Clinical managements of implant periapical lesions: a report of three cases with five to twelve years of follow-up (임플란트 근단 병소의 임상적 접근 방법: 5 - 12년간의 증례 보고)

  • The purpose of this report is to suggest clinical managements of implant periapical lesions by presenting three clinical cases managed by either the infected form or the inactive form with the follow-up period of five to twelve years. One patient with no clinical symptom was regarded as inactive form. Two patients having pain were regarded as infected form and have been under the systemic antibiotic therapy. In one patient, the symptom subsided and the size of radiolucent lesion decreased. However, the other patient showed increased size of lesion causing the implant unstable, which leaded to remove the implant and to replace it. There was neither additional increase of the lesion nor functional problem for all three. It is important to detect implant periapical lesion in early stage before jeopardizing the stable implant and manage properly using systemic antibiotic therapy and surgical approach if needed, depending on infected form and inactive form.

  • A 9-year-old, female Yorkshire terrier and a 10-year-old, female Maltese were referred to the Veterinary Medical Teaching Hospital of Konkuk University. The dogs were presented with severe swelling and cutaneous sinus tract on the left suborbital lesion. Under general anesthesia, we performed physical, intraoral examinations and survey radiography, and diagnosed as periapical abscess in a left fourth premolar tooth. They were treated with tooth extraction, and the sinus tracts were drained with 0.05% chlorhexidine solution. After teeth extractions, medication applied by the result of antibiotic sensitivity test. During follow-up 10 months later, the left suborbital swelling had completely resolved.

A RADIOGRAPHIC STUDY OF FIBRO-OSSEOUS LESIONS IN THE JAWS (악골의 섬유성골병소에 관한 X선학적 연구)

  • This study was undertaken to document and better defined this condition to help clarify this clinical and radiographical appearances by the analysis of clinical and radiographical features of fibro-osseous lesions in the jaws. A study was made of a series of 128 cases with fibro-osseous lesions. The obtained results were as follows. 1. Fibrous dysplasia of the jaws occurred with equal predilection for males and females. But the females occurred in 68% of cemento-ossifying fibroma and 75% of periapical cemental dysplasia. 2. 43% of fibrous dysplasia and 32% of cemento-ossifying fibroma occurred in the 2nd decades and 33% of periapical cemental dysplasia in 5th decades. 3. 62% of fibrous dysplasia occurred in the maxilla, 73% of cemento-ossifying fibroma in mandible, 90% of periapical cemental dysplasia in mandible. 4. 98% of fibrous dysplasia occurred in premolar-molar region, 77% of cemento-ossifying fibroma in molar region, 68% of periapical cemental dysplasia in incisor region. 5. In serial radiographic features, mature stage were 55% of fibrous dysplasia, 45% of cemento-ossifying fibroma, 59% of periapical cemental dysplasia. 6. 87% of fibrous dysplasia had monostotic lesion, 67% of periapical cemental dysplasia had multiple lesions. 7. In fibrous dysplasia and cemento-ossifying fibroma, migration of tooth occurred in 61.7% and 36.4%, retention of tooth occurred in 4.3% and 9.1%, loss of lamina dura occurred in 6.4% and 9.1%, and root resorption had not occurred in fibrous dysplasia, but occurred in 18% of cemento-ossifying fibroma, displacement of mandibular canal occurred in 14.9% and 31.8%.

  • Apical periodontitis is inflammation of the periodontium caused by infection of the pulp canal system. Moreover, a dental periradicular lesion occurs as a periradicular tissue reaction to bacterial infection and consists of periradicular inflammation with alveolar bone destruction and root resorption, a consequence of the interaction between oral flora and the existing host defenses. Many investigations dealing with the pathogenesis and history of periradicular lesions have described histologically, immunologically, biochemically the development of the periradicular lesion;but none of these studies have shown any correlation between this lesion and several factors, the whole body disease in the worldwide.(omitted)

  • Microorganism survived in the root canal after root canal cleaning and shaping procedure is a main cause of root canal treatment failure. There are several mechanisms for the bacteria to survive in the root canal after chemomechanical preparation and root canal irrigation. Bacteria organized as biofilm has been suggested as an etiology of persistent periapical lesion. Recent studies were focus on removal of Enterococcus faecalis biofilm due to the report that the persistence of this bacteria after root canal treatment may be associated with its ability to form biofilm. Several investigations demonstrated that current root canal treatment protocol including use of NaOCl, EDTA and Chlorhexidine as irrigants is quite effective in eliminating E. faecalis biofilm. However, this microorganism still can survive in inaccessible areas of root canal system and evade host immune response, suppress immune activity and produce biofilm. Up to date, there is no possible clinical method to completely get rid of bacteria from the root canal. Once the root canal treatment failure occurred, and conventional treatment incorporating current therapeutic protocol has failed, periapical surgery or extraction should be considered rather than prolong the in effected retreatment procedure.

