일어서기 동작에 대한 동작관찰과 동기화된 전기적 감각자극의 통합적 제공이 뇌졸중 환자의 기능에 미치는 효과 (The Effects of Integrated Provision Action Observation and Synchronized Electrical Sensory Stimulation for Sit-to-stand in Stroke Patients Function)

  • Background: Stroke patients experience multiple dysfunctions that include motor and sensory impairments. Therefore, new intervention methods require a gradational approach depending on functional levels of a stroke patient's activity and should include cognition treatment to allow for a patient's active participation in rehabilitation. Objects: This study investigates the effect of integrated revision of electrical sensory stimulation, which stimulates somatosensory and action observation training, which is synchronized cognition intervention method on stroke patients' functions. Methods: Twenty-one stroke patients were randomized into two groups. The two groups underwent twenty minutes of intervention five times a week for three weeks. This study used an electromyogram to evaluate symmetric muscle activation of lower extremities and muscle onset time when performing sit to stand before and after intervention. A weight-bearing ratio was used to evaluate the weight-bearing of the affected side in a sit to standing. To evaluate sit to stand performance ability, this study performed five timed sit to stand tests. Results: The two groups both showed statistically significant improvement in muscle onset time of lower extremity, static balance ability in a standing position, and sit to stand performance after the intervention (p < 0.05). In addition, the action observation and synchronized electrical sensory stimulation group showed significant improvement in symmetric muscle activation of lower extremities and weight-bearing ratio of the affected side (p < 0.05). Conclusion: action observation and synchronized electrical sensory stimulation (AOT with ESS) can have positive effects on a stroke patient's sit to stand performance, and the intervention method that provides integrated AOT with ESS can be used as new nervous system intervention program.

가상현실 기반의 링 피트 어드벤처 코어 운동이 배가로근, 배속빗근, 배바깥빗근의 두께에 미치는 영향 (Effect of Virtual Reality Based Ring Fit Adventure Core Exercise on the Thickness of the Transverse Abdominis, Internal Oblique and External Oblique Muscle)

  • Purpose : The purpose of this study was to analyze the change in thickness of transvers abdominis, internal oblique, and external oblique when virtual reality based ring fit adventure is applied to young adults in order to investigate the effect of ring fit adventure on core stabilization. Methods : 30 subjects participated in the experiment. Subjects were randomly assigned to two groups. 15 subjects performed ring fit adventure core exercise (experimental group) and 15 subjects bridge and dead bug exercise (control group). The ring fit adventure core exercise program consists of 6 types, 1) bow pull, 2) overhead lunge twist, 3) pendulum bend, 4) seated ring raise, 5) plank, 6) warrior III pose. Each exercise was performed for 5 minutes, for a total of 30 minutes. The bridege and dead bug exercise were performed for 15 minutes each for a total of 30 minutes. All interventions were performed 3 times a week for 4 weeks. Thickness of the abdominal muscles was measured with a ultrasound. The paired t-test was used to compare the thickness of the transverse abdominis, internal oblique, and external oblique before and after intervention, and the comparison between groups was analyzed using the independent t-test. Results : As a result, in the experimental group, thickness of transverse abdominis and internal oblique increased significantly (p<.05), but external oblique decreased significantly (p<.05), and in the control group, thickness of transverse abdominis, internal oblique, and external oblique increased significantly (p<.05). There was a significant difference in external oblique in the difference between groups (p<.05). Conclusion : These study results showed that core exercise using ring fit adventure can reduce external oblique and increased selective muscle activity of transverse abdominis and internal oblique of the deep abdominal muscles, so it is meaningful as an effective intervention for core stabilization.

