침술 후 발생한 표재 비골 신경 및 심 비골 신경의 손상(1예 보고) (Compression Neuropathy of Superficial Peroneal Nerve and Deep Peroneal Nerve Following Acupuncture Treatment (A Case Report))

  • Acupuncture has been widely used to treat a variety of disease and symptoms. But various complications have been reported. Among them, peripheral nerve injuries have been reported less frequently than other complications. The purpose of this report is to describe what we believe to be the first case of delayed superficial and deep peroneal nerve compressive neuropathy caused by fibrotic mass formed by neglected broken acupuncture needle.

  • Objectives: The Purpose of this study is to report the effects of Korean medical treatment on patient of peroneal neuropathy caused by sitting cross-legged. Methods: The patient diagnosed as peroneal neuropathy was treated by Acupuncture, Bee Venom therapy, Herbal medicine. To evaluate the effects of Korean treatment, we used Range of Motion(ROM) of Ankle, Visual Analog Scale(VAS), Numeric Rating Scale(NRS), Ankle-Hind-foot Scale(AHS), Digital Infrared Thermal Imaging(DITI). Results: After treatment, ROM of Ankle, VAS, NRS, AHS were improved. Temperature Difference(${\Delta}T$) was decreased on DITI. Conclusions: This study suggests that Korean medical treatment might be effective on sitting cross-legged patient diagnosed as Peroneal Neuropathy.

체중 감량 중에 발생한 편측 족하수: 증례보고 (Unilateral Foot Drop During Weight Reduction:A Case Report)

  • Peroneal neuropathy scarcely can develop after massive weight reduction. A 21-year-old man complained left foot drop after 28% weight reduction (from 94 kgs to 67.5 kgs). During previous seven months, he played PC games with sitting cross-legged more than seven hours a day. In addition, he started a heavily restricted diet three months ago. Except for those, he had neither any medical history nor trauma to his knee. Electrophysiologic study showed the partial conduction block of left peroneal neuropathy at the fibular head. Four-week well balanced diet and physical therapy improved his foot drop. For the prevention of peroneal neuropathy related to weight reduction, well balanced diet and lifestyle modification are needed.

  • 출생시에 나타나는 신생아 종아리신경병증은 아직 국내에는 보고된 적이 없는 매우 드문 질환으로, 대부분이 자연 치유되는 양성 경과를 취한다. 신생아의 홑신경병증은 대개 산과적 합병증이지만, 출생 전 원인도 고려하여야 한다. 저자들은 볼기태위로 인해 제왕절개술로 만기 태어난 신생아에서 발견된 발처짐을 보고한다. 생후 4일째 시행한 전기생리학적 검사에서 종아리신경의 복합근육활동전위 소실과 앞정간근과 긴엄지폄근의 탈신경전위가 관찰된 것으로 미루어 자궁 내에서 발병한 종아리신경병증으로 추정한다. 조기에 전기생리학적검사를 시행하고 추적검사를 하면 발병시기와 병리적 기전 및 예후를 판단하는데 도움이 된다.

Anatomical Variants of "Short Head of Biceps Femoris Muscle" Associated with Common Peroneal Neuropathy in Korean Populations : An MRI Based Study

  • Objective : In Asians, kneeling and squatting are the postures that are most often induce common peroneal neuropathy. However, we could not identify a compatible compression site of the common peroneal nerve (CPN) during hyper-flexion of knees. To evaluate the course of the CPN at the popliteal area related with compressive neuropathy using magnetic resonance imaging (MRI) scans of healthy Koreans. Methods : 1.5-Tesla knee MRI scans were obtained from enrolled patients and were retrospectively reviewed. The normal populations were divided into two groups according to the anatomical course of the CPN. Type I included subjects with the CPN situated superficial to the lateral gastocnemius muscle (LGCM). Type II included subjects with the CPN between the short head of biceps femoris muscle (SHBFM) and the LGCM. We calculated the thickness of the SHBFM and posterior elongation of this muscle, and the LGCM at the level of femoral condyles. In type II, the length of popliteal tunnel where the CPN passes was measured. Results : The 93 normal subjects were included in this study. The CPN passed through the "popliteal tunnel" formed between the SHBFM and the LGCM in 36 subjects (38.7% type II). The thicknesses of SHBFM and posterior portions of this muscle were statistically significantly increased in type II subjects. The LGCM thickness was comparable in both groups. In 78.8% of the "popliteal tunnel", a length of 21 mm to <40 mm was measured. Conclusion : In Korean population, the course of the CPN through the "popliteal tunnel" was about 40%, which is higher than the Western results. This anatomical characteristic may be helpful for understanding the mechanism of the CPNe by posture.

