  • 본 논문에서는 홈 헬스케어에서 건강측정을 위하여 가장 많이 사용되는 개인 건강 기기(personal health device)인 혈당계, 혈압계, 심박측정계(heart rate monitor), 산소포화도 측정계(pulse-oximeter), 심전도 측정계(ECG monitor)의 5가지 홈 헬스케어 의료기기를 구성하고 표준 핵심 기술 요소인 ISO/IEEE 11073, HL7을 준용하는 게이트웨이 플랫폼을 스마트폰에 적용하여 기존의 홈 헬스케어 시스템을 구성하는 단말기 형태의 게이트웨이나 홈 서버 하드웨어 구성을 필요로 하지 않으면서 집안의 고정된 게이트웨이로 인해서 생기는 위치종속성 문제를 해결할 수 있는 PMG(personal mobile gateway) 기반의 홈&모바일 헬스케어 시스템을 개발하고자 한다.

Controlled Inquiry Rates of Clinical Interviews in Telehomecare

  • Conceived to acquire personal information for an electronic medical record, the clinical interview contains probing questions. The number and type of inquiries are assumed to fulfill medical protocols, and therefore are deemed essential for treatment - but the rate can and should be controlled. High rates of inquiry merely intimidate the patient and affect replies. The purpose of this paper is to mathematically formulate permissible rates of clinical interviews held during telehomecare virtual visits and designed to avoid patient anxiety. Mental stress is derived as a function of the weight of importance assigned by the patient, virtual visit duration, and the rate of questioning in the direction of greater sensitivity. Two operations are of interest: Collecting and recording information by the provider, and maintaining synchrony of questions and answers by the patient. The Lorentz transformation yields the patient’s view of the operational rates. Conservation of information momentum is postulated and applied before and after replies are recorded. It is shown that the weight of importance designated by the patient to collecting and recording personal information is driven by a singularity that depends on the rate of questioning. The findings should serve as a guideline in interviewer training programs.

  • Purpose: The traditional ethical study only suggests a blurred insight on the research using medical big data, especially in this rapid-changing and demanding environment which is called "4th Industry Revolution." Current institutional/ethical issues in big data research need to approach with the thoughtful insight of past ethical study reflecting the understanding of present conditions of this study. This study aims to examine the ethical issues that are emerging in recent health care big data research. So, this study aims to survey the public perceptions on of health care big data as part of the process of public discourse and the acceptance of the utility and provision of big data research as a subject of health care information. In addition, the emerging ethical challenges and how to comply with ethical principles in accordance with principles of the Belmont report will be discussed. Methods: Survey was conducted from June 3th August to 6th September 2020. The online survey was conducted through voluntary participation through Internet users. A total of 319 people who completed the survey (±5.49%P [95% confidence level] were analyzed. Results: In the area of the public's perspective, the survey showed that the medical information is useful for new medical development, but it is also necessary to obtain consents from subjects in order to use that medical information for various research purposes. In addition, many people were more concerned about the possibility of re-identifying personal information in medical big data. Therefore, they mentioned the necessity of transparency and privacy protection in the use of medical information. Conclusion: Big data on medical care is a core resource for the development of medicine directly related to human life, and it is necessary to open up medical data in order to realize the public good. But the ethical principles should not be overlooked. The right to self-determination must be guaranteed by means of clear, diverse consent or withdrawal of subjects, and processed in a lawful, fair and transparent manner in the processing of personal information. In addition, scientific and ethical validity of medical big data research is indispensable. Such ethical healthcare data is the only key that will lead to innovation in the future.

  • Medical information is an important personal information for patients, and it must be protected. In particular, when medical personnel approach electronic medical records, authentication for enhanced security is essential. However, the existing public certificate-based certification model did not reflect the security characteristics of the electronic medical record(EMR) due to problems such as personal key management and authority delegation. In this study, we propose a fingerprint recognition-based authentication model with enhanced security to solve problems in the approach of the existing electronic medical record system. The proposed authentication model is an EMR system based on fingerprint recognition using PEMS (Private-key Escrow Management Server), which is applied with the private key commission protocol and the private key withdrawal protocol, enabling the problem of personal key management and authority delegation to be resolved at source. The performance experiment of the proposed certification model confirmed that the performance time was improved compared to the existing public certificate-based authentication, and the user's convenience was increased by recognizing fingerprints by replacing the electronic signature password.

