내과질환에 동반된 불안증상에 대한 약물치료 (Pharmacological Treatment of Anxiety Disorders in Medical Illness)

  • 진료현장에서 불안증상은 흔하게 접하는 정신과적 병리이다 불안증상과 내과질환의 동시 이환은 비단 진단적인 문제뿐 아니라, 예후와 치료결과에도 관련된 문제이다. 내과질환에 동반된 불안증상은 상황에 따라 치료되어야 하며, 필요한 경우에는 약물치료가 필요하다. 불안을 조절하기 위해 사용되는 약물은 모두 치료제로 고려될 수 있으며, 내과질환의 종류, 이학적 상태 및 불안 정도에 따라 적절한 항우울제나 항불안제를 선택할 수 있다. 경우에 따라서는 항정신병약물이나 직접적인 항불안작용이 없는 약물을 선택할 수도 있다. 환자의 상태에 따라서 불안증상 조절을 위한 약물 사용에 각별한 주의가 필요하다.

  • A large proportion of patients with schizophrenia show a poor response to first-line antipsychotic drugs, which is termed treatment-resistant schizophrenia. Previous studies found that a different neurobiology might underlie treatment-resistant schizophrenia, which necessitates the development of different therapeutic approaches for treating treatment-resistant schizophrenia. This study reviewed previous studies on the pathophysiology of treatment-resistant schizophrenia and the pharmacological intervention, and forthcoming investigations of treatment-resistant schizophrenia are suggested.

기분장애 환자의 비만에 대한 약물치료 (Pharmacotherapy for Obesity in Mood Disorders)

  • 기분장애에서 과체중과 비만의 문제가 일반인구보다 더 심각하고, 심혈관 치명률의 증가와 무관하지 않기 때문에 비만에 대한 치료가 필수적이다. 일반적인 상황에서 비만에 대한 치료와 동일하게 생활습관교정과 같은 비약물치료가 선행되어야 한다. 이 같은 시도가 실패하는 경우에는 약물치료가 필요하다. 기분장애의 비만치료로 공식 승인된 약은 없다. 따라서 일반인구의 비만에 승인된 약을 처방하거나 여러 연구를 통해서 효과가 입증된 약을 처방하게 된다. 여러 치료 지침과 연구 등은 orlistat, metformin, topiramate와 bupropion 등이 효과적임을 지지한다.

  • Pharmacological management is the first choice for treatment of the trigeminal neuralgia patients; however, if this mode of treatment fails a minimally invasive procedure should be performed. One of the most commonly used procedures is conventional radiofrequency lesioning of the Gasserian ganglion. Despite its popularity and success rate, this technique has disadvantages such as diminished corneal reflex, masseter weakness, numbness and anesthesia dolorosa. As a result, many studies have been conducted in an attempt to find a better method of treating trigeminal neuralgia. We report here a case of a trigeminal neuralgia patient that was treated with pulsed radiofrequency lesioning of the supraorbital and supratrochlear nerve due to pain in the frontal head that was refractory to the pharmacological treatments. Following the procedure, the Visual Analogue Scale score for pain decreased to 1-2/10 and the pain relief persisted for 7 months. These results indicate that pulsed radiofrequency treatment of the peripheral nerve may be useful for trigeminal neuralgia patients that do not respond to pharmacological treatments.

신원방우황청심원의 뇌허혈 및 중추신경계에 미치는 영향 (Pharmacological Action of New Wonbang Woohwangchungsimwon Pill on Cerebral Ischemia and Central Nervous System)

  • In order to investigate pharmacological properties of New Wonbang Woohwangchungsimwon Pill (NSCH) and Wonbang Woohwangchungsimwon Pill (SCH), the effects of NSCH and SCH on cerebral ischemia and central nervous system were compared. Cerebral ischemia insult was performed using unilateral carotid artery occlusion in mongolian gerbils. The histological observations showed preventive effects of NSCH and SCH treatments with ischemia-induced brain damage. The ATP in brain tissue was decreased in vehicle-treated ischemic gerbils. This decrease was prevented by SCH treatment. In contrast to what was inhibited by NSCH and SCH treatments. While NSCH and SCH had no effects on the hexobarbital-induced sleeping time, they prevented the seizures induced by electric shock and strychnine. NSCH and SCH showed sedative effect in rotarod and spontaneous activity test. Furthermore, NSCH and SCH showed anti-stress effect. Our findings suggest that the pharmacological profiles of NSCH on cerebral ischemia and central nervous system are similar to those of SCH.

