Based on the literature, status and role the music therapist in America was reviewed for this study. The process of developing a music therapy program in America suggests to us many things: In America, music therapists have sustained a mutually beneficial status with their clients for, over fifty years. Excellence in academic education and clinical training enable music therapists to continue to provide quality music therapy. The magnitude of change in to music therapy in the United States, however creates the challenge of providing real access to music therapy continues in the future. Music therapy is the use of music in the accomplishment of therapeutic aims: the restoration, maintenance, and improvement of mental and physical health. Music therapists work with individuals of all ages who require special services due to behavioral. social. learning, or physical disabilities. Employment may be in hospitals, clinics, day care facilities, schools, community mental health centers, substance abuse facilities, nursing homes, hospices, rehabilitation centers, correctional facilities, or private practices. The American Music Therapy Association (AMTA) was founded in 1998 as a result of a union between the American Association for Music Therapy (founded in 1971) and the National Association for Music Therapy(founded in 1950). Music therapists are highly qualified professionals who have completed approved degree programs and had clinical training in order to receive Board Certification(MT-BC), with the designation of Registered, Certified, or Advanced Certified Music Therapist(RMT. CMT - or ACMT). AMTA provides several mechanism for monitoring the quality of music therapy programs: Standards of Practice. a Code of Ethics, a system for Peer Review, a Judical Review Board, and an Ethics Board. According to the results of this study, the suggestions were as follows: 1. It is concluded that music therapy as a nursing intervention can be effective for the clients. 2. It is a great challenge to develope a music therapy program for nursing intervention however, it is also task and responsibility to further the development of nursing.
Objectives: This study aims to determine fostering stress and mental health state that mothers of handicapped children perceive as primary care givers and to analyze their demand for information assistance in order to release their stress so that it can provide materials that contribute to establishment of assistance system for families with handicapped children. Methods: The research subjects were 340 mothers whose children went to a nursery for special children, 3 general nurseries and 6 special schools in Daegu, and the data were collected using structures questionnaires. The survey analyzed mothers' fostering stress, their demand for fostering information assistance, children's daily activity abilities. Component concepts of each scale was validated by adopting confirmatory factor analysis, and factors affecting demand for fostering information assistance were analyzed by adopting covariance structural analysis. Results: Younger mothers tend to have higher demand for information, and mothers with younger children or children with double handicaps also have higher demand. Mothers under 30 have the lowest demand for public health and medical care assistance and for home and community life assistance, while mothers with children with physical handicaps have the highest. The validity of component concepts was verified by categorizing as cognitive structure models fostering stress, information demand, children's daily activity abilities, and their appropriateness was evaluated through confirmatory factor analysis using structural equation modelling. And then, GFI (more than 0.9), CFI (more than 0.9), TLI (more than 0.9) and RMSAE (less than 0.08) were used to evaluate the appropriateness. It was found that all the component concepts are valid, as every item is within appropriate range. The result of analyzing information demand demonstrated that children's handicap levels significantly affect their mothers' mental health, while fostering stress significantly affect mothers' metal health, information demand. As well, it was confirmed that mothers' mental health has a significant effect on information demand. Conclusions: Therefore, to reduce special children's mothers' uncertainty, helplessness and fostering burden, it is necessary to provide them with information on children's challenges, development and fostering and to offer them quality public health, medical care and welfare assistance along with family and local community life assistance.
Background: Elderly patients with progressive dementia including Alzheimer's disease (AD) are more and more often scheduled to undergo general anesthesia for various pathologies including dental problem. But, there is high risk of deterioration of underlying mental diseases and other co-morbidities. So it is important to implement preventive strategies and take adequate measures to minimize negative perioperative events in these patients. Methods: We reviewed the 17 cases of 11 patients with AD who underwent ambulatory general anesthesia for dental treatment at the clinic for the disabled in Seoul National University Dental Hospital. Results: The mean age was 68 (57-81) years. All of them were diagnosed with AD and some had hypertsnsion, bronchiectasis, urinary incontinence. For anesthesia induction, 3 cases (1 patient) was needed physical restraint, but others showed good or moderate cooperation. Drugs used for anesthesia induction was thiopental (11 cases), propofol (3 cases) and sevoflurane (3 cases). All patients received nasotracheal intubation without difficulties. Mean total anesthetic time was 3 hour 44 min ${\pm}$ 60 min and staying time at PACU was 83 ${\pm}$ 34 min. All the patients except one who showed hypertension discharged without any complication. There was no death or long term hospitalization because of severe complications. Conclusions: If general anesthesia is needed, pertinent diagnostic tests and workup about other medical problems, and appropriate anesthetic planning are essential for safety.
