THE EFFECT OF TWO KINDS OF PERIODONTAL LIGAMENT MANAGEMENT UPON PERIODONTAL HEALING AND ROOT RESORPTION AFTER REPLANTATION IN RATS (쥐치아재식 모델에서 치주인대의 처리방법에 따른 치주조직의 치유와 치근흡수에 관한 연구)

  • The effect of bench drying or removal of the periodontal ligament with NaOCl upon periodontal healing and root resorption after replantation of molars was studied in rats. A total of 40 Sprague-Dawley female rats were used and fed a powdered purina rat chow diet containing 0.4% beta-aminoproprionitrile. The maxillary first molars were extracted and the periodontal ligaments were removed either by bench drying for 15 minutes or by immersion in 2.5% NaOCl solution. The rats were sacrificed at 5, 10, and 21 days by heart infusion. In order to observe the effect of 0.5% stannous fluoride, $10^{-4}M$, $10^{-2}M$, and 1M etidronate disodium on the early stage of periodontal healing, the periodontal ligament was removed with 2.5% NaOCl followed by immersion of the molar in the respective solutions for 5 minutes. The rats were sacrificed after 10 days and the following results were obtained. 1. The removal of the periodontal ligament with 2.5% NaOCl seemed to be more effective than bench drying, since the resorption area in the NaOCl treated group showed a gradual increase whereas a decline in resorption area from 5 days to 21 days was observed in the bench dried group. 2. The application of 0.5% stannous fluoride seemed to enhance the periodontal ligament attachment and active migration of fibroblasts could be observed. 3. The application of $10^{-4}M$, $10^{-2}M$, and 1M etidronate disodium led to a good periodontal ligament attachment. No evident areas of root resorption were found. 4. The use of ${\beta}$-APN made it possible to extract the maxillary first molar with all five roots intact.

Endodontic treatment enhances the regenerative potential of teeth with advanced periodontal disease with secondary endodontic involvement

  • Purpose: The aim of this study was to identify a role for endodontic intervention in enhancing the regenerative potential of the periodontal ligament when combined with periodontal treatment in seriously involved teeth with a secondary endodontic component. Methods: Patients who exhibited radiolucency extending to the periapical region, abnormal electric pulp testing values, and deep probing depth derived from primary periodontal disease with secondary endodontic involvement were included. Intentional root canal treatment was applied to those teeth in which the apical lesions were presumed to communicate with those of the periodontal lesion of the teeth that remained vital. In all three selected cases, regenerative periodontal therapy incorporating either bone graft or guided tissue regeneration was instituted 3 months after the endodontic intervention. Results: Remarkable enhancement in radiographic density was noticeable around the affected teeth as evidenced by changes in radiopacity. There was a significant reduction in the probing pocket depth and gain in the clinical attachment level. Chewing discomfort gradually disappeared from the commencement of the combined treatment. Conclusions: An intentional endodontic intervention may be a worthwhile approach for the sophisticated management of teeth suffering from serious attachment loss and alveolar bone destruction with concomitant secondary endodontic involvement.

  • Cemento-osseous dysplasias are a group of disorders known to originate from periodontal ligament tissue and involve, essentially, the same pathological process. They are usually classified into three main groups: periapical, florid, and focal cemental dysplasias depending on their extent and radiographic appearances. Radiographically, florid cemento-osseous dysplasia (FCOD) appears as dense, lobulated masses, often symmetrically located in various regions of the jaws. The best management for the asymptomatic FCOD patient consists of regular recall examinations with prophylaxis. The management of the symptomatic patient is more difficult. A case of FCOD occurring in a 52-year-old edentulous Korean female is reported which is rare with regard to race and sex.

  • A traumatically intruded tooth is one that is forcefully and abruptly dispaced from its position into the surrounding alveolar bone. Although intrusion of permanent teeth is infrequent, the sequelae compromise the longevity of the tooth and often include pulp necrosis, internal and external root resorption, rupture of periodontal ligament and loss of marginal bone. The purpose of this study was to examine three common management techniques for traumatic intrusion, observation for re-eruption, surgical repositioning & fixation and orthodontic extrusion. In the recent, the accepted treatment was to allow the permanent teeth to reerupt spontaneously for 6-8 weeks. If this did not occur, orthodontic traction was applied. The pulpal status of the teeth was monitored and either calcium hydroxide therapy or conventional endodontics was instituted following pulpal necrosis depending on the maturity of the root end. Pulpectomy and a calcium hydroxide filling were also the treatment of choice if there was evidence of internal or external root resorption. This will reduce the chance of root resorption and provide a period of monitoring prior to a definitive root canal filling.

