일부 요통환자들의 오스웨스터리요통장애지수 및 관련요인 (Oswestry Low Back Pain Disability Index and Related Factors in Patients with Low Back Pain)

  • Purpose: We measured the Oswestry Low Back Pain Disability Index (OLBPDI) and related factors in patients with low back pain. Methods: The sample consisted of 50 patients who received physical therapy at the physical therapy units of the Andong Seoul Sintong Clinic, St. Luke Clinic, and Yeongju Seoul Sintong Clinic in Andong and Yeongju city from October, 2007, to February, 2008. The OLBPDI questionnaire was administered by 5 physical therapists as a cross-sectional study. Student's t-test and analysis of variance (ANOVA/Tukey and Scheffe) were used to analyze OLBPDI score differences. We also used nonparametric statistic analysis (Wilcoxon rank sum test, Median test). Pearson correlation analysis (Spearman correlation analysis) was used to analyze the relationship between OLBPDI and the visual analogue scale (VAS). Multiple regression analysis was performed to determine the effects of independent variables on pain scores as defined by the OLBPDI. Results: The average patient age was 37.1 years (range: 18$\sim$78 years old), and time from onset was 21.7 months (1$\sim$180). OLBPD and VAS scores were 12.70 (3.0$\sim$28.0) and 5.14 (1$\sim$8), respectively. OLBPDI scores were 14.4 in patients taking medicine and 11.57 in those who did not. There was a statistically significant relationship between OLBPDI and VAS (r=0.54, p=0.0001; r=0.55, p=0.0001 by Spearman coefficient). Gender ($\beta$=6.14, p=0.0124), age ($\beta$=-2.01, p=0.0324), weight ($\beta$=0.31, p=0.0222), time from onset ($\beta$=1.54, p=0.0044), and VAS score ($\beta$=1.59, p=0.0004) were significantly associated with OLBPD by multiple regression analysis. Conclusion: Variables associated with OLBPD were gender, age, weight, time from onset, and VAS score. Collecting information on the pain index using OLBPDI was acceptable to patients with low back pain. Further research should explore the pain index by using larger sample sizes and longer follow-up periods.

Physical Functions, Health-Related Outcomes, Nutritional Status, and Blood Markers in Community-Dwelling Cancer Survivors Aged 75 Years and Older

  • Background: A cancer survivor is defined as anyone who has been diagnosed with cancer, from the time of diagnosis through the rest of their life. The purpose of this study was to examine whether physical functions, health-related outcomes, nutritional status and blood markers in community-dwelling cancer survivors aged 75 years and older are different from those who do not have cancer Materials and Methods: Two hundred seventy-five participants were asked by physicians, nurses, and physical therapists, questions regarding cancer history in a face-to-face interview. Data were collected for demographic information, physical functions, such as handgrip strength, knee extension power, abdominal muscle strength, static standing balance, walking speed and the timed-up-and-go test, health-related outcomes, nutritional status, and blood markers. The measured parameters of survivor diagnosed with cancer were compared with those without a history of cancer. Results: Thirty-seven older adults were previously diagnosed with cancer. Female cancer survivors had lower knee extension power (p<0.05), abdominal muscle strength (p<0.05), walking speed (p<0.05), timed-up-and-go test score (p<0.05), and time to spend on walking per day (p<0.05) than older women without a history of cancer. In men, none of the measured parameters were significantly different between cancer survivors and older men with no history of cancer. Conclusions: The present study shows that partial physical function of women cancer survivors aged 75 years and older differs from that in women with no history of cancer.

물리치료사의 사회인구학적 특성이 독립개원의 필요성 인식과 전문직업성에 미치는 영향 (The Effects of Socio-demographic Characteristics of Physical Therapist for the Awareness of Necessity of Opening Independent Clinic & Professionalism of the Physical Therapist)

  • 본 연구는 물리치료사의 사회인구학적 특성이 물리치료 독립개원의 필요성 인식과 전문직업성에 어떠한 영향을 미치는지 알아보고자 실시하였다. 연구 방법은 2015년 서울 경기 지역의 물리치료사 253명을 대상으로 사회인구학적 특성과 전문직업성 척도를 설문조사하였다. 분석 결과, 물리치료사는 사회인구학적 특성에 상관없이 물리치료의 독립개원을 위한 법률 수정이 필요하다고 인식하고 있었다. 물리치료사의 전문직업성 평균은 보통 이상의 수준으로 높지 않았으나 근무기관에 상관없이 연령과 최종학력, 임상경력이 높고 관심분야에 따라 전문직업성이 높게 나타났다. 결론적으로 한국의 물리치료사는 물리치료 독립개원을 위한 법률 개정이 필요하다고 인식하고 있으나 아직 까지는 개정에 필요한 전문직업성은 높지 않았다.