Objectives: The purpose of this study was to identify the factors of satisfaction with regional oral health centers for people with disabilities in Korea. Methods: A survey was conducted with 784 patients who visited regional oral health care centers in Korea. A self-developed questionnaire (29 items) was adapted with a consent form and IRB (Institutional Review Board). All collected data were analyzed for statistics using SPSS version 25.0. Results: The study participants were 452 (57.7%) male patients, 207 (26.4%) aged 20-29 years old, and mostly had physical, developmental, and mental disabilities. The patients were introduced by their acquaintances, and the major reason for visit was specific dental care for the disabled. Most patients used their cars and took half to one hour to commute. The common factor affecting the comprehensive satisfaction was the facility satisfaction. Conclusions: A more comprehensive and standardized questionnaire should be developed for the annual evaluation of all centers. This study presents the basic data that can aid to activate the operation of the four oral health care centers for the disabled, which are scheduled to be implemented in the future, as well as 10 currently operated centers.
A 15-years-old female patient with seizure disorder and pervasive developmental disorder was scheduled for dental treatment under ambulatory general anesthesia. She had past history of pneumonia and herpes encephalitis when she was 3 year old. Because of sever mental retardation and behavior disorder, routine laboratory test was substituted with physical exam and medical records of department of pediatrics. A few days before general anesthesia, she showed slight common cold, but pediatric consult had reported that there was minimal risk in general anesthesia. After 4-hour general anesthesia, she became critically sick with high fever, cough and malaise. After 10-day hospitalization with pneumonia and sepsis, she could go home.
This study was performed on 26 disabled adolescent participants (16 male, 10 female) with the objective of testing changes in equilibrium after engagement in horseback riding. Participants of total 26 persons were divided into three groups as follows: 6 children with Cerebral Palsy (CP), 14 children with Intellectual Disability (ID) and 6 children with Autism (AT). Participants engaged in therapeutic horseback riding (TR) two times per week for 30 minutes per session. The 26 participants demonstrated a considerable increase in equilibrium ability, with an average increase in equilibrium time of $44.22{\pm}50.70$ sec after TR. Equilibrium also increased according to disability group: CP (P < 0.05), ID (P < 0.001), and AT (P < 0.05). TR should be considered as a possible method for improving functionality in the physically disabled. This data may also be usefully applied to the development of a horseback riding program for the improvement of equilibrium in the disabled.
국민건강보험공단 직장가입자들 중 만 20세 이상의 장애인 건강검진 수검자들을 대상으로 2009년도 3,186명, 2013년도 3,611명의 경증장애인들을 조사하였다. 경증 장애인은 장애인 분류표준 기준으로 지체장애, 뇌병변장애, 시각장애, 청각장애, 지적장애, 정신장애, 신장장애, 기타장애 판정을 받은 장애인들 중 3~6등급 장애인으로 그룹화하였다. 연구의 목적은 경증장애인들의 비만이 고혈당, 고혈압, 고콜레스테롤에 미치는 위험성을 살펴보고자 한다. 측정 항목으로는 복부비만, Body Mass Index, 공복시 혈당, 총콜레스테롤, 이완기 확장기혈압을 조사하였으며, 비만도와 각 질환의 위험군과의 관련성을 살펴보기 위해 교차분석과 다항 로지스틱회귀분석을 시행하였다. 그 결과, 남성, 연령이 낮을수록, 소득이 높을수록 복부비만과 BMI가 증가하였다. 또한, 각 질환별 이상군에서 복부비만군과 BMI의 위험도가 높았다. 2009년도는 BMI 비만군이 정상군에 비해 고혈압 위험도는 1.51배 높게 나타났다. 복부비만군은 정상인에 비해 고콜레스롤은 1.59배, 고혈압은 1.26배, 고혈당은 1.54배 더 높은 위험도를 나타냈다. 2013년도는 BMI 비만군이 정상군에 비해 고콜레스테롤은 1.72배, 고혈압은 1.43배 더 높은 위험도를 나타냈다. 복부비만인은 정상인에 비해 고혈당의 위험도가 1.59배 더 높게 나타났다. 장애인에서 비만의 위험도가 정상인에 비해 더 높게 나타난 결과로 보아, 장애인들의 정기적인 건강검진 수검율을 높이고 확대실시하여, 장애인들의 질병양상을 확인하고, 비만예방에 대한 적절한 교육과 관심이 요구되어진다.
