일반적인 자세가 요통에 미치는 영향에 대한 고찰 (Posture and Low Back Pain)

  • Main cause of low back pain is a poor posture. Most low back pains are due to a poor posture. The poor posture induces muscle tension and finaliy low back pain. The poor posture arehabitually trained from the childhood by the environmetal factors. In general, maintaining good posture during working and sleeping hours are the first line of defence against back pam. (1) Supine posture is the easiest posture that relaxes and fixes muscles. Supine posture is thus a starting position for on exercise. Lying down releases the weight pressures of head and shoulder and thus body can be relaxed and extended which are helpful for treating back pain. However, supine posture can increase the pressure in ribcage posture aspect and disphragram due to visceral oragans. (2) Sitting in one position for a long time results in fatigue and relaxation of spinal muscies. Finally, body strength is weakened and sitting posture will become poor. If this poor posture continues for a longer time, pain will be accompanied due to overelongation of muscle ligaments. The habitual poor posture could induce intervertebral disc distortion. If the intervertebral disc is damaged, sitting in one position or movement causes pain. (3) Abnormal lumbar curve induces the tention of abdominal muscle and paravertebral muscle groups as well as tention of lower limb muscle group connected to pelvis. For a person with weak body strength, muscle relaxation increases curvature in lumbar, chest and cervical regions. This will induce a pelvic anterior tilting of the imaginary line between A. S. I. S. and P. S. I. S. Hip joint extensor muscle acts on releasing the pelvic anterior tilting. Contrections of hamstring muscle and femoral muscle recover the imaginary line between A.S.LS. and P.S.I.S. from pelvic anterior tilting. thus, contraction of rectus abdominis muscle are required to maintain the normal lumbar curve.

Immediate Effects of Soft Tissue Massage on Posterior Shoulder Muscle Tightness: A Preliminary Study

  • Posterior shoulder muscle tightness is frequently observed in shoulder impingement syndrome because tightness in the posterior portion of the shoulder muscles can cause anterior and superior translation of the humeral head in relation to the glenoid fossa. The purpose of this study was to determine the immediate effects of soft tissue massage on acromiohumeral distance (AHD), anterior translation of the humeral head, and glenohumeral (GH) range of motion (ROM) in subjects with posterior shoulder muscle tightness. Twenty-seven subjects with greater than $10^{\circ}$ difference in the range of GH horizontal adduction between right and left sides were recruited. The range of GH horizontal adduction and internal rotation were measured by a digital inclinometer. The AHD and anterior translation of the humeral head were measured using ultrasonography. A paired t-test was used to compare AHD, anterior translation of the humeral head, and the range of GH horizontal adduction and internal rotation before and after soft tissue massage. The results showed that AHD increased significantly (p<.05) and the anterior translation of humeral head decreased slightly, but not significantly (p=.40) after the soft tissue massage. Furthermore, the ROM of horizontal adduction and internal rotation in the GH joint increased significantly after the soft tissue massage (p<.05). These findings indicate that soft tissue massage on posterior shoulder muscle tightness is an effective method to increase AHD and ROM in the horizontal adduction and internal rotation of the GH joint.

뇌졸중 편마비환자의 견관절 아탈구 예방에 관한 고찰 (Review of Prevention of Hemipelegic Shoulder Subluxation After Stroke)

  • Purpose : Shoulder subluxation is a very common problem in patients with hemiplegia with stroke. Prevention of the low tone subluxed shoulder has been an issue for physical therapists working with neurological patients for many years. Methods : This study reviewed the literature to definite the management and a cause of shoulder subluxation with hemiplegia patients after stroke. Various modalities have been suggested for realigning the glenohumeral joint, but their use is controversial. The purpose of this paper is to review critically the evidence base in order to inform the clinical decision-making process for physiotherapists working in neurology. Results : Literature has identified supports, strapping and functional electrical stimulation(FES) in the management of low tone shoulders. Following review of this evidence it is suggested that there is a lack of reliable and valid research evidence on which to base conclusions. The modalities with the best supporting evidence for realigning the low tone subluxed glenohumeral joint are the triangular sling, Harris hemi sling and the Rolyan humeral cuff used in a standing position and the lap board and arm trough while the patient is sitting. However, due to soft tissue adaptation with associated lack of movement, over-correction and the need for careful patient positioning these supports need to be evaluated for each patient and should be used only in appropriate situations. Conclusion : Similarly, there is a lack of evidence on the effects of long-term use to this equipment. Electrical stimulation is also thought to have potential in the treatment to subluxed low tone shoulders but additional research is required to clarify the parameters for use and the long-term effects of these forms of management.

