국내 주의력결핍 과잉행동 장애 아동 및 청소년의 약물요법 패턴 및 지속성 (Patterns and Persistence of Pharmacotherapy for Children and Adolescents with Attention Deficit Hyperactivity Disorder in South Korea)

  • Objective: This study aimed to assess treatment persistence in Korean children and adolescents with attention deficit hyperactivity disorder (ADHD) and the factors influencing their adherence to ADHD pharmacotherapy. Methods: The study included patients between 6 and 18 years of age with ADHD who were taking various formulations of methylphenidate and atomoxetine on June 1, 2014. Patients were dichotomized as "persistent" or "non-persistent", depending on whether they continued ADHD therapy for 6 months (therapy persistence). We also investigated if the patients were taking the same medication(s) as before and also classified the patients as "medication persistent" or "non-persistent". Patient' characteristics were correlated with therapy persistence and medication persistence. Multiple logistic regression analyses were performed to assess potential risk factors for treatment persistence. Results: Overall, 3,317 patients were included in the analysis. A majority of patients were taking stimulants (82.0%), 16.2% were taking non-stimulants and 1.8% were taking a combination therapy of stimulants and non-stimulants. After 6 months, 2,290 patients (69.0%) continued to take medication for ADHD with 1,953 patients taking the same medication(s) as 6 months previously. Common positive factors for therapy persistence and medication persistence were identified as younger age, retardation, and developmental delay, and long-acting formulations of methylphenidate as either monotherapy or in a combination therapy may be used. Conclusion: ADHD medications were proven to improve academic performance and social skills of children. Collaboration between patients, parents, school staffs, and prescribers is required to improve the persistent use of ADHD medications.

$Er^{3+}$를 첨가한 $CaZrO_3$ 축광성 형광체의 합성 및 발광 특성 분석 (Synthesis and luminescent properties of $Er^{3+}$ doped $CaZrO_3$ long persistent phosphors)

  • 새로운 $CaZrO_3:Er^{3+}$ 축광성 형광체를 전통적인 고상반응법으로 제조하였으며, 분쇄한 축광성 형광체를 X 선 회절 분석, 광발광 분석, 열발광 분석과 휘도계를 통하여 장잔광 특성을 분석하였다. X 선 회절 분석 결과 순수한 $CaZrO_3$ 결정상을 확인 하였으며, 고온의 질소 분위기에서 합성한 경우 446 nm 와 550 nm의 넓은 발광 피크가 나타났다. 합성한 장잔광 특성의 형광체의 발광 지속시간은 254 nm UV lamp로 여기 시킨 후 어두운 곳에서 6시간 이상 스스로 발광 하였다. 발광 피크는 $Er^{3+}$ 이온의 $^5D_{5/2}{\rightarrow}^4F_{9/2},\;^2H_{12/2},\;^4S_{3/2}{\rightarrow}^4I_{13/2}$ 그리고 $^2G_{9/2}{\rightarrow}^4I_{13/2}$ 전이에 의한 것이며, 잔광 특성은 $CaZrO_3$ 격자 내에 적당한 trap center가 형성 된 것으로 판단된다.

  • 최근 정보보안 환경 변화에 따른 사이버 침해, 사업 기밀유출, 글로벌 보안위협 등의 정보자산에 대한 지속적인 공격으로 위협이 되고 있다. 이는 기존 정보보호시스템에서 대응이 어려운 APT 공격, 우회접근공격 및 암호화 패킷에 대한 공격 등에 대한 탐지와 조치가 가능한 접근에 대해 지속적인 모니터링의 수행이 요구되고 있다. 본 논문에서는 지능형지속위협 공격경로차단을 위한 예방통제(Prevention Control)로 중요한 자산 식별하고 위험을 미리 제거하기 위하여 취약성 분석, 위험분석을 통한 정보통제 정책을 수립하고 서버접근통제, 암호화통신 감시를 통해 탐지통제(Detection Control)를 수립하고 패킷 태깅, 보안플랫폼, 시스템백업과 복구를 통해 교정통제(Corrective Control)를 하여 지능화된 침해대응(Intelligent Violation of Response) 할 수 있도록 정보보호시스템 운영관리 방법을 제안한다.