  • Cemento-Osseous dysplasia is a benign condition of the jaw known to orginate from the periodontal ligament or medullary bone. Its treatment can be categorized according to clinical symptom. If there is no pain or other discomfort, a regular recall examination and having patients maintain proper oral hygeine are the best management. However, if clinical symptoms do exist, proper treatments such as resectioning of the mandible or mandibulectomy are needed. This clinical report describes a rare symptomatic case of cemento-osseous dysplasia which was treated with a conservative surgical method. Using this method, we obtained satisfactory results without additional grafts.

  • A-12year-old boy visited the clinic with chief complaint of anterior maxillary trauma. He was diagnosed with first degree mental retardation and cerebral disorders. By clinical and radiographic examination, intrusion of maxillary central incisors were found. The intrusion was not severe, teeth were luxated with a slight force and the prognosis was followed. After the first year, external root resorption was seen radiographically. Due to difficult behavior management, one visit root canal filling with OrthoMTA(BioMTA, Korea) which is known to generate of cementum and periodontal ligament was planned along with general anesthesia. OrthoMTA was filled from the apex to 1-2mm below cervical area and composite resin used for crown restoration. 6 months after, further resorption, discoloration and mobility was not found. This case is currently checked yearly and further research is needed for inflammatory root resorption and ankylosis.

  • While it is reasonably well known that certain dental procedures increase the temperature of the tooth's surface, of greater interest is their potential damaging effect on the pulp and tooth-supporting tissues. Previous studies have investigated the responses of the pulp, periodontal ligament, and alveolar bone to thermal irritation and the temperature at which thermal damage is initiated. There are also many in vitro studies that have measured the temperature increase of the pulp and tooth-supporting tissues during restorative and endodontic procedures. This review article provides an overview of studies measuring temperature increases in tooth structures during several restorative and endodontic procedures, and proposes clinical guidelines for reducing potential thermal hazards to the pulp and supporting tissues.

  • Dens invaginatus is a developmental anomaly resulting in a deepening or invagination of the enamel organ into the dental papilla prior to calcification of the dental tissues. The most widely used classification of dens invaginatus is the system described by Oehler categorizes invaginations into three classes as determined by how far they extend radiographically from the crown into the root. Oehler's classification type III is that the invagination extends through the root and communicates with the periodontal ligament. There is usually no communication with the pulp. In Type III lesions, any infection within the invagination can lead to an inflammatory response within the periodontal tissues giving rise to a 'peri-invagination periodontitis'. In the cases presented here, we treated two patients who were refered for 'peri-invagination periodontitis' on maxillary lateral incisor with Oehler's type III invagination by different approaches each, and they have shown satisfactory outcomes. Although there are several approaches to the management of dens invaginatus, the most important objective is to preserve the health of the pulp, which can be achieved by early diagnosis and the prophylactic treatment regardless of severity. When disease has developed, decision has to be made whether to treat the invagination and the pulp separately.

MANAGEMENT OF IMPACTED TEETH BY AUTOTRANSPLANTATION IN CHILDREN (소아에서 자가치아이식에의한 매복치의 처치)

  • We decided among extraction, orthodontic traction and autotransplantation such as direction and position of unerupted tooth, degree of developing root apex, eruption space, being of supernumerary tooth or odontoma or cyst when tooth impacted. Autotransplantation is considered when orthodontic traction is unrealistic or when tooth movement can absorb root of neighbor tooth. The prognosis for successful autotransplantation is dependent on a number of factors such as root development, surgical technique, patient's age, endodontic treatment, time and type of splinting, preservation of periodontal ligament and storage medium. Especially when severe osseous defect is being, bone graft considered for reducing of mobility and for assisting recovery. In all cases, chief complaint is unerupted tooth and various causing factors were supernumerary, odontoma, ectopia and so on. Before autotransplantation, space regaining was done if needed and demineralized freezed dried bone and autogenous bone graft was done when there is severe osseous defect by extraction of supernumerary tooth or odontoma. Splinting was removed after 2-3weeks At 3-4weeks after autotransplantation, endodontic treatment was decided. At follow up check, normal recovery was done and there was no inflammatory or replacement root resorption in periapical radiograph.
