가정.방문물리치료 및 학교물리치료의 필요성 및 유형실태에 대한 조사연구 (A Survey of Needs and Types of Home Physical Therapy, Visiting Physical Therapy and School Physical Therapy)

  • The purpose of this survey was to give data and information about type and needs of Home Physical Therapy, Visiting Physical Therapy and School Physical Therapy for physical and nurse. The subjects were 154(99 physical therapists and 55 nurses) who were working at geriatric rehabilitation hospitals and children hospitals. The period of questionary collection was from the 15 of August to the 15th of September 2011. And data was analysis from 99 articles such as journals related to physical therapy, and searched with keyword 'home and visiting physical therapy' by web site and Korea National Assembly Library from 1991 to 2011. The data was analysis with percentage, mean, standard deviation and ANOVA by SPSS PC 12.0. The results were as follows; 1. The definition of 'Home Physical Therapy' has been community based on physical therapy service for the patient who had diagnosis by medical doctor, has been based on medical law. The definition of 'Visiting Physical Therapy' has been community based on physical therapy service at home for the patient who had diagnosis by medical doctor, for the national basic living security, and senior citizen over 65 years who lives alone, has been based on law for community health and law of long term health insurance. The definition of 'School Physical Therapy' has been school based on physical therapy service at school after class for the disabled children who are studying at school, has been based on special education law article 28. 2. As for the knowledge of the Home and Visiting and School Physical Therapy, both groups PT and nurse were 'I do not know'125(81.3%) of the difference the concept of 3 definitions, so it means to need education and information about the different concept of three physical therapy. As for the needs of home and visiting physical therapy, both groups of PT and Nurse were 'needs' 151(98.1%). Physical therapist showed of 'Needs' on visiting physical therapy 35(35.4%), home physical therapy 32(32.3%), and schole physical therapy 32(32.3%). Nurse showed of 'Needs' on home physical therapy 23(41.8%). visiting physical therapy 19(34.5%), school physical therapy 13(23.6%). Therefore it is necessary to have home and visiting physical therapy as for the elderly and disabled person. 3. As for the qualification of Home and Visiting physical therapist, both PT and nurse groups showed as follows; take post graduation education program for home and visiting therapy after became PT : home physical therapist 108(70.1%), visiting physical therapist 106(68.8%). So it means education center or university can be developed post graduation program for home and visiting physical therapist. 4. As for the 'Needs' of school physical therapy, both groups of PT and nurse showed as follows; 'Needs' 142(92.2%), 'Needs superviser education program' 148(96.1%), in PT group showed 'I will participate of education program' 92(92.9%). 5. As for the present states of research papers or report of home, visiting, and school physical therapy was as follows; the 103 papers for 8 fields about' the needs of home and visiting physical therapy' from 1991 to 2011, the 13 papers for 2 fields about school physical therapy from 2001 to 2011, so total papers were 114 articles.

거동 불편 노인의 가족관계와 의.식생활에 관한 연구 -(제2보) 거동 불편 노인의 가족 관계 및 지지 실태 연구- (Studies on Family Caregiving, Clothing and Nutrition of Disabled Elderly -(Part 2) Family Relationship and Caregiving of Disabled Elderly-)

  • This study examines a pattern of family caregiving and its effect on life satisfaction of disabled elderly people. Data were gathered through the use of face-to-face interview method from a sample of 135 elderly aged over 60. One-third of the elderly respondents have no family caregiver. Most of caregivers provide routine household chores and physical help, but persnal care such as bathing and changing clothes was taken by elderly people themselves. The caregiver in an elderly couple is most likely the wife. When a spouse is unavailable, daughters-in law usually assume the role of a primary caregiver. Sex difference has been found in the most comfortable caregiver for the elderly. The most comfortable caregiver for the elderly men is shown to be their spouse, whereas the one for the elderly women is show to be their daughter-in-law. Most of the disabled elderly are emotionally dependent on their son(usually their eldest son), but physically dependent on their spouse, daughter-in-law, or daughter. The caregivers are dominated by women. Among those elderly receiving care from their children, the relationship between the aged their caregiver is found to be closer than the past when the aged were healthy: however, this relationship is found to be almost the same as the past among those elderly receiving care from their spouse. Finally, the degree of family solidarity, satisfaction with received cares, and the relationship with caregivers are found to be closely related to life satisfaction of the elderly respondents.

