A comparative clinical study on oxidized titanium implants and sandblasted large-grit acid etched implants in soft bone

  • Purpose: The aim of this retrospective study was to compare the survival rate of oxidized titanium implants and sandblasted large-grit acid etched implants in soft bone. Methods: 201 oxidized titanium implants were inserted in 84 patients between May 1999 and May 2004. 120 sandblasted large-grit acid etched implants were inserted in 74 patients between December 2000 and May 2004. The patients were followed-up 0${\sim}$5 years in ITI group or 0${\sim}$6 years in BRA group, respectively. The following information was collected from the patient records: age, gender, systemic disease, implant type, number, length and diameter of the implants, their location in the jaws, bone quantity, the number of failed implants, the causes of failure, and advanced surgery for bone augmentation. Results: In the oxidized titanium implants, 8 implants showed early failure, and 1 implant showed late failure, respectively. The cumulative survival rate was 95.48%. In the sandblasted large-grit acid etched implants, 1 implant showed late failure and cumulative survival rate was 99.10%. The cumulative survival rate and the survival rates in the case of the advanced procedure during the implant placement were not significantly different in both groups. Conclusions: Oxidized titanium implants and sandblasted large-grit acid etched implants can be used successfully in soft bone regardless of the surgical methods used during the implant placement. 발치와의 육아조직 이식이 치근이개 결손부의 재생에 미치는 영향 (Effect of extraction socket granulation tissue graft on the regeneration of horizontal furcation defect)

  • An ultimate goal of periodontal therapy is to stop the disease process and to regenerate a functionally-oriented periodontium destroyed as a result of periodontal disease. The purpose of this study was to observe the effect of grafting granulation tissue obtained from extraction socket on the regeneration of horizontal furcation defect. Six dogs were used in this study. All mandibular first and third premolars were extracted. At 2, 3, and 5 days after extraction, tissues were obtained from extraction socket of 1 mongrel dog and examined by light microscope. Granulation tissue obtained at 5 days after extraction was chosen as the graft material. Five days later, horizontal furcation defects were created surgically at mandibular second and fourth premolars in the right and left side of the 5 beagle dogs. The entrance area of the artificially prepared "key hole" defects were about $3\;4mm^2$. By random selections, 2 exposed furcation defects were grafted with granulation tissue obtained from extraction socket as experimental group and 1 furcation defect was as control. The flaps were replaced to their original position and sutured with 4-0 chromic cat-gut. Three dogs were sacrificed 4 weeks and two dogs 8 weeks after surgery, and the prepared specimens were examined by light microscope. At 4 weeks, furcations were filled with epithelial lining and fibrous connective tissue infiltrated with chronic inflammatory cells. New bone formation was observed in all groups. Only experimental group showed new cementum formation. At 8 weeks, new cementum, functional arrangement of new PDL fiber, root resorption, and some ankylotic union of newly formed alveolar bone and root surface were observed in all groups. Experimental group showed that epithelial downgrowth was inhibited and new bone formation was more active compared to control. The success rate of the furcation defect healing was higher in experimental group than control. These results suggested that grafting of granulation tissue obtained from extraction socket which combined with reconstructive periodontal flap surgery may promote periodontal regeneration of horizontal furcation defect.

치아절제술이 시행된 증례의 임상적 평가 : 2년 이상 경과한 증례의 평가 (Clinical evaluation of root-resected teeth clinical outcome over 2 years)

  • Periodontal disease accompany the inflammation around periodontal tissue and generally periodontal destruction is followed, This destruction often makes the molar teeth have furcation defect. And to treat molar furcation involvement, resective surgery such as root resection and ostectomy and regenerative procedure such as guided tissue regeneration were introduced. Also implant can be considered as one of the good treatment methods, Among these treatment alternatives, root resection can be considered as a good procedure in the point of saving one's natural teeth or amount of cost. Therefore the purpose of this article is to evaluate root-resected teeth which were done at least 2 years ago. 70 root-resected teeth in 58 patient who visited Kyungpook National University Hospital were included in this study. They were evaluated by two clinical method. One is subjective evaluation and another is objective evaluation. To evaluate subjective outcome, 58 patients answered to the questionnaire if they experienced tooth extraction, bleeding, swelling, pain, mobility and chewing problem. To evaluate objective outcome, 28 teeth was evaluated according to Langer's criteria. The subjective result showed 82% of success rate and 18% of failure rate. 13 of 70 teeth showed discomfort and were considered as failure, which include chewing problem (39%) and pain (23%). The objective outcome showed that 4 failure (14% failure rate) which were 2 cases of bone loss by periodontal problem, one endodontic problem and one untreatable caries. By these limited results, some of clinical consideration in root resective procedure can be suggested. Periodontal support and less occlusal loading on resected tooth should be evaluated before the procedure, moreover, good oral hygiene is essential. When these factors are considered carefully, the root resection may produce predictive outcomes in the treatment of furcational involvement.

