Survey About Current Status of Pediatric and Adolescent Physical Therapy: Focus on Pediatric and Adolescent Rehabilitation Hospitals in Seoul and Gyeonggi Province (소아 청소년 물리치료 실태 조사: 서울 경기 지역 소아 청소년 재활병원을 중심으로)

  • Objective : This study aimed to investigate the current status of physical therapy in children and adolescents. Methods : Sixty questionnaires from physical therapists treating children and adolescents with disabilities were analyzed. The questionnaire consisted of questions on physical therapy, participants, satisfaction, and the assessment of pediatric and adolescent physical therapy. Descriptive statistics and frequencies were used to investigate the current status, participants, and satisfaction. Differences between physical therapy participation difficulty, importance-ability of major aspects of pediatric and adolescent physical therapy, and therapy goal frequency were analyzed using paired T-test. Results : 11 to 15 cases (66.7%) and one-on-one treatment (95.0%) were performed independently (95.0%). The main ages of the subjects were preschool and school, the diagnoses were brain lesions and developmental delay, and treatment was conducted for up to 20 years or older. Satisfaction with pediatric and adolescent physical therapy was high (70.0%), as was the intensity of work (71.7%). Neurodevelopmental therapy, gait training, and goal-directed rehabilitation were the main treatments, and Gross Motor Function Measures of 88 and 66, respectively, were used. Respondents said that current fee system is inadequate (66.1%) and appropriate fee system is needed. Conclusion : This study extensively investigated the content of and factors related to pediatric and adolescent physical therapy. Based on the current situation, efforts to improve the expertise and continuity of pediatric and adolescent physical therapists and apply the latest treatment techniques are required.

Unilateral Traction Effects for a Herniated Nucleus Pulposus in the Lumbar Disk (요추추간판 수핵탈출증에 대한 편측견인의 치료효과)

  • Markolf and Morris suggests that the deranged disk and torn annulus have an exellent recovery ability from the position of spine extension, where the posterior annulus is not under stress and the gel can move anterioly. McKenzie explains side gliding as a combination of rotation and side bending, his clinical studies indicate that side-gliding can gel laterally. In conclusion, the prone unilateral traction on the opposite side from the patient's pain along with the other treatment appered to have helped reduce those patient's pain. The method of treatment described in this report is suggested for Korean physical therapists who treat patients suspected of posterolateral herniated nucleus pulposus.

  • This thesis is designed to discuss the development of the Korean Academy of Orthopedic Manual Therapy and establish a development strategy for the future. The development has been achieved with the enthusiasm and interest of physical therapists from all over the country over the past 10 years since the establishment and the Academy has been receiving attention from the medical part. The Korean Academy of Orthopedic Manual Therapy will develop a variety of training programs for the research and development of orthopedic manual therapy and will establish a development strategy and master plan for the future in order to lay a foundation for orthopedic manual therapy.

Intertester and Intratester Reliability of Tape Measurement on Lower Extremities (줄자를 이용한 하지부피 측정시 부위별 검사자간, 검사자내 신뢰도 측정)

  • Tape measurement for evaluating edema is frequently used by clinicians. The primary purpose of this study is to determine the intertester and intratester reliability of tape measurement carried out by two physical therapists on lower extremities. The intertester and intratester reliability of six anatomical landmarks measurements were determined in six healthy subjects. The measurements were taken in five sessions by each tester. The intraclass correlation coefficients (2,1) were between .87 and .99 for intertester reliability. Intratester reliability was .93 and .99 for A tester and .82 and .99 for B tester. The average range of variation was .13 to .54 in six landmarks. The reliability of six volume measurements was very high. These results support the use tape measurement as a reliable tool for measuring limb girth.

  • Traction has been used since ancient times in the treatment of painfull spinal conditions, but the literature on traction and its clinical effectiveness Is limited. Traction can be defined as a drawing or pulling tension applied to a body segment. Cervical traction is a technique that applies a longitudinal force of the cervical spine and associated structures. Goals of traction include reduction of radicular signs and symptoms associated with conditions such as disk protrusion, degenerative disk disease, lateral stenosis, muscle spasm, and subluxations. The various mechanical factors most relevant to cervical traction are organized and discussed. The factors presented are 1) angle of pull, 2) Traction force, 3) duration of traction, 4) neck position and clinical application, and 5) frequency of treatment. It should allow physical therapists to adjust traction protocol to match the patient's symptoms and diagnosis. The purpose of this study is to provide a comprehensive overview of the cervical traction and treatment guidelines.

  • This study largely compared the general concept, examination, and treatment of various manual therapy techniques. These various manual therapy techniques, however, are still in the developing stage, so no one approach is yet perfect. Clinically, manual therapy techniques are widely being used to treat the dysfunctional neuromusculoskeletal with the common practice of mobilization, manipulation, exercise, and patient education. Anyone of the above approaches must not be selectively chosen as the best method. Manual therapists should treat patients in the clinic with the full knowledge of these proper manual therapy techniques depending on the patient's symptoms in each of the anatomical, biomechanical, and pathological views.

  • Neurodevelopmental treatment(NDT) is a widely used technique by physical and occupational therapists in the treatment of neuromuscular disorders. Childeren with cerebral palsy are frequently referred for physical and occupational therapy, yet the effectiveness of treatment has not been well-documented. More than 40 years ago, the Bobath introduced a treatment concept for individuals with central nervous system impairment. A theoretical framework for the approach was based on the common belief in the 1940s that the nervous system functions in a hierarchy. Clinical aspects of the NDT approach have grown and changed during the past 40 years. This article details the original NDT concepts and looks at the concept with regard to newer theoretical frameworks of nervous system.

  • The kinesiologic electromyograhy(KEMG) is one of the most common use in physical therapy for understanding normal function and dysfunction of the neuromuscular system. Physical therapists concerned to patterns of muscle response, position, type of muscle contraction, onset and cessation of activity, the level of muscle response in relation to effort, and type of muscle contraction. Also they are use KEMG to assess the exercises for facilitate or for inhibit specific muscle activity. However there is a few references that provide information of KEMG Thus, the purpose of this article is for the use and interpretation of KEMG data. This article will guide beginning researchers for collecting, managing and analyzing of KEMG data.

  • To provide physical therapy service of good quality keeping people healthy and obstain structural reformation coping with the demands of in medical service market to foreign intercourse on 21C, we should make legal system fer the physical therapy practice. Thus we suggest the Ministry of Health and Health and the authorities should, 1. Exclude the provision of physical therapist from the classfication of medical technician on the Medical Technician Law Article 2. and establish the independent Physical Therapist Law 2. Eliminate the provision of physician or dentist's guide the Medical Technician Law Article 1. or reform it to physician or dentist's request so that physical therapists may have a independent practice, or 3. Add the provision of the physical therapy center to the Medical Technician Law, the enforcement ordinances and enforcement regulations, such as the provision of optometrist or dental technician.