Clinical Information Interchange System using HL7-CDA

  • In highly developed society, information and communication technologies are widely used for better medical services. These information and communication technologies should be more and more acceptable in all hospitals for exchange medical records. EMR becomes more convenient than the previously used paper charts. It will be able to record medical institutions every time and dual treatment. Each is different specifications for each medical institution to use the program or document to exchange it. The personal clinic records still does not exchange well. To solve this gap between medical alienation, this paper describes the concepts of HL7-CDA and proposes types of telemedicine system. To resolve time and space constraints, new form of treatment methods presents in future directions after described about related systems. CDA enables electronic medical records to the each medical center and gradually expanded by exchanging the patient's medical records. This paper is using XML-based CDA documents as a hierarchical for medical information exchange standards compliant HL7-CDA documents. It could be possible currently used structural variety of multimedia data. Thus It is able to send and receive HL7-CDA-based medical information and clinical information to identify the medical institutions of medical information with interchange system design and building standards, and through mutual exchange of clinical information.

  • We developed a Smartphone application based on PHR(Personal Health Records) provided by a tertiary hospital to provide users personalized diet, exercise contents. It uses Relevance ratio algorithm of diet and excercise being correlated with diseases listed on PHR to provide personalized contents to management of individual diseases and record the amounts of food taken and exercise along with calories consumed through exercise. Besides, developed U-Health Service also considering users' conditions and conveniences, as a service to use contents and record suitable for the individuals who share the information on restaurants locations, their menus, and nutrition based on users' location information.

  • Rapid integration of information technology into health care systems has included the use of highly portable systems-in particular, personal digital assistants (PDAs). With their large built-in memories, fast processors, wireless connectivity, multimedia capacity, and large library of applications, PDAs have been widely adopted by physicians and nurses for patient tracking, disease management, medical references and drug information, enhancing quality of health care. Many health-related PDA applications are available to both dietetics professionals and clients. Dietetics professionals can effectively use PDAs for client tracking and support, accessing to hospital database or information, and providing better self-monitoring tools to clients. Internship programs for dietetics professionals should include training in the use of PDAs and their dietetics applications, so that new practitioners can stay abreast of this rapidly evolving technology. Several considerations to keep in mind in selecting a PDA and its applications are discussed.

  • MIR system is a nationwide medical record information system that makes medical information available to any hospital and health institution at any time, and information in the system mostly requires high security. In particular, personal information related to patients and doctors, medical technology information and each hospital's digital information are used very frequently and are likely to be modified for illegal use. Thus we need to develop a system equipped with security measures to prevent information leakage while providing medical information service effectively.

The Effect of Ethical Values on the Patient's Personal Information Protection of College Students Majoring in Healthcare Administration (보건행정 전공 대학생들의 윤리적 가치관이 환자 개인정보 보호인지에 미치는 영향)

  • This study was conducted to investigate 820 university students in C and G areas to analyze the effects of ethical values of health administration major college students on the perception of patient personal information protection and to present important basic data for the development of education programs. The data were analyzed through SPSS/WIN 18.0 Program. As a result of analysis, the average of personal information protection of college students majoring in health administration was low at 2.04 ± 0.24, and ethical values were idealistic tendency 2.51 ± 0.32 points, and relativistic tendency was 2.34 ± 0.34 points, which showed a high idealistic tendency. Ethical values were also significantly related to idealistic ethics and relativistic ethics in terms of the level of awareness of patient personal information protection and the perception of patient personal information protection exposure. Therefore, in order to increase the protection of patients' personal information of university students majoring in health administration, the correct ethical values should be established, and systematic and continuous education is needed for this purpose.
