A Case of Fitz-Hugh-Curtis syndrome in a male patient

  • Fitz-Hugh-Curtis syndrome has been described as perihepatitis associated with pelvic inflammatory disease (PID). It is classically seen in premenopausal young women who have right upper quadrant pain, usually but not always accompanied by symptoms of PID, and is frequently confused with biliary tract disease. However, the syndrome has rarely been reported in males. The predominant symptom is right upper quadrant pain, but PID may not be present in male patients. Here, we report a case of Fitz-Hugh-Curtis syndrome in a young male patient, which was diagnosed by serological tests and computed tomography. Fitz-Hugh-Curtis syndrome should be considered as a possible cause of pain in the right upper quadrant in male patients, although such a case is very rare.

오리의 Pasteurella anatipestifer 감염증 발생 (Pasteurella anatipestifer infection in ducklings in Korea)

  • This study was conducted to investigate the cause of a new duck disease occured in southern part of Korea. A meat type duck farm located in Kangjin, Chonnam Province had experienced outbreaks of septicemic disease at around 20 days of age in nearly every batch of ducklings from early spring to early summer in 1989. Main symptoms of the birds were eye and nasal discharge, depression, inappetence, diarrhea and nervous signs such as tremor and ataxia. Some birds died suddenly without any signs. Mortality reached from 20% to 80% in severe cases. The autopsy findings of the affected ducklings revealed consistantly severe airsacculitis, fibropericarditis, perihepatitis and occasionaly enteritis and distended ureter with urate deposit. A rod shaped gram-negative bacterium was isolated purely from brain and liver of the diseased ducks by culturing the specimens on blood agar for 48 hours in candle jar. The isolate neither produced hemolysis nor grew on MacConKey Agar. It formed colony relatively slowly being recognizable at least 36 hours after culturing, reaching colony size of about 1mm in diameter at 48 hours culture. The colony looked iridescent under oblique light and had muddy odor. The isolate did not ferment carbohydrates tested but produced gelatinase, hippuricase and oxidase which were considered as characteristics of P anatipestifer. The isolate induced similar signs and lesions when infected experimentally into ducks of 3 to 38 days age via intraperitoneum or intratrachea. However it did not produce any clinical signs wen inoculated via intranasal route. It produced only mild signs in chicken just injected with a very large dose. The bacteria did not produce any signas or lesions in mice. It was concluded through biochemical and physiological tests and animal inoculation tests that the new disease was caused by P anatipestifer.

닭에서 분리된 조류 병원성 대장균의 항생제 내성 양상 (Antibiotic resistance pattern of avian pathogenic Escherichia coli isolated from chickens)

  • Avian pathogenic Escherichia coli (APEC) is an important bacterial pathogen of chickens and causes colibacillosis such as airsacculitis, perihepatitis, omphalitis, peritonitis, salpingitis, and pericarditis. As the transfer of antibiotic resistance from animal to humans can be possible, surveillance on antibiotic resistance of APEC is very important. A total 34 APEC isolates from diseased chickens during the period from 2007 to 2009 were obtained. The susceptibility of the isolates to 13 antibiotics was determined by disc diffusion assay. Resistance to erythromycin was found in 97.1% of APEC isolated, followed by resistance to tetracycline (85.3%), doxycycline (82.3%), ampicillin (73.5%), sulfisoxazole (67.6%), enrofloxacin (67.6%), ciprofloxacin (64.7%), norfloxacin (61.7%) trimethoprim/sulfamethoxazole (52.9%), gentamycin (26.5%), amoxicillin (8.8%), colistin (5.9%), and amikacin (2.9%). The blaTEM genes were detected in 25 (100%) of the 25 ampicillin-resistant APEC isolates. Among the 29 tetracycline-resistant APEC isolates, tetA and tetB genes were detected in 18 (62.1%) and 9 (31%) isolates, respectively. Twenty six (76.5%) isolates were multiresistant to at least 6 antibiotics and seven (20.1%) isolates were multiresistant to at least 10 antibiotics. This results indicated that multiple antibiotic-resistant APEC is widespread in chicken flocks in Korea.

  • Fitz-Hugh-Curtis syndrome (FHCS)은 골반 내 염증과 동반된 간조직 주위염으로, 담도계 질환과 혼돈되는 우측 상복부 통증이 특징이다. 최근 한국에서도 청소년의 성적 접촉 시작 연령이 빨라짐에 따라 이와 같은 증후군의 발생률 또한 증가할 것으로 예측된다. 저자들은 역동적 복부 전산화 단층 촬영을 이용하여 동맥기의 비정상적인 간주위 피막의 조영 증강 양상으로 진단한 Fitz-Hugh-Curtis Syndrome 2례를 보고하고자 한다.

경남 서부지역 사육오리의 질병 감염 조사 (A study of infectious duck diseases in western Gyeongnam province, Korea)

  • We investigated the prevalence of infectious duck diseases using 156 ducks reared in 18 farms of western Gyeong-nam province. As a result, duck viral hepatitis (12.8%), colibacillosis (7.1%), and fungal disease (9.0%) were detected. However, avian influenza and riemerellosis were not detected. During autopsies, we could grossly observed red swollen liver (12.8%), petechial or ecchymotic hemorrhage on liver (11.5%), and fibrinous perihepatitis (9.0%). Gray-white necrotic spot (23.1%), swollen spleen (22.8%), swollen kidney (20.5%), hyperemia or hemorrhage on tracheal mucous membrane (8.3%), and nodule in long or air sac (9.0%) were also found.
