physical environment). Additionally, the significant factors affecting intention to revisit were age, disease group, length of hospital stay and the 5 quality indices of medical services. Conclusions : The results of this study indicate that rehabilitation long-term care hospitals should try to improve the overall satisfaction of inpatients, especially focusing on the internal physical environment and the quality of the rehabilitation services and medical services, which will lead to a high level of intention for patients to revisit.

The Effect of Thoracic Cage Mobilization and Breathing Exercise of Respiratory Function, Spinal Curve and Spinal Mobility in Elderly with Restrictive Lung Disease

  • The purpose of this study was to examine the effects of combined respiratory physical therapy on respiratory function, spinal curve and spinal mobility for community-dwelling elderlies with restrictive lung diseases. In total, 10 patients participated in an 8-week intervention program of thoracic cage mobilization and breathing exercise in combination. The results of the study are as follows: for respiratory function, the forced expiratory volume in 1 second (FEV1), forced vital capacity(FVC), and FEV1/FVC were significantly improved to $.30{\pm}0.31{\ell}$, $.46{\pm}.42{\ell}$, and $18.10{\pm}11.39%$, respectively (p<.05). For spinal curve, the thoracic curve and the lumbar curve were improved significantly to $-2.20{\pm}1.40^{\circ}$ and $-1.20{\pm}1.14^{\circ}$, respectively (p<.01). For spinal mobility, the thoracic flexion ($3.40{\pm}2.99^{\circ}$), thoracic extension ($3.50{\pm}1.43^{\circ}$), lumbar flexion ($4.50{\pm}4.74^{\circ}$), and lumbar extension($-1.50{\pm}1.84^{\circ}$) were all significantly improved (p<.05). These findings indicate that thoracic cage mobilization and breathing exercise in combination improve the respiratory function, spinal alignment, and spinal mobility in elderly people with restrictive lung diseases.

Effects of Five-month Training of Playing Harmonica on Pulmonary Function in Patients With Neuromuscular Disease: A Pilot Study

  • Background: Progressive muscle weakness is aggravated not only in the skeletal muscles but also in the respiratory muscles in many patients with neuromuscular diseases (NMD). Inspiratory muscle training (IMT) has been reported as therapy for pulmonary rehabilitation to improve respiratory strength, endurance, exercise capacity, and quality of life, and to reduce dyspnea. Objects: The purpose of this study was to determine the effect of playing harmonica for 5 months on pulmonary function by assessing the force vital capacity (FVC), peak cough flow (PCF), maximal inspiratory pressure (MIP), maximal expiratory pressure (MEP), and maximal voluntary ventilation (MVV) in patients with NMD. Methods: Six subjects with NMD participated in this study. The subjects played harmonica once a week for 2 hours at a harmonica academy and twice a week for 1 hour at home. Thus, training was performed thrice a week for 23 weeks. The examiner assessed pulmonary function by measuring FVC in the sitting and supine positions and PCF, MIP, MEP, and MVV in the sitting position at the beginning of training and once a month for 5 months. Results: Both sitting and supine FVC significantly increased after playing harmonica (p=.042), as did MIP (p=.043) and MEP (p=.042). Conclusion: Playing harmonica can be used as an effective method to improve pulmonary function in patients with NMD.

Gender-related Factors Associated with Upper Extremity Function in Workers

  • Objectives: This study aimed to find gender distinctions in terms of the sociology of the population; to determine work-related factors; to analyze gender differences in daily living, work, sports, and art performances; and to identify gender-related factors that limited performance of daily living and work activities. Methods: A questionnaire was designed that included disabilities of the arm, shoulder, and hand (DASH), accident history, disease history, work duration at current workplace, marital status, job satisfaction, job autonomy, and physical demands of the job. Out of 1,853 workers surveyed, 1,173 questionnaires (63.3%; 987 males, 186 females) included responses to DASH disability and DASH optional work and were judged acceptable for analysis. Results: Upper extremity functional limitation during work and daily living was higher for females than males. The limitations for males increased according to their household work time, accident history, work duration, job satisfaction, physical demand, and job autonomy. Meanwhile, female workers' upper extremity discomfort was influenced by their disease history, job satisfaction, and physical demands. In addition, the size of the company affected male workers' upper extremity function, while marriage and hobbies influenced that of female workers. Conclusion: This study addressed sociodemographic factors and work-related factors that affect each gender's upper extremity function during daily living and working activities. Each factor had a different influence. Further studies are needed to identify the effect that role changes, not being influenced by risks at work, have on musculoskeletal disorders.

