손마사지가 경피적관상동맥중재술 환자의 불편감에 미치는 효과 (The Effect of Hand Massage on Discomfort in Patients Following Percutaneous Coronary Intervention)

  • Purpose: This study assessed the effectiveness of hand massage on reducing discomfort of patients undergoing percutaneous coronary intervention(PCI). Methods: The sample consisted of 62 patients admitted to an university affiliated hospital. The 30 patients in experimental group received hand massage developed by Snyder(1995) for 5 minutes on both hands and 32 patients in control group received the usual nursing intervention only. The outcome variable of discomfort was measured 10 minutes before and after the hand massage using Questionnaire and VAS. The data were collected from Feb. 5th to May 17th in 2007, and analysed through Chi-square, and t-test with SPSS WIN 12.0. Results: The level of discomfort measured with the questionnaire was decreased in experimental group, but increased in control group. This discomfort changes in two groups were significantly different(t=4.43, p<.001). The level of discomfort measured with VAS was also decreased in experimental group, but increased in control group. The changes were significant, too(t=5.62, p<.001). Conclusion: It was clear that hand massage could be a useful nursing intervention in reducing the discomfort of patients undergoing PCI.

관상동맥중재술을 받은 지역사회 거주 노인의 건강행위 이행에 영향 미치는 심리사회적 요인 분석 (Analysis of Psychosocial Factors Affecting Health Behavior Adherence among Community-dwelling Older Adults undergoing Percutaneous Coronary Intervention)

  • 본 연구는 관상동맥중재술을 받은 지역사회 거주 노인의 건강행위 이행에 영향을 미치는 심리사회적 요인을 분석하기 위해 수행된 서술적 조사연구이다. 경상남도 J시 G대학교병원에서 관상동맥중재술을 받고 외래를 내원한 환자 126명을 대상으로 2018년 7월 15일부터 10월 2일까지 구조화된 설문지를 사용하여 자료수집 후 SPSS/WIN 21.0으로 통계 분석하였다. 분석결과 대상자 특성에 따른 건강행위 이행은 배우자 유무, 교육수준, 주관적 건강상태, 주관적 삶 만족도에 따라 차이가 있었으며, 의료인 지지, 회복탄력성에서는 정적 상관관계가 있었고, 지각된 스트레스, 지각된 스트레스의 하위영역인 가족관계, 불안과 위축, 빈곤과 재정, 우울과는 부적 상관관계가 있었다. 다중회귀분석 결과 회복탄력성, 지각된 스트레스의 하위영역인 빈곤과 재정, 교육수준, 주관적 삶 만족도가 관상동맥중재술을 받은 노인의 건강행위 이행을 27.9% 설명하는 것으로 나타났다. 이러한 결과를 기반으로 관상동맥중재술을 받은 지역사회 거주 노인의 건강행위 이행을 향상시키기 위한 간호중재 프로그램 개발과 적용이 필요하다.

  • Calcific coronary artery disease is an increasingly prevalent entity in the catheterization laboratory which has implications for stenting and expected outcomes. With new interventional techniques and equipment, strategies to favorably modify coronary calcium prior to stenting continue to evolve. This paper sought to review the latest advances in the management of severe coronary artery calcification in the catheterization laboratory and discuss contemporary percutaneous interventional approaches.

  • 경피적 관상동맥 중재술 시 사용되는 가이드 와이어의 골절은 드문 합병증이다. 골절로 인하여 관상동맥내에 위치하는 가이드 와이어는 혈관 내막 손상과 혈소판의 응집을 촉발하여 혈전 생성을 야기할 수 있다. 좌전하행지에 위치하여 안정 시 흉통을 유발한 가이드 와이어를 성공적으로 제거하고 관상동맥 우회술을 시행하였기에 보고하는 바이다.

  • Major cardiac complication such as acute myocardial infarction can occur unexpectedly in patients without risk factors. We experienced cardiac arrest due to an unexpected acute myocardial infarction in a patient without any risk factors during head and neck reconstructive surgery. The patient was diagnosed with acute myocardial infarction after return of spontaneous circulation. With immediate percutaneous coronary intervention, the patient recovered without complications.

  • Moyamoya disease (MMD) is characterized by progressive steno-occlusive lesions of the distal or proximal branch of the internal carotid arteries, and cerebrovascular symptoms are its major complications. Extracranial vascular involvement including the coronary artery has been reported, and some case reports have described variant angina or myocardial infarction. However, no report has yet described a case of myocardial infarction after coronary artery bypass grafting (CABG). Here, we present a patient with MMD who suffered cardiac arrest caused by myocardial infarction due to a coronary spasm after offpump CABG and who was discharged successfully after treatment with a veno-arterial extracorporeal membrane oxygenator and percutaneous coronary intervention.

Spontaneous resolution of new coronary artery aneurysm following guideline-directed medical therapy after drug-eluting stent implantation

  • Here, we present a case of a 56-year-old man with acute myocardial infarction. The patient underwent percutaneous coronary intervention (PCI) at the left main bifurcation and mid-left anterior descending artery using drugeluting stents. Four months after the PCI, the patient was readmitted for cardiac arrest. Coronary angiography (CAG) revealed stent thrombosis in the left main-to-proximal left anterior descending artery and in-stent restenosis in the left main-to-proximal left circumflex artery. We performed balloon angioplasty at the left main to mid-left anterior descending artery and left main to proximal left circumflex artery stents; subsequently, blood flow was fully restored. However, contrast agent extravasation was observed outside the mid-portion of the left main artery to the proximal left anterior descending artery stent, indicating the presence of a coronary artery aneurysm (CAA) outside the stent. After guideline-directed medical therapy with dual antiplatelet agents and high-intensity statins, follow-up CAG revealed near-resolution of the CAA, absence of stenotic lesions, and good blood flow.

관상동맥질환 위험정도와 혈관탄성의 관계에서 치료지시이행의 매개효과: 경피적 관상동맥 중재술 환자 대상 (Comparison of Vessel Elasticity according to Risk Factors for Coronary Artery Disease, and the Mediating Effects of Treatment Compliance among Patients with Percutaneous Coronary Intervention)

  • Purpose: This study aimed to provide basic data for vascular health of patient who underwent percutaneous coronary intervention (PCI) by verifying the mediating effect of compliance in the relationship between risk level of coronary artery disease (CAD) and blood vessel elasticity. Methods: This is a descriptive study with 115 patients, who underwent the PCI a year ago and visited in the cardiology department from January to March, 2015. The risk level of CAD, blood vessel elasticity and the compliance were measured. For data analysis, SPSS/WIN 21.0 and AMOS (IBM) 21.0 were used. Results: There were a positive correlation with blood vessel elasticity score (i.e. inelasticity of the blood vessel wall) (r=.189) and a negative correlation with compliance (r=-.658) in mediating effect of risk level of CAD. There was a negative correlation between compliance and blood vessel elasticity (r=-.482). The direct effect (${\beta}=-.226$), indirect effect (${\beta}=.415$) and total effect (${\beta}=.186$) of mediating effect of risk level of CAD on blood vessel elasticity were significant. Compliance had a partial mediating effect of risk level of CAD on blood vessel elasticity. Conclusion: The results of this study suggest that managing and preventing moderating effect of risk level of CAD on compliance is helpful in restoring blood vessel elasticity.
