  • The case of the extensive dentigerous cyst treated by intubation technique is reported. 1) An incision is made, and the sterile polyethylene tube is inserted into the prepared opening so that it gently contacts the base of the lesion in 11 year old man. 2) The purpose to maintain vitality of adjacent tooth was achieved by means of intubation in lesion. 3) After 9 months, the extensive periapical lesion was completely replaced with new bone tissue.

  • Myofibroma is a benign tumor composed of fibroblasts and myofibroblasts, occasionally occurring most commonly as a solitary lesion of soft tissue, skin, or bone in children younger than 3 years of age. Solitary lesion of myofibroma is exceedingly rare in adult jaws. This report describes a rare case of myofibroma in the mandible that occurred in a 41-year-old Korean woman.

  • This is a case report of first branchial cleft cyst in 56 year old male patient, which was tentatively diagnosed as acute right submandibular abscess resulted from the periapical lesion of the lower right second molar. The results are as follows, 1. The accompanying ipsilateral inflammatory swelling resulted from the periapical lesion of lower right second molar tooth makes the diagnosis difficult. 2. The onset of this case was very late in comparison to the mean discovering age of branchial cleft cysts. 3. The plain radiography using contrast media is helpful for the diagnosis of cystic lesions within soft tissues. 4. This case in a first branchial cleft cyst(Type I) which occurs less than 1% of all branchial cleft anomalies.

EVALUATION OF CONSERVATIVE TREATMENT BETWEEN AMALGAM FILLING AND STAINLESS STEEL CROWN IN PRIMARY TEETH. (乳齒(유치)에 있어서 Stainless steel crown Amalgam과 充塡(충전)과의 비교연구(比較硏究))

  • The study was performed to evaluate effects of Conservative treatment between amalgam filling and stainless steel crown. The selected subjects for this study was 350 children who have amalgam filling or stainless steel crown. The obtained results are as followings. 1. In amalgam filling teeth, 120 teeth (60.0%) showed marginal leakage, 57 teeth (28.5%) showed fracture of filling material and 28 teeth (14.0%) teeth showed periapical lesion. 2. In crowned teeth, 6 teeth (3.0%) showed hole on the surface of crown, 29 teeth (14.5%) showed avulsion of crown and 10 teeth (5.0%) showed periapical lesion. 3. After 12 months from examining date, 69 teeth (34.5%) showed sound condition in amalgam filling teeth and 155 teeth (75.2%) showed sound condition in crowned teeth.

  • Fusion and gemination are developmental anomalies of teeth that may require endodontic treatment. Fusion may cause various clinical problems related to esthetics, tooth spacing, and other periodontal complications. Additional diagnostic tools are required for the diagnosis and the treatment planning of fused tooth. The present case report describes a case of unilateral fusion of a supernumerary root to an upper permanent central incisor with large periapical lesion in which a conservative approach was used without extraction of supernumerary tooth and obturated with mineral trioxide aggregate to reach a favorable outcome.

A COMPARISON OF PERIAPICAL RADIOGRAPHS AND THEIR DIGITAL IMAGES FOR THE DETECTION OF SIMULATED INTERPROXIMAL CARIOUS LESIONS (모의 인접면 치아우식병소의 진단을 위한 구내 표준방사선사진과 그 디지털 영상의 비교)