The G115 standardized ginseng extract: an example for safety, efficacy, and quality of an herbal medicine

  • Ginseng products on the market show high variability in their composition and overall quality. This becomes a challenge for both consumers and health-care professionals who are in search of high-quality, reliable ginseng products that have a proven safety and efficacy profile. The botanical extract standardization is of crucial importance in this context as it determines the reproducibility of the quality of the product that is essential for the evaluation of effectiveness and safety. This review focuses on the well-characterized and standardized ginseng extract, G115, which represents an excellent example of an herbal drug preparation with constant safety and efficacy within the herbal medicinal products. Over the many decades, extensive preclinical and clinical research has been conducted to evaluate the efficacy and safety of G115. In vitro and in vivo studies of G115 have shown pharmacological effects on physical performance, cognitive function, metabolism, and the immune system. Furthermore, a significant number of G115 clinical studies, most of them double-blind placebo-controlled, have reinforced the findings of preclinical evidence and proved the efficacy of this extract on blood glucose and lipid regulation, chronic obstructive pulmonary disease, energy, physical performance, and immune and cognitive functions. Clinical trials and 50 years of presence on the market are proof of a good safety profile of G115.

구심성, 원심성, 등척성 운동방법에 따른 혈압과 심박수의 변화 (Blood Pressure and Heart Rate Responses to Concentric, Eccentric, Isometric Exercises)

  • The purpose of this study is to determine cardiovascular reponses to concentric, eccentric and isometric exercise applied to the knee extensor muscle group. Exercise types studied were concentric, eccentric and isometric. The subjects were sixty healthy male volunteers who had no hypertension or cardiac disease. Heart rate, systolic and diastolic blood pressure were recorded prior to starting exercise. The subjects also performed 10RM on right lower extremity. A N-K table was used for three exercises to right knee extensors. Each exercise was selected randomly and applied to each subject 10 times in a 10 second. After each exercise, heart rate, systolic and diastolic blood pressure were recorded immediately. Findings were as follows concectric contractions had a greater effect on the increase of systolic blood pressure and heart rate than eccentric or isometric contractions. Diastolic blood pressure is influenced only by isometric contractions. Eccentric contractions have less effect on the increase of systolic blood pressure and heart rate than concentric or isometric contractions. We hope that the results of this experiment can be adapted to exercise programs for patients with cardiac disease.

만성관절염 환자의 삶의 만족도에 관한 연구 (A Study of Life Satisfaction by Chronic Arthritis Patient)