  • The author studied the effect of periapical bone loss on radiographic interpretation by using a dry adult human mandible. Artificial bony lesions were created at the apices of two mandibular second molars and four mandibular bicuspids. The jaw was stabilized and various artificial lesions were radiographed under ideal circumstances. Radiographic pictures were recorded and compared with the size of the actual lesions. The obtained results were as follows: 1. The size of actual lesions were always larger than their radiographic pictures. 2. The size of actual lesion had a greater discrepancy in the molar area where the cortical plates were havier, but the lesions in the bicuspid areas were only slightely larger than their radiographic pictures, and in these areas the cortical plates were quite thin. 3 Periapical lesions located in cancellous bone did not appear on a radiograph, regardless of the size of the created lesion, but when the junctional bone was involved or the cortical plate was perforated, the definite bone destructive change was observed on the roentenogram. 4. Removal of the entire buccal or lingual plate did not affect the trabecular pattern of bone on the roentgenogram. 5. When the cancellous bone and junctional bone were removed simultaneously, altered trabecular patterns were observed on the roentgenogram.

EXPERIMENTAL STUDY ON QUANTITATIVE EVALUATION OF FILM-BASED DIGITAL IMAGING SYSTEM (방사선사진용 디지털 영상시스템의 정량적 평가에 관한 실험적 연구)

  • A digital imaging system using Machintosh Ⅱ ci computer, high resolution Sony XC-77 CCD camera, Quickcapture Frame Grabber Board was evaluated for quantitative analysis of standardized periapical film with aluminum step wedge. The results were as follows: 1. Correlation between Al thickness and gray level was high-positively associated(r²=0.99, p<0.001). 2. Correlation between measured weight of experimental lesion and estimated relative lesion volume by digital subtracted radiography was also high-positively associated (r²=0.98, p<0.001). 3. As exposure time was increased, mean gray level was decreased(p<0.01) and slope of regression line between Al thickness and gray level was also decreased (p<0.01). And when the exposure time was shorter than 0.2 second, the value of r² was relatively low. On the basis of the above results, it is considered that this digital imaging system using a Macintosh Ⅱ ci computer & a high resolution CCD monochrome camera will be useful in evaluating digitized image from standardized periapical film quantitatively.

Screening panoramic radiographs in a group of patients visiting a Health Promotion Center

  • Purpose : To report the incidence of radiological findings from screening panoramic radiographs and verify the validity of the panoramic radiography for screening purposes. Materials and Methods : Six thousand one hundred and sixty panoramic radiographs taken from the patients visiting the Health Promotion Center of CNUH were selected for this retrospective study. Panoramic radiographs were examined into the following pathologic conditions : the presence of periodontal bone loss, dental caries, periapical radiolucencies, retained roots, impacted supernumerary teeth, impacted third molars, odontoma, cystic lesions other than radicular cyst, sialoliths, and mixed radiolucent-radiopaque lesions. Number of pathologic conditions and Prevalence values were recorded. Results: The prevalences of pathologic conditions were $72.9\%$ of periodontal bone loss, $32.2\%$ of dental caries, $11.9\%$ of periapical radiolucencies, $10.8\%$ of retained roots, $0.4\%$ of root fracture, $1.0\%$ of impacted supernumerary teeth, $1.0\%$ of impacted third molars, $0.06\%$ of odontoma, $0.08\%$ of cystic lesion other than radicular cyst, $0.2\%$ of prolonged retention of deciduous tooth, $0.1\%$ of sialolith, and $0.04\%$ of mixed radiopaque and radiolucent lesion. Conclusion : Although the panoramic radiograph should not be used to replace intraoral radiographic and clinical examinations, this study showed that many dental pathologic conditions could be detected on panoramic radiographs. The panoramic radiograph might serve as a diagnostic aid in dental health evaluation programs.

A Study of Usefulness of Panoramic Radiography in Case of Employees' Oral Examination (근로자 구강검진 시 파노라마방사선사진의 필요성에 관한 고찰)

  • This study was conducted to know usefulness of panoramic radiography by comparing clinical evaluation and dental panoramic radiography in case of oral examination. Following results were concluded by analyzing difference between the result of Clinical evaluation and dental panoramic radiography. According to comparison the result of clinical evaluation and panoramic radiography, In Shin's research, dental caries was higher by 23.1%, periodontal disease was 31.9%, in An's research, dental caries was 24.2%. From new point of view from panoramic radiography, impacted tooth was 33.6%, Sinus abnormalities was 11.6%, periapical lesion was 5.4% in Shin's research and periapical lesion was 17.4%, 3rd molar impaction was 15.3% and retained root was 5.3% in An's research. Any kind of caries were not found in oral examination in the 66.7% of patients among patients with dental root caries in An's research. There were misdiagnose in oral examination(even side(59.5%),proximal side(59.5%), seconds caries(44.0%).