Associations of nerve conduction study variables with clinical symptom scores in patients with type 2 diabetes

  • Background: Diabetic peripheral polyneuropathy (DPN) is associated with a variety of symptoms. Nerve conduction studies (NCSs) are considered to be the gold standard of nerve damage assessments, but these studies are often dissociated from the subjective symptoms observed in DPN patients. Thus, the aim of the present study was to investigate the correlations between NCS parameters and neuropathic symptoms quantified using the Michigan Neuropathy Screening Instrument (MNSI). Methods: Patients with type 2 diabetes mellitus (T2DM) with or without symptoms of neuropathy were retrospectively enrolled. Demographic data, clinical laboratory data, MNSI score, and NCS results were collected for analysis; DPN was diagnosed based on the MNSI score (${\geq}3.0$) and abnormal NCS results. Pearson's correlation coefficients were used to evaluate the relationships between MNSI score and NCS variables. Results: The final analyses included 198 patients (115 men and 83 women) with a mean age of $62.6{\pm}12.7$ years and a mean duration of diabetes of $12.7{\pm}8.4$ years. The mean MNSI score was 2.8 (range, 0.0-9.0), and 69 patients (34.8%) were diagnosed with DPN. The MNSI score was positively correlated with the median motor nerve latency and negatively correlated with the median motor, ulnar sensory, peroneal, tibial, and sural nerve conduction velocities (NCVs). When the patients were categorized into quartiles according to MNSI score, peroneal nerve conduction velocity was significantly lower in the second MNSI quartile than in the first MNSI quartile (p = 0.001). A multivariate analysis revealed that the peroneal NCV was independently associated with MNSI score after adjusting for age, sex, and glycosylated hemoglobin A1c (HbA1c) levels. Conclusions: The present results indicate that a decrease in peroneal NCV was responsible for early sensory deficits in T2DM patients.

발처짐 증상으로 발현된 총비골신경의 슈반세포종 (Schwannoma of Common Peroneal Nerve Presenting)

  • Foot drop is usually derived from peroneal nerve injury. Traumatic causes of peroneal nerve injury are more common than insidious causes including metabolic syndromes and mass lesions. We present a case with common peroneal neuropathy due to schwannoma, which is extremely rare. Complete excision of the mass lead to a gradual improvement of the symptoms. Schwannoma should be considered as a cause of common peroneal neuropathy.

전침 및 봉약침 치료로 호전된 특발성 말초신경병증의 증례보고 (A Case of Idiopathic Peripheral Neuropathy Improved by Combined with Electroacupuncture and Bee Venom Acupuncture)

  • The purpose of this study is to report the effects of electroacupuncture and bee venom (BV) acupuncture for a patient suffering with idiopathic peripheral neuropathy. There was a one patient who was diagnosed as idiopathic peroneal neuropathy and tibial neuropathy received electroacupuncture and BV acupuncture at acupoints on peroneal and tibial nerve pathway. The evaluation of clinical outcome was done by range of motion (ROM), manual muscle test (MMT), perimeter of lower leg, neuropathy pain scale (NPS). After treatment, the patient showed improvement in ROM and MMT. The patient's perimeter of lower leg was increased, NPS was decreased. This study suggests that the treatment of acupoints on the damaged nerve pathways could be an effective treatment for peripheral neuropathy, although further studies will be needed.

  • Objectives : The purpose of this study was to explore the effectiveness of Korean medicine treatment and the Muscle Energy Technique on foot drop through a case study with a patient with common peroneal neuropathy. Methods : A patient with severe foot drop after an ankle sprain was hospitalized and treated with acupuncture and traditional medicine and the Muscle Energy Technique. The effect of the treatment was evaluated with the Numeric Rate Scale(NRS), dorsiflexion angle evaluation, and the Manual Muscle Test (MMT). Results : The results of the NRS, dorsiflexion angle evaluation and MMT showed greatly improvementwith results changing from 8 to 2, 0 to 15, and Grade 2 to Grade 4, respectively. Conclusions : This study suggests that Korean medicine treatment and the Muscle Energy Technique could be effective treatments for foot drop with common peroneal neuropathy. However further clinical studies are needed to establish a definite conclusion.

Schwannoma of the Superficial Peroneal Nerve Presenting as Sciatica

  • Schwannomas are benign nerve sheath tumors that can present in various locations and they have variable symptoms. However, schwannoma of the superficial peroneal nerve is rare, and only a limited number of cases have been reported. The authors here describe a case of schwannoma of the superficial peroneal nerve, which was initially considered as a L5 radiculopathy because of its clinical similarity. In the differential diagnosis of nontraumatic and nonarthritic pain of the lower leg and foot, benign tumors, particularly schwannomas of the peroneal nerves should be considered. Treatment by excision can result in relief of the symptoms and maintenance of function.