발달장애 아동을 위한 어머니 건강교육프로그램이 자녀 건강관리행위와 양육스트레스에 미치는 효과 (The Effect of the Health Education Program for Mothers with Developmental Disabled Children on Health Care Behavior and Parenting Stress)

  • 본 연구는 발달장애 아동을 가진 어머니를 대상으로 한 건강교육프로그램을 개발하고 건강관리행위와 양육스트레스 수준을 비교하여 효과를 검정한 비동등성 사전사후 대조군 실험설계 연구이다. 연구대상자는 발달장애 아동의 어머니로 실험군 28명과 대조군 26명으로 총 54명이다. 건강교육프로그램은 전문가 집단 토론과 선행연구들을 토대로 하여 개발하였으며, 프로그램 적용은 주 2회 교육으로 총 4주간 이루어졌다. 프로그램의 전반 2주간은 신체적 건강교육 내용이며, 후반 2주간은 사회심리적 교육내용으로 구성하였다. 사전, 사후, 추후를 통해 자료 수집을 하였으며, 수집된 자료는 repeated measure ANOVA를 통해 분석하였다. 연구결과 프로그램 적용 전, 프로그램 적용 후, 그리고 4주후에 따라 건강관리행위 수준은 향상되고, 양육스트레스는 유의하게 감소하는 것을 확인하였다. 또한 실험군이 대조군에 비해 유의한 차이를 보였다. 이러한 결과는 본 연구를 통해 개발된 건강교육프로그램이 대상자의 건강요구에 적합하였고, 건강관리행위 수준 향상과 양육스트레스를 감소시키는 데 효과적임을 알 수 있었다. 향후에도 발달장애 아동의 건강증진을 위해 다양한 건강교육프로그램 개발과 연구가 지속되어야 할 것이다.

부산대학교병원 소아치과에서 시행한 전신마취에 대한 연구 (A SURVEY OF GENERAL ANESTHESIA IN PEDIATRIC DENTAL CLINIC AT PUSAN NATIONAL UNIVERSITY)

  • The use of general anesthesia as a special method of behavior management is necessary if certain handicapped or disabled child patient to receive dental treatment. This study was designed to report the results of 53 cases of complete oral rehabilitation under general anesthesia. The data were obtained from patients who were provided with dental treatment under general anesthesia for last 3years managed at the Dept. of pediatric dentistry in PNU Hospital. The distribution of age, gender, primary reason for general anesthesia, duration of dental procedure, number of treated tooth and periodic recall check-up were surveyed. In distribution of age, most(78%) were younger than 10 years and mean was 13.0 years. The reasons for providing general anesthesia were lack of cooperation due to various mental and physical handicapped situation(74%), congenital heart disease(13%), combined with medically compromised and behavior problem and others. The average duration of the treatments was 2 hours and 41 minutes and average duration of the anesthesia was 3 hours and 6minutes. The mean number of treated with restoration a children were 16.7 teeth. From the results, total dental rehabilitation under general anesthesia is a favorable modality to improve for disabled children's oral condition.

장애아 문화예술교육을 위한 만화콘텐츠 제작에 관한 연구: 전조작기 단계의 경도 정신지체 아동을 중심으로 (A study on producing cartoon contents for culture and art education for disabled children: Focusing on mild mentally retarded children on stage of pre-operational period)