베이지안 게임모델을 적용한 P-persistent MAC 기반 주기적 안정 메시지 전송 방법 (A Stability of P-persistent MAC Scheme for Periodic Safety Messages with a Bayesian Game Model)

  • 차량통신간 자동차의 정보를 포함하는 메시지인 비콘을 주변의 자동차들에게 주기적으로 전송함으로 차량간의 안전을 보장해야 할 필요가 있다. 그러나 IEEE 802.11p/WAVE 환경에서는 혼잡상황에서 비콘 전송을 경쟁윈도우(Contention window)를 기반으로 한 MAC (Medium Access Control)에서 해결하도록 하였기 때문에 비콘 전송 과정에 충돌이 발생한다. 본 논문에서는 차량 혼잡 상황에서 비콘의 충돌을 막기 위해서 MAC 구조를 슬롯 p-persistent CSMA (Carrier Sense Multiple Access)라 가정하여 전송 확률에 대한 보수행렬(payoff)을 이용한 비 협조적 베이지안 게임이론을 적용하여 베이지안 내쉬 균형점(BNE)을 도출하였다. 본 논문에서는 도출된 BNE를 가지고 포화 상태에서 비콘 전송률을 높일 수 있는 혼잡 제어 알고리즘을 제안했다. 제안된 알고리즘으로 경쟁윈도우크기와 주변 자동차 수에 관련된 함수를 통해 전송 확률을 계산하였다. 시뮬레이션 결과를 통해서 제안된 알고리즘이 안정적으로 동작함을 보였다.

영속적 링크를 제공하는 XRI 기반의 웹 링크 구조 (A Web Link Architecture Based on XRI Providing Persistent Link)

  • 차세대 웹은 표현 중심의 웹을 데이터 중심의 웹으로 이끌 것으로 예측되며, Web 2.0과 시맨틱 웹의 기술적 결합이 될 것이다. 차세대 웹은 시맨틱 처리, 웹 플랫폼과 데이터 결합이 매우 중요한 기술적 요소이다. 이를 가능하게 하기 위해서, 필수적인 기술 중의 하나가 링크 깨짐 (Link Rot) 현상을 해결하는 것이다. 링크 깨짐 현상은 단순히 사용자의 불편함을 야기하는 것에 그치지 아니하고, 정보 시스템 분야에서 데이터 무결성 정보 손실, 서비스결손 등의 다양한 문제들의 원인이 되고 있다. 본 논문에서는 웹의 고질적인 문제점인 링크 깨짐 현상을 해결하기 위한 웹 링크 구조에 대해 제안한다. 제안된 웹 링크 구조는 OASIS에서 제안한 XRI 기반으로 구성되었으며, URL rewriting 기능과 결합하여 영속적 링크 (persistent link)를 지원하도록 설계하였다. 서버-사이드(server-side) 기술로 설계되었기 때문에 기존의 방식에 비해 상호운용성, 투명성, 적용성 면에서 우수하다. 이에 더하여, 해당 구조는 상황인지 링크 변환에 사용될 수 있는 메타데이터 식별 기능을 제공한다.

신생 매일지속두통 환자에 대한 통합 한방치료 증례보고 (Case Report of Korean Medical Treatment for a Patient with New Daily, Persistent Headache)

  • Objectives: This case report presents the effects of Korean medicine treatment in a patient with a new daily, persistent headache. Methods: A 58-year-old female patient with a new daily, persistent headache was treated with Yonggakgyodaeyeong-tang and Kyoungbang Chungsanggyuntong-tang granules, acupuncture, pharmacopuncture, and chuna therapy for 68 days. Headache and neck disability were assessed using a numeric rating scale (NRS), the headache impact test-6 (HIT-6), and the neck disability index (NDI). Results: After 68 days of treatment, headache was relieved, as measured by an NRS decrease from 7 to 4. In addition, the HIT score decreased from 30 to 18, and the NDI score decreased from 42 to 20. Conclusion: These results show that a daily, persistent headache can be relieved with Korean medicine treatment. However, additional well-designed studies are required to confirm these findings.

Modus Operandi: Irrigation of the Modified Eloesser Flap in Heterogeneous Suppurative Lung Pathologies

  • Background: Refractory empyemas with collapsed lung and persistent bronchopleural fistulas pose significant problems to thoracic surgeons and impose a substantial burden in terms of morbidity and mortality. The modified Eloesser flap procedure is a useful palliative option for clearing infections. Herein, we present our experiences with the modified Eloesser flap procedure in mixed suppurative lung pathologies with a new technique of irrigation for persistent infection. Methods: A retrospective review was carried out of 56 patients who underwent the modified Eloesser flap with continuous irrigation at Katurba Medical College. These patients had severe morbidities and were not suitable for major thoracic resection surgery, and electively underwent modified Eloesser flap surgery. Regular follow-up was done at 1, 3, 6, and 12 months. Patients with persistent infections were treated with our continuous irrigation technique. Results: The most important finding was that all patients with active sputum acid-fast bacilli-positive findings became sputum smear-negative during the first month of follow-up. Half (50%) of the patients had a patent stoma. Eleven patients had persistent infections, necessitating continuous irrigation. The infection was fully cleared after 1 month in 9 patients, while 2 patients required second irrigation and continued to receive follow-up. In the remaining 50% of the patients, the stoma closed completely, and the lung expanded fully. Conclusion: The modified Eloesser flap is a simple procedure. In suppurative pathologies, infections were well controlled and the general condition of the patients improved. Our continuous irrigation method showed promising results in patients with persistent purulent discharge.