구강내 과다출혈로 내원한 응급환자에 관한 임상적 연구 (A CLINICAL STUDY ON THE EMERGENCY PATIENTS WITH ACTIVE ORAL BLEEDING)

  • This is a retrospective study on emergency patients with active oral bleeding. The study was based on a series of 135 patients treated as emergency patients at Wonju Christian Hospital, from Jan. 1, 1997, to Dec. 31, 2001. The postoperative bleeding was the most frequent cause of active oral bleeding in emergency room and bleeding from trauma and medically compromised (bleeding disorders) patients were next in order of frequency. In the injury of maxillofacial vessels, peak incidence was occurred in the inferior alveolar vessel (42.9%), followed by the submucosal vessel of lip & cheek, the superior alveolar vessel, and sublingual vessels. The most common disease of bleeding disorders was vascular wall alteration (infection, etc), followed by liver disease, thrombocytopenic purpura, anti-coagulation drugs in order. In the characteristics of dental diseases on bleeding disorders, periodontal disease and alveolar osteitis (osteomyelitis) were more common. The hemostasis was most obtained by use of wound suture, simple pressure dressing, drainage for infection control and primary interdental wiring of fracture. In the complication group, the infrequent incidence was showed in vomiting, hypovolemic shock, syncope, recurred bleeding & aspiration pneumonia. In the uncontrolled oral bleeding, the injured vessels were suspected as skull base & ethmoidal vessels. In this study, authors found that the close cooperation between the dentistry (Oral and maxillofacial surgery) and the medicine (emergency & internal medicine) was the most important for early proper control of active oral bleeding. And then post-operative wound closure, drainage for infection control and previous systemic evaluation of bleeding disorders were critical for the prevention of postoperative bleeding in the local dental clinic.

  • Human Dura mater treated by various processes was used to restore small periodontal defects, large bony defects for improvement of new bone formation, and soft tissue defects and replace the disc of TMJ, etc.. Tutoplast Dura is the solvent-preserved Human Dura mater and sterilized by gamma radiation. In our department, Tutoplast Dura was implanted in 32 patients, from 1994, 6 to 1995, 7. We implanted the Tutoplast Dura at 11 various cysts, 6 implantations, 4 fractures, 3 clefts, 2 TMJ disease, 2 maxillary sinusitis, etc.. We performed the retrospective study about the purpose of Tutoplast Dura implantation, postoperative complication and histologic examination of biopsy specimen at implant second surgery.

재생술식을 이용한 치근단 병소를 동반한 백악질 열리의 치료 (Treatment of cemental tear associated with periapical lesion using regenerative surgery; A case report)

  • Purpose: Cemental tear is a specific type of root surface fracture characterized by a complete separation of a cemental fragment along the cementodentinal junction or a partial split within the cementum along an incremental line. It is suggested to be a factor for periodontal or periapical tissue destruction. The aim of this study is to present a diagnosis and treatment of cemental tear associated with periapical lesion with root canal treatment and regenerative periodontal surgery. Treatments: A 60-year-old male who had a history of sports trauma on the mandibular right central incisor about 10 years ago presented with apical cemental tear. Clinical examination showed a slightly dark yellowish discoloration and sinus tract that was located on the apical labial mucosa. The mobility and percussion were also assessed on the diseased tooth and recorded as $Miller^{\circ}{\phi}s$ Class II and tenderness to percussion. The probing depth was within the normal limit (<3 mm). Radiographic examination revealed a radiolucent lesion at the apical area and extended to distal aspect of the tooth along the fragment of cemental tear. After root canal treatment, periapical surgery was performed. The bony defect was exposed and then the detached root fragment was removed. Apical root resection and retrograde filling with Mineral Trioxide Aggregate (MTA) were accomplished and the bony defect was filled with deproteinized bovine bone mineral (DBBM) and covered with biodegradable collagen membrane. Results: After 9-month follow-up, healing of the mandibular right central incisor was uneventful and no swelling, purulence or pain was revealed in the associated area. Probing pocket depth was favorably stable, and the tooth mobility was decreased to the Miller's Class I. Conclusions: Apical cemental tear associated periapical lesion could be successfully treated with removal of the detached cementum in combination with apical surgery and GTR procedure.

개화성 백악질-골 이형성증: 증례보고 (Florid cemento-osseous dysplasia: a report of two cases)

  • Cemento-osseous dysplasia occurs in the tooth bearing areas of the jaws and is probably the most common fibro-osseous manifestation. They are usually classified into three main groups according to their extent and radiographic appearance: periapical (surrounds the periapical region of teeth and are bilateral), focal (single lesion) and florid (scleroticsymmetrical masses) cemental-osseous dysplasias. Florid cemento-osseous dysplasia clearly appears to be a form of bone and cemental dysplasia that is limited to the jaws. Patients do not have laboratory or radiologic evidence of bone disease in other parts of the skeleton. For asymptomatic patients, the best management consists of regular recall examinations with prophylaxis and the reinforcement of good home hygiene care to control periodontal disease and prevent tooth loss. The treatment of symptomatic patients is more difficult. At this stage, there is an inflammatory component caused by the disease and the process is basically a chronic osteomyelitis involving dysplastic bone and cementum. Antibiotics might be suggested, but are not always effective. Two cases of florid cemento-osseous dysplasia diagnosed in two Korean females are reported with a review of the relevant literature.

