The aim of this document is to evaluate the effect of bite balance on physical fitness and motor capacity to determine the importance of the occlusal stability as a possible action mechanism of occlusal appliance including mouthguards on physical performances. We all remember that day when the prominent athlete was in the news wearing a mouthguard to improve human athletic performance. I once had investigated the determinants of athletic performance in all sorts of sports. Most of the studies had overinterpretation of results and lack of evidence to support the information. However, I discovered great expectations for new possibility in this field and settled for a more academic approach to this intriguing subject. The followings are some examples of the subject 1. Archers who wore occlusal appliances to increase the vertical dimension of occlusion by 2~3mm increased their ability to focus and to maintain good sense of balance. Their accuracy rate has been improved. The appliance was made of acrylic resin (a type of plastic). 2. Canoe players who wore occlusal appliances to increase the vertical dimension of occlusion by 5mm surpassed longtime competitor and won the race with 10 seconds gap. 3. A cycle rider who wore an occlusal appliance to treat his malocclusion surpassed his old record from 10.8 seconds to 10.3 seconds. His muscle reflexes (anaerobic exercise) and endurance (aerobic exercise) has been enhanced. 4. Occlusal appliances had a good effect on athletic performance in most sports such as swimming, diving, and weightlifting. As for the clenching on dentition, people clench their teeth when they push their physical and mental limits during sports activities. Clenching can be induced by physical exertion when maximum muscle strength is retained and this is similarly found in different types of exercises. In addition, restraint of respiratory function is observed. Therefore, creating a occlusal balance with occlusal appliance or mouthguard promoted a favorable influence when an athlete clenches.
중년 비만여성들을 대상으로 다리 근육의 정맥 혈류를 제한한 가압 걷기 트레이닝 그룹 15명으로 연구대상자를 선정하여 3주간의 가압 걷기 트레이닝을 실시한 결과 다음과 같은 결론을 얻었다. 첫째, 가압 걷기 트레이닝을 통해 LBM에서 트레이닝 전보다 후에 1.2 kg이 증가, FM과 %fat에서는 각각 2.8 kg, 3.3% 감소하였으며, 통계적으로도 유의한 차이를 나타냈다($p$<0.05). 둘째, 가압 걷기 트레이닝을 통해 $VO_2max$와 VEmax에서 트레이닝 전보다 후에 각각 3.6 ml/kg/min, 6.0 l/min 증가하였고, HRmax에서는 7 beat/min으로 감소하였으며 통계적으로도 유의한 차이를 나타냈다($p$<0.05). 셋째, 가압 걷기 트레이닝을 통해 심혈관 반응에서 트레이닝 전보다 후에서 운동 강도가 증가함에 따라 CO과 TVC가 증가하는 것으로 나타났으며, 통계적으로도 유의한 차이를 나타냈다($p$<0.05). 이상의 결과로부터 가압 걷기 트레이닝을 통해서 호흡 순환계 및 심혈관 기능의 증가와 근육량 증가에 따른 체성분의 변화를 유발할 수 있다는 것을 제시하였다. 본 연구의 결과를 토대로 향후 연구에서는 연구대상자의 범위를 넓혀 노년층까지 효과를 검증하는 연구와 3주 이상의 운동을 적용시키면 보다 높은 임상학적 효과가 나타날 것으로 판단된다.
The present study was aimed to explore the neuroprotective role of imatinib in global ischemia-reperfusion-induced cerebral injury along with possible mechanisms. Global ischemia was induced in mice by bilateral carotid artery occlusion for 20 min, which was followed by reperfusion for 24 h by restoring the blood flow to the brain. The extent of cerebral injury was assessed after 24 h of global ischemia by measuring the locomotor activity (actophotometer test), motor coordination (inclined beam walking test), neurological severity score, learning and memory (object recognition test) and cerebral infarction (triphenyl tetrazolium chloride stain). Ischemia-reperfusion injury produced significant cerebral infarction, impaired the behavioral parameters and decreased the expression of connexin 43 and phosphorylated signal transducer and activator of transcription 3 (p-STAT3) in the brain. A single dose administration of imatinib (20 and 40 mg/kg) attenuated ischemia-reperfusion-induced behavioral deficits and the extent of cerebral infarction along with the restoration of connexin 43 and p-STAT3 levels. However, administration of AG490, a selective Janus-activated kinase 2 (JAK2)/STAT3 inhibitor, abolished the neuroprotective actions of imatinib and decreased the expression of connexin 43 and p-STAT3. It is concluded that imatinib has the potential of attenuating global ischemia-reperfusion-induced cerebral injury, which may be possibly attributed to activation of JAK2/STAT3 signaling pathway along with the increase in the expression of connexin 43.