영속 메모리를 이용한 스마트폰 버퍼 캐시의 선별적 플러시 정책 (Policy for Selective Flushing of Smartphone Buffer Cache using Persistent Memory)

  • 버퍼 캐시는 스토리지의 느린 속도를 완충하는 중요한 역할을 하지만, 데이터의 유실을 막기 위한 주기적인 플러시 연산으로 인해 스마트폰에서 그 효과가 크게 떨어진다. 본 논문에서는 소량의 영속 메모리에 선택적인 플러시 정책을 적용하여 스마트폰 버퍼 캐시의 플러시 오버헤드를 크게 줄일 수 있음을 보인다. 이는 스마트폰 앱의 I/O 분석 결과 대부분의 파일 쓰기가 소량의 핫 데이터에 집중돼 있는 반면 상당 부분의 파일 데이터는 1회성 쓰기에 국한한다는 점에 근거한다. 제안하는 기법은 플러시 상황 발생 시 자주 수정되는 데이터를 영속 메모리로 우회 플러시하고 그렇지 않은 데이터만을 스토리지로 플러시한다. 이를 통해 스토리지 쓰기량을 크게 줄이는 동시에 영속 메모리의 공간 효율성을 높인다. 인기 있는 스마트폰 앱의 I/O 트레이스를 이용한 재현 실험을 통해 제안하는 기법이 스토리지 쓰기량을 평균 25.8%, 최대 37.8%까지 줄임을 보인다.

Implications for selecting persistent hot spots of schistosomiasis from community- and school-based surveys in Blue Nile, North Kordofan, and Sennar States, Sudan

  • In several schistosomiasis-endemic countries, the prevalence has remained high in some areas owing to reinfection despite repeated mass drug administration (MDA) interventions; these areas are referred to as persistent hot spots. Identifying hotspots is critical for interrupting transmission. This study aimed to determine an effective means of identifying persistent hot spots. First, we investigated the differences between Schistosoma haematobium and Schistosoma mansoni prevalence among school-aged children (SAC) estimated by a community-based survey, for which local key informants purposively selected communities, and a randomly sampled school-based survey. A total of 6,225 individuals residing in 60 villages in 8 districts of North Kordofan, Blue Nile, or Sennar States, Sudan participated in a community-based survey in March 2018. Additionally, the data of 3,959 students attending 71 schools in the same 8 districts were extracted from a nationwide school-based survey conducted in January 2017. The community-based survey identified 3 districts wherein the prevalence of S. haematobium or S. mansoni infection among SAC was significantly higher than that determined by the randomly sampled school survey (e.g., S. haematobium in the Sennar district: 10.8% vs. 1.1%, P<0.001). At the state level, the prevalence of schistosomiasis among SAC, as determined by the community-based survey, was consistently significantly higher than that determined by the school-based survey. Purposeful selection of villages or schools based on a history of MDA, latrine coverage, open defecation, and the prevalence of bloody urine improved the ability for identifying persistent hot spots.

Behavior of F shape non-persistent joint under experimental and numerical uniaxial compression test

  • Experimental and discrete element approaches were used to examine the effects of F shape non-persistent joints on the failure behaviour of concrete under uniaxial compressive test. concrete specimens with dimensions of 200 cm×200 cm×50 cm were provided. Within the specimen, F shape non-persistent joint consisting three joints were provided. The large joint length was 6 cm, and the length of two small joints were 2 cm. Vertical distance between two small joints change from 1.5 cm to 4.5 cm with increment of 1.5 cm. In constant joint lengths, the angle of large joint change from 0° to 90° with increments of 30°. Totally 12 different models were tested under compression test. The axial load rate on the model was 0.05 mm/min. Concurrent with experimental tests, numerical simulation (Particle flow code in two dimension) were performed on the models containing F shape non-persistent joint. Distance between small joints and joint angles were similar to experimental one. the results indicated that the failure process was mostly governed by both of the Distance between small joints and joint angles. The axial loading rate on the model was 0.05 mm/min. The compressive strengths of the samples were related to the fracture pattern and failure mechanism of the discontinuities. Furthermore, it was shown that the compressive behaviour of discontinuities is related to the number of the induced tensile cracks which are increased by increasing the joint angle. In the first, there were only a few acoustic emission (AE) hits in the initial stage of loading, and then AE hits rapidly grow before the applied stress reached its peak. Furthermore, a large number of AE hits accompanied every stress drop. Finally, the failure pattern and failure strength are similar in both approaches i.e., the experimental testing and the numerical simulation approaches.