  • 장애아의 문화예술교육은 매우 중요한 활동임에도 불구하고 '장애유형과 수준에 맞는 지도방법 및 프로그램 부재'로 최근까지도 활발하게 이루어지지 못했다. 이러한 상황에서 만화콘텐츠는 지적 장애아의 문화예술교육을 위한 가장 적절한 교구로서 활용될 수 있다. 만화콘텐츠의 오락적 기능과 장르적 특성은 대상 아동이 보다 자발적이고, 적극적으로 참여할 수 있도록 동기를 유발하고 흥미로운 활동을 위한 교구로서 기능할 수 있다. 물론 일반아동을 대상으로 제작된 만화콘텐츠를 특수교육현장에 그대로 적용하는 것은 적절하지 않으므로 대상 아동에게 적절한 콘텐츠를 개발해야 한다. 만화 해독 능력에 관하여 조형요소 인지, 문자와 기호 인지, 연상과 사고력 등 세 가지 측면으로 살펴본 결과, 연구 대상 아동인 전조작기 수준의 경도정신지체 아동은 그 수준에 맞게 제공된 콘텐츠에 대하여 단순화, 상징화된 만화의 이미지를 실사 이미지보다 더욱 명확하게 이해할 것이며, 문자와 이미지가 서로 의미를 보완하는 만화의 특성은 문자와 기호의 인지를 더욱 용이하게 할 것으로 기대된다. 또한 연상과 사고력 또한 대상 아동의 기억 한계 내에서 구성할 경우 만화의 칸 전개 유형 중 대부분 형태의 연상활동이 가능하였다. 만화의 구성에 있어 주제와 소재는 전조작기 수준의 아동이 이해할 수 있으며, 생활연령 8~13세가 흥미를 유발할 수 있어야 하며, 작품의 분량은 일반아동을 대상으로 한 만화보다 적은 분량으로 수준에 맞게 계획하도록 한다. 문자와 기호가 작품 내에서 보조적인 역할만 하도록 분량 제한하고, 만화 이미지의 스타일 표현과 페이지의 칸 구성 등은 대상아동의 특성과 장애의 정도를 고려하도록 한다. 그러나 이러한 시도가 교육현장에 기여하려면 다양한 만화콘텐츠 제작이 시도되어 장애 유형별 개념과 정서 및 사회 행동적 특성, 인지와 학업성취 능력, 언어 및 신체 건강 특성을 고려한 만화콘텐츠 제작의 구체적 지침을 만들어 내는 것이 필요하며, 개발된 만화콘텐츠가 피상적인 결과물에 머물지 않도록 적용성을 높이기 위해 개발 과정에서 수업 적용 등 대상 아동과 충분한 의사소통이 필요할 것으로 사료된다.

Effect of Horse Riding on Balancing Ability in Children with Cerebral Palsy

  • This study was conducted to investigate the effect of a horse riding program on balancing ability in children with cerebral palsy. Eleven children (five males and six females) diagnosed with cerebral palsy participated. The horse riding exercises (walking and trotting) were conducted twice per week for 30 minutes during 24 weeks. Balancing ability was measured three times at pre, mid, and post-test using an air pad by the same physical therapist. The data were analyzed using a two-way repeated-measures analysis of covariance with time (0, 12, and 24 weeks) using SPSS version 18.0. A comparison of horse riding between patients with hemiplegia and paraplegia was conducted with the Willcoxon signed-ranktest at a predetermined probability rate of 5%. The results showed a significant increase in balancing ability after horse riding than that before horse riding (p<0.01). The average balancing score increased greater in males ($54.59{\pm}84.05$) than that in females ($27.84{\pm}12.67$) after the horse riding exercise program compared to that before the program (p < 0.05). Thus, horse riding exercise was considered an effective to improve balance in children with cerebral palsy. These results provide useful basic data for horse riding for the disabled.

