유한요소기법에 의한 인두의 생체역학모델에 관한 연구 (A study on the biomechanical modeling of human pharynx by using FEM(Finite Element Method))

  • 인두는 구강과 위장, 비강과 폐의 중간에서 능동적으로 구강을 통해 섭취되는 음식물과 비강을 통해 흡입되는 공기의 통로역할을 하는 주요한 기관이다. 본 연구는 유한요소기법을 이용한 인두의 3차원 구조의 재구성 과정을 거쳐 인두의 생체역학모델을 구현하여 각단면에서의 단면적을 유한요소모델의 시뮬레이션 결과에 의한 변위를 이용하여 산출하여 최적화 과정을 거쳐 인두의 기능시 내부에 생성되는 압력의 연속적인 압력분포를 추정할 수 있었다. 즉 인두내의압력에 대한 형상의 변형을 관찰하여 각 단면에서의 단면적을 산출하고 이를 실제의 CT영상자료와 비교하여 최적화 고장을 거쳐 각 부분에서의 추정 압력구배를 구하였다. 모델 시뮬레이션 결과 추정된 압력구배는 10-55 mmHg범위에 분포되어 있으며 전체 인두부 가운데 상부의 4레벨의 압력분포는 아부의 그것과 다른 형태를 보이는 것으로 나타났다. 이러한 인두의 생체역학모델은 인두기능장애를 가진 환자군에 적용하여 비교 분석할 경우 임상자료로서 유용할 것으로 사료된다.

Auto-segmentation of head and neck organs at risk in radiotherapy and its dependence on anatomic similarity

  • Purpose: The aim is to study the dependence of deformable based auto-segmentation of head and neck organs-at-risks (OAR) on anatomy matching for a single atlas based system and generate an acceptable set of contours. Methods: A sample of ten patients in neutral neck position and three atlas sets consisting of ten patients each in different head and neck positions were utilized to generate three scenarios representing poor, average and perfect anatomy matching respectively and auto-segmentation was carried out for each scenario. Brainstem, larynx, mandible, cervical oesophagus, oral cavity, pharyngeal muscles, parotids, spinal cord, and trachea were the structures selected for the study. Automatic and oncologist reference contours were compared using the dice similarity index (DSI), Hausdroff distance and variation in the centre of mass (COM). Results: The mean DSI scores for brainstem was good irrespective of the anatomy matching scenarios. The scores for mandible, oral cavity, larynx, parotids, spinal cord, and trachea were unacceptable with poor matching but improved with enhanced bony matching whereas cervical oesophagus and pharyngeal muscles had less than acceptable scores for even perfect matching scenario. HD value and variation in COM decreased with better matching for all the structures. Conclusion: Improved anatomy matching resulted in better segmentation. At least a similar setup can help generate an acceptable set of automatic contours in systems employing single atlas method. Automatic contours from average matching scenario were acceptable for most structures. Importance should be given to head and neck position during atlas generation for a single atlas based system.

  • Purpose : The lateral cephalometric Korean norms of the skeletal and pharyngeal dimension were analyzed for the diagnosis of obstructive sleep apnea syndrome(OSAS). Materials and Methods : The lateral cephalometric radiographies were taken in male(n=53) and female(n=50), who had a normal profile, class I occlusion, normal ANB $(0{\sim}4^{\circ})$ and normal Wits $Appraisal(-4{\sim}0mm)$. The X-rays were traced by two oral and maxillofacial surgeons twice. The significant differences between male-female and examiners were tested. Results : The angle of mandibular line to Nasion-Sella line was $31.12^{\circ}/33.79^{\circ}$ (in male/in female, p=0.0018). The lower Gonion angle was $73.74^{\circ}/73.74^{\circ}(p=0.9978)$. The length of the soft palate was 36.93mm/34.35mm(p=0.0002). The pharyngeal airway space was 13.42mm/11.55mm at mandibular plane level(PAS(ML))(p=0.0025). The hyoid was placed inferiorly to mandibular plane 10.18mm/7.72mm(p=0.0051). The results from this study are to be used for the diagnosis of OSAS.

Change of the upper airway after mandibular setback surgery in patients with mandibular prognathism and anterior open bite

  • Purpose: It has been reported before that the amount of pharyngeal airway space (PAS) significantly decreases following mandibular setback (MS) surgery in patients with mandibular prognathism (MP). Further, MP patients with an anterior open-bite (AOB) presentation may show a larger decrease in PAS compared with those without AOB. However, studies on postoperative PAS changes in MP patients with AOB remain rare. This study sought to evaluate changes in PAS and hyoid bone positioning following MS surgery in MP patients with and without AOB. Patients and methods: Twenty patients who underwent two jaw surgery involving MS movement were included. Patients were divided into a non-AOB group (n = 10; overbite > 2 mm) and an AOB group (n = 10; overbite < - 4 mm). Three-dimensional changes in PAS and hyoid bone positioning were compared and statistically evaluated pre- and postoperatively using computed tomography (CT). Results: The mean magnitude of MS was 6.0 ± 2.8 mm and 5.6 ± 3.2 mm in the non-AOB group and AOB group, respectively. The oropharyngeal volume and upper hypopharyngeal volume were significantly reduced after surgery in both the groups (p = 0.006 and p = 0.003), while the retroglossal cross-sectional area was significantly reduced only in the AOB group (p = 0.028). Although the AOB group showed a larger decrease in PAS, the difference was not statistically significant between the groups. The position of the hyoid bone showed significant posterior and inferior displacement only in the AOB group, while the vertical displacement of the hyoid bone showed a statistically significant difference between the two groups. Conclusion: PAS was significantly decreased after MS in both the groups, while only the AOB group presented a statistically significant reduction in the retroglossal cross-sectional area. Vertical displacement of the hyoid bone showed a statistically significant difference between the groups, while the PAS change was not. Surgeons should be aware of potential postoperative airway problems that may arise when performing MS surgeries.