  • This study is descriptive research clarifying the relations between physical inconvenience, social aptitude and sense of satisfaction In life of the chronic arthritis patients. This research has been conducted from 115 patients by structured self-reporting questionnaires and direct interview of the adult patients over the age of 16 years old who had been under treatment in 3 University hospitals in Seoul from Oct. 4, 1996 through Oct. 24, 1996 for 20 days. The collected materials have been under t-test and F-test(ANOVA) per variables after computerizing using SPSS package and the correlation between various causes was conducted by pearson's correlation coefficient method. The results are as the follows : 1) Generally the age of starling the disease was 40-60 with 91 in female(79.0%) and male 88(76.5%), the persons who had the religion showed the high rate of being taken ill and the persons with over high school education have fallen ill by 46.1%. The monthly income has shown that 77.5% was over 1,000,000won or over, the periods of the disease in average was 4 years 3 months showing it was the chronic disease, the degree of recognizing the disease was 19.0%, spouse and children were most concerned about their disease and the part of Joint attacked was knee(73.0%), wrist(43.5%) and others which showed that they have been very inconvenient in daily life. 2) The variables affecting to physical inconvenience were by age (F=9.06, p=.000), education level(F=7.14, p=000), economic standards (F=2.18, p=.05), the period of disease(F=3.09, p=.03), hospitalized or not(F=3.23, p=.002), showing such correlations. 3) The variables affecting to the social adaptation were by age (F=3.47, p=.018), education level(F=4.98, p=.001), periods of taken ill (F=419, p:.007), hospitalized or not (F=3.23, p=.004) and the operation or not (F=1.30, p=.028) showing such correlations. 4) The variables affecting to life satisfaction were by sex(t=2.08, p=.045), economic status(F=3.15, p=.01) being able to explain statistically. Through the above correlations, they are required to receive self-management education positively to elevate the effect of rehabilitation treatment and effective nursing arbitration and also they required to be reduced physical inconvenience at the early stage and be helped to be able to lead the quality life in satisfaction by elevating the diverse adaptation to the society by correcting the transformed joint.

흉추 후만증에 관한 임상적 중요성과 측정법 (Clinical Importance and Measurement in Thoracic Kyphosis)

  • Thoracic kyphosis is occasionally used to describe someone with accentuated thoracic curvature, hyperkyphosis is preferred since kyphosis itself refers to the normal sagittal angle of thoracic curvature. The angle of thoracic kyphosis tends to increase with age resulting in hyperkyphosis in some individuals. The persons who suffer from hyperkyphosis are at increased risk for a variety of adverse health outcomes that include musculoskeletal alteration, physical functional limitations, poor quality of life, falls, and even earlier mortality. Hyperkyphosis may develop from vertebral fractures, degenerative disc disease, either muscle weakness, decreased mobility and sensory deficits. The gold-standard orthopaedic technique for assessment of thoracic kyphosis is standing lateral spine radiographs. Other clinical measures are Debrunner kyphometer, inclinometer, flexicurve ruler, arcometer, flexible electrogoniometer and spinal mouse.

전환장애의 치료와 관리 (Treatment and Management of Conversion Disorder)

  • Conversion disorder is a psychologically produced alteration or loss of physical functioning suggestive of a physical disorder. Conversion symptoms are often superimposed on organic disease and can be overlooked. Psychological techniques are central to the management include the following: avoiding confrontation with the patients; avoiding reinforcement or trivializing the symptoms; reviewing results of tests and exams and creating an expectation of recovery; educating the patient before a treatment is begun; evaluating the patient's emotional adjustment and considering it at a treatment; using caution in labeling the condition; considering referral for psychotherapy; establishing particularly a treatment plan and making a definite treatment program; adjusting patient' s environment; letting participate a family at appropriate time; developing a reinforcement program for a treatment of chronic symptoms; developing a home program for outpatients. Use behavior therapy reinforcement may be helpful with more chronic or resistant symptoms, especially when there is a history of vague or excessive somatic complaints or significant secondary gain.

  • The Han Zeong Mag was the typical hydro-therapy of Korea. It looks like hot air bath of western physical therapy. It developed out of the CHO-SEON, King SE JONG(1422). It is warmed fire of pine wood in the room. At first, it runs a temperature about a few hundred and drop to $80^{\circ}C$ through 12 hours then the room air is exceeding dry. After 12h. reheated in the room. Women makes use of that place with bare body. Because for use salt rub of skin. They have low back pain, pain of muscle, muscle spasm, swelling of arthritis, stiffness of joint and it takes effect on the cosmetic of skin. The treatment effect was excellent to pain(28%), next arthritis(25%). But they have not order for treatment of a doctor. It's dangers of heat stroke and heart disease. Imtortant, must educated method of it's use and danger when it's bad use.