뇌병변장애 1급 아동에서 발생한 중증유아기우식증 (SEVERE-EARLY CHILDHOOD CARIES IN CHILDREN WITH GRADE 1 NEUROLOGICAL DISORDER)

  • 뇌병변장애인은 뇌성마비, 외상성 뇌손상, 뇌졸중 등 뇌의 기질적 병변에 기인한 신체적 장애로 보행 또는 일상생활의 동작 등에 상당한 제한을 받는 사람으로 정의된다. 최근에 뇌병변 장애아동의 DMFT는 비 장애아동과 유사하거나 낮다는 연구가 있었다. 그러나, 본 증례는 중증유아기우식증이 나타난 뇌병변장애 1급 아동들을 치료한 것으로, 아동들의 공통점을 통해 중증유아기우식 우식증의 특별한 원인을 찾고자 하였다. 뇌병변으로 인한 일차적인 운동장애와 이차적인 치과치료를 받기 어려울 정도의 쇠약함, 잦은 입원, 당분이 많은 음식과 약물의 섭취, 구강위생관리 소홀, 늦은 치과 내원, 협조도 저하 등이 우식 유발인자를 높이고, 우식 예방인자를 낮추어 치아우식증을 유발할 수 있는 것으로 사료되었다. 뇌병변 장애아동을 치료할 때, 치과의사는 개인적인 우식 위험도 평가를 통해 철저한 우식 예방프로그램을 만들고, 부모와 의학적 주치의는 구강관리의 중요성을 인식하여 조기 치과 내원 및 홈케어에 신경을 써야 할 것이다.

한국 재활간호 현황과 전망 (Current Status and the Future Prospect of Rehabilitation Nursing in Korea)

  • The history of rehabilitation of disabilities in Korea began with the foreigners and missionaries who were interested in it after Korean War. In 1981, Disabled Persons Welfare Act was enacted and the 88 Paralympics brought the nations attention to the welfare and rehabilitation of persons with disabilities. Since then, the facilities and the services for the disabled persons have expanded rapidly and the rehabilitation treatment and nursing intervention are drawing more attention. Against this background, the survey on the current status of disabilities, welfare service, facilities, and rehabilitation nursing was conducted. The results of this survey are as follows. 1. According to the 2000 census of disabilities, the number of persons with disabilities in Korea is estimated at 1,449,500, or 3.09% of the entire Korean population, 0.74% up from 2.35% in 1995. 2. Disability Types in 2000 The 2000 census showed that the persons with disabilities numbered 1,449,496 out of the total population and 1,024,371 persons are registered for disability, making up 70.7% of the estimated disabled population. Among them, physically disabled persons accounted for the largest 41.7% (605,127) and mentally retarded persons stood at the smallest 9% (13,481). 3. Percentage of Disability Presence The survey showed that more than 90% of disability were acquired. However, 44.8% of mental disability and 61.4% of hearing/speaking disability were not acquired after birth. This means that these disabilities happened by congenital cause or birth accident. 4. Yearly Figure of Registered Disabled Persons In 1989, 218,601 persons registered for disability and, in 2000, the number increased by 4.7 times to 1,024,371. These figures are different from the actual number of disabled persons. According to the 1995 census, 1,053,486 were disabled persons but only 378,323registered for disability. And, in the 2000 census, 1,024,371 out of the 1,449,496 of disabled persons registered for disability. 5. Welfare Service for Persons with Disability 62.6% of the total disabled people are registered and physically disabled persons accounted for the highest percentage of 96.7%. 26.5% of non-registered disabled people said that they didnt know the registration procedure. The rest of them replied that they didnt think they were disabled or that registration didnt seem to give any benefits. 6. Welfare Policies for Disabled Persons The welfare benefits given to the disabled are as follows: Issuance of disabled sign for car drivers, Permission to use LPG fuel, Communication fee reduction, Tax exemption related to cars, Reduction of public facility fees, Household allowance, Tax reduction or exemption, Medical allowance and education subsidy for children, and Housing. 7. Current Condition of Welfare Facilities by Disability Type The welfare institutions for disabilities numbered 188 in total and they can accommodate 16,823 persons. Categories of these institutions are physical disability(37), visual disability(10), hearing/speaking disability(14), mental retardation(59), and sanatoriums(68). 8. Human Resource of Rehabilitation of Disabilities Advanced education programs include rehabilitation nursing in its curriculum and this was selected as the program of Korean Academic Society of Nursing in 1990. In November 1997, Korean Academic Society of Rehabilitation Nursing was launched and many academic meeting and seminars were held. This organization is also making efforts to develop the education program for qualified rehabilitation nursing professionals and to develop the standards of rehabilitation nursing practice. In the professionals of the rehabilitation, there are rehabilitation specialist, physical therapist, speech therapist, occupational therapist. It is needed to come up with the measures to supply stable human resources following the demand of disabled persons and to recognize the private certificates for rehabilitation professionals as official ones after reviewing the education and training programs of private institutions. 9. Rehabilitation Nursing 1) Rehabilitation nursing was taught as an independent subject in 11 undergraduate programs and 9 graduate programs. 2) Research on rehabilitation nursing in Korea were 24 experimental research and 11 non-experimental research. The intervention of experimental research were mostly education and exercise rehabilitation programs. 3) In the three rehabilitation hospitals, nursing is divided into two categories, direct nursing and education & counseling. Direct nursing includes tracheostomy or nasogastric tube care, urination and defication, skin care, pain control, complication prevention and care, prevention of injury from a fall, etc.