섭식연하장애와 치과적 관리 (Dental management of the patient difficulty in swallowing)

  • 섭식 연하장애는 수술이나 치료로 인한 기질적 원인, 뇌혈관장애나 뇌성마비, 근육이나 신경장애 등으로 인한 기능적 원인, 그리고 거식증이나 폭식증과 같은 심리적 원인에 의해 발생할 수 있다. 치과의원급에서는 주로 음식물의 인식장애 단계부터 인두로의 전달장애 단계까지의 환자에게 적극적으로 도움을 줄 수 있으며, 그 방법으로는 치의학적 전문지식을 바탕으로 환자의 섭식, 저작과정을 주의 깊게 관찰하여 문제점을 파악하여 각 단계에 적절한 기초 훈련과 섭식 훈련을 실시하는 것이며, 또한 필요시 혀 접촉 보조장치와 같은 장치를 제작하여 환자에게 장착시키고 섭식, 연하 훈련을 시행하여 환자의 연하능력을 개선시켜줄 수 있다. 하지만 무엇보다 가장 중요한 것은 환자가 식전과 식후에 엄격한 구강관리를 시행할 수 있도록 잘 지도하고, 치과의 정기적 방문과 전문적 관리를 통해 구강내 저작기관의 해부학적, 기능적 문제점을 해결해 주도록 하며, 이 때 마다 심리적으로도 환자가 섭식, 연하에 문제가 없도록 세심하게 설명하고 상담해 주는 것으로, 이는 모두 치과의사의 책무라고 할 수 있다.

한국산 놀래기과 어류의 분류학적 검토 (Taxonomical Review of the Korean Labroidei (Teleostei: Perciformes))

  • The perciform suborder Labroidei comprising six families (Cichlidae, Embiotocidae, Pomacentridae, Labridae, Odacidae, and Scaridae) are characterized by having the specialized pharyngeal jaws for food processing, i.e., united fifth ceratobranchials and upper pharyngeal jaw articulating with the basicranium via diarthroses (Stiassny and Jensen, 1987). They usually inhabit in the most tropical and subtropical seas and comprise about 235 genera and roughly 2,274 species worldwide (Nelson, 2006). Concerning the Korean labroid fishes, Mori (1952) had listed 18 genera and 26 species belong to four families in his check list of Korean fishes since Jordan and Metz (1913) firstly reported six genera and seven species in only two families (Embiotocidae and Labridae). Chyung (1977) added two species, Tilapia mossambica and Cirrhilabrus temmincki, to Mori’s list and also classified them into three suborders, i.e., Embiotocina (containing only Embiotocidae), Pomacentrina (Cichlidae and Pomacentridae), and Labrina (Labridae and Scaridae). Subsequently, Lee and Kim (1996) reviewed the Korean labroidfishes taxonomically resulting in 22 genera and 32 species in five families with some taxonomical modifications including a new Korean record. It is remarkable to be added many new Korean recordsto the pomacentrids or the labrids for recent 10 years (Koh et al., 1995; Yoo et al., 1995; Koh et al., 1997; Myoung, 1997; Choi and Kim, 2000; Choi et al., 2002; Kim and Go, 2003). Recently, Kim et al. (2005) briefly described all members of the Korean Labroidei with a color photograph or a figure, recognizing 27 genera and 42 species in five families. In the present study, the current taxonomical status of the Korean labroid fishes including distributional features is summarized based both on specimens collected from the Korean waters and on literature survey to provide bio-information of the Korean native fish species. As a result, the Korean labroid fishes totally consist of 27 genera and 44 species in five families, that is, Cichlidae (1 species), Embiotocidae (3), Pomacentridae (15), Labridae (22), and Scaridae (2). They distributed mainly in the coastal waters of the South Sea, Korea, however, most pomacentrids or labrids occur in the coastal waters of Jeju Island only, although some species were observed in their larval or juvenile stages only from coastal waters of the island. Interestingly, several species are expanding their distribution north to Ulreung and Dok islands in the East Sea, Korea lately.