이 연구에서는 우리나라에서 개발된 의사소통 측정도구의 사용대상, 문항 구성 요인을 알아보고, 보완대체의사소통체계(AAC) 사용자에게 적용된 의사소통 측정도구 분석을 통해 의사소통 측정도구와 AAC의 관련성을 알아보았다. 장애인을 대상으로 언어 및 의사소통 능력을 측정한 경우는 실어증이 월등히 많았으나 다른 장애유형에 적용한 경우는 드문 것으로 나타났다. 의사소통 측정도구의 문항 구성요인은 의사소통제한 요인과 의사소통을 제한하는 신체기능장애 요인을 많이 포함하고 있는 반면, 의사소통 참여 제한 요인과 의사소통을 방해하거나 촉진하는 환경적 요인이 포함된 경우는 많지 않았다. AAC 중재는 지체장애, 자폐성장애, 정신지체, 발달장애, 의사소통장애 순으로 많이 적용되었고, AAC 중재 실험에 사용된 의사소통 측정도구는 "그림어휘력검사", "수용 표현어휘력검사"가 많이 사용된 것으로 나타났다. 결과에 근거하여, AAC 적용 및 개별화교육 지원의 근거를 제공할 수 있는 평가의 필요성과 평가 준거 및 측정문항에 대한 연구가 시도되어야 함을 논의하였다.

This research is to check out the possibility of the systems and the effectiveness of various services and the flexibility of management in Japan. Takurosyo is responsible for a variety function of elderly medical welfare facilities. Within a short time, our country, has entered into a super-aged society, elderly health and welfare facilities are needed. However, because it requires enormous financial, it is difficult to build a new building in reality, However, if remodeling existing buildings, We can build many low-cost small-scale multi-functional welfare facilities such as the takurosyo. Loss and damage have become a vital contemporary issue in climate change studies and actions in developing countries. However, studies are scant on this in the fisheries sector around the world. In Bangladesh, there is no study on the loss and damage in fisheries dependent communities. This study assesses economic and non-economic loss and damage to coastal shrimp farms due to cyclone Bulbul in Gabura Union of Shyamnagar Upazila, Satkhira district, using a mixed method approach. Results show that all shrimp farms' dependent communities are affected by cyclone Bulbul to some extent. About 14%, 57%, and 29% of the farms were totally, heavily and moderately damaged due to farm inundation and dyke damage. The estimated mean loss and damage per shrimp farm was worth USD 4,633. Around 31% and 72% of the farms' fencing nets and traps were lost, which was worth USD 333 per farm. There were also loss and damage to other resources such as houses, solar panels, livestock and agricultural crops where the estimated mean loss and damage per household was worth USD 3,170. This study reported that the rich shrimp farmers encountered proportionately more economic loss and damage than their poor counterparts. However, this does not mean that the poor suffered less. The current study found a range of non-economic loss and damage in different aspects of the shrimp farmers' household members such as unbearable mental pain, deterioration of health, physical injuries, disabilities, etc. and access to services (e.g., inadequate food, lack of safe drinking water, lack of medical facilities, disruption of education systems), social infrastructure (e.g., damage of roads and markets) and disturbance of cultural functions. The findings suggest that urgent short- and long-term actions may be taken to save the aquaculture farms and dependent livelihoods from economic and non-economic loss and damage to cyclones in future.
