Need Assessment for Worksite Health Promotion Program (산업장 근로자의 건강증진 프로그램 요구도)

  • This study was conducted to investigate the kinds of health promotion program which workers want, worker's intention for participation, proper method, time, duration and manager. A self-administered questionnaire method was used to collect data from 412 employees of 3 worksites in Chungpook and Kyungkee. This survey was carried out from Aug. 10 to Aug. 20, 1998. The results of this study are as follows: 1. In male, among health promotion programs, favorite ones were physical fitness(32.2%), periodic health check-up(24.6%), and stress management(18.0%). In female, among health promotion programs, favorite ones were periodic health check-up(26.0%), physical fitness(22.0%), and body weight control(19.5%). 2. The more young subjects are, the more they like physical fitness program, and the female like body weight control program regardless of BMI. Manufacturing worker preferred back pain prevention program to clerical worker did. In female, drinker preferred stress management program to non-drinker did. 3. The more old subjects are, the more they like back pain prevention program, and non-smoker preferred body weight control program to smoker did. 4. In health promotion program format which the subjects wanted, learning of self-examination techniques was 41.1%, worksite screening was 3l.0%, availability of pamphlets and audiovisual materials was 20.0%, presentation of worksite educational sessions was 7.9%, and the most wanted manager for the program was medical doctor and then nurse, physical trainer, psychological counselor. The favorite health promotion program duration was less than 30 minutes(49.6%), and the favorite time was before work(49.6%). 5. Among respondents, 48.5% was smokers, 81.8% was drinkers, 39.9% engaged in the regular exercise, 68.2% engaged in regular diet habit. In vaccination, 50.2%, if possible, wanted to be vaccinated and 37.6% never wanted to be vaccinated. 6. Ex-smoker, ex-drinker, the subjects in the regular exercise, and in the regular diet habit responded they were in good health. There was a significant difference between exercise and health status. Need and intention for participation of health promotion program were high in ex-smoker, ex-drinker, the subjects in the regular exercise, and in the regular diet habit. According to results for this study, if the favorite programs, exercise, periodic health check-up, stress management program, are operated at a proper time and with managers they want, this programs can really raise the participation of employees. And as employees want to learn self-examination techniques if worksite educational sessions are performed, health promotion program can effectively be operated.

Simultaneous Occurrence of Melanotic Macule and Melanoma in the Oral Cavity: A Case Report

  • While oral melanotic macule is regarded as the most frequent oral pigmented lesion without any harmful symptom, oral melanoma is a very rare and life-threatening disease among oral pigmented lesions. Oral melanotic macule has previously been described as benign, with no malignant potential. However, a few case reports have raised the question of malignant potential of oral melanotic macule. In this case report, we present a case of coexistence of oral melanotic macule and oral melanoma. A 66-year-old man with a chief complaint of black pigmentation on gingiva showed the lesion spread out on the right palatal gingiva, the right maxillary buccal gingiva and the right buccal mucosa, appearing to merge with one another. Biopsies were performed on the right palatal gingiva and the right buccal mucosa. While the lesion on the right palatal gingiva was diagnosed as an oral melanotic macule, the other lesion on the right buccal mucosa was diagnosed as an oral melanoma. The present case implied the possibility of malignant potential of oral melanotic macule. Therefore, oral melanotic macule needs careful periodic observation for early detection and prompt treatment of the transformed oral melanoma.

Analysis of Readmission Patients after Lumbar Microdiscectomy (요추간판 미세 현미경 수술 후 재입원 환자의 분석)

  • Objectives : After lumbar microdiscectomy there are relatively higher incidence of readmission because of various postoperative discomfort. Analyzing these readmitted patients in our private hospital where the patients can be readmitted more easily, we expect to find out which factors are associated with improvement of the surgical outcome. Patients and Methoes : After discharge all significant patient's clinical data were registered to our computerized data bank system and periodic follow-up were performed. The authors analysed 651(97.7%) cases followed up over 2 years(average 3 years) out of 666 patients who underwent a initial lumbar microdiscectomy from May, 1994 to April, 1997. Among them, 63 patients(9.7%) were readmitted. Results : Among the patients readmitted, 29 patients(4.5%) were reoperated because of recurred disc herniation, 17 patients were myofascial pain syndrome, 4 patients were discitis, 3 patients were adhesion and one patient was epidural abscess. Treatment results of these readmitted patients showed that success rate of all readmitted patients was 68.3%, reoperation was 58.6% and myofascial pain syndrome was 82.4%. Conclusion : Treatment results of the patients readmitted because of a continuous lumbago or leg pain after microdiscectomy were acceptable through the careful analysis of the causes of the recurrent symptom. Therefore, the incidence of the chronic failed back syndrome can be reduced by efforting a more active management through readmission.

Simultaneous Heat-Massage Therapy for Migraine Without Aura : A Case Report (무전조성 편두통 환자에서 온열과 마사지 동시 치료의 효과: 증례보고)

  • A migraine was a headache disorder characterized by recurrent moderate to severe headaches. The diagnosis was based on clinical signs and symptoms. Medication, physical therapy, nerve block, and nerve stimulation could be applied for treatment. This report described a case of severe migraine without aura that lasted several weeks periodically in a 59-year-old woman. Periodic headache had lasted for more than 14 years, and although she took medicines and nerve blocks, severe pain (VAS 7) was persisted. We recommended her to use the thermo-spinal massage device (CGM MB-1401, CERAGEM Inc., Cheonan, South Korea) continuously three times a week applying in semi-automatic mode around the neck for 40 minutes. There was no change in the pain scale in the automatic mode for the first 4 weeks. Subsequently, the semi-automatic mode of the cervical area was treated for 2 weeks to relieve the pain scale, and it was confirmed that the relieved state maintained for 2 months. This case highlighted the importance in considering thermo-spinal massage devices for managing migraine without aura.