  • The purpose of this study was to compare the diagnostic accuracy of periapical radiographs and their digitized images for the detection of simulated interproximal carious lesions. A total of 240 interproximal surfaces was used in this study. The case sample was composed of 80 anterior teeth, 80 bicuspids and 80 molars which were prepared in order to distribute the surfaces from carious free to those containing simulated carious lesions of varying depths (0.5㎜, 0.8㎜, and 1.2㎜). The periapical radiographs were taken by paralleling technique and film used was Kodak Ektaspeed(E group). All radiographs were evaluated by five dentist to recognize the true status of simulated carious lesion. They were asked to give a score of 0, 1, 2, or 3. Digitized images were obtained using a commercial video processor(FOTOVIX Ⅱ- XS). And the computer system was 486 DX PC with PC Vision and frame grabber. The 17' display monitor had a resolution of 1280×1024 pixels(0.26㎜ dot pitch). But the one frame of the intraoral radiograph has a resolution of 700×480 pixels and each pixel has a grey level value of 256. All the radiographs and digital images were viewed under uniform subdued lighting in the same reading room. After a week the second interpretation was performed in the same condition. The detection of lesions on the monitor was compared with the finding of simulated interproximal carious lesions on the film images. The results were as follows: 1. When the scoring criteria was dichotomous ; lesion present and not present 1) The overall sensitivity, specificity and diagnostic accuracy of periapical radiographs and their digital images showed no statistically significant difference. 2) The sensitivity and specificity according to the region of teeth and the grade of lesions showed no statistically significant difference between periapical radiographs and their digital images. 2. When estimate the grade of lesions ; score 0, 1, 2, 3 1) The overall diagnostic accuracy was 53.3% on the intraoral films and 52.9% on digital images. There was no significant difference. 2) The diagnostic accuracy according to the region of teeth showed no statistically significant difference between periapical radiographs and their digital images. 3. The degree of agreement and reliability 1) Using gamma value to show the degree of agreement, there was similarity between periapical films and digital images. 2) The reliability of each twice interpretation of periapical films and digital images showed no statistically significant difference. In all cases P value was greater than 0.05, showing that both techniques can be used to detect the incipient and moderate interproximal carious lesions with similar accuracy.

  • The aim of this study was to evaluate clinician's detectability in the diagnosis of bone loss in the bifurcation of mandibular molars on periapical radiographs and Digital images. Periapical radiographs were obtained of the first molars in 2 dry mandibles after preparation of bony defects corresponding to degree I, degree II and degree III buccal furcation involvements. The radiographs were randomly presented to 39 clinicians(1 oral radiologist, 4 periodontist, 34 general dentists) who were asked to determine the presence or absence of bone loss. Periapical films were digitized with a TV camera. Digital images were assessed by 15 clinicians(1 oral radiologist, 4 periodontist, 10 general dentists). I. the overall diagnostic accuracy of Digital images for detection of bone loss in the bifurcation of mandibular molars was higher than that of the periapical radiographs. 2. the largest increase in diagnostic accuracy was found between lesion grade II and III on both radiographs and Digital images(P<0.05). 3. there was no significant difference between the standard state and the controlled contrast state on Digital images. 4. the overall diagnostic accuracy of I radiologist and 4 periodontists was better than that of the general dentists for detecting bifurcation involvements.

  • The five different types of bacteria, Bacteroides, Actinomyces, Capnocytophaga, Streptococcus, Fusobacterium which had frequently been recovered in infected canals, were investigated. The purpose of this study was to investigate the bacteriologic status in the traumatized nonvital teeth, and to investigate the effects of bacteria on the size of the le8ion and on the discoloration of teeth. The canal contents of sxiteen traumatized nonvital teeth were sampled and cultured aerobically and anaerobically for growth in five selective agar plates for five bacterial species. The sizes of the radiolucent areas in periapical films were measured and according to the sizes, the samples were divided into two groups. The discoloration of the teeth was checked and according to the existence of the discoloration, the samples were divided into two groups, also. The difference of bacterial colonial numbers in each group was investigated and the following results were obtained. 1. In traumatized nonvital teeth, all of the samples gave bacterial growth except one case. 2. Streptococcus was isolated in four cases but Bacteroides, Actinomyces, Fusobacterium and Capnocytophaga were not isolated. 3. The number of bacterial colonies was not found to be related the size of the lesion periapical films. 4. The number of bacterial colonies was not found to be related the discoloration of teeth.

FLOW CYTOMETRIC ANALYSIS OF LYMPHOCYTE AND CYCLING CELL DISTRIBUTION IN PERIAPICAL LESIONS (유세포 분석기를 이용한 치근단 병소의 임파구 조성 및 CYCLING CELL 분포에 관한 연구)