발목관절 복합체의 가동범위 측정을 위한 중립위치와 측정방법의 신뢰도 (Reliability of the Joint Neutral Position and Measurement Methods of the Ankle Joint Complex Range of Motion)

  • Purpose: To determine the correct measurement methods of the ankle joint complex range of motion for measuring the neutral position and evaluate the rater reliability. In addition, the impact of training on the rater reliability was also assessed. Methods: The subjects were eleven healthy women, who were evaluated by two physical therapists and one physical therapist recorded the results of the study. Standard goniometer was used as the measurement tool. The ankle and subtalar joint neutral position and the active range of motion of the ankle and subtalar joint were measured. Intra-rater reliability and inter-rater reliability measures were analyzed with intraclass correlation coefficients. Results: Intra-rater reliability and inter-rater reliability ranged from high to medium for the neutral position of the ankle joint complex. Intra-rater reliability for dorsiflexion and plantarflexion measurements was medium, while the inter-rater reliability was high. The range of motion of the subtalar joint was measured, and the intra-rater reliability and inter-rater reliability were low and medium, respectively Also, the intra-rater reliability was increased with formal training of the measurement techniques. Intra-rater reliability was reduced in case the raters had not undertaken the training. Conclusion: In summary, the results obtained with the measurement tools and joint measurement of position, indicate the consistency of repeated measurements made by the same observers. Under the same circumstances along with repetition of the same measurement technique during training caused an increase in the rater reliability of formally trained raters.

The Reliability of a Pediatric Balance Scale Based on the Raters' Clinical Work Experience and Test Experience

  • Purpose: To investigate the rater reliability of a Pediatric Balance Scale (PBS) for children with cerebral palsy, and to investigate possible differences among raters according to their clinical work experience and testing experience. Methods: Study participants included 18 children with spastic cerebral palsy who could walk. They were instructed by pediatric physical therapists, two of whom had ten years of clinical work experience and two who had less than one year of experience. The children's ability to achieve physical balance was videotaped for PBS items. The raters watched the tapes and evaluated each child twice. Rater reliability was analyzed using the intraclass correlation coefficient (ICC). Differences between experienced and novice raters were analyzed using a paired t-test. The statistical significance level was set to 0.05. Results: The total PBS scores averaged 45.78~48.00 and 45.72~47.67 for first and second tests. Intra-rater reliability was very high (ICC=0.89~0.99), and the repeated measurement coincidence was high (p>0.05). Inter-rater reliability was high (ICC=0.83~0.84), but there was a bit of a difference in the coincidence (p<0.05). The experienced raters' reliability and coincidence were higher than those of the novices, and there were differences in reliance and coincidence between experienced and novice raters (p<0.05). Conclusion: Inter-rater and intra-rater reliability is very high. However, rater reliability showed defferences depending on clinical work experience and testing experience. When testing pediatric patients with the PBS, the rater's clinical experience and test experience may affect the test results.

뇌졸중 편마비환자의 견관절 아탈구 예방에 관한 고찰 (Review of Prevention of Hemipelegic Shoulder Subluxation After Stroke)

  • Purpose : Shoulder subluxation is a very common problem in patients with hemiplegia with stroke. Prevention of the low tone subluxed shoulder has been an issue for physical therapists working with neurological patients for many years. Methods : This study reviewed the literature to definite the management and a cause of shoulder subluxation with hemiplegia patients after stroke. Various modalities have been suggested for realigning the glenohumeral joint, but their use is controversial. The purpose of this paper is to review critically the evidence base in order to inform the clinical decision-making process for physiotherapists working in neurology. Results : Literature has identified supports, strapping and functional electrical stimulation(FES) in the management of low tone shoulders. Following review of this evidence it is suggested that there is a lack of reliable and valid research evidence on which to base conclusions. The modalities with the best supporting evidence for realigning the low tone subluxed glenohumeral joint are the triangular sling, Harris hemi sling and the Rolyan humeral cuff used in a standing position and the lap board and arm trough while the patient is sitting. However, due to soft tissue adaptation with associated lack of movement, over-correction and the need for careful patient positioning these supports need to be evaluated for each patient and should be used only in appropriate situations. Conclusion : Similarly, there is a lack of evidence on the effects of long-term use to this equipment. Electrical stimulation is also thought to have potential in the treatment to subluxed low tone shoulders but additional research is required to clarify the parameters for use and the long-term effects of these forms of management.

한글판 아동 균형척도(Pediatric Balance Scale)의 측정자간 신뢰도 (Inter-Examiner Reliability of the Korean Version of the Pediatric Balance Scale)

  • The Pediatric Balance Scale (PBS) is a modified Berg's Balance Scale developed as a balance measure for school-aged children with mild to moderate motor impairments. The purpose of this study was to determine the inter-examiner reliability of the Korean version of PBS when applied to children with developmental delays. In this study, PBS was administrated to a total of 79 children with developmental delays (17 with global developmental delay, 31 with cerebral palsy, and 31 with mental retardation) in the Seoul Community Rehabilitation Center. Two pediatric trained physical therapists with longer than 13 year of clinical experience scored the children's performance blind, while replaying videotaped data. The inter-examiner reliability was statistically determined by intraclass correlation coefficients (ICCs). The results of this study revealed that the Korean version of PBS seems to have high inter-examiner reliability when applied to children with movement disorders such as global developmental delay (ICC=.96) and cerebral palsy (ICC=.97); however, it has relatively lower inter-examiner reliability (ICC=.78) for children with developmental delay secondary to mental retardation. therefore, the results support that the Korean version of the PBS could be a useful clinical measurement to assess the balance skills for children with developmental delay who have an adequate level of cognition to enable them to fol1ow the verbal instructions to complete the test.

