Acute upper limb ischemia in a patient with newly diagnosed paroxysmal atrial fibrillation

  • Acute limb ischemia (ALI) due to an embolism is associated with high mortality rate and poor prognosis, and early diagnosis with prompt revascularization is required to reduce the risk of limb amputation or even death. The etiologies of ALI are diverse, and it includes an embolism from the heart and thrombotic occlusion of the atherosclerotic native vessels, stents, or grafts. An uncommon cause of ALI is acute arterial thromboembolism, and atrial fibrillation (AF) is the single most important risk factors for systemic thromboembolism. It is important to correctly identify the source of ALI for secondary prevention, as it depends on the underlying cause. Percutaneous transluminal angioplasty (PTA) has been proven to be a safe and effective treatment for focal atherosclerotic and thrombotic occlusive diseases of the aorta and its major extremity branches. Herein, we report on a 77-year-old female patient with acute upper limb ischemia, treated by PTA using a catheter-guided thrombectomy. He was newly diagnosed with paroxysmal AF (PAF) while evaluation the cause of his acute arterial thromboembolism. We recommend that cardiologists always consider PAF as a possible diagnosis even in patients without any history of AF under ALI because it is possible to develop thromboembolism in clinical practice.

하지동맥 재건술 후 폐쇄성 합병증에 대한 임상적 고찰 (Occlusive Complications after Lower Limb Arterial Bypass Surgery)

  • 배경: 동맥 재건술 후에 발생하는 폐쇄성 합병증은 치료가 어렵고 재발 가능성이 높은 것으로 알려져 있다. 저자들은 동맥 재수술에 관하여 임상양상 및 수술방법, 사용된 이식편, 술 후 결과들을 분석하여 이식편 폐쇄에 영향을 미치는 인자를 알아보고 효율적인 치료 방침을 세우는 데 도움을 얻고자 하였다. 대상 및 방법: 1998년 1월부터 2002년 12월까지 부산대학교병원 흉부외과에서 시행된 173명의 동맥재건술 후 재수술을 받은 33명(55예)을 대상으로 조사하여 임상적 특성과 치초 수술의 유형, 재수술시까지의 경과한 시간, 사용한 이식편, 치료의 결과 등을 후향적으로 분석하였다. 결과: 33명 중 32명이 남자였고 재수술 당시의 평균연령은 63.5세였다. 첫 수술에서 재수술까지의 기간은 11.9개월이었고 원인질환은 동맥경화증이 28예, 버거씨병 5예였다. 동반된 질환은 고혈압 19예, 당뇨병 11예, 심부전 6예 등의 순이었다. 한 환자당 평균 1.67회의 재수술이 시행되었고 치초의 수술유형은 대퇴동맥-슬와동맥 우회술이 19예로 가장 많았다. 수술에 사용되었던 이식편은 PTFE가 25예, Dacron이 6예였고 이식편에 따른 재수술률은 차이가 없었다. 재수술은 혈전제거술이 20예, 혈관 성형술 18예, 우회로 재조성술이 13예, 요부 교감신경절제술이 4예에서 시행되었다. 재수술의 결과는 하지의 기능회복이 15명, 하지보존 7명, 슬상부절단 5명, 슬하부 절단 3명, 사망 3명이었다. 걸론· 하지 동맥 재건술 후 발생하는 폐쇄성 합병증은 유입 동맥이나 유출동맥의 폐쇄가 발생하며 생기는 경우가 많았다 치료의 방침은 최초의 수술방법과 사용된 이식편에 따라 다를 수 있고 무엇보다도 이식편의 감시가 중요한데 이차 수술의 시행빈도가 가장 높은 시기인 술 후 1년 이내의 이식편 감시가 중요하다고 생각한다. 또한 재우회로술 뿐만 아니라 방사선학적 중재술 및 교감신경 절제술과 같은 부가적인 치료도 적극적으로 시행하여야 하지 보존율의 향상을 기대할 수 있을 것이다.