발달장애의 수예공작 프로그램 필요성에 관한 고찰 (A Look at the Need for a Crafts program of Developmental Disabilities)

  • 환경적 오염 및 산업화 등으로 인한 직 간접적인 문제는 장애인, 급 만성질환자 등의 수를 증가시키는 요인이 된다. 이로 인해 발생하는 장애아동의 수도 성인과 비슷한 비율로 매년 증가하고 있다 특히, 발달장애는 전체 아동의 약 5~10%정도에서 발생되는 높은 유병률을 보이고 있다. 발달장애 아동에게 적용하는 치료로는 작업치료, 물리치료, 언어치료, 심리치료, 미술치료 등이 있다. 시지각이란 시각 기관을 통해 외부환경을 인지하는 기능으로 일상생활에서 일어나는 대부분의 행동들이 시지각과 연관된다. 그러나 발달장애 아동은 충분한 시지각의 발달이 이루어지지 않기 때문에 일상과제 수행 면에 많은 영향을 초래하게 된다. 이러한 이유로 아동 작업치료 영역에서 시지각에 대한 평가와 치료에 관한 이해는 매우 중요하다고 할 수 있다. 시지각 기능을 향상시키기 위한 다양한 치료 방법 중에는 감각통합 훈련, 시지각 훈련 프로그램, 수예공작 프로그램 등이 있다. 특히, 수예공작은 주로 손을 사용하여 무엇인가를 만드는 대표적인 활동으로 누구에게나 적용할 수 있다. 뇌에 관한 연구가 활발해지면서 수예공작 활동을 통해 뇌기능을 사용하고 그 기능에 도움을 줄 수 있음이 증명되었으며, 소근육운동 또는 세밀하고 정확한 움직임을 필요로 하는 활동을 수행할 때 시지각 기술이 기반이 되기 때문에 수예공작을 통한 손 기능의 반복적인 사용은 두뇌기능을 발달시켜 시지각 기능 향상에 기여할 것임을 기대 할 수 있다. 이것은 수예공작이 분명한 치료적 가치가 있음을 시사한다. 또한 작업치료 영역에서 시지각의 발달과 시지각의 장애에 대한 중재가 큰 영역을 차지함에도 불구하고 그에 대한 연구가 매우 부족하다. 따라서 본 연구에서는 학위논문의 타당성 검증을 위한 사전연구로서 발달장애 아동의 수예공작 프로그램 수행의 필요성을 소고하고자 한다.

  • Purpose: The purpose of this study was to investigate the effects of modified ankle movement patterns on participants' active dorsiflexion range of motion and leg muscle activity. Methods: This study recruited twenty-five participants, all of whom were healthy individuals with no abnormalities in the ankle or knee joints. The research methodology involved measuring the active dorsiflexion range of motion and muscle activity in each person's legs based on the presence or absence of toe extension while the subjects were in a comfortable, supine position. A statistical analysis was conducted using SPSS 25.0, and a paired samples t-test was employed. The significance level was set at 0.05. Results: When the participants demonstrated the modified ankle movement pattern with a controlled toe grip, there was an increase in their active dorsiflexion angle. However, during the proprioceptive neuromuscular facilitation technique without a controlled toe grip, a higher level of activity was observed in the leg muscles. Conclusion: The results of this study could be used as foundational data for establishing a rehabilitation exercise program designed to enhance range of motion and muscle activation in the ankle joint.

노인의 건강상태 및 간호 요구도 조사 (A Study on Physical Health Status and Nursing Needs of the Elderly)

  • This study was conducted to collect the baseline data of physical health status and nursing needs of the elderly for developing an educational program for nursing professionals. The data was collected from 287 people who were 65 years old or older belonging to a selected 8 out of the 39 welfare centers in Pusan during the period from February 25 to November 30, 1996. Socio-demographic characteristics, frequency of hospitalization, and the prevalence of chronic illnesses were measured to assess the physical health status of the center's elderly admitted to the hospital during the period of data collection. The 5-point Likert-type scale composed of 20 items was used. The data was analyzed with the Statistical Package for the Social Sciences Program (SPSS PC+). The results were summarized as follows : 1. The proportion of elderly subjects with chronic illnesses was 73.6%. Among 6 types of chronic illnesses, hypertension was the most prevalent(23.7%), cardiovascular disease was the second(18.1%), gastrointestinal disease the third(17.1%), musculosskeletal disease the fourth(12.9%), liver and biliary disease the fifth(12.5%), and diabetes the sixth. 2. The average score of the subjects' health status was 3.649 on the 5-point Likert scale. Among the 6 factors related to physical health, personal hygiene was the strongest factor(4.612) ; elimination was the second(4.491) ; sensation the third(4.369), outside activity the fourth(3.675), digestion the fifth(3.331), and sexual life the sixth. 3. Gender, marital status, religion, educational level, family structure, living arrangements, and perceived health status were significantly related to the elderly's physical health status. 4. Among the nursing needs of the elderly, emotional support and care was the strongest need(74.2%) ; physical therapy was the second(73.2%) ; education about diease control the third(64.8%) ; medication the fourth(63.8%) ; and the range of motion exercise the fourth(61.0%).

