This study aimed to investigate the cusp size and morphological characteristics of permanent mandibular molars in Koreans with reference to the hypoconulid, and to analyze the differences and correlations between both sexes as well as between first and second mandibular molars. We obtained data from dental casts of 110 adults (78 males and 32 females). Mesiodistal and buccolingual diameters of first and second mandibular molars, the area of five cusps (protoconid, metaconid, hypoconid, entoconid, and hypoconulid), as well as the total cusp area and occlusal table area were measured. Paired t-test was performed to analyze the morphological differences between first and second mandibular molars and the sex differences between both sexes using SPSS program. Crown diameters and cusp areas of mandibular first molars were larger than those of mandibular second molars in both sexes. The hypoconulid was the most variable in size and morphological pattern among the five cusps, and the first molars showed a higher incidence of hypoconulid than the second molars. Except for the entoconid area of the first molar (p=0.06) and the hypoconulid area of the second molar (p=0.24), all other mean values were larger in males than in females, demonstrating a significant sexual dimorphism. These data suggest that the teeth which develop late in ontogeny tend to be smaller in size and more variable in morphological characteristics.
Retrostpective study of two groups of patients was conducted to evaluate the physiologic drift of the mandibular teeth following the extraction of four first premolars. The concept of physiologic drift, commonly referred to as 'driftodontics', following first premolar extractions has been gaining acceptance in the orthodontic community, the exact nature and amount of drift has not been adequately documented. There were also no guide lines as to when drift should be allowed to now. The purpose of this study was to quantify physiologic drift of the untreated mandibular dentition following extraction of the four first premolars during the early permanent and late permanent dentition stages. The early permanent dentition extraction sample(Group 1) included 26 Patients and the mean age at pretreatment was approximately 13.5 years. The observation period following extraction was approximately 6.96 months. The late permanent dentition extraction sample(Group 2) included 31 patients. The mean age at pretreatment was 21.3 years, followed by a observation period of 7.26 months. During the observation period, except for the extractions, no other mandibular therapy was rendered. Pre-and post-treatment lateral cephalograms and dental casts were analyzed. The obtained results were as follows 1. Group 2 showed marked changes in movements of the mandibular incisors and canines but minimal changes in molars. 2. The amount of changes in movements of the mandibular incisors and canines were significantly greater in Group 1 than in Group 2. The results showed no differences in rates of molar movements between groups. 3. Physiologic drift of the dentition produced desirable changes such as decreased Incisor Irregularity.

Anatomical shape of the mandibular ramus, which includes the area from the rear of the mandibular second molar to the mandibular posterior border and from the mandibular sigmoid notch to the inferior mandibular border, must be carefully considered to perform orthognathic surgery. The locations of the lingula and mandibular foramen in medial side of mandibular ramus are one of the most important factors to decide the location of the horizontal medial osteotomy in sagittal split ramus osteotomy and to select the line of vertical osteotomy in intraoral vertical ramus osteotomy. Sixty-five different Korean human dry mandibles were surveyed. All mandible have permanent dentition including complete eruption of the mandibular second molar. The locations of the lingula and mandibular foramen in medial side of the ramus were identified and following results were obtained. Anterior ramal horizontal distance from lingula was $16.13{\pm}3.53mm(range:8.6{\sim}24.3mm)$, anterior ramal horizontal distance from mandibular foramen was $23.91{\pm}4.79mm(range: 14.1{\sim}39.7mm)$, horizontal width of mandibular foramen was $2.79{\pm}0.95mm(range:1.5{\sim}6.1mm)$, height of lingula was $10.51{\pm}3.84mm(range:3.1{\sim}22.4mm)$, vertical distance from sigmoid notch to lingula was $19.82{\pm}5.11mm(range:9.1{\sim}35.3mm)$. From this study, the result could be used to select the location of osteotomy lines and to decide amount of periosteal elevation to avoid injury of neurovascular bundle, and to accomplish the appropriate split in Korean patients in mandibular orthognathic surgery.
Moebius syndrome (MBS) is a congenital neurologic disorder that causes cranio-facial abnormalities. It involves paralysis of the VI and VII cranial nerves and causes bilateral or unilateral facial paralysis, eye movement disorder, and deformation of the upper and lower limbs. The orofacial dysfunctions include microstomia, micrognathia, hypotonic mimetic and lip muscles, dental enamel hypoplasia, tongue deformity, open bite or deep overbite, maxillary hypoplasia, high arched palate, mandibular hyperplasia or features indicating mandibular hypoplasia. This case report presents a 7-year-old male patient who was diagnosed with MBS at the age 2 years. The patient displayed typical clinical symptoms and was diagnosed with Class II malocclusion with a large overjet/overbite, tongue deformity and motion limitation, and lip closure incompetency. Treatment was initiated using a removable appliance for left scissor bite correction. After permanent tooth eruption, fixed appliance treatment was performed for correction of the arch width discrepancy and deep overbite. A self-ligation system and wide-width arch form wire were used during the treatment to expand the arch width. After 30 months of phase II treatment, the alignment of the dental arch and stable molar occlusion was achieved. Function and occlusion remained stable with a Class I canine and molar relationship, and a normal overjet/overbite was maintained after 9.4 years of retainer use. In MBS patients, it is important to achieve an accurate early diagnosis, and implement a multidisciplinary treatment approach and long-term retention and follow-up.