Pulp and periapical disease as a risk factor for osteonecrosis of the jaw: a national cohort-based study in Korea

  • Purpose: This longitudinal cohort study aimed to evaluate the relationship between osteonecrosis of the jaw and pulp and periapical disease in patients who were administered bisphosphonates. Methods: Using data from a nationwide cohort, we examined the association among dental caries, pulp and periapical disease, and osteonecrosis of the jaw in women aged >50 years who received bisphosphonates for more than 1 year between 2002 and 2015. Because of ambiguities in the diagnosis of osteonecrosis of the jaw in population-based data, we operationally defined and categorized the condition into established and potential osteonecrosis of the jaw. Results: Pulp and periapical disease significantly increased the development of both established and potential osteonecrosis of the jaw (hazard ratio, 2.21; 95% confidence interval, 1.40-3.48; and hazard ratio, 2.22; 95% confidence interval, 1.65-2.98, respectively). Root canal treatment did not have any influence on the development of osteonecrosis of the jaw. Conclusions: Pulp and periapical disease may be a major risk factor for osteonecrosis of the jaw. The study findings suggest that patients should undergo regular dental examinations to detect pulp and periapical disease before or during the administration of bisphosphonates and that root canal treatment should be considered to decrease the risk of osteonecrosis of the jaw.

치주수술 후 지각과민증에 대한 MS Coat와 엘멕스 겔처치의 임상적 효과 (Clinical Effect of MS Coat and Elmex gel on the Dentin Hypersensitivity following Periodontal Surgery)

  • Dentinal hypersensitivity is one of the complicated symptom rather than a disease. It has special reaction or pain over uncovered root by heat, mechanical or chemical stimulations, which are normal with healthy teeth, The purpose of this study is to discover rather MS Coat (oxalate-containing pre-polymerized resin suspension) or Elmex gel(amine fluoride+sodium fluoride) is effective on hypersensitivity after periodontal treatment using NRS(Numerica1 Rating Scales), which it is useful for evaluating pain level. This study has been prepared in Dankook Dental Hospital with generally healthy adult who had been suffered from dentinal hypersensitivity after periodontal treatment. Divided in three groups with saline(control group), MS Coat(test 1 group) and Elmex gel(test 2 group). And then, following evaluations were made at the end of 1 minute, 1 week, 1 month and 3 months. 1. The sequence of higher frequency & severeness of hypersensitivity is water within $7^{\circ}C$, air stream & explorer. 2. As time goes on, 1 minute, 1 week, 1 month & 3 months, severeness of hypersensitivity scored lower with water, air stream and explorer. 3. With explorer, the differences among three groups as time had to seen. 4. With air stream, the sensitivity scored lower after 1min with MS Coat, Elmex, and saline sequence. As time goes on the sensitivity was lower with MS Coat and Elmex than saline, but there was no difference between MS Coat and Elmex. 5. With water in $7^{\circ}C$, the sensitivity was much decreasing with MS Coat and Elmex than saline, but there was no difference between MS Coat and Elmex. As the result, MS Coat and Elmex are effective on hypersensitivity caused by periodontal treatment.

치과 환자로부터 발거된 치아에서의 법랑돌기의 발현빈도 (The Prevalence of Enamel Projection on Molar Teeth Extracted from Dental Patients)

  • 많은 연구에서 대구치의 백악법랑 경계부에 나타나는 법랑돌기의 발현이 치주질환과 밀접한 연관성을 보인다고 하였다. 이 연구는 서울대학교 치과병원에 내원한 환자에서 발거된 치아 660개 중 법랑돌기의 관찰이 가능하고, 제 1, 2대구치의 구별이 가능한 530개의 치아를 대상으로 치면별, 치아별 법랑돌기의 발현빈도를 알아보고자 시행하였다. 법랑돌기의 관찰이 용이하도록 깨끗이 세정한 후, 8배 전자확대경을 이용하여 상, 하악 제 1, 2대구치의 협면, 근심면, 원심면, 설면(구개면)에 나타나는 법랑돌기를 조사하였다. 법랑돌기의 발현빈도는 상악에서 45.49%, 하악에서 39.62%로, 하악에서보다 상악에서 높았으며, 제1대구치는 45.22%, 제2대구치는 39.89%로, 제2대구치보다 제1대구치에서 더 높게 나타났다. 또한 치면별 발현빈도는 전체 대상치아의 협면에서 가장 높게 나타났으며, 상악 제1대구치 협면에서 78.27%, 상악 제2대구치 협면에서 81.23%, 하악 제1대구치 협면에서 76.86%, 하악 제2대구치 협면에서 70.3%의 발현율을 보였다. 이 연구의 결과는 이전의 보고에 비해 높은 법랑돌기의 발현빈도를 보여주고 있으며, 이는 8배 전자확대경을 이용한 관찰에 의한 결과로 생각된다.