The present study aimed to explore the neuroprotective effect and possible mechanisms of rhGLP-1 (7-36) against transient ischemia/reperfusion injuries induced by middle cerebral artery occlusion (MCAO) in type 2 diabetic rats. First, diabetic rats were established by a combination of a high-fat diet and low-dose streptozotocin (STZ) (30 mg/kg, intraperitoneally). Second, they were subjected to MCAO for 2 h, then treated with rhGLP-1 (7-36) (10, 20, $40{\mu}g/kg$ i.p.) at the same time of reperfusion. In the following 3 days, they were injected with rhGLP-1 (7-36) at the same dose and route for three times each day. After 72 h, hypoglycemic effects were assessed by blood glucose changes, and neuroprotective effects were evaluated by neurological deficits, infarct volume and histomorphology. Mechanisms were investigated by detecting the distribution and expression of the nuclear factor erythroid-derived factor 2 related factor 2 (Nrf2) in ischemic brain tissue, the levels of phospho-PI3 kinase (PI3K)/PI3K ratio and heme-oxygenase-1 (HO-l), as well as the activities of superoxide dismutase (SOD) and the contents of malondialdehyde (MDA). Our results showed that rhGLP-1 (7-36) significantly reduced blood glucose and infarction volume, alleviated neurological deficits, enhanced the density of surviving neurons and vascular proliferation. The nuclear positive cells ratio and expression of Nrf2, the levels of P-PI3K/PI3K ratio and HO-l increased, the activities of SOD increased and the contents of MDA decreased. The current results indicated the protective effect of rhGLP-1 (7-36) in diabetic rats following MCAO/R that may be concerned with reducing blood glucose, up-regulating expression of Nrf2/HO-1 and increasing the activities of SOD.
Finding a head of a person in a scene is very important for taking a well composed picture by a robot photographer because it depends on the position of the head. So in this paper, we propose a robust head tracking algorithm using a hybrid of an omega shape tracker and local binary pattern (LBP) AdaBoost face detector for the robot photographer to take a fine picture automatically. Face detection algorithms have good performance in terms of finding frontal faces, but it is not the same for rotated faces. In addition, when the face is occluded by a hat or hands, it has a hard time finding the face. In order to solve this problem, the omega shape tracker based on active shape model (ASM) is presented. The omega shape tracker is robust to occlusion and illuminationchange. However, whenthe environment is dynamic,such as when people move fast and when there is a complex background, its performance is unsatisfactory. Therefore, a method combining the face detection algorithm and the omega shape tracker by probabilistic method using histograms of oriented gradient (HOG) descriptor is proposed in this paper, in order to robustly find human head. A robot photographer was also implemented to abide by the 'rule of thirds' and to take photos when people smile.
Orthodontic treatment is more complicated when both soft and hard tissues must be considered because an impacted maxillary canine has important effects on function and esthetics. Compared with extraction of impacted maxillary canines, exposure followed by orthodontic traction can improve esthetics and better protect the patient's teeth and alveolar bone. Therefore, in order to achieve desirable tooth movement with minimal unexpected complications, a precise diagnosis is indispensable to establish an effective and efficient force system. In this report, we describe the case of a 31-year-old patient who had a labio-palatal horizontally impacted maxillary left canine with a severe occlusal alveolar bone defect and a missing maxillary left first premolar. Herein, with the aid of three-dimensional imaging, sequential traction was performed with a three-directional force device that finally achieved acceptable occlusion by bringing the horizontally impacted maxillary left canine into alignment. The maxillary left canine had normal gingival contours and was surrounded by a substantial amount of regenerated alveolar bone. The 1-year follow-up stability assessment demonstrated that the esthetic and functional outcomes were successful.
취약한 구강 건강은 손상된 저작의 기능을 유발할 수 있다. 음식의 선택에서 저작의 기능이 중요한 역할을 하는지는 아직 까지도 논쟁의 여지가 있을지라도, 손상된 저작의 기능은 부적절한 음식의 선택을 하게끔하며, 따라서 영양의 섭취를 변화시킨다. 만약 이러한 가설이 맞다면, 저작 기능의 결핍은 일반적 건강에 해로운 것일 것이다. 취약한 구강의 건강과 특정한 위장의 질환 사이의 관계는 비록 과학적으로 확립되지는 않았을지라도 제안되어 왔다. 공공 건강의 정책은 자연치아를 보존함으로써 적절한 영양의 섭취를 가능케 하는 것을 강조하여야 하며, 치과의사들은 의치의 사용과 관련된 영양의 위험을 의식하여야 할 것이다.