Supernumerary tooth was resulted from excessive proliferation of dental lamina and associated with familial tendency and a congenital syndrome such as Cleidocranial dysostosis or Gardner's syndrome. Incidence reports identify a range of $0.3{\sim}0.8%$ in primary dentition, $1.0{\sim}3.5%$ in permanent dentition with males being affected twice as frequently as females, maxilla nine times as frequently as mandible. The most common supernumerary tooth is the mesiodens, which located between the maxillary central incisors, and the next common site is the fourth molar and lateral incisors. Supernumerary teeth are uncommon in the mandible, but premolars are the most common supernumerary teeth and occurrence is very rare in the incisor region of the mandible and the incidence is 2%. We need a early diagnosis and appropriate treatment plan because of possiblilty of diastema and eruption failure displacement, rotation of the associated permanent teeth, root resorption and dentigerous cyst with presence of the supernumerary teeth. In this two case, one supernumerary tooth located in the mandibular incisor region, the other supernumerary tooth located in premolar region. We could get normal alignment of mandibular dentition by extraction and orthodontic treatment.

The specimens were ground and the pulp chamber was examined with a magnifier and explored with sharp explorer. The study showed the shape of the pulp chamber, number of root canals, and the type of canal orifice. The results were as follows; 1. In so far as observing the shape of the pulp chamber of the teeth, 58.8% of the teeth were square, 34.2% were triangle and 7.0% were ovoid shape. 2. 58.8% of the specimens have 4 root canal orifices, 34.2% have 3 root canal orifices, 7.0% have 2 root canal orifices. 3. 41.2% of the specimens show 'H' shape, 17.6% show 'Square' shape, 31.6% show 'T' shape, 2.0% show 'reverse-T' shape and 7.0% show 'I' shape.

The purpose of this study was to investigate the effects of cleft on mesiodistal dimensions of permanent teeth in unilateral cleft lip and palate patients. Mesiodistal dimensions of permanent teeth were measured to the nearest 0.01mm on plaster models of 50 subjects with unilateral complete cleft lip and palate, 10 siblings and 50 Controls. The results were as follows : 1. Tooth size discrepancy in the cleft group was significant in all regions except maxillary cuspid, mandibular cuspid and mandibular first premolar. 2. Some of the mesiodistal dimensions of the teeth on the cleft side were significantly smaller than those of their antimeres on the non-cleft side in the cleft group. 3. A comparison of mesiodistal dimensions of the teeth for the right and left sides of the control group showed no statisically significant differences excepts maxillary lateral incisor. 4. Asymmetries of mesiodistal dimensions of the teeth in the sibling group was not found except maxillary first molar.
Objective: The aim of this study was to investigate the incidence of the C-shaped canal of permanent mandibular second molar (PMSM) in Korean sub-population using Cone-Beam CT (CBCT) data and analyze the types of C-shaped canal. Materials & Methods: The protocol for this study was approved by the Institutional Review Board at the Pusan National University Hospital (E-2011039). Among the CBCT images taken of patients who visited the St. Bennedict Dental Hospital (Busan, Korea) from May 2008 to April 2011 for implant surgery and surgical removal of impacted teeth, high-quality CBCTs from 705 patients (361 male and 342 female) were screened and 607 PMSMs of 383 patients were evaluated retrospectively. All PMSMs were anatomically analyzed in detail by using image viewer software (EasyDent; Vatech). PMSMs were evaluated in the axial plane to investigate the shape of root and canals. The C-shaped canals were classified into five types. The total incidence, gender ratio, bilateral and unilateral appearance. and the correlation between right-side and left-side occurrences of C-shaped PMSMs were computed and compared statistically using the chi-square test. Results: Among the 607 PMSMs of 383 CBCTs of 187 females and 196 males, 198 PMSMs(32.6%) had C-shaped root and 158 PMSMs(26.0%) had C-shaped canals. The shape of C-shaped root canals at the furcalion level did not have significant change at the level of mid root (P<0.0001). Female had more prevalence of C-shaped root canals than male (P<0.0001). The prevalence of bilateral occurrence of C-shaped root canals was higher than unilateral occurrence. Conclusions: The occurrence of C-shaped PMSMs among a Korean population was 32.6% and was higher than other countries and ethnicities. Understanding the prevalence of PMSMs with a C-shaped root and/or canal in a Korean population may be useful for successful endodontic treatments.

Ectopic eruption should be understood as a change in the course of the normal eruption path of a dental bud at any moment its origin. An example of this alteration is the dental transposition, a rare and more specific dental anomaly that may be defined as a change of position between two teeth. This case shows ectopic eruption of transposed mandibular lateral incisor beneath primary first molar at the first transitional period of the mixed dentition The crown of the lateral incisor has tipped distally, compelling root resorption and exfoliation of the adjacent primary cuspid and primary first molar. The reason for such eruption is not clearly understood, but it may involve; (1)trauma history, (2)prolonged retention of the deciduous teeth, (3)premature exfoliation of the deciduous teeth, and (4)genetic factor. Treatment is divided into interceptive and definitive treatment. Ectopically erupting mandibular incisor tends to become transposed with the adjacent cuspid and thus seems to warrant early orthodontic intervention. Early treatment may obviate later extraction or transposition of the incisor and canine in the permanent dentition. Timing is an important factor to be considered regarding in the correction of the lateral incisor transposition. This case advocates treatment with an active orthodontic therapy at the early stage of the mixed dentiton, before the eruption of the permanent cuspid.