Spinal Drop Metastasis from a Posterior Fossa Choroid Plexus Papilloma

  • Choroid plexus papillomas (CPPs) are typically considered as benign tumors, with a favorable long-term prognosis. Drop metastasis of CPP into the spinal subarachnoid space is rare. We report a 42-year-old woman who presented with headache and back pain 6 years after removal of a posterior fossa CPP. Magnetic resonance imaging revealed mass lesions in the lumbosacral subarachnoid space and recurrent intracranial tumor. The lesions were resected and histologically diagnosed was CPP. We consider that CPP can spread via cerebrospinal fluid pathways and cause spinal drop metastasis. Therefore, it is necessary to evaluate the whole spinal axis and to perform periodic follow-up examinations in patients with CPP.

  • Prader-Willi syndrome (PWS) is a rare genetic disorder reported rarely in dentistry. Dental practitioners should know the features of PWS because affected patients have a variety of dental symptoms. The current report describes a case of PWS. An 18-year-old male patient presented with traumatic injuries. Initial emergency treatments were performed under sedation, and further treatments were conducted under general anesthesia. After adequate healing, periodic follow-up and dietary management according to the patient's age and nutritional phase were recommended. Dental management of PWS patients consists of active preventive measures in addition to dietary consultation according to age and nutritional phase.

A Case Report of Restless Leg Syndrome in Ischemic Stroke Patient by Acupuncture (뇌경색환자의 하지불안증후군 침치료 1례)

  • Object : The purpose of this study is to report the effect of acupuncture for Restless Leg Syndrome in ischemic stroke patient. Methods : Ischemic stroke patient was admitted who was suffering from a distressing urge to move the legs and triggered by rest or inactivity accompanied with insomnia. The patient was diagnosed with Restless Leg Syndrome. In the point of Differentiation of Syndromes(辨證), we diagnosed this patient as deficiency of yin(陰虛) and treated with acupuncture. Results : After acupuncture, the symtoms including unpleasant sensation or pain, urge to move the leg and insomnia had withdrawn. Conclusions : This case suggest the possiblity of acupuncture for Restless Leg Syndrome and more researches should be followed.

  • Restless leg syndrome(RLS) is a sensorimotor disorder characterized by a distressing urge to move the legs and triggered by rest or inactivity. It is usually accompanied with insomnia. We experienced two cases of RLS among ischemic stroke patients. They complained uncomfortable sensation in their affected leg and urge to move the leg at night. We treated them with electro-acupuncture, moxibustion and herbal medicine until retiring. As a result, the symptoms including unpleasant sensation or pain, urge to move the leg and insomnia had withdrawn. These cases suggest the possibility that we can treat RLS with oriental medicine, especially in the case which is refractory to conventional medication or shows periodic leg movements during sleep.

  • Trigeminal neuralgia is defined as "a sudden, usually unilateral, brief stabbing recurrent pain in the distribution of one or more branches of the fifth cranial nerve" by the International Association for the Study of Pain(IASP). Trigeminal neuralgia is classified as an idiopathic trigeminal neuralgia with no apparent cause and a symptomatic trigeminal neuralgia which is caused by a structural lesion such as brain tumor. Over 80% of the tumors are meningioma, acoustic neuroma, and epidermoid tumors. Symptomatic trigeminal neuralgia can not be excluded even if old-aged patient does not have abnormal neurologic sign and symptom, and good response to pharmacotherapy. Therefore, initial examinations such as MRI or CT are essential to exclude symptomatic trigeminal neuralgia. When compared with CT, MRI, especially gadolinium enhanced MRI, has an increased sensitivity in the detection of intracranial lesions. The most effective medical treatment of trigeminal neuralgia is carbamazepine. The most common side effects of carbamazepine include drowsiness, dizziness, unsteadiness, nausea, anorexia. Hepatotoxicity, bone marrow depression are the most feared side effect of carbamazepine therapy but occurs rarely. It require periodic complete blood cell counts as well as hepatic and renal function tests. It has been recommended that complete blood cell counts is done every 2 weeks for the first 2months and then quaterly thereafter. Oxcarbazepine can be used if neutropenia occurs.

Experimental and Numerical Research on the Airflow Inside Asymmetric Nasal Cavities (비대칭 비강 내 공기유동에 관한 실험 및 수치해석적 연구)

  • Knowledge of the characteristics of airflow in nasal cavities is essential to understand the physiological and pathological aspects of nasal breathing. In our laboratory, a series of experimental investigations on the nasal airflow was conducted; airflow in models of normal and deformed nasal cavities under both constant and periodic flow conditions was studied by PIV. Some of the patients with asymmetric nasal cavities experience pain or discomfort, while other patients with asymmetric nasal cavities do not experience pain. Airflows inside asymmetric nasal cavities with and without obstructions due to a bent nasal septum are investigated both experimentally by PIV and numerically by using the general-purpose FVM code in order to determine the reason for the above-mentioned discrepancy. The comparisons between two cases are tried. Heat and humidity distribution are investigated numerically.