HCCA의 CF-Poll 피기백 알고리즘의 성능평가 (A Performance Evaluation of CF-Poll Piggyback Algorithm for HCCA)

  • HCCA에서 채널 예약 정보를 가진 CF-Poll 프레임을 QoS-Data 프레임에 피기백하여 채널 효율성을 향상시키도록 하였다. 그러나 망 내의 하나의 QSTA라도 물리계층 전송속도가 감소할 경우 CF-Poll 프레임을 피기백한 QoS-Data 프레임은 가장 낮은 물리계층 전송속도를 가진 QSTA에 맞추어 전송해야 한다. 따라서 물리계층 전송 속도가 낮을 때 CF-Poll을 피기백하는 것은 지연을 증가시킬 뿐만 아니라 전체 망 성능을 감소시킬 수 있다. 본 논문에서는 이러한 현상을 QSTA의 채널전송속도 저하에 의한 피기백 문제라고 정의하였으며, 이러한 문제에 의한 성능 감소를 평가하였다. 시뮬레이션 결과로 CF-Poll을 피기백했을 때 임의의 QSTA의 물리계층 전송속도가 낮으면 지연이 최대 25% 증가하였다. 반면 물리계층 전송속도가 높을 경우 지연이 최대 7.8% 감소하는 것을 알 수 있었다. 또한 지연의 측면에서 서비스 트래픽 부하에 따라 전체 QSTA의 최저물리계층 전송속도가 24에서 36Mbps 이상일 때 CF-Poll 피기백의 이득이 발생하는 것을 발견하였다.

Applying Focused and Radial Shock Wave for Calcific Tendinitis of the Shoulder : Randomized Controlled Study

  • Objective: Extracorporeal shock wave therapy (ESWT) is a nonsurgical treatment alternative to surgery for various musculoskeletal diseases that have traditionally been difficult to treat conservatively, including calcific tendinitis, tennis elbow, and plantar fasciitis. This study evaluated the effect of focused and radial shock wave therapy for calcific tendinitis of the shoulder. Design: Randomized controlled study Methods: Forty participants with calcific tendinitis were randomized into focused shock wave therapy (FSWT, n=20) and radial shock wave therapy (RSWT, n=20) groups. Patients were examined before and one week after treatment. Pain intensity was subjectively assessed using the visual analogue scale and function was assessed using the Constant-Murley score (CMS) and range of motion (ROM). Results: The results showed a significant decrease in pain and significant increase in shoulder mobility and function in both groups. However, FSWT was significantly more effective than RSWT, based on CMS and ROM assessment. Conclusions: Although it is possible to raise the energy intensity of RSWT to increase the depth at which the energy becomes dispersed, higher energy intensity is associated with a greater risk of severe neurovascular damage, and that high-intensity stimulation can cause adverse effects such as pain and petechiae. Therefore, FSWT is considered to be a safe and effective method for treating tendinous lesions while minimizing adverse effects. In conclusion, both FSWT and RSWT can reduce pain and increase mobility and function. FSWT can be considered as an alternative for calcific tendinitis of the shoulder.

유도된 감정변화가 위팔두갈래근의 근활성도에 미치는 영향 (Effects of Induced Emotional Changes on Bicep Brachii Muscle Activity)

  • Background: Studies suggest that induced emotional changes can affect the sensory-motor system involved in the practice of muscle activity and movement in physical aspects. Previous studies have shown focused on effects just feedback on muscle activity associated with emotions but rarely have focused induced emotional change on gross motor function such as muscle activity. Objects: The purpose of this study was to compare biceps activity and emotion that before and after viewing a video was induced positive or negative emotion. Methods: The study enrolled 34 healthy male and female who scored at normal points on the Center for Epidermiological Studies-Depression Scale. The study measured over two weeks, showing subjects pleasant and sad videos one by one in a week. We performed to measure the biceps brachii activity which is maximal voluntary isometric contraction (MVIC) and the visual analog mood scale (VAMS) scores before and after one week. The significance level was set to α = 0.05. Results: There was no significant difference in muscle activity of the biceps brachii before and after each video was viewed (p > 0.05). However, the visual analogue mood scale showed an increase in VAMS after viewing each video (p < 0.05). Conclusion: We figured out induced emotional changes are cause actual emotional changes but there are no differences in muscle activity. In this research, watching the video with a short time looks like insufficient to change muscle activity. Nevertheless, there might be different when we check various muscles with sufficient time for viewing the video. Further study is needed to measure a variety of muscles with more time for viewing the video.