  • This study was designed 1) to compare the distributions of periapical inflammatory cells and 2) to identify lymphocytes and compare the lymphocyte distribution with T lymphocyte subpopulation and then 3) to examine the distribution of cycling cell in human dental periapical lesions. From each of the twenty-five human dental periapical lesions observed one small portion was fixed, embeded in paraffin, sectioned serially and stained with HE. The periapical inflammatory cells were counted to obtain the relative concentration of lymphocyte, plasma cell, macrophage and neutrophil. The large part of each lesion was analysed using Flow cytometer and monoclonal antibodies to obtain the relative concentration of T lymphocyte, B lymphocyte, T'helper cell and T suppressor/cytotoxic cell. In addition to that, seven human dental periapical lesions were examined with DNA analysis to observe the distribution of cycling cell. Following results were obtained: 1. 24 cases of the 32 periapical lesions examined were diagnosed as periapical granuloma and the remaining 8 cases as periapical cyst. Lymphocytes comprised 42.1% of total inflammatory cells in periapical granuloma and 41.8% in periapical cyst. Corresponding percentages for macrophages were 33.8% and 30.3%; for plasma cells, 15.9% and 19.0%; for neutrophils, 8.2% and 8.8%. 2. All of the periapical lesions examined had T lymphocyte, B lymphocyte, T helper cell, T suppressor/cytotoxic cell. And in all cases, T lymphocytes were observed predominantly more than B lymphocytes. 3. In 2 cases of the control group only T lymphocytes were found, and in the remaining 2 cases T lymphocytes were observed predominantly. 4. T helper cells were observed predominantly more than T suppressor/cytotoxic cells in all cases of perapical granulomas. 5. T suppressor/cytotoxic cells were observed predominantly more than T helper cells in 4 cases of periapical cysts (total 5 cases were examined) and only in one case T helper cells were more than T suppressor/cytotoxic cells. 6. In control group, T helper cells were predominant in 2 cases and T helper cells were equivalent to T suppressor/cytotoxic cells in one case. In remaining one case T suppressor/cytotoxic cells were predominant. 7. As the result of DNA analysis, the average proliferating indices of the various groups examined were measured as follows: in the control group 5.45%, in periapical granuloma 6.64%, in periapical cyst 10.1%. The highest index was observed in periapical cyst.

A RADIOGRAPHIC STUDY OF THE EXPERIMENTAL LESIONS IN THE MAXILLARY SINUS (상악동의 실험병소에 관한 X선학적 연구)

  • The purpose of this study was to examine the differences in representation of a globular radiopaque mass on the pantomograms and Waters' views and to compare the efficacy of periapical radiograms, pantomograms and Waters' views in detection of defects on the internal walls of the maxillary sinus. This study was performed with dried human skull. For the study of difference of radiopaque mass shadow in the two views, rubber ball with a diameter of 10㎜ was used as the experimental lesion. It was placed successively on the internal wall of the anterior, posterior, medial, lateral walls and floor of the maxillary sinus. To examine the detectability of defects for radiographic techniques, defects were formed in the anterior, posterior, medial, lateral walls, and floor of the maxillary sinus. They were formed with 0.5㎜, 0.75㎜, 1.0㎜, 2.0㎜ and 3.0㎜ sized steel round burs with a slow speed dental handpiece. By subsequently plugging the holes with zinc oxide eugenol paste, radiopaque defects were produced. After that the periapical radiograms, the pantomograms and the Waters' views were taken each and every defect. The obtained results were as follows: 1. Rubber balls placed on each internal wall of the maxillary sinus were correctly depicted on the posterior wall and the floor in case of the pantomogram, and on the anterior wall and the medial wall in case of the Waters' view. 2. On the detectability of defects for each radiographic technique, radiolucent defects were detected in different places for each technique. Periapical radiogram could detect 1.0㎜ defect on the floor of the maxillary sinus, pantomogram could detect 2.0㎜ defect on every internal wall of the maxillary sinus, and Waters' view could detect 3.0㎜ defect on the anterior wall of the maxillary sinus. 3. On the detectability of defects for each radiographic technique, radiopaque defects were detected in different places for each technique. Periapical radiogram could clearly detect 0.5㎜ defect on the floor of the maxillary sinus, pantomogram could detect 0.5㎜ defect on every internal wall of the maxillary sinus, and finally Waters' view could detect 0.5㎜defect on the anterior wall of the maxillary sinus but 0.75㎜ defect on the anterior wall, lateral wall and floor of the maxillary sinus. As the result, the periapical radiogram is the most simple and satisfactory method for investigating in the maxillary sinus. The pantomogram is suitable method for screening of changes in the maxillary sinus. And the Waters' view is available for detect of lesion in the anterior wall of the maxillary sinus. For the purpose of accurate diagnosis and evaluation of lesion in the maxillary sinus, these techniques supplement each other.