Inhibitory Effect of Bacopa monniera on morphine Induced Pharmacological Effects in Mice

  • The effects of the alcoholic extract of Bacopa monniera (BMA) on morphine-induced pharmacological activities were studied in mice. Oral administration of the extract (40 mg/kg) significantly inhibited morphine-induced analgesic tolerance, withdrawal symptoms, hyperactivity, reverse tolerance, Dopamine receptor supersensitivity and apo-morphine-induced climbing behaviour in mice. The results of this study showed that, alcoholic extract of Bacopa monniera (BMA) exerted inhibitory effect against morphine-induced pharmacological effects, suggesting that the extract could be useful in the treatment of morphine toxicity.

The Pharmacological Properties of Silymarin and Its Constituents

  • Silymarin is a standardized extract obtained from the seeds and fruits of Silybum marianum L., or commonly called milk thistle, a member of Carduus marianum family that contains mix of flavonolignans. Some epidemiological and preclinical studies revealed that S. marianum L. has been used for herbal remedies for centuries for its pharmacological activity. In this review, pharmacological studies in vitro and in vivo of silymarin are discussed thoroughly stressing on antioxidant, antimicrobial, antiviral, and anti-carcinogenic aspects of silymarin. In addition, the protective influences of silymarin on some organs such as heart, liver, bone, and neuron tissue are reviewed as well. This review would be useful for further study regarding the potential of natural plant, notably silymarin, and its therapeutic potential in the prevention and treatment of diseases.

인삼의 본초 및 약리 효능과 향후 연구방향 (Herbal and Pharmacological effects of Ginseng Radix and Strategy for Future Research)

  • For the purpose of wide application of Korea Ginseng Radix (KGR) as a tonic and medicinal drug, bibliographical study on Ginseng Radix (GR) was done about its herbal and pharmacological effects known so far. Herbal effect of GR was studied in 28 classical books including Shennongbencaojing(신농본초경), the oldest herbal book in oriental medicine and also its pharmacological effects was compared with its herbal effect. In bibliographical study 38 kinds of efficacies of GR were referred in those books, for example, in fields of gastrointestinal tract, heart, psychology, body fluid, hormone and respiratory tract in order from statistical analysis. GR was chiefly used for supplementing Qi, vital energy in oriental medicine. However, experimental study on respiratory diseases was not reported except lung cancer yet, while GR was used for the treatment of asthma mostly mixed with other herb in oriental medicine. So far research with GR was performed chiefly for isolation of constituents as saponins and evaluation of its efficacy experimentally by KGTR (Korea Ginseng Tobacco Research Institute). From the above studies, to make KGR used widely than before, I suggest study with KGR should be done according to oriental principles such as Qi and tasters and combination theory between herbs excepting typical analytical research with saponins.

  • Phaleria macrocarpa (Scheff.) Boerl. is a dense evergreen tree of the family Thymelaeceae. This plant is popular with the name of Mahkota dewa, which is literally translated as God's Crown. All parts of this plant including fruits, seeds, stem, and leaves have well known therapeutic properties and have been extensively used in traditional medicine for the treatment of various diseases such as cancer, diabetes mellitus, allergies, kidney disorders, blood diseases, stroke, and acne with satisfactory results. Scientific findings on bioactivities of P. macrocarpa also demonstrated different pharmacological properties of various parts of this plant including cytotoxic, antidiabetic, antioxidant, anti-inflammatory, antibacterial, and antihypertensive activities. Phytochemicals studies of P. macrocarpa revealed the presence of several classes of compounds such as benzophenones, terpenoids, xanthones, lignans, acids, and sugars. This review aims to provide a critical overview on botanical description, traditional usage, phytochemicals, and pharmacological activities of P. macrocarpa.