$PGE_1$ 요도좌제의 제조 및 평가 (Preparation and Evaluation of $PGE_1$ Urethral Suppositories)

  • [ $PGE_1$ ] is an endogenous substance of potent vasodialator as well as inhibitor of platelet aggregation. It has been used therapeutically in peripheral arterial occlusive disease and impotence. Intracavernous injection of $PGE_1$ for erectile dysfunction has been established for several years as a treatment option for erectile dysfunction of diverse etiologies, but this mode of administration is limited by penile discomfort, pain at the injection site, inconvenience and noncompliance. As the matter of worse, the $\beta-hydroxy$ moiety of $PGE_1$ is extremely susceptible to dehydration in solution to give inactive $PGA_1$ and $PGB_1$. For the improvement of stability, rapid absorption at action site and the convenience of application, $PGE_1$ was formulated as urethral suppositories of three types of formulations, such as PEG, witepsol, and the mixture of PEG and witepsol. The stability test of $PGE_1$ and the release test in urinary suppositories were performed. Futhermore, the effect of enhancers and vehicle composition on the penetration of $PGE_1$ through excised rat skin was evaluated by permeability coefficient and enhancement ratio.

Femoro-Supragenicular Popliteal Bypass with a Bridging Stent Graft in a Diffusely Diseased Distal Target Popliteal Artery: Alternative to Below-Knee Popliteal Polytetrafluoroethylene Bypass

  • Background: Lesions in distal target arteries hinder surgical bypass procedures in patients with peripheral arterial occlusive disease. Methods: Between April 2012 and October 2015, 16 patients (18 limbs) with lifestyle-limiting claudication (n=12) or chronic critical limb ischemia (n=6) underwent femoral-above-knee (AK) polytetrafluoroethylene (PTFE) bypass grafts with a bridging stent graft placement between the distal target popliteal artery and the PTFE graft. Ring-supported PTFE grafts were used in all patients with no available vein for graft material. Follow-up evaluations assessed clinical symptoms, the ankle-brachial index, ultrasonographic imaging and/or computed tomography angiography, the primary patency rate, and complications. Results: All procedures were successful. The mean follow-up was 12.6 months (range, 11 to 14 months), and there were no major complications. The median baseline ankle-brachial index of 0.4 (range, 0.2 to 0.55) significantly increased to 0.8 (range, 0.5 to 1.0) at 12 months (p<0.01). The primary patency rate at 12 months was 83.3%. The presenting symptoms resolved within 2 weeks. Conclusion: In AK bypasses with a diffusely diseased distal target popliteal artery or when below-knee (BK) bypass surgery is impossible, this procedure could be clinically effective and safe when used as an alternative to femoral-BK bypass surgery.

대동맥하단부-장골동맥의 급,만성 폐쇄성 동맥질환 2례 (Surgical Treatment of Aortoiliac Arterial Occlusion: Report of 2 Cases)

  • The recent development of cardiovascular surgery as well as aortoarteriogaphy has been established excellent operative result with great aid of limb-salvage. However, less consideration or less experience still exists on the regard of vascular accident and vascular disease, as well as vascular surgery in Korea. During the last 13 years, we experienced only two cases of aorto-iliac occlusion,acute and chronic, regardless of having had more than 300 cases of mitral valvotomy and gradual increasing tendency of arteriosclerosis and hypertension in Korea. Therefore it is noteworthy to report the cases in order to promote the consideration for vascular surgery. Case 1; 52 year old female who had 20 years history of mitral stenosis with uricular fibrillation and received medical treatment for recent 1 year in the medical department. 10 days before admission, acute saddle emboli developed and 15 days after the onset, embolectomy through both common femoral arteries on the groin and abdominal approach was made. The progression of emboll to the right popliteal bifurcation was found by arteriography on operating table and retrograde flushing with heparin solution by the polyethylene catheter inserted through posterior tibial artery. The operation was successful, but 9 hours after operation sudden death occurred. Considering this case, first, mitral valvotomy already before might prevent peripheral embolizatlon, secondarily, the more early detection and surgery might also prevent the progression of emboli. Thirdly, although preoperative or postoperatlve heparinization is controversial for mitraI stenosis, heparinization might prevent additional emboli to vital organs in this case Cases 2; 66 year old female who had 4 years history of left hip and calf intermittent claudication and has had rest pain, inability to walk and ischemic necrosis on the the left leg since last 3 months prior to admission to the orthopedic department under the suspicion of herniated disc. Absence of pulsation on the groin and aortography evidenced aortoillac occlusion predominantly on the left side. Thromboendarterectomy was made and the operative result was successful with absence of claudication, healing of ulcer and aortographic patency of occlusive site. This chronic occlusion is considered to result from arteriosclerosis in origin with the evidence of moderate hypertension, x-ray evidence of calcified plaque on the aortic knob and operative finding of palpable plaques.