장애아동 주양육자의 물리적환경과 정서적 환경 관계에서 지적 웰니스의 매개 작용 (A study on the mediating effect of intellectual wellness in the relationship between physical and emotional wellness of caregivers of children with disabilities)

  • 본 연구의 목적은 장애아동 양육자의 물리적·정서적 웰니스 사이에서 지적 웰니스의 매개효과를 조사하여 효율적인 관리 방안 제시에 필요한 기초자료를 제공하기 위함이다. 연구 대상자는 장애아동 양육자 177명이며, 자료수집은 2019년 06월부터 2019년 11월까지 하였다. 자료분석은 PROCESS macro를 이용하였다. 일반적인 특성과 변수간 분석은 Independent t-test 및 ANOVA를 통해 통계적으로 유의한 차이가 있는 변수에 대하여는 Scheffé 사후검증을 실시하였다. 변수간 관련성 분석은 Pearson 상관관계 및 위계 다중회귀분석을 하였다. 연구결과, 장애아동 주양육자의 물리적 웰니스와 지적 웰니스는 장애아동 양육자의 정서적 웰니스에 직접적인 영향을 미치며, 또한 물리적 웰니스와 정서적 웰니스 사이에서 지적 웰니스가 매개효과를 제공하고 있다. 이는 장애아동이 가진 물리적 환경의 영향도 중요하지만, 주양육자에게 적절하고 충분한 지적 교육이 동반되어야 정서적 웰니스 증진이 이루어질 수 있다는 것을 발견하였다. 따라서 다양한 교육 프로그램의 확대는 장애아동 양육자의 웰니스 증진에 기여할 수 있을 것이다.

어린이병원 외래진료부 이용자의 대기 및 휴게행태 연구 - 장애 및 비장애 어린이의 비교 분석을 중심으로 - (A Research on the Waiting and Resting Behaviors of Children in the Outpatient Clinic of a Children's Hospital - Focused on the Comparative Analysis between Children with and without Disabilities -)

  • This study investigates the waiting and resting behaviors of children within a restorative healthcare facility design. In particular, the aim is to compare children with and without disabilities and reveal similarities and differences in terms of their behavioral characteristics and uses of design facilities, related to positive distraction, for restoration in the hospital. Three major common spaces for outpatients in S children's hospital in Seoul were examined including the main lobby and two waiting and resting spaces in the pediatric and rehabilitation medicine departments, respectively. A total of 155 children under the age of 12-67 with physical disabilities and 88 without-were observed while they waited and rested at the three spaces before or after doctors' examination. Basic demographic information and waiting and resting behavioral characteristics were compared between the two groups. The results indicated that the disabled children were more restricted in terms of space, facilities used, and social behaviors. However, regardless of disabilities, the children showed more positive distractions related to cognitive and social behaviors in areas where restorative design elements such as an aquarium, garden, or visual images were available. Based on the results, design implications were discussed to strengthen positive distraction behaviors in children with and without disabilities and to foster the restorative quality of the spaces for waiting and resting in the children's hospital.
