Objectives: We investigated injury of corticostriatal (CStr) tract in patient with mild traumatic brain injury (mTBI), which was demonstrated by DTT. Method: A 44-year-old female with no previous history of neurological, physical, or psychiatric illness had suffered from head trauma resulting from a pedestrian car accident. She complained that could not quickly move the left hand with her intension. After three month's administration, her slowness movement of left hand recovered rapidly to the point that she was able to extend all fingers quickly. Results: On DTT configuration, the integrity of the left CStr tract was well-preserved, however the right CStr tract showed narrowing and partial tearing in the subcortical white matter on a DTT at 25 months after onset. Conclusion: Injury of the right CStr tract was demonstrated in a patient who developed mild motor control problems following mild TBI. We believe that the evaluation of the CStr tract from the secondary motor area for patients who showed unexplained motor control problem is necessary.
Purpose: The purpose of this study was to investigate effects of mobilization combined active movement(SNAGS) on the pain and recovery of function of acute low back pain patients. Methods: The subjects were consisted of 135 patients with acute low back pain. All subjects randomly assigned to mobilization group, stretching exercise group and modality treatment group. The mobilization group received mobilization combined active movement(SNAGS) with modality treatment, exercise group received stretching exercise with modality treatment and modality treatment group received modality treatment. Visual Analogue Scale(VAS) was used to measure patient's pain level and Patient Specific Functional Scale(PSFS) was used to measure patient's functional disability level. Results: The results of this study were summarized as follows : 1. Visual Analogue Scale(VAS) was mobilization group showed significantly decreased more than comparison group(p<.01) and active treatment group showed significantly decreased more than passive treatment group(p<.01). 2. Patient Specific Functional Scale(PSFS) was mobilization group showed significantly increased more than comparison group(p<.01) and active treatment group showed significantly increased more than passive treatment group(p<.01). Conclusion: It maybe suggested that mobilization combined active movement(SNAGS) is beneficial treatment for acute low back pain patient.
무릎 골관절염을 가진 환자에게 근거에 기초한 물리치료 접근법으로 15번 임상 치료하였다. 이 환자의 두가지 중요한 손상을 고려하여(stiffness and pain) 두가지의 물리치료 방법이 시행되었으며 그 결과는 Western Ontario and McMaster Universities osteoarthritis (OA) index (WOMAC)로 측정 되었다. 이 임상 연구를 통해 도수치료와 치료적운동법, 그리고 TENS(Transcutaneous Electrical Nerve Stimulation)의 적용이 무릎 골관절염을 가진 환자의 경직(stiffness)과 통증(pain)을 경감시킬수도 있으며 기능을 향상 시킬 수도 있음을 관찰할 수 있었다.
The purpose of this survey was to give data and information about type and needs of Home Physical Therapy, Visiting Physical Therapy and School Physical Therapy for physical and nurse. The subjects were 154(99 physical therapists and 55 nurses) who were working at geriatric rehabilitation hospitals and children hospitals. The period of questionary collection was from the 15 of August to the 15th of September 2011. And data was analysis from 99 articles such as journals related to physical therapy, and searched with keyword 'home and visiting physical therapy' by web site and Korea National Assembly Library from 1991 to 2011. The data was analysis with percentage, mean, standard deviation and ANOVA by SPSS PC 12.0. The results were as follows; 1. The definition of 'Home Physical Therapy' has been community based on physical therapy service for the patient who had diagnosis by medical doctor, has been based on medical law. The definition of 'Visiting Physical Therapy' has been community based on physical therapy service at home for the patient who had diagnosis by medical doctor, for the national basic living security, and senior citizen over 65 years who lives alone, has been based on law for community health and law of long term health insurance. The definition of 'School Physical Therapy' has been school based on physical therapy service at school after class for the disabled children who are studying at school, has been based on special education law article 28. 2. As for the knowledge of the Home and Visiting and School Physical Therapy, both groups PT and nurse were 'I do not know'125(81.3%) of the difference the concept of 3 definitions, so it means to need education and information about the different concept of three physical therapy. As for the needs of home and visiting physical therapy, both groups of PT and Nurse were 'needs' 151(98.1%). Physical therapist showed of 'Needs' on visiting physical therapy 35(35.4%), home physical therapy 32(32.3%), and schole physical therapy 32(32.3%). Nurse showed of 'Needs' on home physical therapy 23(41.8%). visiting physical therapy 19(34.5%), school physical therapy 13(23.6%). Therefore it is necessary to have home and visiting physical therapy as for the elderly and disabled person. 3. As for the qualification of Home and Visiting physical therapist, both PT and nurse groups showed as follows; take post graduation education program for home and visiting therapy after became PT : home physical therapist 108(70.1%), visiting physical therapist 106(68.8%). So it means education center or university can be developed post graduation program for home and visiting physical therapist. 4. As for the 'Needs' of school physical therapy, both groups of PT and nurse showed as follows; 'Needs' 142(92.2%), 'Needs superviser education program' 148(96.1%), in PT group showed 'I will participate of education program' 92(92.9%). 5. As for the present states of research papers or report of home, visiting, and school physical therapy was as follows; the 103 papers for 8 fields about' the needs of home and visiting physical therapy' from 1991 to 2011, the 13 papers for 2 fields about school physical therapy from 2001 to 2011, so total papers were 114 articles.