Lactate consumption mediates repeated high-intensity interval exercise-enhanced executive function in adult males

  • [Purpose] Lactate is a principal energy substrate for the brain during exercise. A single bout of high-intensity interval exercise (HIIE) can increase the blood lactate level, brain lactate uptake, and executive function (EF). However, repeated HIIE can attenuate exercise-induced increases in lactate level and EF. The lactate levels in the brain and blood are reported to be correlated with exercise-enhanced EF. However, research is yet to explain the cause-and-effect relationship between lactate and EF. This study examined whether lactate consumption improves the attenuated exercise-enhanced EF caused by repeated HIIE. [Methods] Eleven healthy men performed two sets of HIIE, and after each set, 30 min were given for rest and examination. In the 2nd set, the subjects consumed experimental beverages containing (n = 6) and not containing (n = 5) lactate. Blood, cardiovascular, and psychological variables were measured, and EF was evaluated by the computerized color-word Stroop test. [Results] The lactate group had a higher EF (P < 0.05) and tended to have a higher blood lactate level (P = 0.082) than the control group in the 2nd set of HIIE. Moreover, blood lactate concentration was correlated with the interference score (i.e., reverse score of EF) (r = -0.394; P < 0.05). [Conclusion] Our results suggest that the attenuated exercise-enhanced EF after repeated HIIE can be improved through lactate consumption. However, the role of lactate needs to be elucidated in future studies, as it can be used for improving athletes' performance and also in cognitive decline-related clinical studies.

Suppression of stray electrons in the negative ion accelerator of CRAFT NNBI test facility

  • Comprehensive Research Facility for Fusion Technology (CRAFT) is an integration of different demonstrating or testing facilities, which aim to develop the critical technology or composition system towards the fusion reactor. Due to the importance and challenge of the negative ion based neutral beam injection (NNBI), a NNBI test facility is included in the framework of CRAFT. The initial object of CRAFT NNBI test facility is to obtain a H0 beam power of 2 MW at the energy of 200-400 keV for the pulse duration of 100 s. Inside the negative ion accelerator of NNBI system, the interactions of the negative ions with the background gas and electrodes can generate abundant stray electrons. The stray electrons can be further accelerated and dumped on the electrodes or eject from the accelerator. The stray electrons, including the ejecting electrons, cause the unwanted particle and heat flux onto the electrodes and the inner components of beamline (especially the temperature sensitive cryopump). The suppression of the stray electrons from the CRAFT accelerator is carried out via a series of design and simulation works. The paper focuses the influence of different magnetic field configurations on the stray electrons and the character of the ejecting electrons.

Effect of Isometric Horizontal Abduction on Scapular and Shoulder Muscle Activity During Knee Push-up Plus With Different Shoulder Angles in Individuals With Scapular Winging

  • Background: Individuals with scapular winging have a weak serratus anterior (SA) muscle, and to compensate, the pectoralis major (PM) and upper trapezius (UT) muscles excessively activate, which can cause upper extremity dysfunction. This study aimed to compare the effects of isometric horizontal abduction (IHA) on SA, PM, and UT muscle activity, as well as the SA/PM and SA/UT muscle activity ratios during knee push-up plus (KPP) at 90° and 120° of shoulder flexion. Objects: This study aimed to compare the effects of IHA on SA, PM, and UT muscle activity, as well as the SA/PM and SA/UT muscle activity ratios during KPP at 90° and 120° of shoulder flexion. Methods: This study, conducted at a university research laboratory, included 20 individuals with scapular winging. Participants performed KPP with and without IHA at 90° (KPP90) and 120° (KPP120) of shoulder flexion. SA, PM, and UT muscle activity were measured using surface electromyography. Results: PM activity in KPP90 with IHA was significantly lower than KPP90 and in KPP120 was significantly lower than KPP90. UT activity was significantly greater with IHA than without IHA and at 120° than 90° of shoulder flexion. SA/PM muscle activity ratio was significantly higher in KPP90 with IHA than without IHA and in KPP120 than in KPP90. SA/UT muscle activity ratio was significantly lower with IHA than without IHA. Conclusion: KPP90 with IHA and KPP120 are effective exercises to reduce PM activity and increase SA/PM muscle activity ratio. However, applying IHA in KPP90 also reduces SA/UT muscle activity ratio, implying that it would be preferable to apply KPP120 in individuals overusing their UT muscles.

High Temperature Drying of North American Ginseng for Management Decision Making

  • The multi-year production cycle for ginseng can be rapidly depreciated by inferior post-harvest activities. This research examines the character of high temperature drying regimes for North American ginseng root to assist management decision making. The objective is a very rapid drying regime, that will not result in physical or chemical damage to the root and that would not alter the actual dry root weight. Research is presented using drying temperatures of 55, 70 and 105 C. Temperatures above these rapidly cause substantive physical damage to the root samples and seriously compromise the dry root values determined. Temperatures below these behaved quite similar to actual dryer regimes (approximately 38 C). Laboratory results indicate that there are differences between the three temperature regimes tested. Careful usage of the 70 C regime, over a period of two to three days in a convection drying oven, has distinct merit.