Objectives : By analyzing symptoms of tinnitus, we tried to find out the relationship between deficiency and excess(虛實) and visceral pattern identification(臟腑辨證). By testing Tinnitus Handicap Inventory(THI), we evaluated the quality of life of people suffering from various aspects of tinnitus, as well as comparing treatment efficiency. Methods : 52 patients were recruited in this study who was ill with tinnitus. They wrote out the questionnaire about tinnitus and the THI. Results : The vast majority of high-grade tiny noise patients are deficiency type(虛症). The vast majority of low-grade roar patients are excess type(實症). Tinnitus persistence and feeling of ear occlusion were prominent in low-grade roar patients. low-grade roar patients suggest that overall quality of life is lower than high-grade tiny noise patients. In terms of treatment efficiency, spleen-stomach weakness(脾胃虛弱)-type, stomach heat(胃熱)-type and phlegm-fire(痰火)-type was higher than kidney essence depletion(腎精虧損)-type and liver qi depression(肝氣鬱結)-type regardless of the aspect of tinnitus. Conclusions : The present study suggests that tinnitus pattern may be helpful in differentiating patients with tinnitus, and the effectiveness of treatment can be predicted through differentiation.
임플란트 치료 혹은 보철수복치료를 원하는 성인환자의 경우 오랜 기간동안 변화하고 적응된 구강환경으로 인해 다양한 잠재적인 문제들을 가지고 있다. 환자 자신은 특별한 문제나 불편함 없이 지내왔지만 변화된 치아의 교합상태, 치주상태, 교합면 형태 변화 및 치아의 이동 등으로 인해 임플란트 수술 및 보철수복, 혹은 일반 보철 수복 치료를 통해 적절한 교합 상태로 개선하는 것이 어려운 경우가 많다. 치료계획을 세우고 치료하는 술자의 입장에서는 현재 상태를 그대로 유지하는 치료를 하는 것이 편할 수 있지만 치료 후 발생할 수 있는 여러 문제들을 생각하면 임플란트 수술 및 보철수복 전, 그리고 일반 보철 수복 전에 환자의 구강환경을 개선하는 치료를 우선하는 것이 아주 중요한 경우가 많이 있다. 치아의 배열과 교합개선을 위해 치료 전 교정치료가 필요한 경우는 치료에 대한 환자의 동의를 얻는 것이 쉽지 않고 비록 환자가 동의를 하더라도 불편하거나 심미적이지 않은 교정치료방법으로 인해 환자들이 치료를 포기하는 경우도 많이 있다. 디지털이 세계 전 분야의 큰 영향을 주면서 치의학에도 디지털을 활용한 치료방법들이 많이 소개되고 있으며 임플란트 수술가이드, 다양한 수복물 제작, 수술 진단등 치과 전반에도 디지털을 활용한 치료들이 대중화 되고 있다. 치과교정 분야에서는 CAD-CAM을 이용한 간접 접착법의 활용, 진단자료의 활용등으로 시작된 디지털 분야가 투명교정장치의 개발과 사용을 통해 그 영역이 상당히 넓어지고 있다. 투명교정시스템은 심미적이며 사용상의 불편함이 적고, 구강관리가 용이한 장점등을 통해 성인, 중노년 층 환자들이 거부감 없이 사용하고 있으며 이를 통해 임플란트 보철 치료가 필요한 환자들에게 효과적으로 사용되고 있으며 그 사용이 점점 증가하고 있다. 이번 증례보고에서는 교합장애 및 치아의 위치 변화로 인해 임플란트 수복치료가 필요한 50대 여자 환자의 임플란트 보철 복합 증례에서 세라핀 투명교정시스템을 통해 임플란트 수복을 위한 구강환경을 개선하고 임플란트 수술 및 보철수복을 성공적으로 하였으며 이를 보고하고자 한다.
안면비대칭은 안면골의 기형으로 인한 경우가 대부이므로 대개의 경우 수술을 동반한 치료가 이루어지고 있으며 이에 따라 3차원적인 기형인 비대칭을 진단하기 위해서는 안면 구조의 대칭성을 비교 측량하기 위한 시상기준면의 설정이 매우 중요하다. 이 연구의 목적은 이미 사용되고 있는 이러한 골격적 대칭을 비교하기 위한 시상기준면이 안면의 표층에 존재하는 연조직 중심점들과도 일치하는 지를 알아보기 위한 것이다. 총 연구대상은 비대칭군 25명(남 15명, 여 10명), 정상교합군 19명(남 11명, 여 8명)이었다. 실험군과 정상군간의 차이는 단지 Gnathion 부위에서와 Stomion에서만 존재하였으며 3가지 시상중심기준면과의 계측에서 대부분의 연조직 시상기준면은 1mm 내외의 변위를 보였다. 시상중심기준점들과 3가지 시상중심면간의 비교에서 Sa1은 나머지 중심선에 비해 Pronasale에서 변위가 크게 나타났다. 턱(Gnathion)의 변위가 시상중심기준선에서 벗어날수록 크게 영향을 받는 시상중심기준점은 3가지 시상중심기준면 모두에서 stomion 이었다. 3가지 모두의 시상기준면에서 대부분의 연조직의 시상기준점들이 적절한 변위내에 있었으나 Sa1의 경우는 Pronasale의 시상위치에 영향을 줄 수 있었다.