가상현실 기반 운동이 측만 각도 및 근활성도에 미치는 영향 (Effect of Virtual Reality-Based Exercise on Scoliometer and Muscle Activity)

  • The aim of this study is to determine the effect of virtual reality(VR) exercise using XBOX $Kinect^{TM}$ on the scoliometer angle and muscle activities of the trunk. The subjects of the study were 13 young adults who devided into a virtual reality-based exercise group(VREG, n = 7) and a traditional stabilization exercise group(TSEG, n = 6). The VREG received virtual reality game for 5 days a weeks, 30 minutes a day, for a 2 weeks and the TSEG received general trunk stabilization exercise for the same period and frequency. The subjects were measured and compared for muscle activities of trunk stabilization before and after the program. VREG tend to decrease more than TSEG in scoliometer angle. In addition, VREG showed a significant decrease in muscle activities of multifidus. This study presents the impact of virtual reality-based exercise program on trunk balance and stabilization. The following studies need customized programs for subjects that trunk balance and stabilization is required.

산림치유 효과 검증 연구의 주요어 분석을 통한 치유 발현과정 해석 (Interpretation of the Forest Therapy Process and Effect Verification through KeyWord Analysis of Literature on Forest Therapy)

  • 본 연구는 산림치유인자를 활용한 숲 활동이 면역력증진과 건강증진으로 이어지는 산림치유 효과의 발현과정을 이론적 그리고 정성적으로 분석하고 산림치유 개념을 정밀화하고 체계화하기 위하여 수행되었다. 연구분석자료는 산림치유 관련 기관 웹사이트와 2000년에서 2020년 3월까지 산림치유로 검색되는 33편의 석·박사 학위논문과 33편의 전문학술지 논문의 주요어, 2016년 산림치유 체험수기 공모전 입상작에 나타난 명사와 형용사 빈도수에 근거하여 워드클라우드를 생성하였다. 단어 빈도수를 근거로 한 해석으로 산림치유 요인을 도출하고 치유 효과의 발현과정을 추론하여 산림치유에 대한 개념을 정의하였다. 산림 치유력의 원천은 산림치유인자와 함께 숲에 대한 긍정적 경험과 자연에 대한 태도가 중요한 것으로 나타났다. 치유효과는 산림치유프로그램을 통해 극대화되고, 신체적 정신적 회복력과 저항력으로 이어지며, 결과적으로 건강증진, 면역력 증진으로 나타난다. 본 연구는 산림치유란 "대상자들의 숲에 대한 긍정경험 및 태도와 산림의 환경요소를 매개로 대상자들의 심리적, 신체적, 영적 회복력을 위한 건강증진 활동"이라 제안한다.

기능적 과제를 통한 다관절 관절위치감각 훈련이 뇌졸중 환자의 관절위치감각, 균형, 보행능력에 미치는 효과 (The Effects of Multi Joint-Joint Position Sense Training Using Functional Task on Joint Position Sense, Balance, Walking Ability in Patients With Post-Stroke Hemiplegia)