Background: Patient-centered care has recently become highly recommended, because it can improve health outcomes more effectively than problem-oriented care. The goal attainment scale (GAS) is one of the methods used for patient-centered care. It was originally developed as an outcome measurement tool, but it can also be used as a therapeutic intervention when setting quantifiable patient-centered goals. Objects: We sought to identify the effect of setting patient-centered goals on rehabilitation outcomes in patients with subacute stoke using GAS. Methods: Overall, 46 subjects with subacute stroke were divided into experimental ($n_1=23$) and control ($n_2=23$) groups. Subjects in both groups attended physical therapy sessions five times a week for four weeks. Those in the experimental group set goals using goal-attainment scaling. The mobility of each subject was evaluated using the Rivermead mobility index (RMI); daily-living activities, using the K-modified Barthel index (K-MBI); participation, using the Hopkins rehabilitation-engagement rating scale (HRERS); satisfaction, using the patient-satisfaction scale (PSS). Results: Subjects in the experimental group experienced more significant increases in RMI, HRERS, and PSS than those in the control group (p<.05, p<.05 and p<.01, respectively). After four weeks, GAS scores of the experimental group had increased more significantly than those of baseline (p<.01). Conclusion: Setting patient-centered goals is effective in improving the mobility and satisfaction of patients with subacute stoke. Setting patient-centered goals needs to be performed more frequently in clinical settings.
This study was applied using vojta therapy in the patient with hydrocephalus occurred by on traumatic brain injury. Vojta treatment was a recently developed of the brain damage patient treatment which can be applied eariler than the other traditional methods. The results were as follows. 1. Hip joint flexion contracture from $100^{\circ}$ to $15^{\circ}$ was improved on prone position. 2. Left convexity curve on thracolumbar region due to functional scoliosis the normal aligment. 3. Background: An 8-year-old girl had severe neck pain and stiffness after trauma. CT scan showed atlantoaxial rotatory subluxation (AARS). She had conservative treatment because she did not have neurological symptoms and spinal basilar artery dysfunction. Conservative therapy was halter traction twice for 4 weeks. However, pain and stiffness persisted. She had been recommended to have surgery from her physician, but she received manual therapy for non-surgical procedures. Methods: The joint mobilization, muscle energy technique, motor control exercise, and deep neck flexor (DNF) endurance exercise were applied as manual therapy and 10 session for 2weeks. Results: Clinical outcomes were measured at initial baseline, after 2 weeks, and after 6weeks. Active range of motion was completely restored after 6weeks and numeric pain rating scale was completely reduced after 2 weeks. The strength of neck flexor muscle recovered to normal after 2 weeks, and the DNF endurance was improved to 25 seconds after 2 weeks and to 42 seconds after 6weeks. Motor control capacity recovered to 30 ㎜Hg after 2 weeks. Conclusions: This case report describes the immediate and short-term clinical outcomes for a patient presenting with symptoms of neck pain following AARS. Clinical rationale and patient preference aided the decision to incorporate manual therapy as a treatment for this patient. Manual therapy has shown a successful recovery in AARS patients, more research is needed to validate the inference of this case report.

The purpose of this study was to demonstrate cortical reorganization in a 26-year-old right-handed hemiparetic patient with traumatic primary motor cortex (M1) injury, using functional MRI (fMRI). The unaffected (left) primary sensori-motor cortex centered on the precentral knob was activated during unaffected (right) hand movements. However, the medial area of the injured M1 was activated during affected (left) hand movements. It seems that the motor function of the affected hand in this patient was reorganized into the medial area of the injured precentral knob. These investigations provide a great useful information and clinical evidences with the specialized clinician in stroke physical therapy about patient's prognosis and therapeutic guidelines.
Background: The purpose of this study is intended to evaluate the effectiveness and analyse the change on balance, gait and ADL of the stroke patient by obstacle gait training. Design: Randomized controlled trial. Methods: Ten subjects with stroke patient are recruited from C rehablitation hospital located Chungbuck. The subjects which are divided to 2 group as experimental group (N=5) conducted the obstacle gait training and control group (N=5) conducted the general gait training. The gait (10mWT), balance (BBS) and ADL (FIM) were measured before and after intervention. Results: There were no significant differences (p>.05) in sex, age, height, weight and K-MMSE among subjects. There were significant differences (p<.05) in balance and gait between of the experimental group and control group. But there was no significant difference (p>.05) in the ADL between of the experimental group and control group. There were significant differences (p<.05) gait and balance in the experimental group. But there were no significant difference (p>.05) gait, balance and ADL in the control group. Conclusion: Obstacle gait training showed positive effects on the gait, balance of the stroke patient.