육안상 구개열이 없는 구개인두기능부전 환자의 술후 발음 개선 (Postoperative Speech Improvement in the Patients of Velopharyngeal Dysfunction without Definite Cleft Palate)

  • The velopharyngeal dysfunction usually occurs in patients with previous operation of the cleft palate or with submucosal cleft palate. In case of velopharyngeal dysfunction without cleft palate, no study has been made when it comes to operative method and postoperative results. Here, we would like to present the operative methods and the postoperative results with the cases we've experienced. This study is based on seven cases of velopharyngeal dysfunction without cleft palate from 1999 to 2004. Analysis of age, sex, etiology, operative methods, satisfaction rate and speech evaluation was done. The patients were 3 males and 4 females, with an age ranged from 10 to 28 at the time of surgery. The follow-up period was more than six months. One case had bifid uvula, another had atypical anomaly in palate, and five cases had no anatomical abnormality. The palatal lengthening was done on one patient, the levator muscle repositioning on another patient and to the rest of them, the superiorly based posterior pharyngeal flap was done. It was difficult to determine the etiology of the velopharyngeal dysfunction without cleft palate. The speech improvement and the satisfaction rate of the patients and parents were diverse. Although the authors had a problem with statistical analysis between the operative age and the speech improvement, it was reasonable to perform a surgical operation because postoperative speech improvement was observed in most cases regardless of age. There is little statistical correlation, but significantly higher outcomes were observed in palatal lengthening and levator muscle repositioning than in pharyngeal flap.

비강 및 인두질환에 있어 쾌통과 보이스의 자각증상 호전도에 관한 임상적 고찰 (The Clinical Observation on the Effect of Quetong & Voice)

  • Objective: The purpose of this clinical observation was to estimate the efficacy of Quetong & Voice on the abatement of subjective symptoms related nose & pharynx. Methods: We observed 46 patients who visited Kang-Nam oriental hospital with subjective symptoms related nose & pharynx that were prescribed Quetong or Voice. We made an inquiry into the inconvenience, side effect, abatement of subjective symptoms, etc. We used the Wilcoxon signed rank test. Results and Conclusions: 1. The distribution of age & sex was as fallows: 21-30 years(34.8%) were the most, 31-40 years(26.1 %) were the second, 1-10 years(24.0%) were the third. Total patient numbers for men and women were 15 men(32.6%), 31 women(67.4%). 2. The distribution of prescribed aerosols for 46 patients: 5 cases(10.9%) prescribed Quetong, 22 cases(47.9%) prescribed Voice, and 19 cases(41.3%) prescribed Quetong & Voice. 3. The duration of use: 12 cases(26.1 %) used 1 week, 19 cases(41.3%) used 1-2 weeks, 9 cases(19.6%) used 2-4 weeks, 4 cases(8.7%) used 4-8 weeks, and 2 cases(4.3%) used above 8 weeks. 4. The frequency of use: The most patients used 1-5 times(69.6%) or 5-10 times(21.7%) for a day. 5. Quetong & Voice were effective on abatement of subjective symptoms(rhinorrhea, nasal obstruction, pharyngalgia, sneeze or cough, post nasal drip, pharyngeal pruritus, pharyngeal dryness, nasal dryness, and globus hystericus). 6. We must prescribe Quetong to the child or the sensitive patients with discretion becouse of the sting. 7. The side effect was not occurred in all of 46 patients.

악교정술전후의 설위 및 설골의 위치변화와 회귀현상에 대한 연구 (A STUDY OF RELAPSE AND POSITION OF HYOID BONE FOLLOWING ORTHOGNATHIC SURGERY)

  • Although various technical details of the surgical procedures have been improved, Skeletal relapse is the most noteworthy complication of orthognathic surgery. It seems to be an imbalance of the perioral muscular groups resulting from changes in the cavitas oris propria after surgery. Among other factors, it is widely known with the changes of tongue posture, as indicated by the hyoid position. Ten patients that had undergone mandibular setbacks by way of Modified Obwegeser method were evaluated retrospectively. The serial cephalometric films were taken preoperatively, immediately postoperatively, after removal of IMF, and at a subsequent long-term follow-up period. The cephalometric evaluation of tongue posture were based on stable craniofacial landmarks. The relation between the 2-dimensional changes of tongue posture and hyoid position and the relapse of mandibular setback are discussed. Anatomic changes that were found to accompany such setback are as follows. 1.There are 2 cases of relapse in 10 patients at long-term follow-up(20%) 2.The tongue was moved posteriorly and its size was reduced anteriorly and posteriorly at immediate postoperative change and then the mandible shifted slightly toward the preoperative position, but the long was adapted to its new environment due to changing the position of its posterior part, and also the hyoid that moved posterioly and inferiorly was stabilized sightly posteriorly than its original position. 3.On the distance change of the suprahyoid muscle, the distance of P-H, ST-H was increased at immediate postoperative change(p<0.01) and decreased at IMF period(p<0.001), but the distance of H-Me, H-Ge was slightly decreased at IMF and long-term period(p<0.05). 4.On the width change of the pharyngeal air way, the width of the upper part of the pharyngeal space was lightly contracted at IMF and long-term period(p<0.05). 5.On the relation between mandibular setback and tongue posture and hyoid position, the significant correlation was found between the changes of some parts of mandibular setback and those of tongue posture, and not found those of hyoid position.