  • The purpose of this study was to investigate the effect of multi joint-joint position sense (MJ-JPS) training on joint position sense, balance, and gait ability in stroke patients. A total of 18 stroke patients participated in the study. The subjects were allocated randomly into two groups: an experimental group and a control group. Participants in the experimental group received MJ-JPS training (10 min) and conventional treatment (20 min), but participants in the control group only received conventional treatment (30 min). Both groups received training for five times per week for six weeks. MJ-JPS is a training method used to increase proprioception in the lower extremities; as such, it is used, to position the lower extremities in a given space. MJ-JPS measurement was captured via video using a Image J program to calculate the error distance. Balance ability was measured using Timed Up and Go (TUG) and the Berg Balance Scale (BBS). Gait ability was measured with a 10 m walking test (10MWT) and by climbing four flights of stairs. The Shapiro-Wilk test was used to assess normalization. Within-group differences were analyzed using the paired t-test. Between-group differences were analyzed using the independent t-test. The experimental group showed a significant decrease in error distance (MJ-JPS) compared to the control group (p<.05). Both groups showed a significant difference in their BBS and 10MWT results (p<.05). The experimental group showed a significant decrease in their TUG and climbing results (p<.05), but the control group results for those two tasks were not found to be significant (p>.05). There was significant difference in MJ-JPS and by climbing four flights of stairs on variation of pre and post test in between groups (p<.05), but TUG and BBS and 10MWT was no significantly (p>.05). We suggest that the MJ-JPS training proposed in this study be used as an intervention to help improve the functional activity of the lower extremities in stroke patients.

Push-up 동작 시 신체기울기에 따른 견관절 주변근의 수축 개시시간 변화 (The Differences of Shoulder Muscle Activity Onset Time according to Body Tilting Angle in Push-up Exercise)

  • PURPOSE: The purpose of this study was to investigate recruiting order and onset time around shoulder muscle during the push-up according to the body tilting angle. METHODS: Twenty healthy young adult subjects were recruited for this study. They had no neurological and musculoskeletal disease. We used the sEMG for recording onset time of shoulder muscles. Shoulder Muscles were anterior deltoid(AD), posterior deltoid(PD), pectoralis major(PM), upper trapezius(UT). Body tilting angle were measured at 0 degree, 30 degree and 60 degree by using tilting table. Muscles contraction onset time were set by the push-up performed 3 times respectively. Mean of 3 measurements were used. And initiate onset time was decided by the Mean ${\pm}2$ SD in the threshold, more than 25ms. RESULTS: There were significant difference at 0 degree, 30 degree and 60 degree(p<.05). Muscles onset time were same order at 0 degree, 30 degree. UT occurred first of all contraction at 0 degree and 30 degree. And then contracted AD, PD, PM. But, at 60 degree, AD was the first contraction, and PM, UT, PD. CONCLUSION: Muscle recruitment order and onset time according to the body tilting was shown the difference when you do push-up. Therefore, this result, shoulder muscle recruitment pattern of according to the body tilting is different and it has to make effective shoulder exercise program.

Effects of Melatonin on Improvement of Neurological Function in Focal Cerebral Ischemic Rats

  • Acute ischemic stroke results from sudden decrease or loss of blood supply to an area of the brain, resulting in a coinciding loss of neurological function. The antioxidant action of melatonin is an important mechanism among its known effects to protective activity during ischemic/reperfusion injury. The focus of this research, therapeutic efficacy of melatonin on recovery of neurological function following long term treatment in ischemic brain injured rats. Male Sprague-Dawley rats (n=40; 8 weeks old) were divided into the control group, and MCAo groups (Vehicle, MT7 : MCAo+ melatonin injection at 7:00, MT19 : MCAo+melatonin injection at 19:00, and MT7,19 : MCAo+melatonin injection at 7:00 and 19:00). Rat body weight and neurological function were measured every week for 8 weeks. After 8 weeks, the rats were anesthetized with a mixture of zoletil (40 mg/kg) and xylazine (10 mg/kg) and sacrificed for further analysis. Tissues were then collected for RNA isolation from brain tissue. Also, brain tissues were analyzed by histological procedures. We elucidated that melatonin was not toxic in vital organs. MT7,19 was the most rapidly got back to mild symptom on test of neurological parameter. Also, exogenous melatonin induces both the down-regulation of detrimental genes, such as NOSs and the up-regulation of beneficial gene, including BDNF during long term administration after focal cerebral ischemia. Melatonin treatment reduced the loss of primary motor cortex. Therefore, we suggest that melatonin could be act as prophylactic as well as therapeutic agent for neurorehabilitative intervention.