원적외선이 관절염 흰쥐 모델에 미치는 영향과 관련 싸이토카인 예측 프로그래밍 (The effects of Far-infrared on Type II collagen induced mouse and the relative cytokine computerize programming (Emphasis on TNF-a and IL-1b))

  • The purpose of this study was to observe the effects of Far-infrared on mice rheumatoid arthritis induced by type II collagen. The author peformed several experimental tests which were the hematoxylin-eosin tissue stain, the hind paw thickness, and ELSIA of cytokines(TNF-$\alpha$, IL-1$\beta$). On day 14 and 21, FIR-Tx. group decreased in synovial membrane thickness more than control II group FIR-Tx group decreased in hind paw thickness more than control II group. In the ELISA test of TNF-$\alpha$ concentration, control II group significantly increased in the concentration more than normal and FIR-Tx. group on day 7, 14, and 21. In the ELISA test of IL-$\beta$ concentration, FIR-Tx. group decreased in the concentration more than control II group on day 14 and 21. With J's Linear Function Data Process Program, researchers can get the estimated the concentration or the hind paw thickness from this experiment. The estimated results would be a good guide to therapists and researchers, planning Far-infrared application on the rheumatoid arthritis. These results indicated that far-infrared irradiating appeared to reduce the concentration of TNF-$\alpha$ and IL-1$\beta$ at mouse model of autoimmune arthritis. It would be considered that Far-infrared has an effect on relieving rheumatoid arthritis.

선택적 기능 동작 평가의 연구 동향 분석 (Investigation of Research Trends in the Selective Functional Movement Assessment (SFMA))

  • Purpose : Selective functional movement assessment(SFMA) is a movement-based assessment tool designed for clinicians to diagnose and treat pain and functional physical problems. This concept is used for assistance in occupations related to movement, including clinical medical personnel, such as physical therapists, physical education instructors, and athlete coaches. Although this concept is widely used by clinicians, research and scientific proofs are relatively insufficient. This study aimed to review the literature on the understanding, reliability and effectiveness of SFMA, and to consider its role in the future of rehabilitation. Methods : In this review, a total of nine articles were selected according to the eligibility criteria of three major thesis topics. The main topics covered in the literature are reliability and effectiveness of clinical use of SFMA. To gather research articles, we searched official term 'Selective Functional Movement Assessment'. Among the searched 60 studies, nine were mentioned in this study that contained overlapping information and matched our desired topic. We reviewed four reliability analyses, four case reports, and one experimental study. Results : Reliability has an intermediate degree between high raters and within raters. The validity of the SFMA system is influenced by a combination of experience and logic; hence, further improvement is needed. Therefore, if the intervention was effective based on the diagnosis result, then biomechanical evidence is necessary to further support the claim. Conclusion : In future research, to use SFMA as a diagnostic tool with high accuracy, it is necessary to improve the reliability of the main problem through breakout, support for guidelines and validity and efficiency.

COVID-19로 인해 발생한 업무의 피로도와 직무소진의 상관관계 및 직무스트레스와의 회귀분석 연구 -물리치료사를 대상으로- (Correlation Between Work Fatigue Caused by COVID-19 and Occupational Burnout -Regression Analysis of Occupational Stress in Physiotherapists-)

  • Purpose: This study was conducted to investigate the type of physical therapist response work and its intensity, along with the prolonged prevalence of COVID-19, to determine the impact on physical therapists' occupational stress and occupational burnout. Methods: An online survey was conducted with 118 physiotherapists and collected the participants' gender, final educational background, clinical work experience, workplace type, additional workload, and fatigue associated with COVID-19. After excluding one participant who submitted an incomplete questionnaire, 117 respondents were included in the final data. Results: Additional COVID-19-related workloads and response reliability for fatigue, occupational burnout, and occupational stress were shown to have Cronbach's alpha measures of 0.76, 0.89, and 0.87, respectively. Groups who had experienced a new epidemic in the past showed higher fatigue levels (3.06±0.94) than those groups who had no such experience (2.49±0.84; p < 0.05). Correlation analysis of COVID-19 work fatigue and occupational burnout scales showed a positive correlation (r = 0.19; p < 0.05). The regression of occupational burnout and occupational stress showed a regression model of Y = 20.00+0.43X1 (X1: job stress; p< 0.05) and an explanatory power of 24.8% with an adj.R2 = 0.25. Conclusion: Based on the results, it is suggested that various institutions, such as medical institutions, educational institutions, and physiotherapists' associations, should seek ways to manage and alleviate physiotherapists' stress.
